目的:在真实世界的前瞻性队列研究中观察心内科门诊“极高危ASCVD”患者在治疗起始阶段采用联合降脂策略的疗效与安全性。方法:连续入组2021年1月至6月在湘雅医院心内科高脂血症专病门诊就诊的“极高危ASCVD”患者,根据其实际降脂策略分为三组:阿托伐他汀单药组;阿托伐他汀联合依折麦布组;阿托伐他汀联合依洛尤单抗组。主要观察终点为治疗一个月后低密度脂蛋白胆固醇(LDL-C)、脂蛋白a[Lp(a)]、非-HDL-C变化,次要终点为总胆固醇(TC)、甘油三酯(TG)、LDL-C、超敏C反应蛋白(hs-CRP)变化及安全性指标。结果:无论哪种联合方式,在“极高危ASCVD”患者降脂起始阶段联合降脂治疗的疗效均显著优于阿托伐他汀单药组:LDL-C、Log[Lp(a)]、非-HDL-C、TC的降幅更大,差异均有统计学意义(均P0.05)。以“LDL-C<1.4 mmol/L或<1.8 mmol/L”作为降脂达标的标准时,两组联合降脂治疗的LDL-C达标率均高于阿托伐他汀单药组,差异均有统计学意义(均P<0.05);阿托伐他汀联合依洛尤单抗组的LDL-C达标率高于阿托伐他汀联合依折麦布组,差异均有统计学意义(均P<0.05)。单独或联合“LDL下降超过50%”的降幅为达标标准,三组均无人能达标。治疗后三组间的肝脏转氨酶水平变化差异均无统计学意义(均P<0.05);三组间心肌酶同工酶(CK-MB)均下降,但差异无统计学意义(P<0.05)。与阿托伐他汀单药组相比,两联合降脂组治疗组的血糖均下降,差异具有统计学意义(P<0.05);阿托伐他汀联合依折麦布组的血糖比阿托伐他汀联合依洛尤单抗组的降幅更大,差异有统计学意义(P<0.05)。结论:起始联合降脂治疗1个月对心内科门诊“极高危ASCVD”患者的降脂效果及LDL-C的达标率优于阿托伐他汀单药组。以LDL-C<1.4 mmol/L或<1.8 mmol/L为降脂达标目标时,阿托伐他汀联合依洛尤单抗组治疗1个月后LDL-C达标率高于联合依折麦布组;以“LDL下降超过50%”的降幅作为降脂达标标准时,1个月内很难达标。门诊“极高危ASCVD”患者采用起始联合的降脂治疗1个月无不良反应。起始联合降脂策略可用于心内科门诊“极高危ASCVD”患者中LDL-C数值需早期达标的人群,阿托伐他汀联合依洛尤单抗可用于需在1个月内LDL-C<1.4 mmol/L或<1.8 mmol/L的人群。

BACKGROUND Upper gastrointestinal bleeding(UGIB)is defined as bleeding that occurs proximal to the ligament of Treitz and can sometimes lead to potentially serious and life-threatening clinical situations in children.Globally,the cause of UGIB differs significantly depending on the geographic location,patient population and presence of comorbid conditions.AIM To observe endoscopic findings of UGIB in children at a tertiary care center of Bangladesh.METHODS This retrospective study was carried out in the department of Pediatric Gastroenterology and Nutrition of Bangabandhu Shiekh Mujib Medical University,a tertiary care hospital of Bangladesh,between January 2017 and January 2019.Data collected from hospital records of 100 children who were 16 years of age or younger,came with hematemesis,melena or both hematemesis and melena.All patients underwent upper gastrointestinal endoscopy(Olympus CV 1000 upper gastrointestinal video endoscope)after initial stabilization.Necessary investigations to diagnose portal hypertension and chronic liver disease with underlying causes for management purposes were also done.RESULTS A total of 100 patients were studied.UGIB was common in the age group 5-10 years(42%),followed by above 10 years(37%).Hematemesis was the most common presenting symptom(75%)followed by both hematemesis and melena(25%).UGIB from ruptured esophageal varices was the most common cause(65%)on UGI endoscopy followed by gastric erosion(5%)and prolapsed gastropathy(2%).We observed that 23%of children were normal after endoscopic examination.CONCLUSION Ruptured esophageal varices were the most common cause of UGIB in children in Bangladesh.Other causes included gastric erosions and prolapsed gastropathy syndrome.

Colorization is the practice of adding appropriate chromatic values to monochrome photographs or videos.A real-valued luminance image can be mapped to a three-dimensional color image.However,it is a severely ill-defined problem and not has a single solution.In this paper,an encoder-decoder Convolutional Neural Network(CNN)model is used for colorizing gray images where the encoder is a Densely Connected Convolutional Network(DenseNet)and the decoder is a conventional CNN.The DenseNet extracts image features from gray images and the conventional CNN outputs a^(*)b^(*)color channels.Due to a large number of desaturated color components compared to saturated color components in the training images,the saturated color components have a strong tendency towards desaturated color components in the predicted a^(*)b^(*)channel.To solve the problems,we rebalance the predicted a^(*)b^(*)color channel by smoothing every subregion individually using the average filter.2 stage k-means clustering technique is applied to divide the subregions.Then we apply Gamma transformation in the entire a^(*)b^(*)channel to saturate the image.We compare our proposed method with several existing methods.From the experimental results,we see that our proposed method has made some notable improvements over the existing methods and color representation of gray-scale images by our proposed method is more plausible to visualize.Additionally,our suggested approach beats other approaches in terms of Peak Signal-to-Noise Ratio(PSNR),Structural Similarity Index Measure(SSIM)and Histogram.

Background: Acute Kidney Injury (AKI) stands as a prominent postoperative complication in on-pump cardiac surgery, with repercussions on morbidity, mortality, and hospitalization duration. Current diagnostic criteria relying on serum creatinine levels exhibit a delayed identification of AKI, prompting an exploration of alternative biomarkers. Aims and Objectives: This study is designed to overcome diagnostic constraints and explore the viability of serum Cystatin C as an early predictor of Acute Kidney Injury (AKI) in individuals undergoing on-pump cardiac surgery. The investigation aims to establish the relationship between serum Cystatin C levels and the onset of AKI in patients subjected to on-pump cardiac surgery. Primary objectives involve the assessment of the diagnostic effectiveness of serum Cystatin C, its comparison with serum creatinine, and the exploration of its potential for the early identification and treatment of AKI. Methodology: Conducted as a single-center study at the cardiac surgery department of BSMMU in Bangladesh from September 2020 to August 2022, a comparative cross-sectional analysis involved 31 participants categorized into No AKI and AKI groups based on Kidney Disease: Improving Global Outcomes (KDIGO) criteria. Data collection encompassed preoperative, post-CBP (cardiopulmonary bypass) conclusion at 2 hours, postoperative day 1, and postoperative day 2 intervals. Statistical analyses included Chi-squared tests, independent Student’s t-tests, and one-sample t-tests. Significance was set at P Results: The study revealed no significant differences in baseline characteristics between the No AKI and AKI groups, except for CPB time and cross-clamp time. Serum Cystatin C levels in the AKI group exhibited statistical significance at various time points, highlighting its potential as an early detector. Acute Lymphoblastic Leukemia Presenting as Systemic Juvenile Idiopathic Arthritis: Experience from Bangladesh

Background: Acute lymphoblastic leukemia (ALL), the most common paediatric malignancy, is a heterogeneous hematologic disease. ALL patients may present with isolated and persistent osteo-articular complaints, lower incidence of hepatomegaly, splenomegaly or lymphadenopathy without clear laboratory features, and misdiagnosed as systemic juvenile idiopathic arthritis (sJIA). Methods: This was a single center cross sectional study over a period of 4 years. Clinic laboratory profiles of 39 ALL children were compared with 39 age and sex-matched sJIA cases. Result: Among 39 ALL patients 89.7% were initially misdiagnosed as sJIA upon clinical presentation. Majority (66.7%) of ALL patients had oligo-articular joint involvement. In sJIA, small joints of the hands were most commonly involved. The total WBC count was significantly higher in ALL patients (p-value 0.0065). CRP and LDH values between the two groups showed significant differences (p-value 0.00006 and 0.00001 respectively). Conclusion: The presentation of leukemia with arthralgia or arthritis makes the diagnosis difficult for the physicians. The diagnosis of sJIA must be made with caution keeping the possibility of haematological malignancy in mind.

Background: As the half-life of intact parathyroid hormone (iPTH) is very low, it reflects parathyroid insufficiency within minutes to hours after total thyroidectomy. Therefore, iPTH level assessment in the postoperative period can be used to predict the development of hypocalcaemia. The optimal time point to measure serum iPTH is important for the accurate prediction of hypocalcaemia. Aim: This paper aims to evaluate the ability of iPTH as an early predictive marker of hypocalcaemia and determine which time iPTH is more able to predict postoperative hypocalcaemia. Method: This prospective observational study was conducted in the Department of Otolaryngology-Head & Neck Surgery, BSMMU, Dhaka, from July 2020 to December 2021, with 67 patients who underwent total thyroidectomy. iPTH levels were measured on the day before the operation and at 1 hour, 4 hours, and 24 hours after the operation. S.calcium levels were measured on the day before the operation and 1st postoperative day. All the data were compiled and sorted properly and were analyzed statistically. Results: Postoperative hypocalcaemia developed in 18 cases, with an incidence of 26.9%. Pearson correlation showed a significant correlation between postoperative iPTH at 1 hr, 4 h, and 24 hr with 1st postoperative calcium value. The Receiver operating characteristic (ROC) curve was processed for the postoperative iPTH at 1 hr, 4 h, and 24 hr. The sensitivity, specificity, cut-off value, and mean AUC found 93.9%, 94.4%, ≤14.0, 0.988;95.9%, 94.4%, ≤09.5, 0.993 and 91.8%, 94.4%, ≤11.0, 0.993 respectively. Conclusion: iPTH can be used as an early predictor of post-thy-roidectomy hypocalcaemia. 4 hr iPTH showed more sensitivity and specificity for a cut-off value near the laboratory reference range.

Quantifying River Bank Erosion and Accretion Patterns along the Gorai River in Kushtia, Bangladesh: A Geospatial Analysis Utilizing GIS and Remote Sensing Techniques

River bank erosion is a natural process that occurs when the water flow of a river exceeds the bank’s ability to withstand it. It is a common phenomenon that causes extensive land damage, displacement of people, loss of crops, and infrastructure damage. The Gorai River, situated on the right bank of the Ganges, is a significant branch of the river that flows into the Bay of Bengal via the Mathumati and Baleswar rivers. The erosion of the banks of the Gorai River in Kushtia district is not a recent occurrence. Local residents have been dealing with this issue for the past hundred years, and according to the elderly members of the community, the erosion has become more severe activities. Therefore, the main objective of this research is to quantify river bank erosion and accretion and bankline shifting from 2003 to 2022 using multi-temporal Landsat images data with GIS and remote sensing technique. Bank-line migration occurs as a result of the interplay and interconnectedness of various factors such as the degree of river-related processes such as erosion, transportation, and deposition, the amount of water in the river during the high season, the geological and soil makeup, and human intervention in the river. The results show that the highest eroded area was 4.6 square kilometers during the period of 2016 to 2019, while the highest accreted area was 7.12 square kilometers during the period of 2013 to 2016. However, the erosion and accretion values fluctuated from year to year.

Background: Emergence agitation (EA) is a common phenomenon observed in pediatric patients following general anesthesia. This study aimed to assess the efficacy of propofol and fentanyl in preventing EA and to compare their associated complications or side effects. Methods: This prospective randomized observational comparative study was conducted at Dhaka Medical College Hospital from July 2013 to June 2014. The study aimed to evaluate the effects of propofol and fentanyl on EA in children aged 18 to 72 months undergoing circumcision, herniotomy, and polypectomy operations. Ninety children were included in the study, with 45 in each group. Patients with psychological or neurological disorders were excluded. Various parameters including age, sex, weight, American Society of Anesthesiologists (ASA) class, duration of anesthesia, Saturation of Peripheral Oxygen (SPO2), heart rate (HR), respiratory rate (RR), Pediatric Anesthesia Emergence Delirium (PAED) score, duration of post-anesthesia care unit (PACU) stay, incidence of laryngospasm, nausea, vomiting, and rescue drug requirement were compared between the two groups. Results: Age, sex, weight, ASA class, and duration of anesthesia were comparable between the two groups. Perioperative SpO2 and HR were similar in both groups. However, the PAED score was significantly higher in the fentanyl group during all follow-ups except at 30 minutes postoperatively. The mean duration of PACU stay was significantly longer in the fentanyl group. Although the incidence of laryngospasm was higher in the fentanyl group, it was not statistically significant. Conversely, nausea or vomiting was significantly higher in the fentanyl group. The requirement for rescue drugs was significantly higher in the fentanyl group compared to the propofol group. Conclusion: Both propofol and fentanyl were effective in preventing emergence agitation in pediatric patients undergoing various surgical procedures under sevoflurane anesthesia. This research project investigates the current status of water supply, sanitation, and hygiene practices in Munshiganj District, Bangladesh. Data collection involved a structured questionnaire and a reconnaissance survey. Findings reveal that 30% of individuals rely on surface water (hand-tube wells, rivers, and ponds), prioritized as canal > river > pond, while 70% depend on groundwater (subterranean electric motor, deep tube-well). Drinking water is generally sufficient, with 95% reporting adequacy throughout the year. About 45% use hand tube-well water, 28% use deep tube-well water, and 11% use supply tap water for various purposes. Bathing trends include underground water through electric motor > pond > hand tube-well water > river, while for cooking, the order is underground water through electric motor > pond > hand tube-well water > river. Toilet water supply ranks as supply tap water > hand tube-well water > deep tube-well water. Although sanitation awareness is high, some lack knowledge of good hygiene practices. After defecating, handwashing methods include soap, ash, soil, or water. Children’s waste disposal varies, with some discarding it in open areas. Approximately 40% suffer from diseases like Diarrhoea due to unsafe water, primarily affecting children and elders. Training exists, but a significant portion lacks sanitation education. Dry skin or exposure to cold water may cause temporary irritation. Local government involvement in sanitation efforts is less active compared to non-governmental organizations. Results emphasize the need to enhance community awareness of safe water supplies and sanitation practices. .
