Clinical characteristics, treatment and prognosis of patients with idiopathic dilated cardiomyopathy: a tertiary center experience

Background Contemporary heart failure medications have led to considerable improvement in the survival of patients with heart failure. However,limited evidence is available regarding the effect of those medications in patients with idiopathic dilated cardiomyopathy (IDCM),particularly in China. We sought to analyze the trends in clinical characteristics and the prescription rate of recommended therapies and its prognostic impact in patients with IDCM. Methods From 2009 to 2016,1441 consecutive patients (age: 55±14 years,68% men,LVEF: 33%± 12%) fulfilling World Health Organization criteria for IDCM were enrolled in the current retrospective cohort study. Temporal trends of baseline clinical characteristics,treatment and prognosis were analyzed,and potential influential factors were explored. Results Rates of patients receiving angiotensin-converting enzyme inhibitors/angiotensin II receptors blockers,β-blockers,aldosterone receptor antagonists and diuretics increased from 55%,45%,58%,51% in 2009 to 67%,69%,71%,64% in 2016,respectively (P < 0.05);whereas,the proportion of patients receiving digoxin decreased from 39% in 2009 to 28% in 2016 (P < 0.05). The overall proportion of patients with optimal guideline-directed medical therapy (GDMT) was 44.6%;however,that rate increased from 33% in 2009 to 41%,49% and 56% in 2012,2014 and 2016 respectively (P < 0.05). Patients with optimal GDMT had a better outcome than those without,but there was no temporal trend toward improvement in the overall long-term prognosis of IDCM patients with the years. There was a trend towards admission of patients with milder disease and toward increased admission to a cardiology ward with the years. Conclusions An improvement in prescription rates of guideline-recommended medications in IDCM patients was observed. However,it remains suboptimal,and there is still some room for improvement. Resistance to cancer immunotherapy is mainly attributed to poor tumor immunogenicity as well as the immunosuppressive tumor microenvironment(TME)leading to failure of immune *** therapeutic strategies including chemotherapy,radiotherapy,photodynamic,photothermal,magnetic,chemodynamic,sonodynamic and oncolytic therapy,have been developed to induce immunogenic cell death(ICD)of cancer cells and thereby elicit immunogenicity and boost the antitumor immune ***,many challenges hamper the clinical application of ICD inducers resulting in modest immunogenic ***,we outline the current state of using nanomedicines for boosting ICD of cancer ***,synergistic approaches used in combination with ICD inducing nanomedicines for remodeling the TME via targeting immune checkpoints,phagocytosis,macrophage polarization,tumor hypoxia,autophagy and stromal modulation to enhance immunogenicity of dying cancer cells were *** further highlight the emerging trends of using nanomaterials for triggering amplified ICD-mediated antitumor immune *** reticulum localized ICD,focused ultrasound hyperthermia,cell membrane camouflaged nanomedicines,amplified reactive oxygen species(ROS)generation,metallo-immunotherapy,ion modulators and engineered bacteria are among the most innovative *** challenges,merits and demerits of ICD inducer nanomedicines were also discussed with shedding light on the future role of this technology in improving the outcomes of cancer immunotherapy.

目的探讨影响急性心肌梗死合并左室射血分数降低患者远期死亡的危险因素,建立预测模型并进行验证。方法这项回顾性队列研究共纳入1013例于2010年12月-2019年6月在四川大学华西医院住院并诊断为急性心肌梗死合并左室射血分数降低的患者,使用Excel软件的RAND函数将研究人群随机分为3组,其中2组合并为模型建立队列,1组为模型验证队列。终点事件为全因死亡,随访工作截至2021年1月20日。使用Cox比例风险模型筛选影响患者远期死亡的危险因素,建立基于这些因素的预测模型并进行验证。结果中位随访时间为1377 d,共296例患者死亡。多因素Cox回归分析示年龄≥65岁[风险比(hazard ratio,HR)=1.842,95%置信区间(confidence interval,CI)(1.067,3.179),P=0.028]、Killip分级≥Ⅲ级[HR=1.941,95%CI(1.188,3.170),P=0.008]、氨基末端脑钠肽前体≥5598 pg/mL[HR=2.122,95%CI(1.228,3.665),P=0.007]、未行经皮冠状动脉介入治疗[HR=2.181,95%CI(1.351,3.524),P=0.001]、未使用他汀类药物[HR=2.441,95%CI(1.338,4.454),P=0.004]、未使用β受体阻滞剂[HR=1.671,95%CI(1.026,2.720),P=0.039]是远期死亡的独立危险因素。建立预测模型,根据总分分为低危组(0~2分)、中危组(4~6分)、高危组(8~12分)3个风险组。受试者工作特征曲线[曲线下面积=0.724,95%CI(0.680,0.767),P<0.001]和Hosmer-Lemeshow检验(P=0.108)、Kaplan-Meier生存曲线(P<0.001)结果表明该预测模型有良好的预测能力和拟合度,对不同组别也有良好的区分度。并于模型验证队列中得到验证[曲线下面积=0.758,95%CI(0.703,0.813),P<0.001]。结论年龄≥65岁、Killip分级≥Ⅲ级、氨基末端脑钠肽前体≥5598 pg/mL、未行经皮冠状动脉介入治疗、未使用他汀类药物、未使用β受体阻滞剂是急性心肌梗死合并左室射血分数降低的患者远期死亡的独立危险因素,基于此建立的预测模型有良好的预测能力。

Quality Assessment of Online Patient Education Materials for Diabetic Foot

Background: The quality of online Arabic educational materials for diabetic foot syndrome (DFS) is unknown. This study evaluated Arabic websites as patients’ sources of information for DFS. Methods: The study assessed patient-related websites about DFS using a modified Ensuring Quality of Information for Patients (EQIP) tool (score 0 - 35). Specific terms were searched in Google to identify DFS websites;eligibility criteria were applied to 20 pages of search results to select the included websites. Data on country of origin, source types and subtypes, and website traffic were extracted. Additional therapeutic information regarding prevention and conservative, pharmacological, and surgical treatments was also recorded and analyzed. Results: Among 559 websites, 157 were eligible for inclusion. The median EQIP score was 16 out of 35, indicating poor quality in one of three domains (content, identification, or structure). Most sources originated from Arab countries (75.8%) were non-governmental (94.9%), and were medical information websites (46.5%). High-scoring websites were significantly more likely than low-scoring websites to describe information on prevention (30.9% vs. 2.9%, p = 0.001), conservative treatment (34.1% vs. 13%, p = 0.002), or pharmacological treatment (32.5% vs. 16.8%, p = 0.024). There were increased odds of scoring high if a website provided information on prevention (OR = 12.9, 95% CI [1.68 - 98.57], p = 0.014). Conclusion: Most Arabic online patient information on DFS is of poor quality. Quality control measures are needed to ensure accurate health information for the public.

A Computational Study of Cavitation Model Validity Using a New Quantitative Criterion

The predictive capability of two different numerical cavitation models accounting for the onset and development of cavitation inside real-sized diesel nozzle holes is assessed on the basis of the referenced experimental *** calculations performed indicate that for the same model assumptions,numerical implementation,discretization scheme,and turbulence grid resolution model,the predictions for differently applied physical cavitation submodels are phenomenologically distinct from each *** present a comparison by applying a new criterion for the quantitative comparison between the results obtained from both cavitation models.

Investigation of the Flow Pattern inside a Diesel Engine Injection Nozzle to Determine the Relationship between Various Flow Parameters and the Occurrence of Cavitation

In diesel engines the fuel injection system produces the spray, which directly affects the combustion of the fuel, which in turn determines the production of pollutants. In spite of this, the details of this causal relationship remain unclear. There is, however, a lack of quantitative experimental data for determining and visualizing the cavitation inside real size diesel injector nozzle. The present work is devoted to analyze analytically the flow pattern inside the nozzle of a diesel engine working with hydrocarbon fuel (Diesel fuel) and to predict the relationship between the various flow parameters and occurrence of fuel cavitation in such nozzles. Basic physical parameters affecting this phenomenon are identified and quantified while the effect of nozzle geometry, fuel injection pressure, and engine cylinder temperature upon the flow pattern and occurrence of cavitation in such nozzles are assessed. In this study, a commercial computational fluid dynamics (CFD) package (FLUENT-T grid) is used while a computational grid is generated for the real geometry of diesel injector nozzle using (ANSYS). The suitability of the generated computational grid to give reliable results is examined using the suitable procedures and techniques. The results indicated that, cavitation modeling has reached a stage of maturity and it can usefully identify many of the cavitation structures present in internal nozzle flows and their dependence on nozzle design and flow conditions. The qualitative distributions and comparison of cavitation inception and distribution as well as flow parameters at the nozzle exit are also studied.

Numerical Study on the Hydrogen Fueled SI Engine Combustion Optimization through a Combined Operation of DI and PFI Strategies

As the practicability of a hydrogen-fueled economy emerges, intermediate technologies would be necessary for the transition between hydrocarbon fueled internal combustion engines and hydrogen powered fuel cells. In the present study, the hydrogen engine efficiency and the load control are the two main parameters that will be improved by using the combined operation of in-cylinder direct fuel injection (DI) and port fuel injection (PFI) strategies to obtain maximum engine power outputs with acceptable efficiency equivalent to gasoline engines. Wide open throttle (WOT) operation has been used to take advantage of the associated increase in engine efficiency, in which the loads have been regulated with mixture richness (qualitative control) instead of volumetric efficiency (quantitative control). The capabilities of a 3D-CFD code have been developed and employed to simulate the whole engine physicochemical process which includes the hydrogen injection through the intake manifold (PFI) and/or the hydrogen DI in the engine compression stroke. Conditions with simulated PFI, PFI + DI and DI have been analyzed to study the effects of mixture preparation behaviors on the hydrogen ignition and its flame propagation inside the engine combustion chamber. Numerically, the CFD code has been intensively validated against experimental engine data which provided remarkable agreement in terms of in-cylinder pressure history evaluation.

Possibility of using the beet dyes as a laser gain medium

Nowadays the dye lasers play as an important tool and are used in many applications including spectroscopy, medicine and dermatology. This research was carried out to study the possibility of using the beet dyes as a laser gain medium. The fluorescence quantum yield was determined by the comparative method with rodamine b as an organic dye standard. The value of the fluorescence quantum yield was found about (0.14) and the fluorescence quantum yield was developed until reaching about (0.323). The increasing of fluorescence quantum yield of dye solution as a result of increasing the viscosity of solvent was observed clearly. The study concluded that the beet dyes are so sensitive to fluorescence and it is very suitable to be used as a laser gain medium.

在常压定容室内,应用ND:YAG激光阴影法、高分辨率CCD照相机和粒径分析软件,对不同含水量的柴油乳化液雾油滴的闪急沸腾与微爆现象以及油滴粒径分布进行了试验测量与变参数对比分析,探讨了在130℃温度条件下,乳化油含水量对乳化油喷雾液滴闪急沸腾和微爆的影响,揭示了微爆效应在喷雾雾化和液滴碎化中的作用机理。试验结果表明,包覆于油中的水相颗粒受热闪急沸腾所形成的高压蒸气团冲破无水边界层是导致油滴发生微爆和进一步破碎分裂的首要原因;微爆现象随乳化油含水量的增高而趋强;喷雾索特平均直径(SMD)的细化率随乳化油含水量和喷雾贯穿距的增加而增大,且乳化油含水量对喷雾SMD细化率的影响程度也随喷雾贯穿距的增加更加显著。
