Characteristics and outcomes of Bahraini pediatric patients sent abroad for bone marrow transplantation:A ten-year retrospective cohort study

BACKGROUND Bone marrow transplantation(BMT)is a breakthrough procedure for patients with hematological and oncological conditions,particularly when all other treat-ments *** indications vary between patients and settings,and its outcomes depend on the donor type,transplantation facility,and center *** coun-tries where transplantation facilities are lacking,sending patients abroad for transplantation might be a safe and effective alternative to leaving the patient to face eventual disease morbidity or even mortality if the procedure is not per-formed ***,studies evaluating BMT abroad are *** To assess the clinical characteristics of patients who underwent BMT overseas and analyze the factors affecting their survival *** We conducted a retrospective cohort study of all Bahraini pediatric patients who underwent BMT between 2013 and *** records from Salmaniya Medical Complex and Overseas Treatment Office were *** demographics,RESULTS Of the 75 listed patients,62(82.7%)underwent BMT and were included,10(13.3%)did not,and 3(4.0%)were awaiting *** patients were male(n=33,53.2%).The mean age at transplantation was 7.8±4.9 *** main indication for treatment was acute myeloid leukemia(AML)(n=15,36.6%).Six patients(9.7%)required *** the 68 transplants,60(88.2%)involved conditioning,mostly a combination of fludarabine and total body irradiation(n=7,11.7%).Most patients underwent allogeneic transplantation(n=48,77.4%),primarily from related donors(n=47/48,97.9%).The most common complication was infection(n=51,79.7%).follow-up averaged 3.3±2.5 *** overall survival rate was 77.4%.Survival odds were better for non-AML patients and Middle Eastern centers(P=0.015 and P=0.032,respectively).CONCLUSION Bahraini males with AML primarily underwent allogeneic ***-AML patients and those transplanted in the Middle East had better survival rates,despite high complication rates

Mode of travel and the decision to allow children to walk or bike to schools—The Abu Dhabi experience

The study investigates school travel to and from schools as perceived by parents in Abu Dhabi, United Arab Emirates. far fewer children are cycling or walking to school, and more and more are being driven to school by car. Traffic conditions, environmental factors, distance to school, road’s infrastructures, walking or biking with groups, schools efforts to educate children about active travel, and preferences of parents to accompany children to school were significantly perceived as important. The absence of active travel culture in the community was seen as a significant factor. Parent’s gender, child’s gender, child’s grade level, number of cars owned by the family, nationality and number of children were significantly associated with parents’ decisions to allow active travel to school. Results show that Abu Dhabi must establish aggressive strategic plans to promote active-commuting to school as the main mode of transportation. Current conditions in Abu Dhabi City are not conducive to active travel to school.

Polyethylene glycol functionalized carbon nanotubes/gelatin-chitosan nanocomposite: An approach for significant drug release

This research work blooms the new idea of developing a safe and controlled drug releasing matrix using multi-walled carbon nanotubes(MWCNTs).In aqueous solution,uniform and highly stable dispersion of MWCNTs was obtained after secondary functionalization with polyethylene glycol(PEG)which was studied by fourier transmission infrared spectroscopy(fTIR)and thermogravimetric analysis(TGA).Solution casting method was used to prepare MWCNTs/gelatin-chitosan nanocomposite films and the effect of MWCNTs on physico-mechanical,thermal and water uptake properties of the nanocomposites were *** of MWCNTs into the porous gelatin-chitosan matrix showed interesting stiffness and dampness along with developed microfibrillar structures within the pore walls intended at being used in tissue engineering of bone or cartilage.A common antibiotic drug,ciprofloxacin was incorporated into nanocomposite *** evaluation of the effect of MWCNTs on drug release rate by dissolution test and antimicrobial susceptibility test was *** release of the drug was found at early stages(~1 h),but the rate was reduced afterwards,showing a sustained *** was observed that for all microorganisms,the antibacterial activities of drug loaded MWCNTs/gelatin-chitosan nanocomposites were higher than that of drug loaded gelatin-chitosan composite films containing no *** statistical studies by ANOVA techniques also showed remarkable difference between the antibacterial activities,exhibited by MWCNTs-incorporated and non-incorporated composite films.

1,2,3-Triazole incorporated coumarin derivatives as potential antifungal and antioxidant agents

A series of novel ethyl-7-（（1-（benzyl）-1H-1,2,3-triazol-4-yl）methoxy）-2-oxo-2H-chromene-3-carboxylates 8a-h as potential antifungal agents were synthesized via click chemistry. The antifungal activity was evaluated against five human pathogenic fungal strains, such as Candida albicans, fusarium oxysporum, Aspergillus flavus, Aspergillus niger and Cryptococcus neoformans. Compound 8c, 8d, 8e and 8h were found to be equipotent against C. albicans when compared with miconazole and compound 8f was found to be two-fold more active compared with miconazole and equipotent to fluconazole against ***. The coumarin-based triazole derivatives were also evaluated for antioxidant activity and compound 8a was found to be potent antioxidant when compared with standard drug. furthermore,molecular docking study of the newly synthesized compounds was performed and results showed good binding mode in the active site of fungal C. albicans enzyme P450 cytochrome lanosterol 14 ademethylase. Moreover, the synthesized compounds were also analyzed for ADME properties and showed potential to build up as good oral drug candidates.

朝天椒果实的准确识别是实现智能采摘的关键步骤,针对其生长环境复杂、果实大小不一、遮挡重叠等造成识别准确率低的问题,本研究提出一种基于改进YOLOv7的朝天椒果实识别方法。以YOLOv7为基础模型,设计了一种含有残差结构的AM_f模块,将其融合在YOLOv7主干网络中;基于空间、通道注意力机制的结构改进得到SAM_f、SE_ECA模块,分别将其融合在主干网络和颈部网络中,并进行结构简化,同时将SPP_CSP结构中的SPP替换为SPPf,实现参数计算量的精简,最终得到改进型YOLOv7模型——YOLOv7-f。采用对比试验对YOLOv7-f模型的识别效果进行验证分析,结果表明,YOLOv7-f模型对朝天椒果实的识别平均精度均值为80.07%,与YOLOv7模型相比,YOLOv7-f模型在识别时间加快23.4 ms的前提下,平均精度均值提升了1.06个百分点,而且模型大小也减少77.94 MB。YOLOv7-f模型实现了朝天椒果实识别精度和速度同步提升,为朝天椒果实智能采摘提供技术支撑。

Review on the Estimating the Effective Way for Managing the Produced Water: Case Study

Water manufactured is the primary waste source in the oil and gas industry. Because of the rising amount of waste worldwide, the environmental effect of wastewater has become a primary environmental concern in recent years. The vast amounts involved have resulted in considerable costs to the industry for handling produced water. This research explains the wide variety of choices for water management. This research’s first phase was water minimization techniques, consisting of three different applications made in three different wells (Well 1, Well 2 and Well 3) and water recycling and reuse by two techniques. In Well 1, Mechanical shut-off technique was applied using through tubing bridge plug and 5 m cement dumped above it to isolate the watered out zone;as per water oil ration plot the water cut is decreased from 100% to 4% and the production is increased from 0 to 400 bcpd. In Well 2, Chemical shut-off technique using a polymer called Brightwater has been used to block channeling through high permeability intervals after PLT log detected it, and the result was brilliant, the water cut decreased from 60% to 25%, also the oil production increase from 500 to 3000 bopd. In Well 3, downhole separator installed in it using workover (unfortunately, this technique is not applied in middle east till the moment so this application is taken from an oil field in Canada)and the result was perfect, the water cut decreased from 70% to 28%, also the oil production increase from 44 to 100 bopd. This study tried to clarify and compare the most widely used water management techniques using one of the Western Desert (W.D.) (enhanced for oil recovery, constructed wetland).

Vitamin D Deficiency and Pseudofractures in Child-Bearing Egyptian Women: Successful Medical Treatment Helps to Avoid fractures and Surgical Interference

Background: Recent evidence for the effects of vitamin D and recognition of the high prevalence of its deficiency has increased the interest in it. Vitamin D-sub nutrition may contribute to the risk of a wide range of disorders. Methods: The females in the child-bearing period attending the endocrinology and orthopedic out-patient clinics complaining of pain and/or tenderness at the groin were evaluated. Patients with chronic metabolic or skeletal illness, primary hyperparathyroidism, and patients receiving drugs that interfere with bone mineral metabolisms were excluded. One hundred accepted and consented to participation in the study. All the participants were subjected to full history taking, clinical evaluation, laboratory investigations including serum Ca, Ph, PTH, ALP, TSH, f T4, f T3, Cortisol level, and 25(OH)D level. Plain X-ray was done for the regions of bony tenderness. The subjects were divided into 2 groups based on the presence or absence of pseudo-fractures (looser zones). They were treated and followed up till normalization of the laboratory parameters and healing of the looser zones. Results: The mean age for patients was 30.45 ± 5.8, their mean 25(OH) vitamin D level was 14.7 ± 5.9 ng/ml, the mean PTH was 195.7 ± 162.6, and looser zones were evident in the X-rays of 34 patients. The serum 25(OH)D showed significant negative correlations with veiling, parity, localized hip pain, and tenderness, ALP, PTH, but, a significant positive correlation with sun exposure, dairy products consumption, vitamin D supplementation, and serum calcium levels. The independent factors associated with hypovitaminosis D were the lack of sun exposure, lack of vitamin D supplementation, and the presence of pseudo- fracture (p ≤ 0.009, 0.038, and 0.001 respectively). No surgery was required in any of our patients. Conclusions: Vitamin D deficiency is common in our region. Efficacy and safety of treatment of hepatitis C virus infection in renal transplant recipients

AIM: To assess the efficacy and safety of combinedpegylated interferon and ribavirin therapy in hepatitis C virus (HCV) infection in renal transplant recipients. METHODS: This is a retrospective chart review of post renal transplant patients who were positive for anti-HCV and HCV-RNA, and who have received treatment with combination of pegylated interferon and ribavirin between October 2003 and December 2008. Only patients with stable graft function and absence of evidence of cirrhosis and who received the therapy for continuous 48 wk were included. Nineteen patients (13 male and 6 female) were identified and included. The patient's complete blood count, liver and kidney prof ile, and cal- culated glomerular f iltration rate (GfR) were monitored every 6-8 wk while on treatment. HCV-RNA was tested at 12 wk for early virological response, at 48 wk for end of treatment response (ETR), and then retested at 24, and 48 wk after completion of therapy for sustained virological response (SVR). Liver biopsies were obtained before treatment from all patients and graft kidney biopsies were performed as required. RESULTS: Of the entire cohort, 9 patients (47.4%) showed an ETR and 8 had SVR (42.1%). Of the 8 patients with abnormal alanine aminotransferase (ALT) levels at baseline, 78.9% had their ALT normalized (including the virological non responders). ALT was normal in all responders at the end of therapy and at 24 wk post therapy (100%). Only one patient (5.3%) developed an increase in creatinine and decline in GfR from baseline towards the end of treatment. This patient's kidney biopsy revealed borderline rejection. There was no impact on response by HCV-genotype, initial HCV RNA load, age or sex of the patient or duration post transplant before commencement of therapy. All patients tolerated treatment in the same way as non-transplant with no unusual or increased occurrence of side effects.
