Periodontal Tissue Status among mental Retardation Patients with Fixed Prosthodontic Appliances

Background: The mental, physical and social impediments cause limitation of chances in normal life as an equal level with others of the community;particularly in relation to maintain good oral hygiene and periodontal health among the disabled patients who were receiving fixed partial denture. So the present study was performed to evaluate the periodontal tissue status among mental retardation patients with fixed prosthodontic appliances. Subjects and methods: A total of 400 patients between the ages 18 and 50 years were clinically selected from outpatient clinics, college of dentistry, King Khalid University and rehabilitation center in Abha city, Saudi Arabia. They were divided into two equal groups as the following: group I: Two hundred mental retardation patients without fixed prosthodontic appliances (control group) and, group II: Two hundred mental retardation patients had fixed prosthodontic appliances for at least a year ago. A complete periodontal clinical examination was performed and the following indices were recorded: plaque index (PLI), gingival index (GI), and clinical attachment loss (CAL). All data was recorded and analyzed by ANOVA test. Results: The clinical results of the present study confirmed the relationship among the mental retardation, fixed prosthodontic appliances and destruction of periodontal tissues. It has been found that the severity of periodontal disease increases among the study group compared to the control group. Statistically significant differences were found in PLI, GI and CAL in the comparison between group I and II (p Conclusion: There was a negative relationship between mental retardation with a status of periodontal tissues and oral hygiene among the patients under fixed prosthodontic management.

Prevalence and Correlates of Dyslipidemia among Adults in Saudi Arabia: Results from a National Survey

Data were collected and analyzed from a cross-sectional study using the World Health Organization’s STEPwise approach, to estimate the prevalence of various types of dyslipidemia and determine their associated factors among adults in Kingdom of Saudi Arabia. The study population included 4490 Saudi adults aged 15 years and older who were selected by a stratified, multistage, cluster random sampling technique. Lipid profile was determined by spectrophotometrically by biochemical methods, high total cholesterol (TC) was defined as TC of 5.2 mmol/L or more, hypertriglyceridemia as serum triglycerides level 1.7 mmol/L or more and. low high-density lipoprotein cholesterol (HDL-C) as serum HDL-C 0.9 mmol/L or less, LDL-C 3.35 mmol/L or more and TC/HDL ratio 5 or more. Of the 4490 subjects (94.4 % of total sample) included in the final analysis, 51% were females, 23% of study subjects were less than 25years and 11% were 55 or more years of age. The overall prevalence of dyslipidemia ranged from about 20% to 40%. The highest prevalence was for triglycerides where about 44% of all subjects were affected. About a fifth of the subjects had high level of total cholesterol. males had significantly higher prevalence of all types of dyslipidemia than female except LDL-C and TC. Higher dyslipidemia prevalence of total cholesterol and triglycerides were significantly found in older subjects, illiterates and housekeepers. Lower prevalence rates for HDL-C was significantly observed in retired and youngest subjects. There were significant regional variations and significantly higher prevalence of dyslipidemia among hypertensives, diabetics, obese/overweight, smokers and physically inactive subjects. There were no significant differences according to income or fruits and vegetable consumption. The Impact of a Structured Clinical Pathway on the Application of management Standards in Patients with Diabetic Ketoacidosis and Its Acceptability by medical Residents

Diabetic Ketoacidosis (DKA) is a serious and potentially a fatal complication of diabetes mellitus. Tools to guarantee proper, evidence-based, guideline implementation are of paramount importance and an essential element for quality patient care. Clinical pathways represent one such tool that clearly promotes the implementation of guidelines and research evidence into clinical practice. The aims of this study were to measure quantitatively and qualitatively the impact of a specially structured Resident-friendly, DKA clinical pathway on the application of evidence-based management standards and its acceptability by the treating resident physicians. A retrospective chart review of patients who were admitted prior to and after the launching of the clinical pathway and a questionnaire assessment of resident’s acceptance of the pathway format were undertaken. Eighty one episodes of DKA in a total of 58 patients fulfilled the criteria for inclusion in the study. Thirty seven admissions were on the pathway (45.7%) and 44 were not (54.3%). Documentation of severity indices of patients who were admitted under the pathway were significantly improved with a trend for a shorter hospital stay. The duration of intravenous insulin therapy, intensive care unit consultation and diabetes educator involvement in patient care were not different between the two groups. Residents found the pathway user-friendly, educationally very valuable, reduced their workload and had a positive effect on their DKA management skills.?Conclusions: Use of specially structured, resident-friendly pathway led to significant improvement in documentation of DKA severity indices and empowered our residents with evidence-based knowledge and skills to deal with this serious diabetic complication.

为研究稻谷的石墨烯低温远红外干燥特性及其对稻谷干燥品质的影响，以辐射温度、排粮流量和除湿风量为影响因素，以干燥速率和应力裂纹指数增值为评价指标，用自制的循环式石墨烯低温远红外干燥机进行稻谷干燥试验，通过BBD(Box-Behnken设计)响应面法，分析了低温远红外干燥对稻谷干燥品质的影响以及工艺参数优化。结果表明：影响稻谷干燥特性和品质的最主要因素是辐射温度，其次是排粮流量和除湿风量。随着辐射温度的升高，稻谷的干燥速率和应力裂纹指数增值逐步增大，整精米率则逐步降低。与同温度的热风干燥相比，石墨烯低温远红外干燥的平均干燥速率和干燥品质均有显著提高。经优化后，稻谷最佳石墨烯低温远红外干燥工艺条件为：辐射温度43℃、排粮流量4kg/min、除湿风量193m3/h，此时应力裂纹指数增值为9，整精米率为79.75%，稻谷干燥品质最佳。这说明利用石墨烯低温远红外干燥稻谷，可以明显提高干燥速率并改善稻谷干燥品质。

Background: Bystander cardiopulmonary resuscitation (CPR) is a vital element of patient survival. Therefore, the aim of the present study was to assess sources of knowledge about CPR and its association with demographical characteristics in Saudi Arabia. methodology: Data about CPR were obtained from 440 Saudi volunteers living in the city of Hail, Northern Saudi Arabia. Results: Out of the 440 study subjects, 257/440 (58.4%) individuals were found to know about CPR. Out of 257 individuals, 154/257 (60%) were males and 103/257 (40%) were females. Conclusion: The level of awareness toward CPR in general population of Northern Saudi Arabia, which requires more efforts at community basis. The most effective source for raising awareness is the appropriate utility of social media, as well as, implementing sustainable program for delivering CPR training courses evolving all community settings.

目的:筛选并优化没食子温敏原位凝胶的制备工艺,研究其表征、流变学行为、热分析和体内滞留性并建立质量标准。方法:(1)对没食子药材进行性状鉴定和常规检查;UV-Vis法测定没食子水提物中鞣质和多酚含量;分别采用TLC定性、HPLC定量分析没食子水提物中GA、mG和EA 3个有效组分。(2)以水提物为原料药,采用单因素试验考察P407、P188和HPmC用量对胶凝温度(T)的影响;以T为指标,在单因素试验基础上采用Box-Behnken Design(BBD)响应面法优化P407、P188和HPmC的用量;依据BBD试验结果,进一步采用Franz扩散池法,以24 h体外释药性能为指标,确定没食子温敏原位凝胶最佳制备工艺。(3)以最佳工艺制备的没食子温敏原位凝胶为主要研究对象,采用SEm观察微观形态;FTIR分析辅料是否影响没食子固有特征峰;采用流变仪进行剪切速率扫描、屈服应力扫描、触变性扫描、决定线性黏弹区扫描、频率扫描和温度扫描,分析相变前后的流变学行为;DSC分析相变前后的热稳定性;荧光成像法考察直肠局部滞留性。(4)以最佳工艺制备的没食子温敏原位凝胶为研究对象,采用目测法观察外观,p H计测定p H值,离心法考察均一性,流变仪测定黏度、T,恒温水浴锅测定胶凝时间,按2020版《中国药典》附录方法测定粒度;TLC定性、HPLC定量测定有效组分GA、mG、EA,薄层生物自显影技术(TLC-Bio)确定其抗氧化活性,建立其质量标准草案。结果:(1)没食子药材呈类球形,直径0.8 cm至2.5 cm,表面有短柄,呈灰黄色、橄榄绿,有许多角状突起,质坚,不易破碎。没食子粉末呈浅黄色,平滑,无特殊气味,味苦而涩;没食子药材中的水分和水溶性浸出物平均值分别为9.72%和74.43%,没食子药材过80目筛的细粉在1 h时吸水率最高;没食子水提物的制备工艺确定为没食子药材粉碎,过80目筛,料液比1:8,浸泡1 h,蒸馏水回流提取3次每次0.5 h,浸膏得率为82%;水提物中多酚和鞣质的含量分别为0.0213 g·g和0.0126 g·g,换算成生药中含量分别为0.8537 g·g和0.7136 g·g;TLC定性分析最佳展开系统为氯仿-乙酸乙酯-甲醇-甲酸(5:4.5:1:2);测定有效组分GA、mG和EA的HPLC条件为:色谱柱ZORBAX SB-C18(4.6 mm×250 mm,5μm),以0.2%磷酸溶液(A)-甲醇(B)溶液为流动相梯度洗脱,洗脱程序0～10 min,10%→30%B,10～26min,30%→68%B,26～28 min,68%B,28～29 min,68%→10%B,29～35min,10%B,检测波长258 nm,进样量10μL,流速1.0 m L·min,柱温:25℃。没食子水提物中GA、mG、EA平均含量分别为131.05 mg·g、5.01 mg·g、6.12 mg·g。(2)没食子温敏原位凝胶最佳制备工艺为:P407(24.07%,w/v),P188(1.22%,w/v),HPmC(0.60%,w/v),T为(32.8±0.4)℃;没食子温敏凝胶体外累积释药曲线符合一级动力学方程Q=119.68(1-e)(r=0.9968),且在24 h内完全释放。(3)SEm观察微观形态,空白凝胶呈多孔相连的三维网状结构、有贯穿的圆形空洞、片层结构较松散且内部结构质地均匀,载药凝胶中没食子水提物均匀分散于凝胶片层结构中、凝胶孔隙填平;FTIR分析显示辅料未对没食子固有特征峰产生影响;流变学研究表明没食子温敏原位凝胶在4℃、25℃时具有牛顿流体、无屈服应力,在37℃时具有假塑性流体性质、有屈服值且相变前后均无触变性,制备的凝胶具有温敏性和一定的滞留性;DSC分析没食子温敏原位凝胶相变前后在生理温度范围内稳定且没食子与辅料互不干扰;溶胶动力学实验表明没食子水提物的加入降低了溶胶动力学速率,提高了粘附性,进而降低了凝胶中的药物释放速率;新吲哚菁绿(IR-820)标记的没食子温敏原位凝胶组12 h仍可观察到荧光信号,对照组1 h时药物已基本遍布全身,表明没食子温敏原位凝胶可延长IR-820的局部保留时间。(4)没食子温敏原位凝胶为黄棕色均一透明溶液,p H值4.27±0.2,匀一性良好,胶凝温度(33.2±1.2)℃、胶凝时黏度103 Pa·s,胶凝时间(130±15)s,粒度符合要求;TLC斑点清晰、分离度良好、专属性强,HPLC定量检测出GA、mG、EA分别在4.039～517(r=0.9997)、1.656～106(r=0.9997)、1.609～103(r=0.9995)μg·m L范围内线性良好,TLC-Bio表明GA、mG和EA均具有抗氧化活性。结论:(1)所建立的TLC定性和HPLC定量方法可用于分析没食子水提

Institutional Review Board Compliance with National Regulatory Body: A Successful Experience from Saudi Arabia

Background: The Institutional Review Board (IRB) or Research Ethics Committees (RECs) is established to review the research proposals and ensure that participants’ ethical standards, scientific merit, and human rights are protected. Purpose: The authors report the experience of the REC at Qassim Region, Saudi Arabia over 10 years period. methods: All proposals submitted to Qassim REC during the period 2008-2017 were studied using a 30 items data collection form based on The National Committee of Bioethics Regulations. Data extracted included;principal investigator characteristics, numbers of proposals reviewed, applications completeness, approval decision status, reported ethical issues, classification of the ethical review, and committee review duration. The structure, workload, and review process of Qassim REC were addressed redundant. Results: During 10 years, Qassim Research Ethics Committee (QREC) witnessed a progressive increase in the number of submitted proposals, from 9 to 149 proposals. Out of 508 submitted applications, 439 (86.4%) proposals were eligible for ethical review. Of these, 50 (11.4%) proposals were incomplete due to nonresponse of the principal investigators to the QREC comments. The final decision was made for 389 (88.6%) completed proposals. The approval rate was 85.4%, while the rejection rate was only 1.1%. The median time taken for ethical review was 13 days. Proposals that underwent full board review had a long review duration (median: 19 days) in comparison to the expedited review (median: 10 days). Incomplete Committee requirements, unclear research methodology, or possible ethical violation opportunities were the main reasons for delayed decisions. Conclusion: The workload of the Qassim ethics committee is high and growing progressively. However, the process indicators as per National Bioethics Committee rules were satisfactory. The Relationship between Serum 25-Hydroxyvitamin D and Resistin Levels in Saudi Diabetic Patients

Objectives: The aim of this study was to investigate the relationship between the levels of 25-hydroxyvitamin D and the adipokine resistin in Saudi diabetic patients and those at high risk to develop atherosclerotic cardiovascular diseases. methodology: One hundred and sixty seven subjects were recruited for the cross-sectional study at King Fahad medical City, Saudi Arabia. Blood samples were analyzed for biochemical parameters. Serum 25-hydroxyvitamin D and resistin levels were measured for all the participants. Result: Fifty six type-1, 55 type-2 diabetic patients and 56 healthy controls were recruited. Serum 25-hydroxyvitamin D level in the control group was 44.21 ± 41.80 μg/ml compared to 50.67 ± 35.60 μg/ml in the diabetic group (P = 0.427) with no significant different between type-2 diabetes mellitus and type-1 diabetes mellitus (P = 0.628). Resistin level in the control group was 14.00 ± 7.39μg/ml compared to 20.21 ± 16.94 μg/ml in the diabetic group (P ≤ 0.01), no significant difference between type-2 diabetes mellitus and type-1 diabetes mellitus (P = 0.817). Resistin in those taking vitamin D supplementation was 21.34 ± 18.27 μg/ml;and in those not taking vitamin D supplementation, it was 17.4 ± 11.19 μg/ml;(P = 0.237). There was significant negative correlation between 25-hydroxyvitamin D and resistin in type-2 diabetes mellitus (P ≤ 0.043) and no significant correlation was found between vitamin 25 (OH) D and resistin level in type-1 diabetes mellitus (P = 0.538). Blood urea and creatinine showed significant positive correlation with vitamin 25 (OH) D in one hand and resistin level on the other hand. Discussion and Conclusions: Several in vitro and in vivo studies have confirmed that the vitamin 25 (OH) D have numerous important functions in the body. The circulating vitamin 25 (OH) D and resistin levels were associated with general inflammation in renal diseases.
