Association of E-cadherin (CDH1) gene polymorphisms and gastric cancer risk

AIM:To investigate the associations between CDH1 gene polymorphisms and gastric cancer(GC) risk predisposition.METHODS:We analyzed four CDH1 polymorphisms(+54 T>C,-160 C>A,-616 G>C,-3159 T>C) in an Omani population,by extraction of genomic DNA from the peripheral blood of 192 patients with GC and 170 control participants and performed CDH1 genotyping using DNA sequencing.RESULTS:CDH1-160-AA genotype was associated with an increased risk of GC(OR = 3.6,95% CI:1.1-11.8)(P = 0.03).There was no significant association between the other polymorphisms and GC risk.The haplotype analysis of +54 T>C,-160 C>A,-616 G>C,-3159 T>C genotypes revealed that the OR of CCGC and CAGC haplotypes was 1.5(95% CI:0.7-3.5) and 1.5(95% CI:0.2-3.0),but did not reach statistical significance.CONCLUSION:The current study suggests that the-160 AA genotype was associated with an increased risk of GC in Oman.

although gastrointestinal stromal tumors(GISTs)are rare,with an incidence of 1/100000 per year,they are the most common sarcomas in the peritoneal cavity.Despite considerable progress in the diagnosis and treatment of GIST,about half of all patients are estimated to experience recurrence.With only two drugs,sunitinib and regorafenib,approved by the Food and Drug Administration,selecting treatment options after imatinib failure and coordinating multidisciplinary care remain challenging.In addition,physicians across the Middle East face some additional and unique challenges such as lack of published local data from clinical trials,national disease registries and regional scientific research,limited access to treatment,lack of standardization of care,and limited access to mutational analysis.although global guidelines set a framework for the management of GIST,there are no standard local guidelines to guide clinical practice in a resource-limited environment.Therefore,a group of 11 experienced medical oncologists from across the Gulf and Levant region,part of the Rare Tumors Gastrointestinal Group,met over a period of one year to conduct a narrative review of the management of GIST and to describe regional challenges and gaps in patient management as an essential step to proposing local clinical practice recommendations.

Multidisciplinary treatment approach of a patient with amelogenesis imperfecta: A case report

Introduction: Amelogenesisimperfecta (AI) is a developmental disturbance, genomic in origin, which interferes with normal enamel formation of both primary and permanent dentitions in the absence of a systemic disorder. Three major categories can be recognized clinically, namely hypoplastic, hypomaturation and hypocalcified. Timely and comprehensive intervention is critical to spare the patient from psychological consequences of these disfiguring conditions. Clinical Report: A 10-year-old boy was presented with dissatisfaction with appearance of his teeth sensitivity and poor masticatory efficiency. His medical history was noncontributory. Composite restorative material was selected as a suitable replacement of the defective structures because of its esthetics and high sustainability. Treatment Objectives: The multidisciplinary approach was oriented toward achieving functional and esthetic rehabilitation of these teeth with minimal chair-side time. Treatment Outcomes: The use of composite restorative material resulted in successful and satisfactory enhancement of the patient’s esthetic appearance, as well as protection against further wear and sensitivity thereby improving his functional demands. The longevity of the treatment outcome required meticulous maintenance of oral hygiene and patient compliance. Conclusion: Management of a patient with AI is a challenge for the clinician. Treatment options vary considerably depending on several factors such as age of the patient, socioeconomic status, severity of the disorder, and most importantly, the patient’s cooperation. Composite restorative material is considered an excellent conservative transitional treatment for protection of AI weakened teeth.
