AIM: To determine whether ophthalmic photographers(OPs) can conduct virtual clinics(VCs) in medical retina appropriately and whether this increases clinic ***: Three OPs underwent a training programme to learn how to assess and manage macular edema secondary to diabetes or retinal vein occlusion. The 300 consecutive patients over a 7-month observation period in 2018 were assessed in VCs by both OPs and medical retina consultants. The degree of agreement in treatment decisions between doctors and photographers, as well as adverse events, was recorded. The change in number of patients seen in VCs over two years was also ***: There was 100% agreement in management decisions between doctors and photographers during the 300-patient observation period. No adverse events were recorded. In 2017, 572 patients were seen in VCs by doctors. After OP clinics were introduced in 2018, this number increased by 24% to 709 patients seen by both photographers and doctors. There was a significant increase in mean number of patients seen per month between 2017(47.7±11.7) and 2018(59.1±14.4;P=0.045, 95%CI-22.5 to-0.296).CONCLUSION: OPs can manage certain medical retina patients in VCs appropriately and can increase clinic capacity significantly for particular conditions.

The selection of working fluid significantly impacts the geothermal OrC’s *** a mixture as a working fluid is a strategy to improve the output of geothermal *** the current study,modelling and thermodynamic analysis of OrC,using geothermal as a heat source,is carried out at fixed operating *** model is simulated in the Engineering Equation Solver(EES).An environment-friendly mixture of fluids,i.e.,r245fa/r600a,with a suitable mole fraction,is used as the operating *** mixture provided the most convenient results compared to the pure working fluid under fixed operating *** impact of varying the evaporator pressure on the performance parameters,including energy efficiency,exergy efficiency and net power output is *** system provided the optimal performance once the evaporator pressure reached the maximum *** efficiencies:Energy and Exergy,and Net Power output of the system are 16.62%,64.08%and 2199 kW for the basic cycle and 20.72%,67.76%and 2326 kW respectively for the regenerative cycle.

Barrett's esophagus:Prevalence and risk factors in patients with chronic GErD in Upper Egypt

AIM： To determine the prevalence and possible risk factors of Barrett＇s esophagus （BE） in patients with chronic gastroesophageal reflux disease （GErD） in EI Minya and Assuit, Upper Egypt. METHODS： One thousand consecutive patients with chronic GErD symptoms were included in the study over 2 years. They were subjected to history taking including a questionnaire for GErD symptoms, clinical examination and upper digestive tract endoscopy. Endoscopic signs suggestive of columnar-lined esophagus （CLE） were defined as mucosal tongues or an upward shift of the squamocolumnar junction. BF was diagnosed by pathological examination when specialized intestinal metaplasia was detected histologically in suspected CLE. pH was monitored in 40 patients. rESULTS： BE was present in 7.3% of patients with chronic GErD symptoms, with a mean age of 48.3 ± 8.2 years, which was significantly higher than patients with GErD without BE （37.4 ± 13.6 years）. Adenocarcinoma was detected in eight cases （0.8%）, six of them in BE patients. There was no significant difference between patients with BE and GErD regarding sex, smoking, alcohol consumption or symptoms of GErD. Patients with BE had significantly longer esophageal acid exposure time in the supine position, measured by pH monitoring. CONCLUSION： The prevalence of BE in patients with GErD who were referred for endoscopy was 7.3%. BE seems to be associated with older age and more in patients with nocturnal gastroesophageal reflux.

BACKGrOUND Childhood cancer survivors are potentially at a higher risk of infection with hepatitis C virus (HCV).The effects of all-oral direct-acting antiviral therapy (DAA) on both the HCV infection as well as the state of cancer remission have not been well investigated in this *** To test the effects of dual sofosbuvir/daclatasvir (SOF/DCV) therapy in the treatment of chronic HCV in survivors of hematologic malignancy in pediatric age *** We conducted a prospective,uncontrolled,open-label multicenter study.A total of 20 eligible,chronic HCV,genotype-4,infected children who had been in continuous complete remission from hematologic cancer (leukemia/lymphoma) for at least one year were included in the *** patients were treated with combined SOF/DCV for 12 *** were monitored throughout the study till 12 wk after end of treatment for safety and efficacy outcomes including the sustained virologic response 12 (SVr12) rate,hematological indices,liver and kidney *** The intent-to-treat SVr12 rate was 20 of 20 (100%;95%CI: 84%-100%).All patients showed normalized liver enzymes from *** hematological indices,liver and kidney functions were kept normal throughout the *** fatalities or treatment-emergent serious or severe adverse events were reported throughout the *** SOF/DCV combined therapy could be used safely and effectively in the treatment of chronic HCV genotype-4 infection in leukemia/lymphoma treated *** relapses were detected during treatment and throughout the follow up period for either the original malignant disease or the HCV infection.

Soil organic matter (SOM) is a fundamental soil constituent. The estimation of this parameter in the laboratory using the classical method is complex time-consuming and requires the use of chemical re-agents. The objectives of this study were to assess the accuracy of two laboratory measurement setups of the VIS-NIr spectroscopy in estimating SOM content and determine the important spectral bands in the SOM estimation model. A total of 115 soil samples were collected from the non-root zone (0-20 cm) of soil in the study area of the Triffa Plain and then analysed for SOM in the laboratory by the Walkley-Black method. The reflectance spectra of soil samples were measured by two protocols, Contact Probe (CP) and Pistol Grip (PG)) of the ASD spectroradiometer (350-2500 nm) in the laboratory. Partial least squares regression (PLSr) was used to develop the prediction models. The results of coefficient of determination (r2) and the root mean square error (rMSE) showed that the pistol grip offers reasonable accuracy with an r2=0.93 and rMSE=0.13 compared to the contact probe protocol with an r2=0.85 and rMSE = 0.19. The near-Infrared range were more accurate than those in the visible range for pre-dicting SOM using the both setups (CP and PG). The significant wavelengths contributing to the pre-diction of SOM for (PG) setup were at:424, 597, 1432, 1484, 1830,1920, 2200, 2357 and 2430 nm, while were at 433, 587, 1380, 1431, 1929, 2200 and 2345 nm for (CP) setup.

Determination of Clopidogrel Carboxylic Acid in Human Plasma by LC-MS/MS

Background: Clopidogrel, is a thienopyridine derivative labeled for use to prevent thrombosis after coronary artery stenting. Pharmacokinetics of clopidogrel is studied indirectly by quantification of carboxylic acid which is a major metabolite of Clopidogrel. Objective: The aim of this work is to develop and validate a rapid, simple and sensitive LC/MS/MS assay method for the determination of Clopidogrel carboxylic acid in human plasma using Clopidogrel-D4-carboxylic acid as internal standard. Methods: Analytes was extracted from 200 μl of plasma by a simple liquid-liquid extraction using diethyl ether – n-hexane (80:20, v/v). The chromatographic separations were achieved on a C18 column using Methanol, de-ionized water and formic acid as a mobile phase at flow rate of 0.5 ml/minute. Analysis was monitored by multiple reactions monitoring mode based on m/z transition of 308.10→113 for Clopidogrel carboxylic acid and 312.10→129 for internal standard. result: The method had a total run time of about 4 minutes. The lower limit of quantification (LLOQ) was 25 ng/ml showing good linearity over the working range of 25 – 3000 ng/ml (r ≥ 0.999). The intra- and inter day accuracies were 90% - 98% and 92.138% - 96.889% respectively (deviation within acceptable range ≤ 10%).Conclusion: It was shown that this method is suitable for pharmacokinetic study following oral administration of Clopidogrel and can be successfully applied to the therapeutic drug monitoring of Clopidogrel in Clopidogrel-treated patients.

Changes in the sucrose metabolism of Cur-cuma longa *** were studied under treatment with different triazole compounds viz.,triadimefon(TDM)and propiconazole(PCZ).Plants were treated with TDM at 15mg/L and PCZ at 10mg/L separately by soil drenching on 80,110,and 140 days after planting(DAP).The plants were harvested randomly on 90,120,and 150DAP to determine the effect of both the triazoles on sucrose metabolizing enzymes and phenol *** sucrose metabolism was studied by analyzing sucrose metaboliz-ing enzymes like sucrose synthase and sucrose phosphate *** the analyses were assayed in leaves and tubers of both control and treated *** was found that both of the triazole compounds had profound effects on these parameters.

Salt stress is one of the major limitations to modern agriculture that negatively inﬂuences plant growth and *** tolerant cultivar can provide excellent solution to enhance stress tolerance with plantﬁtness to unfavorable ***,this study was aimed to screen salt tolerant sorghum genotypes through evaluating of different morphological,biochemical,and physiological attributes in response to salinity *** this study,we have been evaluated total six sorghum genotypes including Hybrid sorgo,Debgiri,BD-703,BD-706,BD-707,and BD-725 under salt stress(12 dS m^(-1) NaCl).The response variables included length and weight of root and shoot,root:shoot ratio(rSr),photosynthesis(A),transpiration rate(E),elemental concen-trations(K^(+),Na^(+) and K^(+)/Na^(+)),photochemical efﬁciency of photosystem II(F_(v)/F_(m)),water use efﬁciency(WUE)and pigment content(chlorophyll a,and b).The results revealed that saline environment signiﬁcantly reduced all response variables under study of sorghum genotypes,however,Hybrid sorgo remained unmatched by recording the maximum root and shoot *** same genotype recorded higher photosynthetic efﬁciency which was attributed to Na^(+) extrusion,K^(+) uptake and higher K^(+)/Na^(+) ratio(1.8 at stress),while these mechanisms were not fully active in rest of ***,this study also implies the involvement of proline in imparting tolerance against saline environment in Hybrid sorgo ***,BD-703 remained the most salt sensitive genotype as evident from the minimum morphological growth traits and the least biosynthesis of ***ﬁndings open new research avenues for salt stress alleviation by identifying elite salt-to-lerant genotypes of sorghum for breeding programs.
