Controversy of hand-assisted laparoscopic colorectal surgery

Laparoscopically assisted colorectal procedures are time-consuming and technically demanding and hence have a long steep learning curve. In the technical demand, surgeons need to handle a long mobile organ, the colon, and have to operate on multiple abdominal quadrants, most of the time with the need to secure multiple mesenteric vessels. Therefore, a new surgical innovation called hand-assisted laparoscopic surgery (HALS) was introduced in the mid 1990s as a useful alternative to totally laparoscopic procedures. This hybrid operation allows the surgeon to introduce the non-dominant hand into the abdominal cavity through a special hand port while maintaining the pneumoperitoneum. A hand in the abdomen can restore the tactile sensation which is usually lacking in laparoscopic procedures. It also improves the eye-to-hand coordination, allows the hand to be used for blunt dissection or retraction and also permits rapid control of unexpected bleeding. All of those factors can contribute tremendously to reducing the operative time. Moreover, this procedure is also considered as a hybrid procedure that combines the advantages of both minimally invasive and conventional open surgery. Nevertheless, the exact role of HALS in colorectal surgery has not been well defined during the advanced totally laparoscopic procedures. This article reviews the current status of hand-assisted laparoscopic colorectal surgery as a minimally invasive procedure in the era of laparoscopic surgery.

The need for interval appendicectomy (I.A) after successful conservative management of appendiceal mass has recently been ***,emergency appendicectomy for appendiceal mass is increasingly performed with equal success and safety to that performed in non-mass forming acute *** is an increasing volume of evidence-although mostly retrospective-that if traditional conservative management is adopted,there is no need for routine I.A except for a small number of patients who continue to develop recurrent *** the other hand,the routine adoption of emergency laparoscopic appendicectomy (LA) in patients presenting with appendiceal mass obviates the need for a second admission and an operation for I.A with a considerable complication *** also abolishes misdiagnoses and deals promptly with any unexpected ileo-cecal ***,it may prove to be more cost-effective than conservative treatment even without I.A due to a much shorter hospital stay and a shorter period of intravenous antibiotic *** emergency LA is to become the standard of care for appendiceal mass,I.A will certainly become 'something' of the past.

AIM:To investigate the stability of the seven housekeeping genes:beta-actin(ActB),glyceraldehyde-3-phosphate dehydrogenase(GAPDH),18s ribosomal unit 5(18s),cyclophilin A(CycA),hypoxanthine-guanine phosphoribosyl transferase(HPRT),ribosomal protein large P0(36B4)and terminal uridylyl transferase 1(U6)in the diabetic retinal tissue of rat ***:The expression of these seven genes in rat retinal tissues was determined using real-time quantitative reverse transcription polymerase chain reaction(RT-qPCR)in two groups;normal control rats and streptozotocininduced diabetic *** stability analysis of gene expression was investigated using geNorm,NormFinder,BestKeeper,and comparative delta-Ct(ΔCt)***:The 36B4 gene was stably expressed in the retinal tissues of normal control animals;however,it was less stable in diabetic *** 18s gene was expressed consistently in both normal control and diabetic rats’retinal *** this gene was the best reference for data normalisation in RT-qPCR studies that used the retinal tissue of streptozotocin-induced diabetic ***,there was no ideal gene stably expressed for use in all experimental ***:Identifying relevant genes is a need for achieving RT-qPCR validity and reliability and must be appropriately achieved based on a specific experimental setting.

A study was carried out to estimate the actual evapotranspiration(ET)over a 1074 km2 of the humid area of Perak State(Malaysia),where water and evaporation cycle deeply influences the climate,natural resources and human living *** from both Terra and Aqua platforms of the Moderate Resolution Imaging Spectroradiometer(MODIS)sensor were used for ET estimation by employing the Surface Energy Balance Algorithm for Land(SEBAL)*** a part of the accuracy assessment process,in-situ measurements on soil temperature and reference ET(ET0)were recorded at the time of satellite *** order to enhance the accuracy of the generated ET maps,MODIS images were subjected to sub-pixel analysis by assigning weights for different land surface cover(urban,agriculture and multi-surface areas)*** weighting process was achieved by integrating ET from pure pixels with the respective site-specific ET0 of each land *** enhanced SEBAL model estimated ET exhibited a good correlation with the in-situ measured Penman-Montieth ET0,with R2 values for the Aqua and the Terra platforms of 0.67 and 0.73,***,the correlation of the non-enhanced ET maps resulted in R2 values of 0.61 and 0.68 for the Aqua and the Terra platforms,***,the results of this study revealed the feasibility of employing the sub-pixel analysis method for an accurate estimation of ET over large areas.

随着网络威胁和攻击越来越普遍,并迅速演变,支持恶意软件和反对恶意软件开发商之间的战斗似乎远远没有接近尾声。此外,人们对于远程和虚拟通信计算机和电子系统的需求,随着covid-19和社会距离的加大而变得越来越大,从而推动了需求曲线向上。这为网络罪犯创造了机会,并导致恶意软件攻击数量显著增加。现代的恶意软件采用各种复杂和不受怀疑的新手段来获得访问,并使被攻击计算机系统不被发现。但是,使用经典的方法来处理恶意软件检测,如基于签名,AV扫描,和其他方法来防止恶意软件攻击已被证明是过时的,不再有效。因此,处理恶意软件是计算机科学与工程,电子通信和技术发展的世界主要挑战之一。执法人员、商业实体和一般用户等利益攸关方遭受了多次严重损失。数据科学在网络安全中的重要性,在预测未来可能的威胁以减轻和预防恶意软件攻击方面,永远都不会被高估。数据科学、机器学习和人工智能在恶意软件预防和控制方面的一些应用已经在学术界提出并在现实世界中实现。我们的工作集中在一个探索性的实现机器学习的恶意软件检测算法,该算法失踪了 PE文件DLL信息,N-Grams,和深度学习技术。我们使用公共恶意软件数据集进行实验,并详细介绍了 ML技术和算法的概述和工作流程的恶意软件检测。并且,数据科学在网络安全中的应用的短暂下降也被探讨.

1) Background: Osteoarthritis (OA) is defined as a degenerative joint disease that mainly affects the bone. This study aims to evaluate the effect of low-intensity continuous ultrasound (LICUS) treatment on the knee of osteoarthritis patients through home-based intervention using the LICUS medical device. 2) Methods: The clinical trials were designed in a single-arm, open-label, and intervention study. Thirty-five participants, including those who dropped out (12%), were screened and enrolled. The patients received LICUS (1.1 MHz, 1.5 W/cm2, collimated beams) on the knee by the instructions of the investigator at home (5 min/session, 3 times/day, for four-weeks). Outcome measures were assessed using the Visual Analog Scale (VAS) as a primary endpoint and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) as a secondary endpoint to evaluate pain relief and functional recovery of the knee between pre-treatment (baseline) and post-treatment (four-weeks). 3) Results: Knee pain scores measured using the VAS and WOMAC indices were significantly reduced after a four-week treatment with LICUS compared to baseline. Knee stiffness and functional capacity were significantly reduced after the LICUS application. In addition, there were no reports of adverse effects during the study period. 4) Conclusion: Long-term and home-based application of LICUS can be recommended as an alternative option for the treatment of OA patients, as evidenced by the effect of pain relief and knee function recovery.
