A matched-pair analysis of laparoscopic versus open pancreaticoduodenectomy: oncological outcomes using Leeds Pathology Protocol

BACKGrOUND： Laparoscopic pancreaticoduodenectomy（LPD）is a safe procedure. Oncological safety of LPD is still a matter for debate. This study aimed to compare the oncological outcomes,in terms of adequacy of resection and recurrence rate following LPD and open pancreaticoduodenectomy（OPD）.METHODS： Between November 2005 and April 2009, 12LPDs（9 ampullary and 3 distal common bile duct tumors）were performed. A cohort of 12 OPDs were matched for age,gender, body mass index（BMI） and American Society of Anesthesiologists（ASA） score and tumor ***： Mean tumor size LPD vs OPD（19.8 vs 19.2 mm,P=0.870）. r0 resection was achieved in 9 LPD vs 8 OPD（P=1.000）. The mean number of metastatic lymph nodes and total number resected for LPD vs OPD were 1.1 vs 2.1（P=0.140）and 20.7 vs 18.5（P=0.534） respectively. Clavien complications grade I/II（5 vs 8）, III/IV（2 vs 6） and pancreatic leak（2 vs 1）were statistically not significant（LPD vs OPD）. The mean high dependency unit（HDU） stay was longer in OPD（3.7 vs 1.4 days,P〈0.001）. There were 2 recurrences each in LPD and OPD（logrank,P=0.983）. Overall mortality for LPD vs OPD was 3 vs 6（log-rank, P=0.283） and recurrence-related mortality was 2 vs *** was one death within 30 days in the OPD group secondary to severe sepsis and none in the LPD ***： Compared to open procedure, LPD achieved a similar rate of r0 resection, lymph node harvest and longterm recurrence for tumors less than 2 cm. Though technically challenging, LPD is safe and does not compromise oncological outcome.

Colorectal cancer is a leading cause of cancerrelated mortality,with nearly half of the affected patients developing liver *** three decades,liver resection(Lr)has been the primary curative strategy,yet its applicability is limited to about 20%of *** transplantation(LT)for unresectable metastases was attempted unsuccessfully in the 1990s,with high rates of perioperative death and *** is now more interest in this strategy due to improvements in systemic therapies and surgical techniques.A significant study conducted by the Oslo group showed that patients receiving liver transplants had a 60%chance of survival after five *** better results have been achieved by using advanced imaging for risk stratification and further refining selection criteria,especially in the Norvegian SECA *** review carefully charts the development and history of LT as a treatment option for colorectal cancer liver *** revolutionary path from the early days of exploratory surgery to the current situation of cautious optimism is traced,highlighting the critical clinical developments and improved patient selection standards that have made LT a potentially curative treatment for such challenging very well selected cases.

Long-term outcomes of pediatric liver transplantation in acute liver failure vs end-stage chronic liver disease:A retrospective observational study

BACKGrOUND Children with acute liver failure(ALF)who meet the criteria are eligible for super-urgent transplantation,whereas children with end-stage chronic liver disease(ESCLD)are usually transplanted *** liver transplantation(PLT)in ALF and ESCLD settings has been well described in the literature,but there are no studies comparing the outcomes in these two *** To determine if there is a difference in post-operative complications and survival outcomes between ALF and ESCLD in *** This was a retrospective observational study of all primary PLTs performed at a single center between 2000 and *** and ESCLD groups were compared for pretransplant recipient,donor and operative parameters,and post-operative outcomes including graft and patient *** Over a 20-year study period,232 primary PLTs were performed at our center;195 were transplanted for ESCLD and 37 were transplanted for *** ALF recipients were significantly older(median 8 years vs 5.4 years;P=0.031)and heavier(31 kg vs 21 kg;P=0.011).Living donor grafts were used more in the ESCLD group(34 vs 0;P=0.006).There was no difference between the two groups concerning vascular complications and rejection,but there were more bile leaks in the ESCLD ***-transplant patient survival was significantly higher in the ESCLD group:1-,5-,and 10-year survival rates were 97.9%,93.9%,and 89.4%,respectively,compared to 78.3%,78.3%,and 78.3%in the ALF group(P=0.007).However,there was no difference in 1-,5-,and 10-year graft survival between the ESCLD and ALF groups(90.7%,82.9%,77.3%vs 75.6%,72.4%,and 66.9%;P=0.119).CONCLUSION Patient survival is inferior in ALF compared to ESCLD recipients;the main reason is death in the 1st year post-PLT in ALF *** the ALF children overcome the 1st year after transplant,their survival stabilizes,and they have good long-term outcomes.

The CNO cycle is the main source of energy in stars more massive than our *** process defines the energy production,the duration of which can be used to determine the lifetime of massive *** cycle is an important tool for determining the age of globular *** proton capture via p+^(14)N→^(15)O+γ,at energies of astrophysical interest,is an important process in the CNO *** this project,we apply a potential model to describe both non-resonant and resonant reactions in the channels where radiative capture occurs through electric E1 *** employed the r-matrix method to describe the ongoing reactions via M1 resonant transitions,when it was not possible to correctly reproduce the experimental data using the potential *** partial components of the astrophysical S-factor are calculated for all possible electric and magnetic dipole transitions in ^(15)*** linear extrapolated S-factor at zero energy(S(0))agrees well with earlier reported values for all transition types considered in this *** on the value of the total astrophysical S-factor,depending on the collision energy,we calculate the nuclear reaction rates for p+^(14)N→^(15)O+γ.The computed rates agree well with the results reported in the NACrE II Collaboration and most recent existing measurements.

reservoir quality evaluation of the Narimba Formation in Bass Basin,Australia:Implications from petrophysical analysis,sedimentological features,capillary pressure and wetting fluid saturation

The evaluation of reservoir quality was accomplished on the Late Paleocene to Early Eocene Narimba Formation in Bass Basin,*** study involved combination methods such as petrophysical analysis,petrography and sedimentological studies,reservoir quality and fluid flow units from derivative parameters,and capillary pressure and wetting fluid saturation *** and diagenetic features are affecting the reservoir ***,compaction,and presence of clay minerals such as kaolinite are found to reduce the quality while dissolution and secondary porosity are noticed to improve *** is believed that the Narimba Formation is a potential reservoir with a wide range of porosity and *** ranges from 3.1%to 25.4%with a mean of 15.84%,while permeability ranges between 0.01 mD and 510 mD,with a mean of 31.05 *** on the heterogenous lithology,the formation has been categorized into five groups based on permeability *** I showed an excellent to good quality reservoir with coarse *** impacts of both textural and diagenetic features improve the reservoir and producing higher reservoir quality index(rQI)and flow zone indicators(FZI)as well as mostly mega *** non-wetting fluid migration has the higher possibility to flow in the formation while displacement pressure recorded as *** II showed a fair quality reservoir with lower petrophysical properties in macro *** irreducible water saturation is increasing while the textural and digenetic properties are still enhancing the reservoir *** III reflects lower quality reservoir with mostly macro pores and higher displacement *** may indicate smaller grain size and increasing amount of cement and clay *** IV,and V are interpreted as a poor-quality reservoir that has lower rQI and *** textural and digenetic features are negatively affecting the reservoir and are leading to smaller pore size and pore throat radii(r35)

An asymmetrical multilevel inverter with minimum voltage stress and fewer components for photovoltaic renewable-energy system

The enhanced power quality provided by multilevel inverters (MLIs) has made them more appropriate for medium- and high-power applications, including photovoltaic systems. Nevertheless, a prevalent limitation involves the necessity for numerous switches and increased voltage stress across these switches, consequently increasing the overall system cost. To address these challenges, a new 17-level asymmetrical MLI with fewer components and low voltage stress is proposed for the photovoltaic system. This innovative MLI configuration has four direct current (DC) sources and 10 switches. Based on the trinary sequence, the proposed topology uses photovoltaics with boost converters and fuzzy logic controllers as its DC sources. Mathematical equations are used to calculate crucial parameters for this proposed design, including total standing voltage per unit (TSVPU), cost function per level (CF/L), component count per level (CC/L) and voltage stress across the switches. The comparison is conducted by considering switches, DC sources, TSVPU, CF/L, gate driver circuits and CC/L with other existing MLI topologies. The analysis is carried out under various conditions, encompassing different levels of irradiance, variable loads and modulation indices. To reduce the total harmonic distortion of the suggested topology, the phase opposition disposition approach has been incorporated. The suggested framework is simulated in MATLAB (r)/Simulink (r). The results indicate that the proposed topology achieves a well-distributed stress profile across the switches and has CC/L of 1.23, TSVPU of 5 and CF/L of 4.58 and 5.76 with weight coefficients of 0.5 and 1.5, respectively. These values are notably superior to those of existing MLI topologies. Simulation results demonstrate that the proposed topology maintains a consistent output at varying irradiance levels with FLCs and exhibits robust performance under variable loads and diverse modulation indices. The temporal-spatial expression and functional analysis of three gustatory receptor genes in Solenopsis invicta using sweet and bitter compounds
