Epithelioid angiosarcoma is a rare vascular tumour with dark prognosis at short term. The primitive bone localization is exceptional and sometimes presents itself under a unifocal form. We report a clinical case of pelvic multifocal primitive bone epithelioid angiosarcoma (iliac crest and hip) diagnosed on a young male adult (45 years old). The radiological investigations put in evidence geographic osteolytic lesions (type IC) and, diagnostic was given thanks to the histological examination of the surgical biopsy pieces. The evolution was marked by the appearance of pulmonary and lymph node metastases with a death occurring in less than 4 months. Extremely rare tumour, bone epithelioid angiosarcoma is a variant of angiosarcoma characterised by an aggressive nature whose evolution is always fatal. Through this observation we report our experience on the diagnostic approach;and using a literature review we discuss the therapeutic modalities of this tumour with very bad prognosis.

The relationship between diabetes and periodontitis is established and described as a bidirectional influence, whereas that with dental caries is still controversial. The objective of this study was to analyze the relationship between blood glucose control and oral diseases in a population of diabetics followed at the “Centre Anti Diabétique d’Abidjan” (CADA). The dental condition of the diabetics included in this cross-sectional study was assessed by the DMF index, periodontal condition by the CPITN and oral hygiene by the OHIS index. Diabetes data were extracted from CADA patient medical records. Blood glucose control has been defined by an average of the blood glucose values of the last 3 months less than or equal to 1.26 g/l. Regression models were constructed to estimate the risk of diabetes imbalance adjusted for all characteristics collected. The sample of 356 diabetics consisted of 53.7% women with an average age of 55.6 years (range: 12 - 85 years). The average DMF index was 7.9 (Std: 6.6) with an estimated prevalence of caries of 77.0%, that of periodontitis of 53.1%. The proportion of subjects with good glycemic control was estimated at 41.3%. The characteristics associated with glycemic control were: dry mouth (p = 0.005), type of diabetes treatment (p < 0.00014) and duration of diabetes (p = 0.039) and periodontal status assessed with the CPITN (p = 0.014). The results of this study confirmed the link between periodontitis and glycemic control of diabetes described in the literature but didn’t find significant association between dental caries and diabetes.

Diffuse large B-cell lymphoma (DLBCL) is the most common non-Hodgkin lymphoma, characterized by high clinical and biological heterogeneity. Patients typically present with progressive lymphadenopathy, extranodal disease and may also experience fever, night sweats and unexplained weight loss. We report here the case of a 16-year-old female with osteoarticular pain, dizziness, and dyspnea on exertion. Clinical examination showed no lymphadenopathy. Complete blood count (CBC) revealed pancytopenia and marrow smears found to be hypocellular. Initial diagnosis favored secondary myelofibrosis. Diagnosis of bone marrow involvement by DLBCL was retained on bone marrow histology and immunohistochemistry which showed infiltration of large B lymphoid cells. The patient was treated by immunochemotherapy R-CHOP regimen. This case highlights a very rare and atypical circumstance of discovery of DLBCL with myelofibrosis as an initial symptom. Prognosis value of this presentation and management difficulties are also discussed.

Primary Malignant Bones Cancers in Cote d’Ivoire: Epidemiological and Histopathological Aspects

Primary bones cancers are rare and ubiquitous malignant tumors. Aim: Our study aimed was to describe the epidemiological and histopathological aspects of primary malignant bone cancers in Ivory Coast. Material and method: This is a retrospective and descriptive study of primary malignant bone cancers diagnosed over the period 1990 to 2013 (24 years). The studied parameters were: epidemiological aspects (frequency, age, sex, topography) and histological aspects. Results: Over the study period, primary malignant bone cancers accounted for 1.93% (n = 228) of cancers. The average age was 27.02 years with extremes of 2 and 85 years. The sex ratio was 1.65. They were in the skull and maxilla in 43.9% (n = 105), lower limbs (23.8%), spine and ribs (19.8%), pelvis (5.3%) and upper limbs (2.3%). The clinic was dominated by pain and alteration of general state. Radiology was variable depending on the histological type. In children, primary malignant bone cancers were found in the maxillary (77.9%) and the lower limb (15.8%). Burkitt’s lymphomas represented 36.4% (n = 66) of the histological types followed by 22.4% osteosarcomas (n = 51). In adults at histology, sarcomas were the most common (10%), followed by cancers of the hematopoietic and lymphatic organs (8.09%). Conclusion: Primary malignant bone cancers are rare in Ivory Coast. They most often affect male children with maxillofacial involvement and predominant lower extremity. Burkitt’s lymphoma and osteosarcoma are the often diagnosed histological types.

Ceramic Properties of Three Specimens of Alluvial Clays Used in Local Constructions from Mbouda Clay Deposit, West Cameroon

The Mbouda alluvial deposit is located at the foot of the Bamboutos mountains (West Cameroon) where three types of clayey materials are widespread. The populations collect these clays in their natural state in view of constructions using fired bricks or compressed blocks. Unfortunately, these buildings are not strong. This study investigates the causes of the strengthlessness of buildings and suggests solutions to overcome the difficulty. The research content includes field and laboratory studies. The methodology consists of sampling black (AN), white (AB) and red (AR) clays specimens identified in the study area and analysing them simultaneously at MIPROMALO (Cameroon) and at ACME LAB in Vancouver (Canada). The results obtained show a high sand content in the samples AN (64%), AB (55.2%), AR (30.9%). The compressive strength of the built specimens is low at 900˚C considered as the traditional firing temperature AN (0.94 MPa), AB (5.25 MPa), AR (2.18 MPa). The mineralogical series are identically made by kaolinite, chlorite, gibbsite, quartz, muscovite, biotite, goethite, magnetite and hematite. Silica (SiO2) presents higher contents AN (52.87%), AB (48.02%), AR (47.68%) followed by alumina (Al2O3) AN (29.96%), AB (28.13%), AR (24.72%). The other elements are poorly represented.

Radiotherapy of Oligoprogressive Lesions in Castration-Resistant Prostate Cancer: Impact on Second-Generation Hormone Therapy

Background: The therapeutic standard for oligoprogressive prostate cancer resistant to castration is second-generation hormone therapy. This systemic treatment is expensive. There are oligoprogressive lesions accessible to radiotherapy. Objectives: To study the impact of radiotherapy of oligoprogressive lesions on the implementation of second generation hormone therapy. Patients and Methods: A retrospective study from 2012 to 2020 was carried out. All patients with oligoprogressive prostate cancer who had received radiotherapy on one or more lesions in progression were collated. Survival was calculated using the Kaplan-Meier method. Results: 8 patients were treated with stereotactic and conformational radiotherapy between August 2012 and August 2020 in the context of oligoprogressive prostate cancer resistant to castration. The median age at diagnosis of oligoprogression was 73 years with a median PSA level of 3.11 ng/ml. Nine lesions were diagnosed with PET scan PSMA. All the lesions were treated by radiotherapy with different regimens. After a median follow-up of 12.5 months, 7 patients showed a biochemical response to treatment with a median decrease in PSA of 67%. The median survival without clinical or biochemical progression was 7 months. The median survival without the need for further systemic treatment was 9 months. Introduction: Tramadol* is a synthetic opioid agonist used as an analgesic. Despite its minor potential of addiction, it created recently a lot of concern. Its misuse may be motivated by a need to control the pain or by looking for her euphoric character. The aim of our study was to proceed a psychopathological reading of a case of addiction to Tramadol in a sickle cell patient with histrionic personality traits. Methods: We report the case’s management of a patient with addiction to Tramadol in a sickle cell patient with histrionic personality traits. The diagnosis was based on the DSM IV criteria. The review was done in the model of semi-structured interviews. Observation: Miss DP is 23 years old, sickle cell patient (AS profile). It is addressed to us for the management of an excessive consumption of Tramadol* started 4 years ago. Because of the difficulties of care in outpatient, we conducted hospitalization. The interviews highlight the histrionic personality traits and disruption of family dynamics. Discussion: Painful chronic diseases require a long analgesic treatment. It is a factor of dependence specially when the patient has a personality disorder. Multidisciplinary treatment between psychiatrist and addiction specialist and medical doctor is a guarantee of success.

Clinical Case of Bilateral Rupture of the Patellar Tendon of the Late Diagnosis

Defined as a continuity solution on the muscular-osseous-tendinous chain of the knee, ensuring the extension of the leg on the thigh, the traumatic rupture of the knee’s extensor apparatus is often reported in literature. They are dominated by patellar fractures. The patellar or quadricipital tendon damage is rare and even exceptional in a bilateral topography. The aim of this work was to highlight the clinical case of bilateral rupture of the patellar tendon of late diagnosis, occurring in an adult with a particular field. It was an adult, who presented a bilateral rupture of the knee extensor apparatus following a minimal traumatism. Anamnesis found a chronic kidney disease correctly follow-up. The diagnosis was delayed so was the surgical management (Krackow’s technique and protection by a metallic frame). At 8 months post surgery the functional result is acceptable. The bilateral rupture of the patellar tendon stays a rare pathologic entity, which always seems associated at a particular field. The diagnosis delay (and so did the care) is an important bad prognosis factor. Treatment of this old form (unknown) is exclusively surgical.

Base Thickness Optimization of a (n+-p-p+) Silicon Solar Cell in Static Mode under Irradiation of Charged Particles

In this work, we propose a method to determinate the optimum thickness of a monofacial silicon solar cell under irradiation. The expressions of back surface recombination velocity depending the damage coefficient (kl) and irradiation energy (øp) are established. From their plots, base optimum thickness is deduced from the intercept points of the curves. The short-circuit currents Jsc0 and Jsc1 corresponding to the recombination velocity Sb0 and Sb1 are determinated and a correlation between the irradiation energy, the damage coefficient and optimum thickness of the base is established.

Introduction: The assessment of health care needs must be reproducible, independent of the operator, hence the use of indexes like the Index of Orthodontic Treatment Need (IOTN). The aim of this study was to assess both, the prevalence of malocclusions among patients received in the Dental Surgery Unitat Yalgado Ouédraogo Teaching Hospital and their care needs according to IOTN index. Materials and Methods: It is a prospective, cross-cutting diagnostic study of malocclusions at Yalgado Ouédraogo Teaching Hospital (CHUYO), conducted from January 1 to December 31, 2016. The examiner with an examination tray records on the 12- to 16-year-old patients’ medical recorded, the variables based on the polychromatic pictures series, the IOTN rule, and the dental health component table. Results: On the whole, 81 patients were included, with 60.5% of girls and 39.5% of boys. 16- and 12-year-old patients were dominant. The average age was 14.05 years old. Various types of malocclusions were encountered. The need for orthodontic care was 56.8%. For 14 patients, 17.28%, their aesthetic component score is higher than the assessment made by the orthodontist. Conclusion: Aesthetics was an important factor in the need for orthodontic care. The aesthetic deficit entails a psychological impact that should be taken into account in the assessment of care needs.
