Background: Diagnosis of autoimmune diseases (AID) is challenging, due to overlapping features with other non-immune disorders. Anti-nuclear antibodies (ANA) are sensitive screening tests but anti-deoxyribonucleic acid-antibody (anti-DNA), and anti-extractable nuclear antigens (anti-ENA) are specific for AIDs. We aimed to look at ANA patterns in our patients and correlated them with anti-ENA for proper interpretation and better patient management cost-effectively. Methods: A retrospective study was conducted over 1 year from January to December 2022 who were tested for ANA at biology medical laboratory of Pasteur Institute of Dakar. Anti-ENA and anti-DNA results were also analyzed for ANA-positive patients. Statistical analysis was performed using STATA 14.0, p Results: 216 patients were analyzed. Women predominated at 79.2% and mean age was 48 years [CI 95%, 46 - 50], with extremes of 10 and 89. Most represented age group was [41 - 60] with 38%. ANA was positive in 27 (12.5%) of patients, 59.2% of whom were strongly positive (titer of 1/1000, 1/3200 or 1/6400). The most common pattern was nuclear speckled, which was found in 77.8% of samples. Anti-ENA and anti-DNA positivity in ANA-positive patients was found respectively in 63% (17/27) and 1.4% (3/27) of the samples analyzed. Most commonly identified anti-ENA was anti-Sm 29.6%, anti-SSA 29.6%, anti-Ro-52 25.9%, anti-RNP 18.5% and anti-SSB 14.8% which was associated with speckled pattern. Association results indicated a significant relationship between both tests and between ANA titer in the anti-ENA- and ANA-positive patients (p 0.001). Conclusions: ANA, Anti-ENA and anti-DNA antibodies are essential for AIDS diagnosis. However, the testing repertoire should follow an algorithm comprising of clinical features, followed by ANA results with nuclear, mitotic, and cytoplasmic patterns, anti-ENA, and anti-DNA for a more meaningful, and cost-effective diagnostic approach.

Purpose: High urinary retention (HUR) can negatively impact renal function. Our study aimed to present the epidemiological, diagnostic, and therapeutic aspects of HUR in a Senegalese academic hospital. Patients and Methods: We conducted a retrospective study of 70 patients with HUR associated with renal failure from January 2017 to December 2020. Parameters examined included: age, sex, coexisting conditions affecting renal function, clinical symptoms, diagnostic tests, causes of HUR, urinary diversion, and patient outcomes. Results: The average age was 66, with a majority of male patients (87%). Twenty-three patients had pre-existing medical conditions. Oligo-anuria was the most common reason for detecting HUR (70%). Half of the patients had an ECOG score ≥ 2. The mean creatinine level was 50.7 mg/l. Nineteen patients exhibited hydroelectrolytic disorders. Bacterial colonization was observed in 25 patients. Ultrasound and computed tomography were the most frequently performed imaging tests (100% and 62.8%, respectively). Sixty-seven patients had ureterohydronephrosis (UHN), with bilateral UHN in 88.6% of cases. Pelvic cancers (47.1%) were the primary cause of HUR, primarily bladder cancers (27.1%). Nephrostomy was the most common urinary drainage method (50%), particularly for obstructions due to pelvic cancer (88.6%). The majority of patients (52.8%) regained normal renal function after drainage. Nineteen deaths occurred among elderly patients with compromised general health. Conclusion: Urinary drainage significantly improved renal function for most patients. Pelvic cancer emerged as the leading cause of HUR. Nephrostomy was the predominant drainage method.

Background: Type 2 diabetes is a metabolic disease characterized by chronic hyperglycemia. The latter plays an essential role in inflammation by stimulating the production of pro-inflammatory cytokines or by increasing their secretion by an oxidative mechanism. These cytokines control the hepatic synthesis of an inflammatory protein called C-Reactive Protein (CRP). High or average values of CRP would have a predictive value of cardiovascular diseases and type 2 diabetes. The relationship between low CRP values and the risk of developing type 2 diabetes and its cardiovascular complications is not sufficiently studied. The hsCRP could serve as a predictive biomarker of risk of onset, follow-up and prognosis for type 2 diabetes and its cardiovascular complications. To answer this problem, we conducted this study, the aim of which was to study the predictive role of hsCRP in the risk of occurrence of type 2 diabetes and its cardiovascular complications. Materials and Methods: This is a prospective and cross-sectional case-control study involving 200 participants including 100 control women and 100 women with type 2 diabetes (mean age was respectively 49.89 years ± 8.26 & 51.92 years ± 7.18;p = 0.066). The interviews were conducted on the basis of a questionnaire. Physical examination collected biometric data and cardiovascular constants. The biochemical parameters such as hsCRP were analyzed by an automated Abbott device. Results: We noted that hsCRP was significantly higher in type 2 diabetic subjects compared to control subjects (p (rho = 0.40, p systolic blood pressure (rho = 0.30, p = 0.003), diastolic blood pressure (rho = 0.28, p = 0.006), total body fat (rho = 0.48, p Conclusion: hsCRP has a prognostic value in the evaluation of cardiovascular risk. It seems to play an important role in the pathogenesis of type 2 diabetes mellitus and its cardiovascular complications. It could thus be considered as a biomarker for the screening, monitoring and prognosis of type 2 diabetes mellitus.

Background: Extended-spectrum beta-lactamase (ESBL) producing bacteria are a real public health problem, particularly in Africa. Among these ESBLs, there are the Muenchen Cefotaximase (CTX-M) described all over the world of which the most frequent is the CTX-M of group 1 particularly the CTX-M-15 variant. The objective of this study was to determine the prevalence of CTX-M group 1 ESBL-producing Escherichia coli strains and to test their antibiotics susceptibility profile. Methodology: A retrospective cross-sectional descriptive study was conducted to detect ESBL-secreting Escherichia coli strains by the synergy test. Identification of CTX-M type ESBL from group 1 was performed using the NG-Test CTX-M rapid diagnostic test (NG-Biotech®). Antibiotic susceptibility profile was determined using CA-SFM/EUCAST guidelines 2019. Data entry and statistical analysis were performed with Excel version 2010 and SPSS 20.0 respectively. Results: Eighty-two ESBL-producing Escherichia coli strains were tested. A group 1 CTX-M ESBL was detected in 75.6% of the strains (n = 62). These strains were highly resistant to cefotaxim (100%), aztreonam (100%), ceftazidim (85.4%) and cefepim (66.1%). They were also resistant to quinolones, gentamycin and sulfadoxine-trimethoprim combination. However, these strains showed sensitivity to ertapenem (100%), cefoxitin (69.3%), tigecyclin (66%), and amikacin (66.1%). The combination of piperacillin and tazobactam was active on 30.6% of the strains against 6.4% for the combination of amoxicillin and clavulanic acid. Conclusion: The CTX-M type ESBL of group 1 was present in the majority of ESBL-producing Escherichia colis trains. Despite the production of this enzyme conferring resistance to most beta-lactam antibiotics, some antibiotics remain active to treat infections caused by these germs.

Inland fishing predominates in the Kedougou region, which has no coastline. This type of fishing does not yet meet the local population’s demand for fish products. In response to this concern, a Community Agricultural Estate housing an integrated fish farm was set up in the commune of Bandafassi, in the village of Itato. Since its creation, this production unit has been faced with the problem of sourcing high-quality, low-cost, monosex male Oreochromis niloticus fry. In order to overcome this constraint, the present research focuses on the contribution of Garcinia kola to the productivity and economic profitability of the Itato farm. The aim of the research is to assess fish production in the experimental set-up and the production costs of tilapia in a controlled environment. The comparative study of the various production factors shows disparities only in the input factor, where scenarios 2 and 3 use additional products. These are 17-α-methytestosterone for scenario 2 and Garcinia kola for scenario 3. These products significantly interfere with fish production, with a fairly high mortality rate for scenario 2 (25% for two production cycles/ cohort2 (B5, B6, B7 and B8) treated with 17-α-methytestosterone). As for scenarios 1 and 3, mortalities are 5% with or without recourse to additional products (G. kola). In addition, average fish production for the three (03) scenarios is estimated at 28687.5 kg/2 cycles. It varies from one scenario to another, i.e. 30937.5 kg/2 cycles for scenarios 1 and 3 and 24187.5 kg/2 cycles for scenario 2. It is therefore higher in scenarios 1 and 3 than in scenario 2. This difference is due to the fairly large losses of individuals in scenario 2. Furthermore, the analysis of the profit and loss accounts for tilapia production varies from one scenario to another depending on the type of farm: 476 Franc CFA for scenario 1, 610 Franc CFA for scenario 2 and 472 Franc CFA for scenario 3 (F CFA = franc of the French Colonies of Africa). Vaginal Carriage of Group B Streptococcus in Pregnant Women in Rural Areas in Senegal

Vaginal carriage of Group B Streptococcus (GBS) is a maternal and child health issue. Our objective was to determine the prevalence of GBS carriage;identify the factors associated with this carriage and determine the antibiotic sensitivity of the isolated strains. We conducted a cross-sectional and prospective study in rural Senegal (in the health district of Sokone). Socio-demographic, clinical and gynaeco-obstetrical data were collected. Vaginal swabs were taken by the midwives on specific settings in order to test for GBS and other High Risk Vaginal Bacteria (HRVB). Antibiotic susceptibility testing was done according to the recommendations of the CA SFM/EUCAST 2020. In total, 100 pregnant women were targeted and 97 pregnant women were included. Their age ranged from 18 to 40 years with 64.9% (63/97) of participants belonging to the “20 - 30” age group. The overall prevalence of Group B Streptococcus carriage was 15.5% (15/97). However, the proportion of women with at least one high risk infectious bacteria was 29.89% (29/97). No statistically significant differences were found between GBS carriage and the potential factors studied. However, the study also looked for the presence of other high-risk bacteria and coinfections were indeed found between GBS and E. coli and Staphylococcus aureus. Antibiotic susceptibility testing shows that GBS strains were fully susceptible to penicillin G, erythromycin, clindamycin, chloramphenicol, rifampicin and vancomycin. Sensitivities to norfloxacin and gentamycin were 73.3% and 86.7% respectively. In contrast, high resistance to tetracycline (86.7%) was observed. GBS carriage remains a major public health issue because of its consequences for the mother and the newborn. Correct screening and proper monitoring of strain susceptibility remain one of the most effective means of patient management and care.

Purpose: In Sub-Saharan Africa, an important proportion of incident HIV cases occur among heterosexual serodiscordant couples (HSDC) but the majority of HIV negative partners can remain seronegative. These are called HIV-exposed seronegative (HESN). We aimed to compare immune activation (IA) levels between HESN, their HIV infected counterparts (HIV+ partners) and HIV unexposed uninfected individuals (HIV-neg Controls) and to evaluate the association between sexual exposure to HIV (SEHIV) and IA. Methods: We conducted a cross-sectional study in Dakar, Senegal on 148 participants recruited between November 2013 and February 2014: 40 HIV-neg Controls, 54 HESN and 54 HIV+ Partners. SEHIV was evaluated individually using questionnaires. IA level was measured by plasma level of β2-microglobulin (β2m). Logistic regression was used to estimate odds ratios (OR) and 95% confidence intervals (CI) for the different associations. Results: The median levels of β2m were 1.57 mg/l (IQR: 1.37 - 1.77), 2.14 mg/l (IQR: 1.76 - 2.43) and 2.24 mg/l (IQR: 1.80 - 3.17) for HIV-neg Controls, HESN and HIV+ partners, respectively. After adjustment, HESN had similar levels of IA with HIV+ partners but significantly higher than that of HIV-neg Controls (adjusted OR = 6.28;95% CI: [2.19 - 18.00]). The association between IA and SEHIV was evaluated in the HIV negative individuals. High frequency of SEHIV was associated with a β2m > 2.2 mg/l (OR = 6.56;95% CI: [1.71 - 25.21]);significantly more than median cut off value of >1.81 mg/l. Conclusions: Our study shows that, despite being uninfected with HIV, HESN individuals show a high level of IA, which was depended on the level of SEHIV.

Until recently,potassium(K)has not received considerable attention because of the general belief that soils contain ample amounts of this element.In addition,low rates of K fertilizer application in agriculture have led to rapid depletion of K in the rhizosphere soil in many underdeveloped countries.This results in various negative impacts,including preventing optimum utilization of applied nitrogen and phosphorus fertilizers.To compensate for these losses,massive use of K fertilizers in agriculture has been suggested.Potassium fertilizers are manufactured from rock minerals,particularly sylvite(KCl)and carnallite(KCl·MgCl2·6H2O).Unfortunately,to date,there is no cost-effective technology available for converting rock minerals into potassic fertilizers.Potassium-solubilizing microorganisms(KSMs)can release K from soil/minerals into plant-available forms,which could be a sustainable option.The possibility of using KSMs as efficient biofertilizers to improve crop production has been increasingly highlighted by researchers.In this review,the existing forms of K in soils and their availability and dynamic equilibrium are discussed.In addition,different K fertilizers and their advantages and disadvantages for crops are described.Furthermore,the microorganisms usually reported as K solubilizers,the research progress on KSMs,and future insights on the use of these KSMs in agriculture are reviewed.Screening and analyses of the published literature show that organic acid production is the common mechanism of K solubilization by bacteria and fungi.This review may serve as a proposal for the future research avenues identified here.

Hepatitis B virus (HBV) infection is highly endemic in Senegal. Vaccination of all children against HBV was introduced in 1999 and included in Expanded Programme on Immunisation in 2005. The aim of this study was to assess the prevalence and immune status against HBV in patients received at Pasteur Institut in Dakar, Senegal. Methods: Between January 2016 and December 2020, patients aged between 1 and 96 years received laboratory were included in the study. Serum samples were analysed for HBV serology (HBs antigen: HBsAg, HBs antibody: HBsAb and HBc antibody: HBcAb) using ARCHITECT? analyser. Patients with anti-HBs antibody levels (HBsAb ≥ 10 IU/l) were considered seroprotected against HBV. Results: A total of 5629 patients were analysed with a mean age of 39 years and extremes from 1 to 96 years. The most represented age group was 31 - 45 years with 38.4%. HBsAg was present in 520 patients (9.2%) and was signed by sex and age group. Anti-HBc antibodies were found in 52.7% of patients and 1603 (28.48%) had isolated anti-HBs antibodies reflecting proportion of people vaccinated at the time of the study. However, 2143 patients (41.9%) had no seroprotection (HBsAb 10 IU/L) and 640 (12.6%) had strong seroprotection defined as HBsAb > 1000 IU/L. Conclusion: Our results show a significant presence of virus in Senegalese population and low vaccination coverage, especially in adults. Evaluation of HBsAb levels and provision of HBV booster shots should be considered for children in Senegal.
