The diagnosis and management of ovarian cystic tumor is sometimes difficult. In this manuscript, we wish to summarize this issue by showing three cases. Ultrasound and Color Doppler are very useful to diagnose the presence of this tumor but also to distinguish between benign and malignant. Pelvic MRI is also a strong tool and is sometimes mandatory. HE4 and CA125 are often useful as a serum diagnostic marker of malignancy, but not always so. Recently, laparoscopic management has gained popularity;however, great caution should be exercised during laparoscopic operative procedures, as this may lead to intraperitoneal malignant cell spreading/proliferation when the tumor is malignant. During the surgery of malignant tumor, irrespective of laparoscopic or laparotomic, intraperitoneal rupture should be avoided. Since the final diagnosis must be made by histological examination, the patient must always be informed of this possibility.

Epidemiology of Fetal Death and Analysis of Causes According to CODAC Classification at Amath Dansokho Regional Hospital of Kedougou, Senegal

Objectives: This paper aims to study the epidemiology and causes of fetal deaths in utero at Regional Hospital Amath Dansokho of Kedougou (RHADK). Methodology: This was a retrospective epidemiological study conducted at the Maternity Ward of the Regional Hospital Amath Dansokho of Kedougou from June 01, 2022 to June 30, 2023, including all patients seen for delivery care. Data were analyzed using Statistical Package for Social Science (SPSS 22, Windows version). The parameters studied were the frequency of in-utero fetal death, sociodemographic characteristics, pregnancy and delivery data, neonatal data and cause-of-death classification according to the Cause of Death and Associated Conditions (CODAC) classification. Results: We recorded 1628 deliveries, with 231 cases of in-utero fetal death, a frequency of 14.2%. Fetal death occurred most frequently in multiparous women (64.5%). The majority of patients (72.3%) were transferred. 51.9% of patients with fetal death had at least 3 antenatal visits. On admission, fetal heart rate was absent in 73.2% of patients. The etiology of in-utero fetal death was dominated by maternal factors (high blood pressure, anaemia and diabetes), which accounted for 36.9% of deaths, followed by placental pathologies (retroplacental haematoma) and intrapartum pathologies (uterine rupture, abnormal presentation). Conclusion: In-utero fetal death can be prevented, and is mainly due to direct obstetric complications. The focus should be on the prevention and management of hypertensive disorders and their complications during pregnancy, the fight against anaemia and, above all, the rapid and correct management of dystocia.

When contributing to participatory research, farmers usually appreciate the performance of cowpea varieties using qualitative scores. The score they attribute to each variety are based on local knowledge. The specific criteria they individually use to attribute a score are not well described. The objectives of this work were to: 1) identify and describe exhaustively the local criteria used by farmers to measure the agronomic performance of cowpea;2) assess the variability and statistical structure of these farmer criteria across local contexts;3) and analyze the association between these farmer criteria and the classical agronomic measurement. To achieve these objectives, an augmented block design was implemented across fifteen locations in the regions of Maradi, Dosso and Tillabéri, representing a diversity of local contexts. From a set of 36 cowpea varieties, fifteen varieties were sown per location, including five varieties (controls) common to all locations. In each location, two replicates were sown in randomized Fisher’s blocks. After agronomic measurement and participatory evaluation (scoring of varieties by farmers), a group survey (focus group) was conducted in each location to identify the criteria considered by farmers to found their discretional scoring of varieties during the participatory evaluation. The analysis of the data identified, across locations, thirteen criteria defined by farmers to characterize the agronomic performance of cowpea. Some of these criteria were different according to location. Farmers ranked the three varieties with the best performance for each agronomical trait (Top 3 varieties). A comparison of the farmer ranking with the ranking based on agronomic measurements revealed similarity and complementary between both methods. This study highlighted the importance of considering both local and scientific knowledge in local varietal evaluations.

During this research work we developed another approach to digital mapping using the pixelation technic. This unprecedented digital mapping of the basin MSGBC in Senegal required the compilation of numerous geological data consisting of seismic lines and oil and hydraulic log reports. These spatial reference data include geological information from the surface to the top of the Campanian. The mapped terrains are composed of the Post-Paleocene Complex (PPC), the Paleocene, the Maastrichtian, and the Campanian. The nearest neighbor method has been used to establish the spatial distribution of the different geological formations. Histograms of values were used to determine the confidence intervals of the mapping. They were used to locate areas of low relative error and to apply the 3D digital mapping technique. For instance, Diender Guedj has been mapped at 1:25,000. The result of this mapping is extracted and processed using the DBMS (MySQL) software. The latter allowed both to determine Paleocene gab and update data. And then the database is processed. The programming languages PHP and Javascript have been used to simulate a website.

Assessment of the Management of Post-Tonsillectomy Pain at the ENT-CFS Department of Ignace Deen National Hospital

Introduction: Tonsillectomy is described as one of the oldest surgical procedures according to the authors of the surgical treatise (Aulus Amida and Paul of Aegina in the 6th and 7th centuries) found in the Vatican library. The contraindication of codeine in children has changed the management of post tonsillectomy pain. The aim of this study was to assess the management of post tonsillectomy pain in our developing country context. Methods: This was a prospective, analytical study lasting 6 months (September 2019-February 2020), carried out in the ENT/CFS department of the Ignace Deen National Hospital. We included in this study all patients who underwent a tonsillectomy in the department during the study period and who agreed to participate in the survey. Results: 34 patients were included in our study, i.e. a frequency of 25% of surgical interventions. The mean age of our patients was 18.06 ± 12 years with extremes of 03 years and 45 years. Recurrent hypertrophic tonsillitis with sleep disturbances was the most frequent indication for surgery in our study, at 47.06% (n = 16). We performed an isolated tonsillectomy in 52.9% (n = 18) of cases. Analgesia was multimodal using the WHO Step I and II analgesics in 44.1% (n = 15) and 55.9% (n = 19), respectively. Pain control was satisfactory in all patients. Conclusion: The evaluation of post tonsillectomy pain is essential for optimal management. In this indication, multimodal analgesia, involving several levels I and II analgesics, provided satisfactory pain control.

On the northern coast of the Cape Verde peninsula, Lake Retba is one of the few depressions that have preserved its water body due to the drought that plagued the Sahel in the 1960s. The aim of this article is to understand the resilience of the lake to the drought. For this purpose, a deep knowledge of interactions between the different components of the hydro-system, namely: lake, dune tablecloth, ponds and ocean is necessary. The use of electrical resistivity tomography (ETR) and chemical and isotopic tools yielded conclusive results. Electrical tomography of resistivity (ETR) shows throughout the coastal dune a superposition between fresh and salt water, with even a predominance of salt water at the level of the old canal. The chemistry of the major ions and isotopes confirms this contact between the saltwater bison of the sea and the lake by showing a superposition of the fresh/brackish waters on the salty waters of the bevel, which, in turn, are found in the very salty waters influenced by the lake. This translates a feeding of the lake by the sea through the bevel. At the canal level, this feeding is done, on the surface, by means of ponds whose water characteristics show that they constitute an outcrop of the bevel. The freshwater body of the coastal dune that floats on salt water in the form of a lens also flows into the lake. The feeding of the lake, by the sea through the bevel and the ponds, and by the water table, is therefore effective and attested by all the methods used in this work. It allowed the resilience of the lake against the drought. This inflow of seawater into the lake is at the origin of its salinization and therefore of the salt that settles on the bottom. This study shows that the survival of the lake also depends on safeguarding the ponds located in the north of the lake.

基于WRF-chem模式对北非2018年3月下旬的典型强沙尘暴过程进行模拟,分析了此次强沙尘发生季节、持续时间、局地特征以及传输路径的关键动力系统与动力机制。鉴于起沙是沙尘暴发生的关键点之一,并且起沙主要取决于风力和下垫面沙源性质,本文测试了三种起沙参数化方案的影响,并将模拟结果与卫星MODIS监测及其再分析资料MERRA-2进行了对比,又经系列统计方法检验。结果显示,宏观思路的起沙方案GOCART比AFWA和UoC两种起沙方案更适合此次大尺度强沙尘暴数值模拟(锋面跨度接近60个经度)。综合沙尘暴关键系统的动力机制分析和数值模拟结果显示,强沙尘暴关键系统为深厚的西风槽、沙尘冷锋锋面和锋后的地面高压反气旋。北非中部深厚的西风槽为后倾槽,该系统稳定,造成沙尘暴持续时间长。沙尘暴锋后反气旋中的下沉气流抑制了扬沙向高层扩散,造成低层能见度恶劣。沙尘锋区结合了动力、热动力以及湿热动力不稳定,因此锋区风力大,地面沙尘驱动力强。而西风槽和强大反气旋依托环流形势,提供了沙尘传输到三大洲的长途输送力。

Perceptions of Actors on the Community-Based Maternal and Neonatal Health Services Project at the Kolda and Sedhiou Regions

Introduction: The health of mothers and children remains a major concern in developing countries. Infant and maternal mortality rates, although declining, are still high. Access to health services is a major obstacle to reducing maternal and infant mortality. To support the Ministry of Health and Social Action in the implementation of its Strategic Plan for Reproductive Health (2012-2015), the NGO Micronutriment Initiative has developed a project based on community-based maternal and neonatal health services (CBMNHS) in the Kolda region. The general objective of this study is to collect the perceptions of the actors on the Community-Based Maternal and Neonatal Health Services project in the Kolda and Sedhiou regions. Method: This was a qualitative study consisting of an evaluation of the activities carried out within the framework of the CBMNHS project at the level of the intervention zone. The study focused on support groups, community actors, post nurses and implementers of the intervention. Sampling was comprehensive for community actors and nurse heads. Focus groups and in-depth individual interviews were used to assess the perceptions of women and support groups, as well as to better understand the dynamics of the intervention. The content analysis was performed to exploit the qualitative data. Results: All local and national health authorities interviewed are unanimous about the relevance of the project. The project’s strategies are essentially community-oriented. Thus, a large number of committees have been set up. A total of 1258 support groups were set up during the project, distributed as follows: 260 grandmother groups (Grandmothers Strategy);266 groups of pregnant women (Pregnant Women’s Solidarity Circles);248 Care Group;468 community watch and alert circles and 16 Future Fathers’ Groups (Future Fathers’ Solidarity Circles). Intrauterine Device in the Immediate Postpartum:Study Comparing Insertion after Cesarean Section and Vaginal Delivery

Objective To compare the safety, side effects and failure rates of contraception copper intrauterine device (IUDs) in the immediate postpartum period after a vaginal delivery and during a cesarean section. Patients and methods This was a descriptive and analytical study, conducted in Gynecology and Obstetrics departments hospital Pikine and Abass Ndao hospitals. It compared two groups of women with IUDs in the immediate postpartum period after a vaginal delivery and during cesarean section. Our sample consisted of 215 patients. We had divided them into two groups: group 1 consisted of 115 patients who delivered by cesarean section;Group 2 consisted of 100 patients who delivered vaginally and whom the insertion was made in post placental or within 48 hours after birth. The variables studied were the sociodémographic characteristics, characteristics of pregnancy and childbirth, insertion procedures, follow up with the assessment of side effects and complications. Results and comments The socio-demographic characteristics were superimposed in both groups. Nine patients were lost to follow up, 4 in group 1 and 5 in group 2. The complaints such as pain (3.7% for Group 1 against 2.6% for group 2), vaginal bleeding (2.1% for group 2, and 1.8% for group 1) were rarely reported and stackable in both groups. After 6 months of follow up, the menses was observed in 42.8% of group 2 against 43.1% of group 1. The abnormal menstrual flow, evaluated by the Higham score, was noted in 29.6% of group 2 and 25% of group 1, and was dominated by excessive bleeding. They motivated IUD removal in two patients. A significant difference was found in the perception of IUD strings, it was effective in 95.4% of patients in group 2 against 50.5% for group 1. The evictions were 9 at 3-month follow-up 7 related group 2 and 2 for Group 1. It was partial (isthmic IUD) and total expulsion. Obstetric Hemorrhage during the Third Trimester of Pregnancy: Experience in a University Hospital in Guinea

Aims: Obstetric hemorrhage, especially during the 3rd trimester of pregnancy, causes maternal, fetal and neonatal mortality and morbidity. We attempted to characterize its clinical features in Guinea. The objectives of this study were to describe the socio-demographic characteristics of the patients, identify the causes and contributing factors, describe the management and evaluate the maternal-fetal prognosis in such patients. Methods: We retrieved and analyzed patients with 3rd trimester hemorrhage whom we managed at Ignace Deen National Hospital, Guinea during 1-year period (1st of December 2019-30th of November 2020). Results: We experienced recorded 401 patients with 3rd trimester obstetric hemorrhage out of 5468 deliveries during the corresponding period;the rate being 7.33%. The main causes were as follows: placental hematoma (65.33%), placenta previa (27.68%) and uterine rupture (6.99%). The socio-demographic profiles were as follows: the age group of 25 - 29 years (28.42%), married (94.51%), uneducated (50.12%), and with a liberal profession. (43.64%) and pauciparous (30.42%). The conditions were considered to be preventable by managing risk factors during the prenatal consultation (PNC): 7.73% underwent no PNC. Cesarean accounted for 84.78% of patients. Prognosis was as follows: 14 maternal deaths (3.45% of a fatality), 34.66% of anemia, and 16.95% of hemorrhagic shock. Fetal/neonatal prognoses were poor. Conclusion: Obstetric hemorrhage during 3rd trimester remains the main cause of poor outcomes in Guinea. This study identified that this type of hemorrhage still represents an important cause of maternal and fetal morbidity and mortality in developing countries.
