Endoscopic Transsphenoidal Surgery for Pituitary Adenoma. Early Experience in Sohag University Hospitals

Objective: To report the results of our first series of patients undergoing a purely endoscopic endonasal approach for pituitary surgery in Sohag University Hospitals. Patients and Methods: We reviewed 20 consecutive patients during the period from January 2017 to January 2020 with pituitary adenoma who underwent purely endoscopic transsphenoidal resection of their lesions. The patients’ clinical outcomes, degrees of tumor removal, and complications were reported. Results: There were 18 primary and 2 recurrent adenomas (19 nonfunctioning and 1 functioning GH secreting adenoma). The average degree of gross total removal for tumors was 70%, C.S.F leak occurred in 20%, diabetes insipidus in 10% and sellar hemorrhage in 5%. There was no post-operative mortality, vascular injury, sphenoiditis, septal perforation, epistaxis or general complication. Conclusion: A purely endoscopic approach for pituitary adenoma treatment is a safe and effective alternative to the traditional microscopic procedure.

Intracranial Fungal Infections: Overview from Two Large Tertiary Hospital in Upper Egypt and Literature Review

Purpose: Fungal infections of the central nervous system (CNS) are potentially lethal conditions with high morbidity and mortality. In this review, we summarise the most common clinical manifestations, diagnostic methods, and treatment strategies for intracranial fungal infection at two tertiary care teaching hospitals. Material and methods: Prospective hospital study is carried out at Department of Neurosurgery;Assiut and Suhaj University Hospitals between January2010 to January 2018 (Minimum 12-months follow-up). Radiographs and hospital data of 74 patients with proven intracranial fungal infections were gathered and analyzed. There were no exclusion criteria: age, gender, clinical presentations, immunity status, radiological findings, laboratory, and microbiological data, types of management and outcome. In surgically treated patients, diagnosis was confirmed by pathologic evaluation. Gathered data were coded and entered into a computer and analyzed using SPSS version 22. Results: The greatest number of the patients had 40 to 60 years old (49;66%) and the mean age was 44 years. There was an overwhelming male patient’s ranged preponderance 66%;49 cases. Sixty-three patients (85%) were immunosuppressed;11 cases (15%) were immunocompetent. The most common causes of immunosuppression were diabetes 27 patients;43%, on chemotherapeutic agents 19 patients;31%, on corticosteroid 16 patients;25% and AIDS in one patient;1%. Five different fungal types were identified but Cryptococcus spp. was the most common cause of CNS fungal infection, occurring in 39 patients (53%). This was followed by Candida spp. in 14 patients (19%), Aspergillus in 11 patients (15%), Blastomyces in 7 patients (9%) and Coccidiosis in 3 patients (4%). Headache was the most common presenting symptom, occurring in 33 patients (45%). Other relatively common symptoms were nausea or vomiting 11 patients (15%), fever 10 patients;(13%), seizures 9 patients (12%), acute mental status changes 8 patients;(11%)

In present work,the effects of UV-B treatments on quality of fresh-cut spinach leaves were *** this purpose,spinach leaves grown in glasshouse were harvest,cleaned and placed into polystyrene foam dishes and wrapped by stretch *** packaging,freshcut spinach leaves were treated with UV-B irradiation and were stored at 5℃ temperature for 12 *** soluble solids,visual quality,L^*a^*b^* values,and also hue angle values of samples were determined during *** soluble solids content of samples in control group has been higher than those from samples in UV-B treatments,however,visual quality was lower than the other *** to L*values of the samples,it is found that lower ultraviolet irratiation(UV-B)treatments caused yellowing of samples,and also same result was obtained by b^* and a^* values of fresh-cut spinach ***,hue angles values of samples treated with UV-B for 6min was higher compared with the other ***,yellowing of fresh-cut spinach leaves was decreased,and visual quality scores of samples was increased through ultraviolet irradiation treatments.

This paper considers a multi-state repairable system that is composed of two classes of components,one of which has a priority for ***,we investigate the well-posedenss of the system by applying the operator semigroup ***,using Greiner’s idea and the spectral properties of the corresponding operator,we obtain that the time-dependent solution of the system converges strongly to its steady-state solution.

Chromosomal instability(CIN)is a hallmark of cancer and is associated with tumor cell *** triggers a chain reaction in cells leading to chromosomal abnormalities,including deviations from the normal chromosome number or structural changes in *** arises from errors in DNA replication and chromosome segregation during cell division,leading to the formation of cells with abnormal number and/or structure of *** in DNA replication result from abnormal replication licensing as well as replication stress,such as double-strand breaks and stalled replication forks;meanwhile,errors in chromosome segregation stem from defects in chromosome segregation machinery,including centrosome amplification,erroneous microtubule-kinetochore attachments,spindle assembly checkpoint,or defective sister chromatids *** normal cells,CiN is deleterious and is associated with DNA damage,proteotoxic stress,metabolic alteration,cell cycle arrest,and ***,despite these negative consequences,CiN is one of the hallmarks of cancer found in over 90%of solid tumors and in blood ***,CiN could endow tumors with enhanced adaptation capabilities due to increased intratumor heterogeneity,thereby facilitating adaptive resistance to therapies;however,excessive CiN could induce tumor cells death,leading to the"just-right"model for CIN in *** the complex nature of CIN is crucial for understanding the dynamics of tumorigenesis and for developing effective anti-tumor *** review provides an overview of causes and consequences of CIN,as well as the paradox of CIN,a phenomenon that continues to perplex ***,this review explores the potential of CIN-based anti-tumor therapy.

Aqueous ruthenium was detected in real-time under ambient conditions using microwaveassisted laser-induced breakdown spectroscopy(MW-LIBS).A 10 mJ laser energy and 750 W microwave power were directed at an open liquid jet sample of *** was observed that,for liquid flow,the coupling efficiency between the microwave and the laser-induced plasma was limited to 43%.The improvement in the ruthenium’s signal-to-noise ratio with MW-LIBS,with respect to LIBS,was *** on MW-LIBS,the limit of detection for aqueous ruthenium was determined to be 957±84 ppb.

In this paper, we consider point spectra of the operator corresponding to the M/M/1 queueing model with working vacation and vacation interruption. We prove that the underlying operator has uncountable eigenvalues on the left real line and these results describe the point spectra of the operator. Then, we show that the essential growth bound of the C0-semigroup generated by the operator is 0 and therefore it is not quasi compact, the essential spectral bound of the C0-semigroup is equal to 1. Moreover, our results imply it is impossible that the time-dependent solution of the model exponentially converges to its steady-state solution.

The viscosity of a substance or material is intensely influenced by the temperature,especially in the field of lubricant engineering where the changeable temperature is well *** this paper,the problem of temperature-dependent viscosity on mixed convection flow of Eyring Powell fluid was studied together with Newtonian heating thermal boundary *** flow was assumed to move over a vertical stretching *** model of the problem,which is in partial differential equations,was first transformed to ordinary differential equations using appropriate *** approach was considered to reduce the complexity of the ***,the transformed equations were solved using the keller box method under the finite difference scheme *** validation process of the results was performed,and it was found to be in an excellent *** results on the present computation are shown in tabular form and also graphical *** major finding was observed where the skin friction and Nusselt number were boosted in the strong viscosity.

Objective: The objective of this study was to examine the effect of hypothyroidism on hearing function in patients surgically treated for differentiated thyroid cancer and subsequently experienced hypothyroidism during preparation for follow up I-131 scintigraphy scan by either recombinant human thyroid stimulating hormone(rhTSH) treatment or thyroid hormone withdrawal(THW).Methods: A total of 55 patients undergoing I-131 scintigraphy scan following surgeries for differentiated thyroid cancer were included in the study, including 25 patients prepared by administration of recombinant TSH(rhTSH Group) and 30 patients by thyroid hormone withdrawal(THW Group).Results: Air conduction thresholds at 1, 2 and 4 kHz for both ears were higher during hypothyroid period than during euthyroid period for patients in the THW group(p k for hearing loss.
