In software-defined networking(SDN)networks,unlike traditional networks,the control plane is located separately in a device or *** of the most critical problems in these networks is a controller placement problem,which has a significant impact on the network’s overall *** paper attempts to provide a solution to this problem aiming to reduce the operational cost of the network and improve their survivability and load *** researchers have proposed a suitable framework called kernel search introducing integer programming formulations to address the controller placement *** demonstrates through careful computational studies that the formulations can design networks with much less installation cost while accepting a general connected topology among controllers and user-defined survivability *** researchers used the proposed framework on six different topologies then analyzed and compared with Iterated Local Search(ILS)and Expansion model for the controller placement problem(EMCPP)along with considering several evaluation *** results show that the proposed framework outperforms the ILS and ***,the proposed framework has a 38.53%and 38.02%improvement in reducing network implementation costs than EMCPP and ILS,respectively.

不明原因的消化道出血（OGIB）是指内镜表现为阴性的持续性或复发性出血,约占所有消化道出血的5%。不明原因的消化道出血可分为显性出血和隐性出血。显性出血是指初次内镜检查阴性（食管胃十二指肠镜和结肠镜检查）和影像学检查阴性（小肠灌肠造影）后复发或持续的肉眼可见的出血,如呕血、便血或黑便。而隐形消化道出血是指大便潜血阳性,伴或不伴有缺铁性贫血。使用胶囊内镜（CE）和球囊辅助肠镜（BAE）之前,消化道出血由Treitz韧带近端或远端分类。随着新型小肠镜技术的引入,消化道出血重新分类为上消化道出血（Vater壶腹近端）、中消化道出血（Vater壶腹至回盲瓣）及下消化道出血（结肠）。标准内镜的高漏检率和较老诊断模式对小肠评估的局限性影响OGIB的评估,而胶囊内镜、气囊辅助肠镜、螺旋肠镜和CT小肠造影的引入克服了这些局限性。胶囊内镜检查目前被推荐为双向内镜检查阴性后评估OGIB的第三种选择。气囊辅助小肠镜检查对OGIB的诊断和治疗都很有价值。在腔内和腔外小肠检查中,CT小肠造影优于小肠X线检查。小肠诊断和内镜治疗的进步已经很大程度上取代了外科手术,形成了无创评估及内镜治疗OGIB的趋势。我们报道了一例21岁患者,男性,黑朦、间歇性便血,摄入海鲜和酒精后加重,既往治疗没有明显改善,生命体征平稳,内镜显示回肠有一个边缘隆起的溃疡和两个肠腔,病理结果为粘膜慢性活动性炎症伴糜烂,诊为meckel’s憩室穿孔。行回盲部切除术,随访6个月,目前病情稳定。我们发现,酒精和海鲜的摄入是出血诱因,酒精可能会腐蚀胃壁和回肠。治疗导致的小肠血管反复扩张和术后急性和慢性回肠粘膜炎症也是出血的原因之一。几次手术后,患者痊愈并出院。

The Afya Jijini program targeted improved access to and quality of health services in Nairobi City County through the strengthening of local health systems. The program took a systems thinking approach recognizing that strong, coordinated commitments and contributions from various levels of multiple sectors such as health, public works, and education would be essential to the program’s success. Consistent with the World Health Organization’s proclamation that the provision of safe water, sanitation, and hygiene (WASH) are prerequisites for all public health endeavors, WASH-related efforts were woven into nearly all facets of the Afya Jijini program. IRC is a nonprofit located in the Netherlands focused on systems thinking approaches for sustainable WASH improvements. IRC’s 9 building blocks are tools to assist in breaking down the complexities of systems, identifying key actors and factors that contribute to sustainable improvements, and setting clear program indicators and targets for success. This conceptual manuscript seeks to explore the Afya Jijini program through the lens of the IRC’s building block approach to systems thinking. Examples of Afya Jijini programming aligned with each building block are presented to assist program planners and guide future interventions. Afya Jijini could have benefited from increased attention to several key building blocks, including institutions, finance, and learning and adaptation.

Early repolarization syndrome(ERS), demonstrated as J-point elevation on an electrocardiograph, was formerly thought to be a benign entity, but the recent studies have demonstrated that it can be linked to a considerable risk of life- threatening arrhythmias and sudden cardiac death(SCD). Early repolarization characteristics associated with SCD include high-amplitude J-point elevation, horizontal and/or downslopping ST segments, and inferior and/or lateral leads location. The prevalence of ERS varies between 3% and 24%, depending on age, sex and J-point elevation(0.05 m V vs 0.1 m V) being the main *** patients are sporadic and they are at a higher risk of having recurrent cardiac events. Implantable cardioverter-defibrillator implantation and isoproterenol are the suggested therapies in this set of patients. On the other hand, asymptomatic patients with ERS are common and have a better prognosis. The risk stratification in asymptomatic patients with ERS still remains a grey area. This review provides an outline of the up-to-date evidence associated with ERS and the risk of life- threatening arrhythmias. Further prospective studies are required to elucidate the mechanisms of ventricular arrhythmogenesis in patients with ERS.

The study took a comprehensive approach to understanding mastitis in lactating camels, a disease that significantly impacts milk quantity and quality. This complex and multifactorial disease poses a significant challenge to dairy camel farming, particularly in Somalia. The studys objective was to determine the prevalence of mastitis and the risk factors associated with it among lactating camels on dairy farms in Benadir, Somalia. To achieve this, a cross-sectional study was conducted from May to September 2022 in the Benadir region of Somalia. A total of 96 lactating camels underwent examination using the California Mastitis Test (CMT) to identify clinical and subclinical mastitis cases. Additionally, a questionnaire survey was conducted among 20 farm employees/owners to gather information on hygiene practices, parity, lactation stage, tick infestation, and the presence of udder/teat lesions. Data regarding parity and lactation stage were obtained through owner interviews. The data collected on specifically designed forms were recorded and entered into a Microsoft Excel spreadsheet for analysis using SPSS version 20 statistical software, ensuring a comprehensive and reliable analysis of the data. The studys findings revealed a significant prevalence of camel mastitis, with 34.4% (33/96) of lactating camels affected and clinical and subclinical cases constituting 5.2% and 29.2%, respectively. Quarter-level prevalence was 46.3%, with clinical and subclinical mastitis at 4.7% and 41.6%, respectively. The study also identified significant associations (P < 0.05) among risk factors such as stage of lactation, parity, and age, indicating a higher susceptibility to mastitis in older camels during early lactation compared to those in late lactation and younger camels. Introduction: Healthcare workers in Mogadishu, Somalia face significant occupational injury risks, particularly needle stick injuries, with 61.1% reporting incidents. This poses a serious threat to their health, leading to infections such as hepatitis B, hepatitis C, and HIV. Despite the high prevalence of injuries, awareness of Post-Exposure Prophylaxis (PEP) accessibility is relatively high, with 84.0% of respondents aware of it. However, there are gaps in knowledge and implementation, as evidenced by variations in availability of PEP. Improving workplace safety measures, providing comprehensive training on injury prevention and PEP protocols, and ensuring consistent availability of PEP in healthcare facilities are crucial steps to safeguard the well-being of healthcare workers in Mogadishu, Somalia. Methods: A cross-sectional study was conducted among hospital workers in Mogadishu, Somalia, focusing on professionals from various healthcare facilities. The study targeted nurses, doctors, laboratory personnel, and pharmacists. Purposive sampling was employed, resulting in a sample size of 383 calculated using Fisher’s sample size formula. Data were collected using coded questionnaires entered into Microsoft Excel 2019 and analyzed with SPSS software to generate frequencies and proportions, presented through frequency tables and pie figures. Results: The study in Mogadishu, Somalia, examined the prevalence of occupational injuries and knowledge of Post-Exposure Prophylaxis (PEP) accessibility among healthcare workers. Findings indicate a high prevalence of injuries, with 61.1% reporting incidents, predominantly needle stick injuries (60.6%). Despite the majority seeking prompt medical attention (72.0%), work-related illnesses affected 53.2% of respondents, notably work-related stress (59.5%). Study on Breeding Practices and Reproductive Performance of Black-Head Somali Sheep under Traditional Management System: The Case of Awbarre District, Eastern Ethiopia

The study was conducted in the Awbarre district of the Fafen zone of the Somali regional state of Ethiopia. The objective of the study was to assess the breeding practices and reproductive performance of Black-head Somali sheep under a traditional management system. Purposive and simple random sampling techniques were used to select targeted kebeles and households, respectively. A total of 120 households were selected from four kebeles, each of 30 households, based on the production system and sheep population. Semi-structured questionnaires, group discussions, key informants interviews and field observations were used to generate the required data. The primary purpose of keeping sheep was for income generation, followed by saving as a future asset. The majority (89.2%) of the respondents separated male and female animals during herding. The selection criteria for breeding rams were appearance, growth, pedigree, and color while for breeding ewes were appearance, adaptability, pedigree, color, and lamb growth. The overall weaning age of Black-head Somali sheep in the study area was 3.7 months for both males & females. The castration of male sheep was common for the purpose of fattening, fattening with breeding control and breeding control as well. The castration is mainly performed during the summer and autumn and the methods of castration were both traditional and modern methods, the traditional castration method being the most important one in pastoral areas. The age of sexual maturity was 7.64 months for rams and 8.97 months for ewe’s male and female lambs in the pastoral area and 8.42 & 8.38 for rams & ewes in agro-pastoral and overall lambing interval was 11 months. On average, the ewe of Black-head Somali sheep in pastoral & agro-pastoral could produce 9.49 & 9.57 lambs, respectively in their lifetime.
