Located south of the West Bank, Wadi Al-Samen is considered one of the most important sources of groundwater recharge for the eastern aquifer in Hebron. It is polluted by sewage originating from domestic and industrial consumption in the Hebron area. Water quality assessment is an important criterion for achieving sustainable development. To evaluate water quality, twenty samples were collected from groundwater sources for two seasons and were analyzed for Physical properties (Total dissolved solids (TDS), Electrical conductivity (EC), potential hydrogen (pH), Temperature (T)), Four major cations (Mg2+, Ca2+, Na+ and K+), and the Major anions (HCO-3, Cl-, and SO2-4);geochemical methods such as Piper scheme were used for the sample result analysis. To characterize wastewater components, six samples were collected from the Wadi discharge for two seasons and were analyzed (potential hydrogen (pH), Electrical Conductivity (EC), Total Dissolved Solid (TDS), Total Suspended Solids (TDS), Total Suspended Solids (TSS), Biological Oxygen Demand (BOD5), and Chemical Oxygen Demand (COD). The results of nitrate levels showed that 20% of the ground water samples exceeded the standard limit of the World Health Organization (WHO). The quality of drinking water was assessed using the Water Quality Index (WQI), which suggests that 10% of samples are classified from poor to very poor. The abundance of cations from highest to lowest was found to be: Ca;Mg;Na, and for the anions it is HCO3;Cl;SO4. The dominant hydrochemical facies of 35% of collected aquifer samples reveal that Ca-Mg-Na-Cl-HCO3 are in the domain. Evaluation of irrigation suitability was performed using parameters of Sodium adsorption ratio (SAR), electric conductivity (EC), and Salinity. Productivity Improvement Model without Increasing Operation’s Risk and Fatigue

Productivity improvement of an operation without increasing operation risk and operation fatigue that increase the needed relaxation allowance is an important subject in process design. This research subject stimulates researchers to focus on improving the productivity of the whole production process by changing the technique of performing significant operations in the process. However, two important issues that affect the implementation of any new technique were not considered in the pervious research works. These are the risk magnitude of the new technique on the workplace environment and the fatigue level that affect human's health. In this paper, a model was developed that maximize the productivity of the production process by selecting the best technique to perform significant process operations among proper candidate techniques that improve these operations productivity while minimizing these operations risk and fatigue.

Upper Gastrointestinal Bleeding at a Public Referal Hospital in Malawi

Background: Upper gastrointestinal bleeding (UGIB) is a medical emergency. Timely and appropriate treatment can be lifesaving. Where medical equipment and supplies are inadequate, management of upper gastrointestinal bleeding is challenging. Methods: A retrospective review of charts of patients who were admitted during the year 2010, with a diagnosis of Upper Gastrointestinal Bleeding (UGIB), was done at Kamuzu Central Hospital, Lilongwe, Malawi. A Rockall score was applied to determine mortality risk. Results: A total number of 187 records (119 men and 68 women, mean age of 40.7 ± 15.3 years) were reviewed. The mortality rate was 23.5%, with a non-significant gender difference. Bleeding oesophageal varices were the most common clinical cause of UGIB (42.8%), with more males (63.1%) than females affected. About 40% of patients had no cause of UGIB indicated in their records. 14 (7.5%) patients had a normal oesophagogastroduodenoscopy (OGD). Oesophageal tumor was present in 2.7% of the subjects as a cause of UGIB. Access to endoscopy, for diagnosis and therapeutic intervention, and surgery (Hassab procedure) was available to less than 50% of the patients. Sixteen patients (9.5%) had surgery after endoscopy due to lack of variceal banding materials.?Conclusion:?Upper gastrointestinal bleeding is an important and common clinical problem at Kamuzu Central Hospital. Oesophageal varices seem to be the commonest cause of UGIB. Inadequacy of resuscitation materials and perhaps timely diagnostic and therapeutic endoscopic and surgical interventions are important limiting factors to favourable patient outcome. Work towards regular provision and supply of interventional resources regarding UGIB management may improve patient outcome.

Prescribing patterns of antibiotics at two teaching hospitals in Sanandaj

We report here the results of a prospective analysis of the pattern of antibiotics use over a one month period(May 2009)at two teaching hospitals in Sanandaj,western part of *** on antibiotic usage were obtained from the pharmacy *** was coded,computed and analyzed using SPSS version 11.5.A total of 1336 and 556 patients from Beasat and Tohid hospital respectively received *** patients represented 758(41.5 %)and females 1080(58.5 %)of the total number of patients evaluated by physicians at the two hospitals during the one month *** of antibiotics varied from hospital to hospital;Beasat hospital had the highest *** ward-wise distribution of antibiotic use showed that the prevalence of prescribing was highest in the post partum ward(66.58%).Similarly,among Tohid hospital wards,men surgical ward and women surgical ward prescribed antibiotics significantly more than all other *** route of administration of antibiotics was parentrally for 85.5% and 88.2% at Beasat anf Tohid hospitals *** distribution of use of individual antibiotics showed that cepfzoline was the most frequently prescribed antibiotics used at both the ***(childbirth)and pneumonia were the most frequent diagnosed pattern for which an antibiotic was prescribed at Beasat and Tohid hospital *** conclusion our study suggests that there is considerable scope for improving prescribing pattern among the practitioners and minimizing the use of *** improvement would be facilitated by providing feedback,prescriber education and hospital formulary.

Simple Quantitation of Flucytosine in Low Volume Serum Samples and Evaluation of Common Endogenous Interferences by HPLC-UV

5-Fluorocytosine (5-FC) is used for the treatment of several infections. It is extremely important to monitor blood level concentration for maximum activity to avoid its side effects. A simple, faster, and more accurate analytical method is developed and validated using high-performance liquid chromatography with UV detection in a very low-volume serum sample. Exactly 50 μL of serum was precipitated with 5% trichloroacetic acid. After mixing and centrifugation, 20 μL of supernatant was injected into the HPLC column. Detection was performed at 280 nm. The method is very specific and free from interfering substances due to different drugs and their different circulating metabolites. The limit of detection (LOD) and limit of quantification (LOQ) were found to be 0.50 μg/L and 1.0 μg/L, respectively. The method was linear in the range of 5 - 150 μg/L in the serum sample. In method comparison, the correlation coefficient r2 was 0.999 and the percentage recovery was 90% - 105% on four levels of the quality control samples. Within run and between run precision was found to be less than 2.2% at four different concentrations (5, 25, 50, and 100 μg/L). A simple, faster, and more accurate HPLC-UV method is developed which is very useful for monitoring 5-FC concentration in low volume serum samples without evaporation step and ion exchange chromatography within minutes.

Validation Models of Turbulent Flow and Heat Transfer

A FEM (finite element method) based investigation is carried out for the determination of confined turbulent flow and associated heat transfer in a long straight channel. The effects of such flow in a zone close to a solid boundary have been investigated, by developing a finite element modeling technique based on near wall zone in which it replaces the traditional use of the empirical laws. The validity of the wall technique was investigated and well compared with other standard techniques.

On Turbulent Flow Near a Wall

A finite element method is advocated for representing equations governing confined turbulent flow. In particular, a technique for the effective mapping of regions close to the solid boundaries has been adopted for predicting the distribution of the pertinent variables in this zone in a smooth straight channel. In the present work, the validity of the imposed technique has been tested when two types of flow were examined, pressure flow and the combination of pressure and Coquette flow with a one equation model used to depict the turbulent viscosity.

Aim: To determined risk of increases Incidence of Thyroid cancer in solitary thyroid nodules so Preoperative distinction between benign and malignant in solitary thyroid nodules is important and helps to avoid unnecessary surgery and its adverse effects, such as hypothyroidism, hypocalcemia, and recurrent nerve injury. Methods: Descriptive perspective analyzed data over a period of 6 years April 2015-April 2021 in Saudi Hospital at Hajjah, Yemen. 226 thyroid operations for 207 patients, 135 patient’s diagnosis as Solitary thyroid nodule and 72 patients as Multi nodular goiter. Patients with a clinically solitary thyroid nodule were included in the study group. Results: 135 cases of clinically detected solitary thyroid nodules, 126 female and 9 male patients, between 14 - 65 years age, median 41 years and mean 39.76 years, (94, 41) patients respectively Right side thyroid effect more than Left side, Fine needle aspiration cytology (FNAC) sensitivity, specificity and accuracy was (61%, 72%, 64%) respectively. Postoperative histopathology reported 100 (74%) patients as having benign thyroid nodules and 35 patients (26%) as having malignant thyroid nodules. Postoperative transient hypocalcemia was observed in 9 patients (7%), and temporary hoarseness was observed in 3 patients (2%). Conclusion: The incidence of malignancy in solitary thyroid nodules is high. Rapid growth by history and hard fixed nodule by examination and hypoechoic, micro calcification and cervical lymphadenopathy on Thyroid Ultrasound frequently in malignant nodules. Male risk factors for thyroid cancer but age, number and size of nodules not included as risk factor. Fine needle aspiration cytology (FNAC) is more helpful for diagnosing if aspiration under Ultrasound guidance and reading by experience histopathologists. Asthma is a chronic inflammatory disorder of the airways characterized by recurring episodes of reversible airway obstruction, hyper-responsiveness, wheezing, breathlessness and coughing. Clinical diagnosis of asthma is based on the pattern of clinical symptoms and pulmonary fuction tests. Asthma affectes 5% - 10% of the population and the number of worldwide cases is approximately 300 milliones. The incidence of this disease is increasing particulry in western countries [1]. It is the cause of a huge economic burden to national healthcare services. In a minority of cases, asthma is potentially fatal. After a period when fatalities appeared to be increasing [2], in recent years asthma-related mortality has progressively declined due to the develop- ment of specific asthma disease management programs, as well as the extensive use of in- haled corticosteroids [3]. Inflammation of the airways is a central component in asthma. In- flammation is associated with infliltration of the airway wall with eosinophiles and or neutron- philes mast cell degranulation and T cell active- tion. Other pathological features include, sub- basement membrane thickening, loss of epithet- lial cell integrity, goblet cells hyperplasia In- crease in airway smooth muscle mass. Eosino- phils are thought to be vital in the development of airway hyperreactivity, with the eosinophil cationic protein playing a crucial role [4]. The fact that treatment of asthma with corticos-teroids reduces eosinophils numbers and decreases airway reactivity further supports this hypothesis.
