Long-term hydrochemical monitoring and geothermometry:understanding groundwater salinization and thermal fluid contamination in Mila’s basin,Northeastern Algeria

The regular hydrochemical monitoring of groundwater in the Mila basin over an extended period has provided valuable insights into the origin of dissolved salts and the hydrogeochemical processes controlling water salinization.The data reveals that the shallow Karst aquifer shows an increase in TDS of 162 mg L^(-1) while the ther-mal carbonate aquifer that is also used for drinking water supply exhibits an increase of 178 mg L^(-1).Additionally,significant temperature variations are recorded at the sur-face in the shallow aquifers and the waters are carbo-gaseous.Analysis of dissolved major and minor elements has identified several processes influencing the chemical composition namely:dissolution of evaporitic minerals,reduction of sulphates,congruent and incongruent car-bonates’dissolution,dedolomitization and silicates’weathering.The hydrogeochemical and geothermometric results show a mixing of saline thermal water with recharge water of meteoric origin.Two main geothermalfields have been identified,a partially evolved water reservoir and a water reservoir whosefluid interacts with sulphuric acid(H_(2)S)of magmatic origin.These hot waters that are char-acterized by a strong hydrothermal alteration do ascend through faults and fractures and contribute to the contamination of shallower aquifers.Understanding the geothermometry and the hydrogeochemistry of waters is crucial for managing and protecting the quality of groundwater resources in the Mila basin,in order to ensure sustainable water supply for the region.A conceptual model for groundwater circulation and mineralization acquisition has been established to further enhance under-standing in this regard.

A 61-year-old woman had cholangitis secondary to two cholelithiases[one in the common bile duct(CBD)and one in the cystic duct(Mirizzi syndrome)],which was treated by antibiotics.The patient subsequently underwent a shincterotomy with endoscopic retrograde cholangiopancreatography(ERCP)and,finally,a cholecystectomy by coelioscopy.The postoperative course was marked by biliary leakage.She was transferred to our hospital due to haemorrhagic shock and abdominal pain.

Evaluating the influence of a structured nursing protocol on targeted outcomes in rheumatoid arthritis patients

Objective:Rheumatoid arthritis(RA)requires comprehensive management.Structured nursing protocols may enhance outcomes,but evidence is limited.This study evaluated the effect of a structured nursing protocol on RA outcomes.Materials and Methods:In this one-group pre-post study,30 Egyptian RA patients completed assessments before and after a 12-week nursing protocol comprising education,psychosocial support,and self-management promotion.Assessments included clinical evaluation of joint counts,erythrocyte sedimentation rate(ESR),and C-reactive protein(CRP)and patient-reported Arthritis Self-Efficacy Scale(ASES),Health Assessment Questionnaire(HAQ),Visual Analog Scale(VAS)for pain,and Hospital Anxiety and Depression Scale(HADS).Results:The study demonstrated significant improvements in both clinical-and patient-reported outcomes.Joint count decreased from 18.4±4.2 to 14.2±3.8(P<0.001),ESR from 30.1±6.8 mm/h to 25.5±6.8 mm/h(P<0.01),and CRP levels from 15.2±3.6 mg/L to 11.8±2.9 mg/L(P<0.01)postintervention.Patient-reported outcomes showed a marked increase in ASES score from 140±25 to 170±30(P<0.001)and reductions in HAQ from 1.6±0.4 to 1.3±0.3(P<0.01),VAS pain score from 7.8±1.7 to 6.2±1.2(P<0.001),and HADS anxiety and depression scores from 11±3 to 8±2(P<0.05)and 10±2 to 7±1(P<0.05),respectively.Conclusion:A structured nursing protocol significantly improved clinical disease activity,physical functioning,pain,self-efficacy,and emotional well-being in RA patients.A multifaceted nursing intervention appears beneficial for optimizing RA outcomes.

Takotsubo Cardiomyopathy Triggered by Acute Pancreatitis: A Case Report

Takotsubo cardiomyopathy is a heart condition that is widely known to be caused by stress. It presents with symptoms that are similar to a myocardial infarction even though the coronary arteries are clear. This case report details the clinical characteristics, diagnostic assessment, and management plan of a 55-year-old male patient with a past medical history of alcoholism who arrived at the emergency department with the typical symptoms of acute pancreatitis. The case demonstrates the progression of Takotsubo cardiomyopathy, which was triggered by acute pancreatitis in the context of alcoholism, and underlines the significance of early detection and management to enhance the patient’s outcomes.

Outcome of Nurses with Occupational Dermatitis

Background: Occupational dermatitis is considered as the second most common occupational disease. It accounts for 25% of all lost workdays. Several international studies reported a prevalence of occupational dermatitis in healthcare workers between 17% and 55%. This study aims to identify factors that affect the professional outcome of nurses suffering from occupational dermatitis. Methods: This was a multicenter cross-sectional study concerning nurses declared having occupational dermatitis in the central region of Tunisia. A synoptic sheet related to socio-professional and administrative data was completed. A self-administered Questionnaire going over medical and occupational characteristics was completed during a direct interview. Results: The study involved forty nurses working in four public hospitals in the center of Tunisia. Only 37 workers were included in the study. A professional reclassification was introduced among 19 workers (51% of study population). Work-station adaptation was requested in 14 cases (38%). Exposure to allergens in the workplace was eliminated in 20 cases. Two study participants were transferred to other departments (5.4%) and three people retired (8.1%). A statistically significant association was found between professional reclassification and a history of allergic manifestations (p = 0.003). Similarly, a significant association was found between professional reclassification and the allergic agent (p = 0.014). Workstation layout was significantly associated with a history of allergic manifestations (p = 0.039), the palm hand location (p = 0.04), professional eviction (p Conclusion: This study identified the main factors influencing the occupational outcome of nurses suffering from occupational dermatitis. This outcome depended on a history of atopy (especially allergic rhinitis) and sensitization to allergens (thiuram mix).

An investigation into the feasibility of a hybrid generator-photovoltaic-wind farm with variable load profile:case of headland south-west of Morocco

A hybrid system proposed by three different specifications for the equipment of a tourist lodge in the headland of south-west Morocco was sized by analysing the limits of load profile constraints,such as hour-to-hour variability(HHR),day-to-day variability(DDR)and the operating reserve rate(ROR).Based on the three-factor Doehlert matrix recommendations,the simulations employed an energy-sizing tool for hybrid renewable-energy systems.Testing was conducted with DDR at 5-30%,HHR at 10-30%and ROR at 0-20%.Under these conditions,a second-order polynomial relationship with a correlation rate of~90%was found between the net present cost(NPC)of the system,the levelized cost of electricity and the various constraint factors.The first specification,SPC(1),composed of generators and batteries,was introduced to control and validate the simulation independently of renewable energy,which showed a positive manifestation with the imposed constraints.The analysis expanded by introducing solar and wind energy resources.The SPC(2)configuration added PV modules to the SPC(1)and the SPC(3)configuration added wind turbines to SPC(2).The effect of DDR,HHR and ROR in the trials was significant by linear regression.At the same time,only DDR had a significant quadratic regression.The others,with their pairwise interactions,were insignificant.The desirability procedure made it possible to calculate the maximum limits of load profile constraint variables leading to targets of LCOE=0.41 US$/kWh and NPC=US$320080.1 of the load profile constraints:the DDR=15.47%and the HHR=26.55%at an ROR rate of 17.77%.

The Effect of Swirl Intensity on the Flow Behavior and Combustion Characteristics of a Lean Propane-Air Flame

The effect of swirl number(Sn)on the flow behavior and combustion characteristics of a lean premixed propane FlameФ=0.5 in a swirl burner configuration was numerically verified in this study.Two-dimensional numerical simulations were performed using ANSYS-Fluent software.For turbulence closure,a standard K-εturbulence model was applied.The turbulence-chemistry interaction scheme was modeled using the Finite Rate-Eddy Dissipation hybrid model(FR/EDM)with a reduced three-step reaction mechanism.The P1 radiation model was used for the flame radiation inside the combustion chamber.Four different swirl numbers were selected(0,0.72,1.05,and 1.4)corresponding to different angles(0°,39°,50°,and 57.8°).The results show that the predicted model agrees very well with the experimental data,especially with respect to the axial and radial velocity and temperature profiles.An outer recirculation zone(ORZ)is present in the combustor corner at Sn=0 and an inner recirculation zone(IRZ)appears at the combustor centerline inlet at a critical Sn=0.72.When the Sn reaches an excessive value,the IRZ moves toward the premixing tube,leading to a flame flashback.The flame structure and its length are strongly affected by changes in the Sn as well as the formation of NOx and CO at the combustor exit.

Mechanical agitation in baffled vessels with turbines plays a vital role in achieving homogeneous fluid mixing and promoting various transfer operations.Therefore,designing vessels with optimal energy efficiency and flow dynamics is essential to enhance operational performance and eliminate flow perturbations.Hence,the present research focuses on a numerical investigation of the impact of inclined slots with different angles installed at the side-wall of a cylindrical vessel equipped with a Rushton turbine.This study explores power consumption and vortex size while considering various rotation directions of the impeller with different rotation speeds.The numerical simulations are conducted for Reynolds numbers ranging from 104 to 105,using the RANS k-εturbulence model to govern the flow inside the stirred vessel,accounting for mass and momentum balances.The results have shown that the installation of slots reduces power consumption and vortex size compared to conventional vessel configu-rations.Moreover,increasing the slot angle from 0 to 32.5°further reduces energy consumption and vortex size,especially with negative rotation speeds.On the other hand,increasing the Reynolds numbers leads to a decrease in power consumption and an increase in vortex size.The present research therefore proposes a design for con-structing Rushton-turbine stirred vessels offering optimal operation,characterized by reduced energy consumption and minimized vortex size.

Model Identification and Control of Evapotranspiration for Irrigation Water Optimization

Water conservation starts from rationalizing irrigation,as it is the largest consumer of this vital source.Following the critical and urgent nature of this issue,several works have been proposed.The idea of most researchers is to develop irrigation management systems tomeet the water needs of plants with optimal use of this resource.In fact,irrigation water requirement is only the amount of water that must be applied to compensate the evapotranspiration loss.Penman-Monteith equation is the most common formula to evaluate reference evapotranspiration,but it requiresmany factors that cannot be available in many cases.This leads to a trend towards behavior model estimation.System identification with control is one of the most promising applications in this axis.The idea behind this proposal depends on three stages:First,the estimation of reference evapotranspiration(ET0)by a linear ARX model,where temperature,relative humidity,insolation duration and wind speed are used as inputs,and ET0 estimated by Penman-Monteith equation as output.The results show that the values estimated by thismethodwere in good agreement with the measured data.The second part of this paper is tomanage the quantity of water.For this purpose,two controllers are used for testing,lead-lag and PID.To adjust the first controller and optimize the choice of its parameters,Nelder-Mead algorithm is used.In the last part,a comparative study is done between the two used controllers.
