Macrofaunal Paleocommunities and Microfacies Analysis from the Upper Cretaceous in North Eastern Desert of Egypt

The Upper Cretaceous successions of Wadi Umm-Khayshar,southern Galala Plateau,North Eastern Desert,Egypt,are composed of highly diversified faunas and subdivided from base to top into the Galala(Middle-Late Cenomanian),Maghra El Hadida(Latest Cenomanian-Late Turonian),Matulla(Coniacian-Santonian),and Sudr(Campanian-Maastrichtian)*** ammonite biozones are recorded in the studied section:Neolobites vibrayeanus,Vascoceras cauvini(late Cenomanian),Vascoceras proprium,Choffaticeras segne,and Coilopoceras requienianum(Turonian)*** paleoenvironments of the Upper Cretaceous succession are interpreted based on detailed study and microfacies analysis that reflect a deposition in a homoclinal ramp platform ranging from intertidal to deep *** paleoecology of the Cenomanian-Turonian succession in the studied section is explained based on a detailed study of macrofaunal associations and sedimentary *** analysis(Q-mode cluster analysis,using the Raup-Crick Paired group method)of 1029 macrobenthic specimens,including 45 species of mollusks and echinoids yielded five macrofaunal associations(A-E)that are described and interpreted as remnants of *** nektonic elements are represented by 235 cephalopod specimens of 20 species;heatmap cluster analyses show the distribution of these specimens during the Upper Cenomanian-Turonian *** parameters(substrate consistency,rate of sedimentation,water energy,surface-water productivity,and oxygen availability)controlled the distribution of the studied *** five associations are divided into two major groups:low-stress associations(A,B,C,and D),and a highstress association(E).The low stress associations are recorded from two different habitats:(1)a high-energy,firm substrate habitat,dominated by epifaunal bivalves,large epifaunal gastropods and a regular echinoid;(2)a low-energy,soft substrate habitat dominated by infaunal bivalves and *** high stress associ

Objectives:This study examined the levels of shared leadership and missed nursing care and their relationship among nurses in ***:A cross-sectional survey was *** February to April 2024,340 nurses worked in all inpatient care units at Alexandria Main University *** Shared Leadership and Missed Nursing Care Survey was used for ***:The overall score of shared leadership was(72.62±4.30),which imitates a high perception of shared leadership among nurses;the dimension of delegation achieved the highest average mean score(3.66±0.26),followed by collaboration(3.64±0.31),the vision dimension scored the lowest mean score(3.59±0.33).The elements of missed nursing care had a total score of(46.72±5.69),and the dimension of secondary care achieved the highest average mean score(3.74±0.31).The reasons for missed nursing care had a total mean score of(22.40±1.59),and the dimension of labor resources achieved the highest average mean score(3.20±0.22).Male nurses,less than 30 years old,married,held a bachelor’s degree in nursing sciences,less than five years of experience in the nursing profession,and less than five years of experience in the current working unit had higher total scores of the shared leadership and lower total scores of missed nursing care(P<0.001).A negative correlation existed between shared leadership and two dimentisons(the elements of nursing care[r=-0.383],the reasons for missing nursing care[r=-0.047])(P<0.001).Conclusions:The study’s findings can help as a theoretical underpinning for nursing leaders to stand in an environment that reduces the incidence of missed nursing care by encouraging teamwork,responsibility,workload management,and empowerment among nurses.

昆虫体内共生菌可通过多种方式对寄主生物学及生理学产生影响,根据共生菌在昆虫体内存在的部位可将其分为内共生菌与外共生菌,它们以共生、寄生或病原菌的形式与寄主建立联系。大头金蝇Chrysoma megacephala(双翅目:丽蝇科)是一种全球范围内分布的昆虫,也是法医昆虫学中最常见的丽蝇科昆虫之一。近年来,大头金蝇幼虫被广泛应用于作物秸秆和有机废弃物的转化,其成虫可对植物进行授粉,作为授粉昆虫。为满足自身卵子发生及幼虫生长发育的营养需求,大头金蝇需在脊椎动物伤口、尸体、花瓣及粪便上取食,在这种复杂的取食环境中,其体内共生菌发挥着重要作用。大头金蝇肠道共生物种类丰富,对于维持寄主适合度必不可少,然而,其肠道共生物的多样性以及共生物对宿主代谢和繁殖的影响尚不清楚。在本研究中,我们利用16S rRNA高通量测序技术(Illumina MiSeq technology)对大头金蝇不同发育阶段相关细菌的菌群组成及多样性进行了分析;对大头金蝇幼虫肠道共生物进行了分离鉴定,筛选到几株纤维素降解菌,评估了纤维素酶活;将分离的共生菌添加到大头金蝇饲料中,研究其对寄主适合度的影响。主要结果如下:1.利用高通量测序技术研究大头金蝇肠道细菌多样性利用高通量测序技术(MiSeq technique),对大头金蝇雌成虫、卵、发育6h的初孵幼虫、发育72h的幼虫、鸡肉(大头金蝇产卵基质)共生菌菌群结构及组成进行分析。测序共获得423个OTU,其中51个OUT在各样品中均存在,测序结果共注释了 15个门,28个纲,74个目,138个科,260个属的细菌类群。其中,变形菌门、厚壁菌门、放线菌门及拟杆菌门细菌在各样品中均存在。沃尔巴克氏体Wolbachia、漫游球菌Vagococcus、葡萄球菌Staphlyococcus是样品中优势菌群,这些共生菌可能对大头金蝇卵子发生及幼虫的发育具有重要作用。2.大头金蝇肠道共生菌的分离与鉴定采用连续稀释平板计数法对大头金蝇肠道可培养共生菌进行分离,并对分离的细菌进行16s rRNA基因测序,结合NCBI-Blast工具对其进行鉴定。结果显示,大头金蝇幼虫肠道菌群多样性丰富,变形菌门、拟杆菌门和厚壁菌门细菌是大头金蝇不同发育阶段的优势菌群,变形菌门细菌在各发育阶段中含量最多。变形杆菌属细菌是一龄幼虫及三龄幼虫肠道优势菌,占比分别高达25%、30%,假单胞菌是二龄幼虫肠道优势菌(21%)。共生菌对幼虫发育及寄主生物学具有重要作用,后续对分离到的8种共生菌进行深入研究,以期利用益生菌促进大头金蝇的大量饲养。3.大头金蝇肠道内纤维素降解菌的分离及功能验证为了研究在富含纤维素的麦麸饲料中肠道细菌对幼虫存活率的影响,我们利用羧甲基纤维素钠(CMC)培养基从大头金蝇幼虫体内分离纤维素降解菌。基于细菌形态学分析及16S rRNA分子鉴定,分离到10株细菌,分别属于假单胞菌属Pseudomonas、不动杆菌属Acinetobacter、无色杆菌属Achrcmobacter和产碱菌属Alcaligenes细菌。通过测量菌落及纤维素降解圈直径对分离到的细菌纤维素降解能力进行评估。结果表明:(1)荧光假单胞菌Pseudomonas fluorescens及浅黄色假单胞菌Pseudomonas lurida在CMC培养基上长势最好,菌落直径均大于8mm;(2)对不同细菌的纤维素降解圈大小进行比较,其中浅黄色假单胞菌、解蛋白不动杆菌Acinetobacter proteolyticus、猴假单胞菌Pseudomonas simiae所产生的纤维素降解圈直径依次为19.97 mm、6.97 mm、5.88 mm,浅黄色假单胞菌的降解圈与菌落直径比值(D/d)最大(2.46);(3)猴假单胞菌纤维素酶活最高。浅黄色假单胞菌、解蛋白不动杆菌及猴假单胞菌可能具备纤维素降解能力,提高幼虫在富含纤维素培养基上的存活率。4.大头金蝇肠道益生菌对寄主适合度的影响对上述自大头金蝇肠道内分离得到的8种细菌进行富集培养,待其吸光度值(OD600)为0.5时进行饲料添加试验。试验设置三组处理:共生菌组(正常幼虫饲喂含菌饲料)、无菌组(抗生素处理后的幼虫饲喂含菌饲料)及对照组(正常幼虫饲喂常规饲料),饲喂5天后幼虫均已化蛹,随后将蛹转移至无菌沙子内待其羽化。试验记录大头金蝇不同发育阶段发育历期、幼虫存活率及蛹重。结果显示:(1)各处理组的幼虫发育历期无显著差异;(2)与无菌组及对照组相比,共生菌可促进幼虫发育,提高幼虫存活率;共生菌组幼虫存活率为73%,蛹羽化率为91%;无菌组分别为60%,83%,而对照组为70%,76%;(3)蛹的干重数据与幼虫存活率趋势相似。上述研究为益生菌在大头金蝇大规模饲养技术中的应用奠定了基础。总之,本文通过高通量测序表明沃尔巴克氏体、漫游球菌和葡萄

Comparative study of the mechanical behavior of the superior thoracic artery and abdominal arteries using the finite elements method

The human body has been the subject of thorough researches—not only from medical perspective but from a technical one as well—are very rich, amongst them we find blood circulation system comprising: the heart, the arteries and the veins. The overriding role of these researches is to explain some cardiovascular pathology and provide an aid tool for the endoprothesis positioning in blood vessels while treating them. In this study we have developed a digital pattern using the common (engineering technique of the) finite element method (FEM) to simulate the mechanical behavior of the thoracic aorta and the abdominal aorta below the kidney under blood pressure effect. This pattern calculates the displacements, the stresses (constraints) and the deformations of the two arteries’ walls enabling us to know their experimentally determined mechanical and geometric properties. This pattern could be applied to detect the aneurysm and dissection phenomena.

Assessment of nurses’workplace silence behaviour motives:A crosssectional study

Objectives:The motives that lead to nurses’workplace silence behaviour are essential to understand,as the failure to voice their concerns and maintain silence may cause various adverse outcomes,including burnout,turnover,job dissatisfaction,low productivity,and mental and moral *** study aimed to identify the level and influencing motives of the workplace silence behaviour among nurses at the National Guard Hospital in ***:A descriptive cross-sectional survey was conducted in January and February *** participants completed a questionnaire with two parts:the sociodemographic data form and the Workplace Silence Behavior Nursing Motives *** analysis was performed by descriptive statistics and the chi-square ***:The overall level for all workplace silence behavior motives is moderate(3.29±0.69),with Avoidance Motives dimension receiving the highest scores(3.62±0.75)and Organization-Oriented Motives dimension receiving the lowest(3.15±0.86).Chi-square test showed that Saudi nationality(X^(2)=0.015,P=0.019)and monthly income(X^(2)=0.045,P?0.013)are factors in high-moderate levels of silence ***:The overall level of workplace silence behaviour motives was found to be moderate,nurses have higher monthly salaries and non-Saudi nationals tend to have higher levels of workplace silence behaviour *** study results provide a reference for nursing managers to pay attention to nurses’workplace silence behavior and take corresponding intervention measures.

目的旨在调查埃及护士专业共享治理及职业动机的现状,并探讨其相关性。方法采用横断面调查法,于2022年5—8月选取埃及亚历山大大学附属医院内科、外科和ICU工作的724名护士为研究对象。采用专业护理治理指数(Index of Professional Nursing Governance,IPNG)和职业动机量表进行调查。结果本组护士IPNG总分为(109.18±22.76)分(量表得分范围50~250分,50个条目),表明护士感知专业治理水平较低。其中获取信息维度的均分最高,为(2.81±0.76)分;其次是设定目标和解决冲突的能力维度,为(2.75±0.53)分;参与委员会组织维度均分最低为(1.65±0.37)分。职业动机的总分为(69.82±9.70)分(量表得分范围25~105分),表明护士职业动机处于中等水平,其中职业洞察力维度均分最高为(3.56±0.34)分,职业适应力维度均分最低为(3.07±0.49)分。男性、年龄在30岁以下、本科学历、在ICU工作的护士IPNG和职业动机量表总分较高(P<0.001)。相关分析结果显示,护士专业共享治理得分与职业动机得分呈正相关(r=0.239,P=0.003)。结论研究结果可以为护理管理者提供理论依据,重新规划管理模式,并制订相应策略,从而为护士提供更好的职业规划。

Evaluation of Bilateral Mental Nerve Block as an Alternative to General Anesthesia for Resection of Lower Lip Tumors

Objective: Nerve blockade is commonly used in oncosurgical procedures as an alternative to general anesthesia for older patients and those with significant medical problems. We report a single tertiary oncology center experience in performing various lip resections and reconstructive techniques using this technique. Patients and methods: sixty patients with lower lip tumors were enrolled into this study with exclusion of refusing, non-compliant and critically ill patients. A solution of 4 ml 2% Lidocaine and epinephrine was put in a 5 ml syringe. Half the solution (2 ml) was injected into each side with a 23 gauge needle and after 5 minutes the surgical resection was carried out after testing for anesthesia. Results: The mean age was 68 ± 6.2 years. The mean ASA score was 3 ± 0.75. There were 4 cases who expressed painful sensation and their operations were completed with fentanyl increments. Mean hospital stay was 1 ± 0.75 days. There were three cases of wound gaping who were treated with secondary closure in an outpatient basis. One patient expressed postoperative acute ischemic heart pain that was managed with anti-ischemic measurements. Conclusion: Bilateral mental nerve block is a safe and effective alternative to general anesthesia in lower lip tumors’ resection especially in older patients and those with poor tolerance for general anesthesia provided that there is no need for cervical nodal dissection.

The Effectiveness of Intraoperative Frozen Section Analysis of Safety Margins in Breast Conserving Surgery and the Role of Surgeon in Decreasing the Rate of Positive Margins

Introduction: Nowadays the more accepted surgical option for treating early breast cancer is breast conserving surgery. The main challenge in this type of surgery is to get free safety margins without need of second surgical operation, so many breast surgeons have started to depend on intraoperative frozen sections to ensure free safety margins. Aim of work: To assess our policy that we prefer to depend on intraoperative frozen section analysis to get free safety margin in breast conserving surgery from the 1st surgery, and its oncologic outcome. Patients and Methods: This is a retrospective study conducted in Oncology Center—Mansoura University (OCMU), where the data of 219 patients with breast cancer, who were managed by breast conserving surgery with intraoperative frozen section analysis of the safety margins, was analyzed. Results: The intraoperative frozen section analysis of safety margin was negative from the start in 183 (83.6%) patients, while it was positive in 36 patients (16.4%). Intraoperative decision of margin re-excision was applied for 29 patients (13.2%) in order to reach negative margin, modified radical mastectomy was offered for 4 patients (1.8%), while nipple sparing mastectomy with immediate breast reconstruction using latissimus dorsi flap was offered for 3 patients (1.4%). The postoperative paraffin results were typical with intraoperative frozen section analysis results in 216 patients (98.6%) and different results were obtained in only 3 patients (1.4%) who were managed by modified radical mastectomy in a second operation. Only 4 patients had local recurrence (1.8%) during the period of follow-up duration which was ranged from 1 to 86 months with mean ± SD (22.3 ± 14.1). Does local vaginal estrogen after tension-free transobturator vaginal tape reduce overactive bladder symptoms in postmenopausal women? A prospective randomized, controlled study

Egypt is mainly covered by desert and only 4% of total lands is arable, mainly the alluvial plain of the Nile and its delta. In order to cope with the increasing population, the government of Egypt has been pressed into the development of Sinai Peninsula. So, it is indispensable to evaluate the potential groundwater resources for the development of the Sinai Peninsula. The key components are a conceptual model and a groundwater model with certain mathematical components. The conceptual model is a system analysis of the hydrogeological understanding of how water flows into, through and out of a groundwater system. Based on available borehole data, the study area is characterized to reach to the most representative conceptual models. Finally, a finite difference groundwater model was applied utilizing the graphic user interface GMS were used. In order to handle problems at regional scale, automated parameter estimation (PEST) was used in GMS. Moreover, recharge was parameterized using zones by defining these zones several factors were considered;for example, surface geology, density of vegetation, general land use, and LANDSAT image. However, Groundwater flow model successfully calibrated. Calibrated groundwater model helped to identify the heterogeneity in the aquifer.
