Hepatitis C virus genotypes in north eastern Algeria: A retrospective study

AIM: To determine the frequency of various hepatitis C virus (HCV) genotypes present in patients from north eastern Algeria. METHODS: This is a retrospective cross-sectional study of 435 HCV infected patients from northeast Algeria, detected in the Sadelaoud laboratory and diagnosed between January 2010 and December 2012. The patients were diagnosed with HCV infection in their local hospitals and referred to be assessed for HCV genotype before the antiviral treatment. Demographic information (sex, age and address), genotype, subtype and viral load were retrieved from the patient medical records. The serum samples were tested by the type-specific genotyping ***: The majority of the patients (82.5%) were from the central part of the examined region (P = 0.002). The mean age of the patients studied was 53.6 ± 11.5 years. HCV genotype 1 was the most frequent (88.7%), followed by genotypes 2 (8.5%), 4 (1.1%), 3 (0.9%) and 5 (0.2%). Genotype 6 was not detected in these patients. Mixed infection across the HCV subtypes was detected in twenty patients (4.6%). The genotype distribution was related to age and region. Genotype 1 was significantly less frequent in the ≥ 60 age group than in the younger age group (OR = 0.2; 95%CI: 0.1-0.5, P < 0.001). Furthermore, genotype 1 was more frequent in the central part of the examined region than elsewhere (P < 0.01). CONCLUSION: The HCV genotype (type 1b was dominant) distribution in Algeria is different from those in other northern countries of Africa.

Background: Inappropriately ovarian cancer cannot be detected until an advanced stage. Radical debulking surgery is considered the cornerstone in the management of advanced ovarian cancer pointing to complete tumor resolution. Unless optimal debulking cannot be achieved, these patients gain little benefit from surgery. Neoadjuvant chemotherapy (NACT) has been recommended as a novel therapeutic modality to a diversity of malignant tumors when the disease is not willing to optimal surgical resection at the time of diagnosis or the patient who unfit for aggressive debulking surgery. The purpose of this study is to compare survival in the patient with advanced ovarian cancer (stage III/IV) underwent primary debulking surgery followed by adjuvant chemotherapy (PDS-ACTR) to those who received neoadjuvant chemotherapy followed by interval debulking surgery (NACT-IDS). Results: Neoadjuvant chemotherapy (NACT-IDS) showed significant complete cytoreduction and decreased in surgical morbidity in comparison to primary debulking surgery (PDS-ACTR). NACT-IDS showed significant improvement in progression-free survival (P-value 0.002) and overall survival (P-value 0.03) in comparison to PDS-ACTR. Response to NACT and residual volume were the two independent prognostic factors for overall survival. Conclusion: NACT-IDS for advanced ovarian cancer (III/IV) resulted in higher frequency of complete resection with no residual tumor, less post-operative surgical morbidity and significant increase progression-free survival and overall survival. Both responses to NACT and residual tumor volume were the two independent prognostic factors for survival in ovarian cancer.

Anticoagulation in Pregnant Women with Mechanical Heart Valve Prostheses: 25-Year Experience at a Tertiary Care Hospital in a Developing Country

The aim of this retrospective study was to compare the different anticoagulation regimens used in pregnant women with prosthetic heart valves. We reviewed 86 pregnancies in 57 women from 1987 to 2011. The patients were divided into two groups: group A (39 pregnancies) had oral acenocoumarol throughout pregnancy;and in group B (47 pregnancies), acenocoumarol was replaced by subcutaneous heparin during the first trimester. Both groups received heparin at the time of delivery. The valves replaced were mitral (59.65%), aortic (12.28%), or both (28.07%). 74 pregnancies (86.04%) resulted in live births, 9 (10.46%) had stillbirths, 1 (1.16%) had spontaneous abortion and 2 (2.32%) underwent therapeutic abortions. The live birth rate was higher in women on heparin (87.23%) compared with those on acenocoumarol (84.61%). No malformations appeared in the 74 newborns, except for one case of hydrocephalus. There was one maternal death due to acute mitral valvular thrombosis while on heparin in the first trimester. Hemorrhagic complications occurred in 13 patients in the postpartum period, 4 of whom required transfusion. No anticoagulant regimen can be said to be entirely safe for use during pregnancy as there is a degree of risk with each regimen. Heparin does not offer a clear advantage over oral anticoagulation in the pregnancy outcome.

Objective:The aim of the study was to report the experience of National Cancer Institute(NCI),Cairo University,Egypt,in managing various benign and malignant renal tumors with nephron sparing surgery(NSS),and to assess its safety and ***:Reviewing the literature for NSS,and records of patients who underwent NSS in the period from January 2000 to December 2009 at National Cancer Institute,Cairo University regarding the patient and tumor related characteristics,the indication for NSS,operative technique,postoperative complications,full histopathological data,and follow up ***:The total number of patients was *** age at surgery was 30.7 years(range 1.5-65 years).Five patients had bilateral tumors during *** mean tumor size was 4.5 cm(range 1-9 cm).All patients had normal preoperative kidney *** patients had an absolute indication for NSS,6 patients had a relative indication,and 4 patients had an elective *** the 5 patients with bilateral tumors underwent bilateral simultaneous *** ischemia was used in 8 patients,1 patient was exposed to warm ischemia,manual compression was used in 2 patients,and no vascular control was applied in 6 *** were encountered in 2 patients,one of them had urinary leakage which needed reoperation,and the other had subcutaneous hematoma which was treated *** analysis revealed Wilm's tumor(8 patients),angiomyolipoma(4 patients),renal cell carcinoma(4 patients),and hydatid cyst(1 patient).All patients had negative surgical *** patients with Wilm's tumor,the mean follow up was 21.4 months(range 0-94 months),2 patients had local recurrence,and 1 patient had distant *** patients with RCC,the mean follow up was 15.3 months(5-33 months),no patients had local recurrence or distant *** patients had normal kidney functions during postoperative and follow up ***:NSS is a feasible safe procedure

Spontaneous cholecystocutaneous fistula is a rare complication of chronic calculous cholecystitis, because currently gallstones are diagnosed and treated at an early stage. This occurrence is possible even if it seems actually to be rare. We report a case of spontaneous cholecystocutaneous fistula of Anterior Abdominal Wall with subcutaneous gallstones admitted in the medical oncology unit of HASSAN II University Hospital. An 64-year-old woman, presented with a large subcutaneous abscess in the right subcostal area with swelling. An abdominal computed tomography scan showed a subcutaneous gallstone and communication between the abscess and the gallbladder. Cholecystectomy was performed and the abdominal wall abscess was drained externally. This case report demonstrates that maintaining a high degree of suspicion of this rare entity is helpful in achieving correct preoperative diagnosis, and that computed tomography scan should be performed in all cases of unexplained abdominal wall suppuration.

Split-dose menthol-enhanced PEG vs PEG-ascorbic acid for colonoscopy preparation

AIM:To compare the efficacy and palatability of 4L polyethylene glycol electrolyte(PEG)plus sugar-free menthol candy(PEG+M)vs reduced-volume 2 L ascorbic acid-supplemented PEG(Asc PEG).METHODS:In a randomized controlled trial setting,ambulatory patients scheduled for elective colonoscopy were prospectively *** were randomized to receive either PEG+M or Asc PEG,both splitdosed with minimal dietary *** was assessed on a linear scale of 1 to 5(1=disgusting;5=tasty).Quality of preparation was scored by assignment-blinded endoscopists using the modified Aronchick and Ottawa *** main outcomes were the palatability and efficacy of the *** outcomes included patient willingness to retake the same preparation again in the future and completion of the prescribed ***:Overall,200 patients were enrolled(100patients per arm).PEG+M was more palatable than Asc PEG(76%vs 62%,P=0.03).Completing the preparation was not different between study groups(91%PEG+M vs 86%Asc PEG,P=0.38)but more patients were willing to retake PEG+M(54%vs 40%respectively,P=0.047).There was no significant difference between PEG+M vs Asc PEG in adequate cleansing on both the modified Aronchick(82%vs77%,P=0.31)and the Ottawa scale(85%vs 74%,P=0.054).However,PEG+M was superior in the left colon on the Ottawa subsegmental score(score0-2:94%for PEG+M vs 81%for Asc PEG,P=0.005)and received significantly more excellent ratings than Asc PEG on the modified Aronchick scale(61%vs 43%,P=0.009).Both preparations performed less well in afternoon vs morning examinations(inadequate:29%vs 15.2%,P=0.02).CONCLUSION:4 L PEG plus menthol has better palatability and acceptability than 2 L ascorbic acidPEG and is associated with a higher rate of excellentpreparations;*** identifier:NCT01788709.

The Mediterranean Sea is home to an interesting biodiversity. The current cumulative and descriptive study aims to enumerate some relatively large marine mammals, reptiles, and fishes that have been spotted, caught, by-caught, or stranded on the coast of the Gaza Strip, Palestine, which extends about 42 km along the Mediterranean Sea. This 20-year study from 2003 to 2022 relied much on frequent field visits, meetings, and discussions with stakeholders, following local media sites and social media pages, and photography. The study recorded at least 26 giants or relatively large marine organisms on the Mediterranean coast of the Gaza Strip, Palestine. Certainly, the coming years may bring other marine organisms of relatively large sizes. Marine mammals included three species of cetaceans with the Fin Whale (Balaenoptera physalus) being the largest mammal and even the largest animal ever recorded in this study. Marine reptiles included three species of sea turtles, the largest of which is the Leatherback Sea Turtle (Dermochelys coriacea), which is in fact the world’s largest sea turtle. The bony fishes were represented by ten species, the largest of which was the Ocean Sunfish (Mola mola), which is the largest bony fish in the world. The cartilaginous fishes included ten species with the largest specimens encountered were the Shortfin Shark (Isurus oxyrinchus), Bluntnose Sixgill Shark (Hexanchus griseus), Scalloped Hammerhead Shark (Sphyrna lewini), and Giant Devil Ray (Mobula mobular). Seabirds were not included in the study. The Alexandria Pompano (Alectis alexandrinus) and the Silver-Cheeked Toadfish (Lagocephalus sceleratus), which are bony fishes, appear to be the relatively smallest marine organisms recorded here. In conclusion, the role of the various parties must be coordinated to ensure the sustainability of human activities and their compatibility with the task of conserving local marine biota, including the gigantic or relatively large ones.

Endoscopic variceal ligation has emerged as a superior alternative to endoscop ic injection sclerotherapy, however, the “single-shot" mechanism of the genera lly used Stiegman-Goff ligator made the procedure tedious and time-consuming a nd required overtube placement, associated with discomfort and potentially life -threatening complications. In this study we describe our experience with the S aeed Six-Shooter (multiple-ligation device). Fifty con- secutive patients with variceal bleeding were prospectively studied. After ini tial endoscopic ligation, subsequent sessions were every 2 weeks. Study outcomes were: the ability to control active bleeding, the frequencies of rebleeding, th e number of treatment sessions and time required for irradication, the percentag e eradication of varices, complications, and mortality. Active bleeding was cont rolled in all eight (100%) patients. Four (8%) patients rebled, three from eso phageal varices, and one from portal hypertensive gastropathy. Esophageal varice s were eradicated in 47 (94%) patients (3.1 ±1.3 sessions). Time needed till e radication was 6.2 ±1.9 weeks. Chest pain was reported in two (4%), low, grade pyrexia in two (4%), and pneumonia in one (2%) patient. There were three deat hs, none due to *** Six-Shooter is a safe and efficient device f or the endoscopic ligation of esophageal varices which has overcome the limitati ons of the single-shot ligator: (1) Visualization is better (the endoscopic “t unnel vision" and internal light reflection from the stainless-steel banding cy linder of the single-shot device are avoided); and (2) the use of an overtube i s no longer necessary and serious complications can be avoided.

Obesity is globally viewed as chronic relapsing *** surgery offers the most efficient and durable weight loss ***,weight regain after surgery is a distressing issue as obesity can *** procedures were originally designed to reduce food intake and catalyze weight loss,provided that its role is marginalized in long-term weight ***,it is essential to establish a scientifically standardized applicable definitions for weight regain,which necessitates enhanced comprehension of the clinical situation,as well as have realistic expectations concerning weight ***,several factors are proposed to influence weight regain as psychological,behavioral factors,hormonal,metabolic,anatomical lapses,as well as genetic ***,there is a growing evidence of utilization of scoring system to anticipate excess body weight loss,along with characterizing certain biomarkers that identify subjects at risk of suboptimal weight loss after ***,personalized counseling is warranted to help select bariatric procedure,reinforce self-monitoring skills,motivate patient,encourage mindful eating practices,to avoid recidivism.
