Medical Expulsive Therapy (MET) for Large Distal Ureteral Stones: A Prospective Study Comparing Three Drugs

Introduction: One of the most common disorders of the urinary tract is Urolithiasis. Twenty percent of lithiasis are located in the ureter of which 68% are seen in the distal ureter. The concept of medical expulsive therapy (MET) has been developed with enough knowledge of the ureter physiology in order to make easier the spontaneous expulsion of the stone. The aim of this study was to evaluate the efficacy and safety of three different drugs for the treatment of pelvic ureteral stones. Materiel and Methods: Between October 2017 and November 2018, 90 adult patients presenting with low or non-obstructive pelvic ureteral stones sized 8 to 10 mm were included. They were prospectively randomized, using computer-based randomization charts, into three equal groups: treatment with ketoprofen 100 mg once daily (Group I), silodosin 8 mg once daily (Group II) and tadalafil 5 mg once daily (Group III). The aim was to compare spontaneous expulsion of stone between those drugs Results: The mean expulsion time from the start of MET was 11.5 ± 3.27 days for ketoprofen group, 10.71 ± 3.98 days for silodosin group and 10.57 ± 3.40 days for tadalafil group. But these differences were also not significant (P = 0.79). The use of analgesics (grade II) was higher in groups II and III compared to group I, but without significant difference (23.33% in group I, 33.33% in group II and 40% in group III, p = 0.38). Discussion: The overall chance of spontaneous passage is low when the stone diameter is sized more than 7 mm. A wide range of spontaneous passage rates have been reported in the literature, varying from 71% to 98% for distal ureteral stones less than 5 mm and 25% - 53% for stone sized 5 to 10 mm with a mean expulsion time of more than 10 days. Conclusion: The three drugs have a low expulsion rate for 8, 9 and 10 mm pelvic ureteral stones with a higher adverse event rate for the NSAID group.

Hydrological Modeling of Aguibat Ezziar Watershed (Morocco), Comparative Study of Two Different Hydrological Models

This study aims to compare the performance of two hydrological models, conceptual (HEC-HMS) and spatial (ATHYS) on the Aguibat Ezziar watershed. The comparative analysis is based on the performances of simulation in terms of Nash-Sutcliffe and RSR. The study requires the collection of a series of data as inputs models namely rainfall data, water quantity, soil occupation, DTM and requires also a calibration in order to evaluate these models in validation phase. The simulation results were obtained from the validation phase aiming to replicate the operation of watershed Aguibat Ezziar, and present a suitable adjustment perspective of the observed hydrograph. These results show that the objective is achieved and a model distributed like ATHYS plays an effective role to improve the efficiency and present a high advantage in anticipation of runoff volume comparing with other models.

Biocontamination Risk of <i>Staphylococcus aureus</i>in Dairy Manufacturing Presence of Two Types of UHT Milk

The biofilm formation on equipment surfaces in dairy manufacturing is a major concern for industry and consumers alike, which may affect the safety and quality of dairy products. In order to identify the bio-contamination risk of materials commonly used in dairy manufacturing, adhesion of Staphylococcus aureus on glass coated by two kinds of UHT milk (whole milk and skimmed milk) was investigated. It is known that adhesion is mainly governed by surface physicochemical properties, for that, the effect of milk components on physicochemical properties of glass and bacterial surfaces were exanimated through contact angle measurements. MATLAB software was used to evaluate the ability of S. aureus adhesion on glass. The hydrophobic quantitative and electron acceptor characteristics of the glass appear to increase with the presence of fat in milk, while its electron donor property decreases with this component. Water management in morocco faces significant challenges in both levels, quality and quantity, in order to address these challenges and to secure the rational and efficient management of water resources, it is necessary to try to understand the different issues involved in the water cycle works! The large number of hydrological studies and techniques used at the Sebou watershed are limited to water quality, and the use of the simpler empirical models, that, do not permit to take into consideration all the factors affecting flow and cycle of water. In the purpose of using the hydrological model SWAT to study the hydrological functioning of the Sebou watershed (Morocco) which faces many problems regarding the management of its water resources like all the other watersheds in our country, this study focuses on the creation, organization and generation of input data (soil, soil occupation, climate…) using a GIS (ARCGIS) then, the configuration and calibration of the model and after that, the evaluation of the results. The main reason behind this work is to measure the adaptability of this model to the selected area in order to generalize the model to the whole Sebou watershed. The results are satisfying;they show that this model can represent the overall hydrological regime of the pilot basin Bab-Merzouka. This model can open up new perspectives regarding the effects of climate change, and the changes caused by humans on water resources, in terms of both quantity and quality.

Jejunojejunal Intussusception following Jejunostomy

Intussusception is an invagination of a segment of the gastrointestinal tract into an adjacent one. Jejunojejunal intussusception is a rare complication of jejunostomy tube placement with an incidence of 1%. We are reporting a case of 35-year-old man who was suffering from severe oral, lesions due to Ingestion of acide agents. He received Witzel jejunostomy for early feeding. Ileus developed postoperatively and plain X-ray of the abdomen showed distended small bowel loop. Abdominal computed tomography revealed target sign as well as the feeding tube in a dilated jejunum and intussusception was diagnosed. Exploratory laparotomy was required due to failure of expectant therapy. Reduction of intussusception was done during exploratory laparotomy. The jejunostomy feeding was continued and the postoperative course was uneventful.

Exploration and Mapping of Metalliferous Black Shales in the Saghro Temporary Subsidence Zones (Anti-Atlas, Morocco)

The Saghro massif constitutes a vast metallogenic province with numerous deposits and shows of base metals (lead, zinc, copper) and precious metals (gold and silver), besides various useful substances (talc, pyrophyllite, barite, fluorite). Silver/lead occurrences are concentrated along the Cryogenian Imiter series and moderately at Boumalne and Sidi Flah. Copper occupies the plutonic intrusions and intrusive rocks of the East-Central Saghro while barite deposits are widespread throughout the Cambrian cover of the East Saghro in contact with the Ediacaran basement. To justify this distribution, the new contributions of the cartography and the organic geochemistry of the black shales of Jbel Saghro have clearly shown the particularity of the Imiter black shales in terms of the richness in organic matter (TOC = 0.18%), the blackish color and the friability. The Boumalne and Sidi Flah groups present some similarities with the Imiter group, such as the sub-equatorial structuring, the friable pelites and the richness in organic matter (Boumalne TOC = 0.11% and SidiFlah TOC = 0.16%), which is a quite good show that requires to reinforce the exploration works. For Western Saghro in the Iknioun and Qalaa’t M’Gouna groups, the variations in the thickness of the volcanic cover show an irregular paleotopography with hard, greenish, organic-poor pelitic sediments (TOC = 0.01 to 0.04%). We can conclude that the formation of Imiter-type silver concentrations requires the combination of the sedimentological, the volcanic and structural factors. For Imiter-type silver these factors are: a fine pelitic and argillic casing deposited in a confined environment, a basic volcanism source of metals and other intermediate to acid generated by the hydrothermalism and heat, a convenable paleotopography and a network of fracturations to trap the mineralizations.

Impact of cytochrome P450 2C19*2 polymorphism on the clinical cardiovascular events after stent implantation in patients receiving clopidogrel of a southern Tunisian region

Introduction: The concept of clopidogrel resistance, first described in biology is being strengthened by recent data from clinical epidemiology. The cardiologists have been sensitized to this concept because of its possible involvement in the occurrence of coronary stent thrombosis. Purpose of the study: The purpose of this study was to investigate the genetic variant of the gene CYP 2C19 inour population and to assess the involvement of this genetic profile in the occurrence of major cardiovascular events (MACE) during the follow-up period. Methods: Our prospective study was conducted between May 2009 and September 2010 including 100 patients admitted to the cardiology department for percutaneous coronary stenting. The patients were divided into 2 groups: those with at least one CYP2C19*2 allele (*2 carriers) and non-carriers. Results: The mean age of our patients was 56.7 years ± 10, 5. No remarkable differences in the baseline characteristics were noted between the two groups. The prevalence of CYP2C19*2 allele in our population was 11.5%. Hospital mortality was estimated at 3%. No statistically significant differences were noted between the two groups regarding the occurrence of intra hospital MACE. The mean follow up was 7.5 ± 4.87 months for the entire study population. The rate of MACE during the follow-up of patients receiving clopidogrel was 8.2% throughout the study population: 5.3% in the *2 non-carriers versus 18.2% in the *2 carriers with a statistically significant difference (p = 0.075) at the risk of error of 10%. Concerning the occurrence of stent thrombosis, there was no significant statistical difference between the two study groups. Conclusion: From these results it is suggested that CYP2C19*2 polymorphism is associated with increase in the occurrence of MACE among Tunisian patients receiving clopidogrel. A larger study is needed to assess the role of genotyping in the evaluation of the phenomenon of clopidogrel resistance.

Let F, N、A and N2 denote the properties of being finite, nilpotent, abelian and nilpotent of classes at most 2, respectively. Firstly we consider the class of finitely generated FTV-groups. Wc show that the property FC is closed under finite extensions, and extend this result to finitely generated NF-groups. Secondly we prove that a finitely generated NF-group G is in the class ((FC)F, oo) if and only if G is an FA-group. Finally we prove that a finitely genera ted ATF-group in the class ((FC)F,∞)^* is an FM-group. Moreover, G/Z2(G) is finite.
