Objective:To determine the prevalent pathogens responsible for spontaneous bacterial peritonitis(SBP) and their sensitivity pattern,to test the efficiency of different culture techniques in microbial isolation,and to study the diagnostic predictors of such ***:One hundred eight SBP episodes from 92 adult patients were compared to 88 cirrhotic ascites patients cross - matched with age and sex without *** fluid was subjected to cytological,biochemical examination and culture on both conventional and blood culture bottles at the bedside for bacterial identification and antimicrobial susceptibility ***:The prevalence of SBP was 25.02%.Logistic regression analysis revealed that;previous SBP episode,low ascitic fluid protein levels, high serum creatinine and low serum albumin levels were the independent significant predictors of *** forty - five per cent of SBP episodes were detected by conventional culture compared to 73.15%by modified technique with a significant *** - negative bacteria were the cause of SBP in 46(58.23%) culture positive *** coli and Staphylococcus aureus were the most commonly detected organisms. BACKGROUND Diffusion-weighted magnetic resonance imaging has shown promise in the detection and quantification of hepatic fibrosis. In addition, the liver has numerous endogenous micro-RNAs(miRs) that play important roles in the regulation of biological processes such as cell proliferation and hepatic *** To assess diffusion-weighted magnetic resonance imaging and miRs in diagnosing and staging hepatic fibrosis in patients with chronic hepatitis *** This prospective study included 208 patients and 82 age-and sex-matched controls who underwent diffusion-weighted magnetic resonance imaging of the abdomen, miR profiling, and liver biopsy. Pathological scoring was classified according to the METAVIR scoring system. The apparent diffusion coefficient (ADC) and miR were calculated and correlated with pathological *** The ADC value decreased significantly with the progression of fibrosis, from controls(F0) to patients with early fibrosis(F1 and F2) to those with late fibrosis(F3 and F4)(median 1.92, 1.53, and 1.25 × 10^(-3) mm^2/s, respectively)(P = 0.001).The cut-off ADC value used to differentiate patients from controls was 1.83 × 10^(-3) mm^2/s with an area under the curve(AUC) of 0.992. Combining ADC and miR-200 b revealed the highest AUC(0.995) for differentiating patients from controls with an accuracy of 96.9%. The cut-off ADC used to differentiate early fibrosis from late fibrosis was 1.54 × 10^(-3) mm^2/s with an AUC of 0.866. The combination of ADC and miR-200 b revealed the best AUC(0.925) for differentiating early fibrosis from late fibrosis with an accuracy of 80.2%. The ADC correlated with miR-200 b(r =-0.61, P = 0.001), miR-21(r =-0.62, P = 0.001), and miR-29(r = 0.52,P = 0.001).CONCLUSION Combining ADC and miRs offers an alternative surrogate non-invasive diagnostic tool for diagnosing and staging hepatic fibrosis in patients with chronic hepatitis C.

Objectives:To assess the knowledge,attitude,and practice(KAP)towards COVID-19 and its associated factors among house ***:A multicentered crosssectional study on house officers was carried out using an online self-administered *** data and data related to KAP towards COVID-19 were ***,KAP level of the house offices was recorded and factors associated with good KAP were ***:This study included 555 house officers from 10 different hospitals with 128 males and 427 *** half of the house officers had good knowledge and a favorable attitude(46.1%and 51%,respectively)on ***,82.5%of them were committed to good practice of preventive ***,percentages of good knowledge and practice among those with chronic diseases or those who knew someone infected with SARS-CoV-2 were higher,but the differences were not statistically significant(P>0.05).In addition,the percentages of females with good practice were higher than those of males,with no statistical difference(P>0.05).Conclusion:House officers showed a good KAP towards *** aware of gaps in KAP can help to effectively contain the current pandemic.

Combining Committee-Based Semi-Supervised Learning and Active Learning

Many data mining applications have a large amount of data but labeling data is usually difficult, expensive, or time consuming, as it requires human experts for annotation. Semi-supervised learning addresses this problem by using unlabeled data together with labeled data in the training process. Co-Training is a popular semi-supervised learning algorithm that has the assumptions that each example is represented by multiple sets of features （views） and these views are sufficient for learning and independent given the class. However, these assumptions axe strong and are not satisfied in many real-world domains. In this paper, a single-view variant of Co-Training, called Co-Training by Committee （CoBC） is proposed, in which an ensemble of diverse classifiers is used instead of redundant and independent views. We introduce a new labeling confidence measure for unlabeled examples based on estimating the local accuracy of the committee members on its neighborhood. Then we introduce two new learning algorithms, QBC-then-CoBC and QBC-with-CoBC, which combine the merits of committee-based semi-supervised learning and active learning. The random subspace method is applied on both C4.5 decision trees and 1-nearest neighbor classifiers to construct the diverse ensembles used for semi-supervised learning and active learning. Experiments show that these two combinations can outperform other non committee-based ones.

Background:Nasal continuous positive airway pressure(NCPAP)is frequently used in preterm ***,there is no consensus on when and how to wean them from *** sources:Based on recent publications,we have reviewed the criteria of readiness-to-wean and factors affecting weaning success.A special focus is placed on the methods of weaning from NCPAP in preterm ***:Practical points of when and how to wean from NCPAP in preterm infants are *** infants are ready to be weaned from NCPAP when they are stable on a low NCPAP pressure with no(or minimal)oxygen *** used to wean from NCPAP include:sudden weaning of NCPAP,gradual decrease of NCPAP pressure,graded-time-off NCPAP(cycling),weaning to high or low flow nasal cannula,and a combination of these *** best strategy for weaning is yet to be ***-off NCPAP increases the duration of NCPAP and length of hospital stay without beneficial effect on success of *** decrease of NCPAP pressure is more physiological and better tolerated than cycling-off ***:Further studies are needed to reach a consensus regarding the optimal timing and the best method for weaning from NCPAP in preterm infants.

Novel CYP1B1 mutations and a possible prognostic use for surgical management of congenital glaucoma

AIM: To identify CYP1B1 gene mutations and evaluate their possible role as a prognostic factor for success rates in the surgical management of Egyptian congenital glaucoma ***: Totally 42 eyes of 29 primary congenital glaucoma patients were operated on with combined trabeculotomy/trabeculectomy with mitomycin-C and followed up at 1 d, 1 wk, 1, 6 and 12 mo postoperatively. Genomic DNA was extracted from peripheral blood leukocytes. Coding regions of CYP1B1 gene were amplified using 13 pairs of primers, screened for mutations using single-strand conformation polymorphism followed by sequencing of both strands. Efficacy of the operation was graded as either a success [maintaining intraocular pressure(IOP) less than 21 mm Hg with or without antiglaucoma medication], or a failure(IOP more than 21 mm Hg with topical antiglaucoma medications). RESULTS: Seven novel mutations out of a total of 15 different mutations were found in the CYP1B1 genes of 14 patients(48.2%). The presence of CYP1B1 gene mutations did not correlate with the failure of the surgery(P=0.156, odds ratio=3.611, 95%CI, 0.56 to 22.89); while the positive consanguinity strongly correlated with failure of the initial procedure(P=0.016, odds ratio=11.25, 95%CI, 1.57 to 80.30). However, the Kaplan-Meier survival analysis revealed a significantly lower time of IOP control in the subgroup with mutations in CYP1B1 versus the congenital primary glaucoma group without mutations(log rank test, P=0.015).CONCLUSION: Seven new CYP1B1 mutations are identified in Egyptian patients. Patients harboring confirmed mutations suffered from early failure of the initial surgery. CYP1B1 mutations could be considered as a prognostic factor for surgery in primary congenital glaucoma.
