There are several reports on the prevalence and importance of milk allergens in the induction of allergic diseases, whereas its role in the induction of autoimmune disease was rarely studied. So, the present work aimed to study the diagnosis and the efficacy of oral antigen specific immunotherapy (OAIT) on two groups of autoimmune disease patients namely, rheumatic arthritis (RA), group II, and systemic lupus erythematosus (SLE), group III, who has allergens for milk as an effective treatment option. From the assessment of the data obtained and the clinical outcome of RA and SLE patients after food elimination strategy and milk immunotherapy, it was evident that there were significant reductions (P < 0.001) in the levels of total IgE, specific IgE, Rheumatic factor (R.F), C-reactive protein (C.R.P), Anti-nuclear antibodies (ANA) and anti-double stranded DNA antibodies (Anti-ds DNA) compared to pretreated levels. Moreover, the immunotherapy induced “blocking antibodies” by remarkable highly significant increasing in IgG, phagocytic inhibition test (PIT %), Complement component 3(C3), and Complement component 4 (C4) levels. More improvement was noticed in the SLE as a result of the immunotherapy. Conclusion: milk desensitization is a gained interest and a safe valuable effective treatment of autoimmune conditions in Egypt.

Objective: There are clinical and laboratory associations between thyroid and liver diseases. Hepatitis C virus (HCV) is known to be responsible for both hepatic and extrahepatic diseases. The most frequent and clinically important endocrine extrahepatic diseases are thyroid disorders and type 2 diabetes mellitus. We aim to study the relationship between the serum level of thyroid hormones (THs) and the severity of liver disease in patients with chronic hepatitis C virus (CHC) infection. Methods: 60 patients with CHC infection were selected for the study. They were divided into two groups: with or without liver cirrhosis. Those with liver cirrhosis were further subdivided according to the Child-Turcotte-Pugh scoring system. Serum levels of free T3 (FT3), free T4 (FT4) and TSH were measured to all patients. Results: There was decrease in the FT3 and FT4 levels and increase in the TSH levels in patients with CHC with cirrhosis when compared to patients with CHC without cirrhosis. Conclusion: Thyroid profile abnormalities were seen in cirrhotic HCV patients when compared to non-cirrhotic patients. The abnormalities in the serum level of THs (decreased FT3, FT4, and increased TSH) are strongly associated with the severity of liver damage and advancing of the child score.

This work represents the extraction of oil with high free fatty acid content from spent bleaching earth using sub-critical water technology as a greener production pathway. The extraction efficiencies under different conditions were investigated. The studied parameters include temperatures in the range of 180 to 270°C, the feed to solventfeed-to-solvent (in this case water) ratio (1:1, 1:2, 1:3, 1:4 and 1:5) and extraction times in the range of 5-60 minutes. The results showed that the optimum temperature, feed to solventfeed-to-solvent ratio, and extraction time were 270°C, 1:3, and 20 minutes, respectively. In another experiment, the extracted free fatty acids were converted into mono-, di-, and triglycerides through esterification with glycerol to increase the value added of the extracted products. The kinetics of the extraction process was found to be corresponding to an irreversible consecutive unimolecular-type first order reaction, consisting of the extraction step followed by the decomposition reaction step. Both reaction rates of extraction and decomposition were estimated using the reaction rate equations utilizing the nonlinear regression method. The apparent activation energy was calculated to be 46.1 kJ·mol-1. This result indicates a diffusion controlled reaction. For more exploration and deep understanding of the extraction mechanism, other thermodynamic parameters were also calculated and analyzed including,ΔH#, ΔS#, and ΔG# of the extraction step.

AIM To investigate the clinical utility of serum annexin A2(ANXA2) as a diagnostic marker for early hepatocellular carcinoma(HCC).METHODS This study was performed in HCC Clinic of Ain Shams University Hospitals, Cairo, Egypt and included: Group 1: Fifty patients with early stage HCC(Barcelona Clinic Liver Cancer stage A); Group 2: Twenty five patients with chronic liver disease; and Control Group: Fifteen healthy, age-and sex-matched subjects who were seronegative for viral hepatitis markers. The followinglaboratory investigations were done: Viral hepatitis markers [hepatitis B surface antigen and hepatitis C virus(HCV) antibodies], HCV RNA in HCV antibody-positive patients, serum alpha fetoprotein(AFP), and serum ANXA2 *** In this study, 88% of HCC patients(n = 44) were HCVpositive, while HBV infection represented only 8% of all HCC patients(n = 4); and two patients were negative for both viral markers. A highly significant difference was found between patients with HCC and chronic liver disease as well as controls with regard to serum ANXA2 levels(130, IQR 15-240; 15, IQR 15-17; and 17, IQR 15-30 ng/m L, respectively). The area under the curve of ANXA2 was 0.865; the cut-off value was established to be 18 ng/mL with a diagnostic sensitivity of 74% and a specificity of 88%, while the sensitivity and specificity of AFP at the cut-off value of 200 ng/dL were 20% and 100%, *** Serum ANXA2 may serve as a biomarker for the early detection of HCC.

A Reconfigurable UWB Bandpass Filters with Embedded Multi-Mode Resonators

The two proposed filters described here satisfy the Federal Communications Commission Ultra-wideband (FCC-UWB) specifications and also control the center frequency and bandwidth of the filters passband. These filters consist of two distinguishing parts, Electromagnetic bandgap (EBG)-embedded multiple- mode resonator (MMR) and interdigital coupled lines to realize high performance in the operation band with a compact size of 14.0 mm × 10.1 mm. The main advantage of the two proposed filters is that three different bands are tuned. The 1st tuned band is from 3.5 GHz to 11.4 GHz for the first filter and from 3.1 GHz to 11.6 GHz for the second proposed filter, respectively. The 2nd tuned band is from 3.5 GHz to 7.5 GHz for the first filter and from 3.1 GHz to 7.8 GHz for the second proposed filter, respectively. While the 3rd tuned band of the first proposed filter is from 3.5 GHz to 5.9 GHz and from 3.1 GHz to 5.8 GHz for the second proposed filter. The bandwidth of the filters can be changed by increasing the length of the outer open circuited stubs which are controlled by using switching matrix equipment (mini circuit, replacement of PIN diodes). To validate the design theory, a reconfigurable UWB bandpass filters (BPFs) with EBG Embedded MMR are designed, fabricated and measured. Good agreement is found between simulated and measured results.

AIMTo investigate the diagnostic ability of a non-invasive biological marker to predict liver fibrosis in hepatitis C genotype 4 patients with high accuracy. METHODSA cohort of 332 patients infected with hepatitis C genotype 4 was included in this cross-sectional study. Fasting plasma glucose, insulin, C-peptide, and angiotensin-converting enzyme serum levels were measured. Insulin resistance was mathematically calculated using the homeostasis model of insulin resistance (HOMA-IR). RESULTSFibrosis stages were distributed based on Metavir score as follows: F0 = 43, F1 = 136, F2 = 64, F3 = 45 and F4 = 44. Statistical analysis relied upon reclassification of fibrosis stages into mild fibrosis (F0-F) = 179, moderate fibrosis (F2) = 64, and advanced fibrosis (F3-F4) = 89. Univariate analysis indicated that age, log aspartate amino transaminase, log HOMA-IR and log platelet count were independent predictors of liver fibrosis stage (P CONCLUSIONOur predictive model is easily available and reproducible, and predicted liver fibrosis with acceptable accuracy.

Objective: The purpose of this study to determine the prevalence of both pre-hypertension and hypertension and risk factors associated with newly diagnosed Saudi military active duty service ***: Community-based screening of 1283 Saudi military active duty service personnel was conducted during the period September-December 2007 at six randomly selected out of a total of 15 military units of Taif region,Western of Saudi *** tools included self-administrated questionnaire,general physical examination,anthropometric measurements and assessment of blood ***: All participants were Saudi *** age ranged from 19-56 years old with mean ± SD of 37.2 ± *** applying the Joint National Committee on prevention,detection,evaluation,and treatment of high hypertension(JNC-7) criteria,214(17.3%) were considered *** logistic regression analysis showed that obesity as measured by body mass index(OR=2.71 CI: 1.39-5.28),positive family history(OR=1.46 CI:1.03-2.06),ever smoking(OR=1.45 CI:1.05-2.02),and increased waist circumference(OR=1.04 CI:1.02-1.06) were the significant predictors of workplace newly diagnosed hypertension among military active duty ***: Pre-hypertension is a common hidden problem and it predicts the development of frank *** of the current study support the recommendation of lifestyle modification for pre-hypertension ***,further prospective studied are required to determine the role of pharmacotherapy in pre-hypertension.

＜正＞ As a result of focusing ultrafast Ti:Sapphire laser pulses in a gas. a plasma column is created. Its density does not exceed some limiting value as a result of the defocusing by the electrons resulting from muluphoton ionization and the involvement of high order nonlinearities. The spectrum of the radiation from the excited species in the column has little contribution from the plasma continuum and the line broadening is less than that of the spectra radiated from a plasma generated by a long laser pulse. Thus. the spectroscopic source obtained using short laser pulses is an excellent source for spectroscopy, （?）2000 Published by Elsevier Science B.V.

The regeneration of pore water (PW) nutrients was investigated and the contribution of benthic nutrient fluxes to the overlying bottom water (BW) was examined. Dissolved inorganic nutrients (NO-2, NO-3, PO3-4 and SIO4-4) were measured in PW and BW in El Mex Bay and surround drains during spring 2010. Nutrient concentrations gradiance in PW with overlying seawater were evaluated according to Fick’s Law. Average inorganic nutrient fluxes were estimated using the pore water gradient concentration across the sediment-water interface. Calculated nutrient fluxes had averages of ﹣7.24, ﹣1.36, ﹣7.86 and ﹣1.33 in El Mex Bay. Additionally, the fluxes in the drains were ﹣34.39, ﹣32.28, ﹣53.20 and ﹣117.6 mg·m﹣2·day﹣1 for NO-3, NO-2, PO3-4 and SIO4-4, respectively. Mineralogical studies of sediment samples by using IR, X-ray analysis were carried out to identify the chemical structure of sediments. The results revealed that calcite, aragonite and quartz are the dominant minerals. On the other hand, differential thermal analysis (DTA) was used to evaluate and discuss different kinetic parameters such as Ea#, ΔG#, ΔH#, ΔS#, Z and Tm support the view of the extra stability of these sediments.
