AIM To assess the levels of different immune modulators in patients with hepatocellular carcinoma(HCC),in relation to other hepatic *** Eighty-eight patients were included in the current study and represented patients with HCC(20),liver cirrhosis(28) and chronic hepatitis(CH;25),and normal controls(nC;15).Peripheral blood was isolated for immunophenotyping of active myeloid dendritic cells(m DCs;CD1 c and CD40),mature inactive myeloid cells(CD1 c and HLA),active plasmacytoid cells(p DCs;CD303 and CD40),mature inactive p DCs(CD30 and HLA),active natural killer(nK) cells(CD56 and CD161),active nK cells(CD56 and CD314) and inactive nK cells(CD56 and CD158) was done by flow *** levels of interleukin(IL)-2,IL-10,IL-12,IL-1β,interferon(IFn)-α,IFn-γ and tumor necrosis factor(TnF)-αR2 were assessed by *** Active m DCs(CD1 C+/CD40+) and inactive m DCs(CD1 c+/HLA+) were significantly decreased in HCC patients in relation to nC(P n on active p DCs was decreased in HCC patients(P nd its level was not significantly changed among other *** p DCs(CD303+/HLA+),inactive nKs(CD56+/CD158+) and active nKs(CD56+/CD161+) were not statistically changed among the four groups studied;however,the latter was increased in CH(P nKG2 D was statistically decreased in HCC,CH and cirrhosis(P nd it was not expressed in 63%(12/20) of HCC *** was significant decrease of IL-2,IFn-α and IFn-γ(P nd a significant increase in IL-10,IL-1β,and TnF-αR2(P nd P n HCC *** was inverted correlation between IL-12 and IL-1β in HCC(r =-0.565,P ng correlation between p DCs(CD303+/CD40+) and nKs(CD56+/CD161+;r = 0.512,P nactive m DCs(CD1 c+/HLA+) and inactive nK cells(CD56+/CD158+;r = 0.945,P nCLUSIOn nKG2 D,CD40,IL-2 and IL-10 are important modulators in the development and progression of HCC.

Recurrent Miscarriage: Hysteroscopy-Assisted Management

Objective: To apply office hysteroscopy in assessment/management of patients with recurrent miscarriage, thus to/or not to recommend office hysteroscopy as a routine procedure in such cases. Study design: Retrospective analysis. Setting: University hospital’s outpatient abortion clinic. Subjects & Methodology: Patients’ records during the period between March 2015 and January 2017 for subjects with at least 2 previous miscarriages, who had undergone office hysteroscopy were reviewed. Results: Cases with 2 previous miscarriages (n = 95) were assessed and compared with those with 3 or more miscarriages (n = 105). Abnormal uterine findings were diagnosed in 24.1% of the former, and 43.8% of the latter group. The prevalence of uterine lesions among cases with 2 recurrent miscarriages was 42.1%;meanwhile, for subjects with 3 or more consecutive miscarriages, it was 43.8%. Conclusion: In addition to safety profile, simplicity and outpatient basis of use, outpatient hysteroscopy in recurrent miscarriages would be an added-value to practitioners as a diagnostic and therapeutic tool.

Moringa oleifera is well known as a highly nutritious plant and a water purifier in fish *** fish has many impressive qualities making them very suitable for ***,this study aimed to evaluate the effect of supplementation of Moringa oleifera aqueous extract(MOAE)at varying concentrations(0,100,200,and 400 mg/kg diet for 90 days)on growth performance,hematological,and some biochemical parameters of nile ***,histological changes and immunohistochemical expression of caspase 3 in gills and livers were *** enhancements(Pn the final weight,weight gain,specific growth rate,red blood cells,hemoglobin,packed cell volume,white blood cells,and total serum protein were observed in fish fed 200 mg MOAE/kg diet compared with other ***,a significant reduction(Pnd kidney function tests and non-significant changes in lipid profile were *** the other hand,fish fed 400 mg MOAE/kg diet revealed disturbance in the corresponding *** histological degenerative changes were detected in the gills and liver of fish fed 400 mg MOAE/kg *** comparison,fish fed 200 mg MOAE/kg diet showed the best histo-architectures of these organs compared with other *** immune expressions of caspase 3 were determined where gills and liver of fish fed 400 mg MOAE/kg diet revealed extensive number of positive caspase 3 cells indicating severe apoptotic ***,a dietary MOAE at 200 mg/kg diet is recommended to improve the growth performance,hematological parameters,liver and kidney functions in nile ***,caution must be considered when using MOAE at a concentration over 200 mg/kg diet.

Integrated Action of Mixture Rates and nitrogen Levels on Quantity and Quality of Forage Mixture from Egyptian Clover and Barley in Sandy Soil

Effect of seeding rate mixture from Egyptian clover (Trifolium alexandrinum L.) E.C. and barley (Hordum vulgare L.) B, n-fertilization levels and their interaction on quantity and quality of forage yield. Two field experiments were conducted during winter season of 2012/2013 and 2013/2014 in sandy soil at research and production station of national Research Centre, Al nubaria district, Al Behaira Governorate, Egypt. The experiment included two factors first was five mixtures from seeding rates (24 kg EC/fed. – 50 kg B/fed. – 18 kg EC + 12.5 kg B/fed. – 12kg EC + 25 kg B/fed. – 6 kg EC + 37.5 kg B/fed.) and second factor was three n levels (30-45-60 kg n/fed.). Two cuts were obtained at 60 and 100 DAS. Results indicated significant differences between mixture rates for all studied characters in both cuts 60 and 100 DAS. Significant differences between n levels for all studied characters at 60 DAS, also, forage yield /fed. and DM% at 100 DAS. It can be introducing forage mixture of 18 kg EC + 12.5 B had balanced character in forage yield/fed., DM%, carbohydrate % and protein % at both cuts 60 and 100 DAS.

Correlation between IL28B/TLR4 genetic variants and HCC development with/without DAAs treatment in chronic HCV patients

In Egypt,Sofosbuvir(SOF)in combination with Dataclasvir(DCV)is the broadly used DAAs with excellent therapeutic *** study is designed to explore the relation between IL28B/TLR4 genetic variants and each of the followings;HCC development post SOF/DCV treatment,progression to HCC in naı¨ve patients and SOF/DCV therapy outcome.A total of 493 blood samples were collected(controls(n = 70);HCV patients treated with SOF/DCV(n = 252)of whom 65 patients developed HCC,187 patients didn’t develop HCC(125 responders,62 relapsers);naı¨ve HCV patients(n = 171)had early(n = 48),late liver fibrosis(n = 21)and HCC(n = 102)).Both SnPs were genotyped using a TaqMan 50 allelic discrimination *** IL28B rs12979860 SnP,the C allele was significantly correlating with the response rate more than T allele(OR 1.9,95%CI 1.29e2.9,p=0.004),while at TLR4 rs4986791 SnP,no association was found(OR 6.5,95%0.57e75.28,p = 0.09).Both SnPs couldn’t detect the probability for HCC emergence after *** naı¨ve patients,the protective alleles were detected in their lowest frequency in HCC patients(p = 0.1,for rs12979860 and,p = 0.001 for rs4986791).SOF/DCV combination improved SVR rates in HCV genotype 4a infected patients regardless of IL28B genotype,with the best rates in those lacking the T allele.

Annexin A2 as a biomarker for hepatocellular carcinoma in Egyptian patients

AIM To investigate the clinical utility of serum annexin A2(AnXA2) as a diagnostic marker for early hepatocellular carcinoma(HCC).METHODS This study was performed in HCC Clinic of Ain Shams University Hospitals, Cairo, Egypt and included: Group 1: Fifty patients with early stage HCC(Barcelona Clinic Liver Cancer stage A); Group 2: Twenty five patients with chronic liver disease; and Control Group: Fifteen healthy, age-and sex-matched subjects who were seronegative for viral hepatitis markers. The followinglaboratory investigations were done: Viral hepatitis markers [hepatitis B surface antigen and hepatitis C virus(HCV) antibodies], HCV RnA in HCV antibody-positive patients, serum alpha fetoprotein(AFP), and serum AnXA2 *** In this study, 88% of HCC patients(n = 44) were HCVpositive, while HBV infection represented only 8% of all HCC patients(n = 4); and two patients were negative for both viral markers. A highly significant difference was found between patients with HCC and chronic liver disease as well as controls with regard to serum AnXA2 levels(130, IQR 15-240; 15, IQR 15-17; and 17, IQR 15-30 ng/m L, respectively). The area under the curve of AnXA2 was 0.865; the cut-off value was established to be 18 ng/mL with a diagnostic sensitivity of 74% and a specificity of 88%, while the sensitivity and specificity of AFP at the cut-off value of 200 ng/dL were 20% and 100%, *** Serum AnXA2 may serve as a biomarker for the early detection of HCC.

Assessment of the Hydrochemical Characteristics of the Suez Gulf Coastal Waters during 2011-2013

The coastal region of the Suez Gulf, is one of the most densely industrialized zones in Egypt. In order to assess the quality of Suez Gulf coastal waters for the sustainable use and development, and consequently for the national income, 12 field campaigns were carried out in 3 years (2011-2013). The results of beach litter cleared out that the shoreline of the Suez Gulf, especially Ras Gharib area was subjected to many factors which undoubtedly affected the rate of man-made litter accumulation. The most abundant visible items were general litter as they represented 30.64% of the total items encountered. Oil, seaweeds as well as old and new tar constituted 15.41%, 14.85% and 11.84%, respectively. The increase on the levels of water temperature, Chl-a, TSM, DO, BOD, DOM, COD, ammonium ion, nitrite, nitrate, total phosphorus, silicate and total nitrogen developed a eutrophic region. In general, dissolved inorganic nitrogen (DIn) species were almost 9 times higher in the northern parts compared to the south-ern ones revealing that the northern parts of the Gulf were more affected by human impacts compared to the southern area. Moreover, concentrations of most studied metals such as Zn, Mn, ni, Cu, Cr, Cd, Pb, and Hg were higher in northern parts com-pared to southern parts of the Gulf. The concentrations of dissolved metals in the northern and southern Suez Gulf followed the following order: Fe ≥ Zn ≥ Cu > Pb > Mn > ni > Cr > Cd ≥ Hg. Except for Hg, concentrations of metals were in the accepta-ble ranges recommended internationally. Hg concentrations especially in the northern Suez Gulf were slightly higher than the permissible levels. As the northern parts of the Gulf were more suffered from oil pollution, total petroleum hydrocarbons revealed higher concentrations in the northern parts compared to the southern parts of the Gulf. Investigation of Self Excited Torsional Vibrations of Different Configurations of Automatic Transmission Systems during Engagement

The vehicle drive line system is subjected to torsional vibration from different sources of the system such as;engine fluctuating torque, Hook’s joint and the final drive. However, the essential source is the friction torque induced in the friction elements, during their engagement. In the automatic transmission system, the planetary gear set includes several friction elements such as;clutch, band brakes, and one way clutch. During the engagement, severe torsional vibration is induced by friction which is noticeable by the passengers in the form of what so called vehicle shunt. In present paper, a torsional vibration model for Drive Line (DL) system includes three different configurations of automatic transmission is constructed. A computer program using MATLAB subroutines is implemented to obtain the system response. Effect of system parameters on the dynamic behavior and stability has been investigated. The system damping and the trend of the friction coefficient have an essential effect on the dynamic behavior and stability of the system. The system response is now predictable with change of the system parameters which opens up the opportunity in future to control the vibration level.

Relationship between peripheral arterial disease, carotid intima-media thickness and C-reactive protein in elderly diabetic patients

Purpose: To study the association between carotid intima media thickness (CIMT) and high sensitivity CRP (hs-CRP) level as markers for PAD in elderly patients with DM. Subjects: A case-control study on 90 participants aged 60 years and older divided into 60 cases (30 patients with DM alone and 30 patients with DM and comorbidities) and 30 healthy controls. All were assessed by measuring CIMT, ankle brachial index (AB), and markers for cardiovascular disease such as high-sensitivity CRP (hs-CRP), total cholesterol (TC), triglycerides (TG), high density lipoprotein (HDL), and low density lipoprotein (LDL). Results: hs-CRP levels showed statistically significant difference being highest among patients with DM and comorbidities and lowest among controls (P nificantly higher among cases than controls. ABI was able to detect PAD in many asymptomatic patients. Color changes were present in only 43.30% (n = 26) of positive PAD cases while delayed wound healing, claudication pain, rest pain, cold extremities, and trophic changes were present in 23.30% (n = 14), 16.71% (n = 10), 16.71% (n = 10), 45.00% (n = 27), and 21.7% (n = 13). Using logistic regression analysis revealed that DM, CIMT, and hs-CRP were independent predictors for PAD (OR = 4.194, 7.236, 1.003;P value = 0.044, 0.25, 0.031) after adjustment of other coronary risk factors such as sex, smoking, hypertension, TC, and TG. Conclusion: Diabetic elderly have higher prevalence of asymptomatic PAD thannon-diabetics using solely ABI. DM, CIMT, and hs-CRP are independent predictors for the occurrence of PAD. Hs-CRP levels are highest among diabetics with comorbidities.

Doppler Study of Uterine Artery and Ultrasonography of Endometrial Thickness in Perimenopausal and Postmenopausal Bleeding

Background: Transvaginal color Doppler sonography of the uterine artery has been reported useful for evaluation of abnormal uterine bleeding in peri-/post-menopausal women. It may differentiate physiological from malignant endometrial changes. Objective: Evaluating the endometrial thickness and uterine artery Doppler as an initial diagnostic tool to identify patients with abnormal endometrial pathology. Methods: This is a prospective case control study consisted of total 50 women with peri-(n = 35) and post-(n = 15) menopausal bleeding, with 20 women consisting control group (no bleeding) (peri-(n = 10) and post-menopausal (n = 10)). Double layer endometrial thickness and uterine-artery-Doppler waveforms were measured, with the latter being S/D ratio, RI and PI. These were related to histological findings (endometrial biopsy). Result: Patients with abnormal uterine bleeding (both peri- and postmenopausal) had a significantly higher mean endometrial thickness than the control women. Study group showed a significantly lower value of all Doppler indices (S/D ratio, RI and PI). In study group, patients with malignant endometrial pathology showed significantly thicker endometrium and significantly lower Doppler indices than those with benign pathology. Conclusion: The combination of endometrial thickness and uterine artery color Doppler pattern might predict uterine endometrial malignancy, although whether it can discriminate (screen) patients requiring endometrial biopsy is not evident. Further study is needed.
