Retrograde intrarenal surgery for lower pole stones utilizing stone displacement technique yields excellent results

Objective:To evaluate the long-term stone-free rate(SFR)of retrograde intrarenal surgery(RIRS)in the treatment of lower pole renal calculi using only basket relocation and identify independent predictors of stone-free ***:All consecutive patients undergoing RIRS lower pole renal calculi at a single highvolume tertiary center were analyzed *** pole stones were relocated to the upper pole,where laser lithotripsy was *** patients were followed up in the clinic following the surgery and yearly *** stone-free status was assessed with a combination of an abdominal ultrasound and abdominal X-ray,or an abdominal non-contrast computed tomography if the stones were known to be ***:A total of 480 consecutive patients who underwent RIRS for treatment of lower pole renal calculi,between January 2012 and December 2018,were analyzed from a prospectively maintained database of 3000 *** a median follow-up time of 18.6 months,the mean SFR was 94.8%.The procedures were unsuccessful in 26(5.4%)patients due to unreachable *** median stone size of the unreachable stones was 12 mm(range 10e30 mm).Multivariable logistic regression analysis revealed two predictors of SFR for lower pole stones:a small cumulative stone burden(odds ratio[OR]:0.903,95%confidence interval[CI]:0.867e0.941,peoperative ureteral stent insertion(OR:0.515,95%CI:0.318e0.835,pZ0.007).Conclusion:The long-term SFR of RIRS for the treatment of lower pole stones with basket displacement with appropriate patient selection is high.

Micropunch grafting for healing of refractory chronic venous leg ulcers

Background: Chronic venous ulcers are a serious problem for both patients and physicians. The CeAP classification(clinical manifestations(C), etiologic factors(e), anatomic distribution of disease(A), and underlying pathophysiologic findings(P)) for chronic venous disorders(CVD) was developed in 1994. Published papers on CVD use all or part of the CeAP. Micropunch grafts, which are a variation of skin grafts, have been used with great success in plastic surgery for both esthetic and reconstructive purposes. This study aimed to assess the outcomes of micropunch grafting in the treatment of chronic refractory venous ***: Patients with chronic venous ulcers who did not respond to conservative treatment for more than 6months were included in the study. All patients underwent ulcer coverage by micropunch skin grafts using a micrometer. The patients were discharged on the same day as the surgery. Micropunch skin grafts were manually counted per square centimeter intraoperatively and during follow-up using Dermlite DermatoschopeTM II Pro *** satisfaction was assessed by using a ***: Twenty patients underwent the surgery. Most of the patients were middle-aged males. The ulcers were predominantly small-and medium-sized. The mean operation time was 167.00 ± 86.01 min. After 6 months, the survival rate of the planted micrografts was 84%. Three ulcers had graft loss and two patients had an infection at the ulcer site, which was managed conservatively. The patients were followed-up for 14 ***: Micropunch grafting is a useful and convenient method for the treatment of difficult venous ulcers and can be performed on an outpatient basis.

Sedimentation Processes at the Navigation Channel of the Liquefied Natural Gas (LNG) Port, Nile Delta, egypt

Liquefied Natural Gas (LNG) port is located at Abu Qir Bay on the northwestern coast of the Nile delta, egypt. The port was constructed in 2004 to export liquefied natural gas worldwide. The offshore basins of this port including the turning and berthing areas (15-m depth) are connected to the deep water by a 15-m depth dredged channel that extends 4 km offshore. However, the navigation channel and its contiguous basins have experienced problematic shoaling that might affect the navigation activities of gas tankers. Sedimentation processes have been investigated by analyses of waves, currents, bathymetry, grain size of seabed and channel dimensions. Sedimentation rates are estimated using a developed numerical model. Sedimentation rate fluctuates between 0.048 × 106 m3/month and 0.388 × 106 m3/month, with an annual sedimentation rate of 1.977 × 106 m3/yr. The variance in the sedimentation rates between winter and summer resulted in increasing of current speed and direction flowing towards offshore. The sedimentation process is influenced by the temporal variability in the direction and intensity of the predominant waves, currents, orientation of navigation channel, basin breakwaters, seafloor morphology and sediment sources. Due to the geographic location of LNG port it lays within a sediment sink for sediments supplied from different alternating directions by several pathways, flowing towards the N-W, S-W, N-e, and S-e quadrants. Most of these currents components are substantially effective in transporting fine-grained sediment towards the navigation channel axis and contiguous basins. Together with these currents, the predominant NW and Se waves acting to agitate and stirrup sediments in the vicinity of the port, and thereby accelerating sedimentation rates.

Investigation of Self excited Torsional Vibrations of Different Configurations of Automatic Transmission Systems during engagement

The vehicle drive line system is subjected to torsional vibration from different sources of the system such as;engine fluctuating torque, Hook’s joint and the final drive. However, the essential source is the friction torque induced in the friction elements, during their engagement. In the automatic transmission system, the planetary gear set includes several friction elements such as;clutch, band brakes, and one way clutch. During the engagement, severe torsional vibration is induced by friction which is noticeable by the passengers in the form of what so called vehicle shunt. In present paper, a torsional vibration model for Drive Line (DL) system includes three different configurations of automatic transmission is constructed. A computer program using MATLAB subroutines is implemented to obtain the system response. effect of system parameters on the dynamic behavior and stability has been investigated. The system damping and the trend of the friction coefficient have an essential effect on the dynamic behavior and stability of the system. The system response is now predictable with change of the system parameters which opens up the opportunity in future to control the vibration level.
