Midterm Results of Leaflet Augmentation in Mitral Valve Repairin Rheumatic Valves Experience in One Center

Patients with severe mitral regurgitation (MR) should undergo surgery when they present symptoms or if asymptomatic when there is objective evidence of left ventricular dysfunction. In this work, we analyze the midterm results of leaflet augmentation in mitral valve repair of rheumatic valves with gluteraldehyde preserved autologous pericardium. Patients and Methods: In our department 48 patients were exposed to mitral valve repair by leaflet augmentation either anterior or posterior beside other technique and all patients supported by flexible annuloplasty ring and followed for five years clinically and by echocardiography. Results: Age of the patients ranging from 12 to 47 years, mean age 25.9 ± 8.9 and there were 12 males (25%) and 36 females (75%) with male to female ratio of 1:3. All patients presented with shortness of breath (100%);with 14 patients were in NYHA class III (29.17%) and 34 patients were in NYHA class IV (70.83%). During follow-up period 5 patients needed reoperation by valve replacement, causes of reoperation were restrictive valve motion in one patient, left atrial thrombus in 1 patient and sever mitral regurgitation in 3 patients. Freedom from reoperation was 87.5%. At 5 years, (92.9%) were in New York Heart Association functional class I, three patients (7.1%) were in class II. Echocardiography at follow-up showed satisfactory mitral valve function. Conclusion: leaflet augmentation is a simple and reproducible method of valve repair for rheumatic MR with good midterm result.

Grape (Vitis vinifera L.) is assumed as one of the most valuable and popular fruit crops all over the world. In this study, ten of local grape germplasm were characterized considering phenotypic diversity and genetic features under Egyptian conditions. Selected local grapes;Baltim Eswid, Edkawy, Matrouh Eswid, Bez El-Naka, Bez El-Anza, Romy Abiad, Romy Ahmer, Gharibi, Fayoumi and Banaty, were spread geographically among seven national governorates. A total of 58 attributes were characterized grape germplasm for the distinctness of vine parts. For molecular characterization, 9 nuclear SSR loci were analysed. Data revealed a broad sense of similarity at the level of studied morphological attributes, where the highest similarity (0.93) was between Romy Ahmer and Romy Abiad. A total of 24 alleles, ranging from 124 - 253 bp in size, were detected at the nine tested loci with an average of 2.67 alleles per locus. The amplification products in all of the 9 SSRs loci showed polymorphism among the 10 grapevine cultivars. The genetic relatedness among most of the cultivars was in accordance with their identification based on ampelographic properties. Data of phenotypic and molecular analysis revealed high variability of Vitis germplasm in Egypt and contributed substantially to our awareness of valuable local grape genetic resources which are on the verge of extinction.

A comparative study of liquid and solid inner contact roxatidine acetate ion-selective electrode membranes

A comparative study was conducted using two designs of a roxatidine acetate （ROX）-selective electrode; a conventional liquid inner contact called electrode A and a graphite-coated solid contact called electrode 13. The fabrication of electrodes was based on roxatidine-tetraphenylborate （ROX-TPB） as an ion-association complex in a PVC matrix using different plasticizers. Electrode A has a linear dynamic range of 2.2 ×10^-5 mol/L to 1.0 ×10^-2 mol/L, with a Nernstian slope of 54.7 mV/decade and a detection limit of 1.4 ×10^-6 mol/L. Electrode B shows linearity over the concentration range of 1.0×10^-6 mol/L to 1.0×10^-2 tool/L, with a Nernstian slope of 51.2 mV/decade and a limit of detection of 1.1×10^7 mol/L which is remarkably improved as a result of diminishing ion fluxes in this solid contact, ion-selective electrode. The proposed sensors display useful analytical characteristics for the determination of ROX in bulk powder and its pharmaceutical formulation. The present electrodes show clear discrimination of ROX from several inorganic, organic ions, sugars, some common drug excipients and the degradation product （3-[3-（1-piperidinyl methyl） phenoxy] propyl amine） of ROX. Furthermore, the proposed electrodes were utilized for the determination of ROX in human plasma, where electrode B covers drug Cmax which indicated its applicability to pharmacokinetic, bioavailability and bioequivalent studies. The results obtained by the proposed electrodes were statistically analyzed and compared with those obtained by a reported HPLC method. No significant difference for either accuracy or precision was observed.

Role of relevant immune-modulators and cytokines in hepatocellular carcinoma and premalignant hepatic lesions

AIM To assess the levels of different immune modulators in patients with hepatocellular carcinoma(HCC),in relation to other hepatic *** Eighty-eight patients were included in the current study and represented patients with HCC(20),liver cirrhosis(28) and chronic hepatitis(CH;25),and normal controls(NC;15).Peripheral blood was isolated for immunophenotyping of active myeloid dendritic cells(m DCs;CD1 c and CD40),mature inactive myeloid cells(CD1 c and HLA),active plasmacytoid cells(p DCs;CD303 and CD40),mature inactive p DCs(CD30 and HLA),active natural killer(NK) cells(CD56 and CD161),active NK cells(CD56 and CD314) and inactive NK cells(CD56 and CD158) was done by flow *** levels of interleukin(IL)-2,IL-10,IL-12,IL-1β,interferon(IFN)-α,IFN-γ and tumor necrosis factor(TNF)-αR2 were assessed by *** Active m DCs(CD1 C+/CD40+) and inactive m DCs(CD1 c+/HLA+) were significantly decreased in HCC patients in relation to NC(P < 0.001).CD40+ expression on active p DCs was decreased in HCC patients(P < 0.001),and its level was not significantly changed among other *** p DCs(CD303+/HLA+),inactive NKs(CD56+/CD158+) and active NKs(CD56+/CD161+) were not statistically changed among the four groups studied;however,the latter was increased in CH(P < 0.05).NKG2 D was statistically decreased in HCC,CH and cirrhosis(P < 0.001),and it was not expressed in 63%(12/20) of HCC *** was significant decrease of IL-2,IFN-α and IFN-γ(P < 0.001),and a significant increase in IL-10,IL-1β,and TNF-αR2(P <0.01,P < 0.001 and P < 0.001;respectively) in HCC *** was inverted correlation between IL-12 and IL-1β in HCC(r =-0.565,P < 0.01),with a strong correlation between p DCs(CD303+/CD40+) and NKs(CD56+/CD161+;r = 0.512,P < 0.05) as well as inactive m DCs(CD1 c+/HLA+) and inactive NK cells(CD56+/CD158+;r = 0.945,P < 0.001).CONCLUSION NKG2 D,CD40,IL-2 and IL-10 are important modulators in the development and progression of HCC.

Characterization of Radiation Prepared Copolymer and Studies of Their Biodegradability

Blends of biodegradable copolymer based on starch, poly lactic acid (PLA), poly vinyl alcohol (PVA) and natural rubber (NR) have been prepared. Gamma radiation induced synthesis and modification of polymer hydrogel was studied. The polymer blends have been chemically surface modified by glycerol. The modified polymer blends have been investigated for swelling ratio, tensile strength and 9 scanning electron microscopy. The swelling ratio of polymer blends increased significantly after surface modification with glycerol. The swelling of polymer was decreased as a function of (NR) content in polymer blends. The gel fraction (PVA-starch-PLA) and (PVA-starch-NR) blends increased by increasing the radiation doses (kGy) to reach the maximum amount of (~99%) and (~88.2%), respectively. Addition of 2.5% (PLA) led to greater increase of the swelling ratio than 10% (NR) to blends and the maximum swelling was found at dose (5 kGy). At concentration of glycerol (5.0% w/w), tensile strength decreased and elongation at break % increased. The polymers degrading microorganisms were isolated from soil samples. The degradation ability of the microbial isolates for each polymeric material was tested on agar plates. Among these isolates, the most efficient degrader isolates for prepared blends in MSM shaking flasks were selected and the degradation was confirmed by scanning electron microscopy.

Rubber band ligation for 750 cases of symptomatic hemorrhoids out of 2200 cases

AIM: To study the results for the treatment of symptomatic hemorrhoids using rubber band ligation (RBL) method. METHODS: A retrospective study for 750 patients who came to the colorectal unit from June, 1998 to September, 2006, data was retrieved from archived fi les. RBL was performed using the Mc Gown applicator on an outpatient basis. The patients were asked to return to out-patient clinic for follow up at 2 wk, 1 mo, 6 mo and through telephone call every 6 mo for 2 years). RESULTS: After RBL, 696 patients (92.8%) were cured with no difference in outcome for second or third degree hemorrhoids (P = 0.31). Symptomatic recurrence was detected in 11.04% after 2 years. A total of 52 patients (6.93%) had 77 complications from RBL which required no hospitalization. Complications were pain, rectal bleeding and vaso-vagal symptoms(4.13%, 4.13% and 1.33% of patients, respectively). At 1 mo there were a significant improvement in mean SF-36 scores over baseline in five items, while after 2 years there were improvement in all items over baseline, but not significant. No significant manometeric changes after band ligation. CONCLUSION: RBL is a simple, safe and effective method for treating symptomatic second and third degree hemorrhoids as an out patient procedure with signifi cant improvement in quality of life. RBL doesn't alter ano-rectal functions.

Endoscopic management of Dieulafoy's lesion using Isoamyl-2-cyanoacrylate

Dieulafoy's lesion (DL) is a rare but important cause of obscure gastrointestinal bleeding that may be over-looked during diagnostic endoscopy. Mortality rates are similar to those of other causes for gastrointestinal bleeding. Diagnosis by upper endoscopy is the modal-ity of choice during acute bleeding. In the absence of active bleeding, the lesion resembles a raised nipple or visible vessel. There are no guidelines regarding effective selective therapy for DL, when diagnosed, en-doscopist experience is the major determinant of the treatment strategy. Following our strategy, an expert endoscopist with a skilled assistant should have a high rate of successful DL diagnosis when an obscured gas-trointestinal lesion is suspected. Cyanoacryltes com-pounds have been used successfully in management of Gastric varices and DLs. To our knowledge, there have been no previous reports regarding use of isoamyl-2-cyanoacrylate (AMCRYLATE ; Concord Drugs Ltd.,Hyderabad, India) as an effective therapy for gastric DL without serious complications. In our case study, Isoamyl-2-cyanoacrylate (AMCRYLATE) was effective and safe for treating DL. Surgical wedge resection of the lesion should be considered as a therapeutic option if endoscopic therapy fails.
