Outcomes and Toxicology of Herbal Drugs in Alcoholic Hepatitis-A Single Center Experience from India

Background and Aims:We aimed to study clinical out-comes and liver biopsy features of alcoholic hepatitis(AH)patients on complementary and alternative medicines(CAMs)and to analyze the retrieved drugs for chemical and toxic components linked to drug-induced liver ***:We retrospectively assessed clinical,biochemical and liver bi-opsy features of AH patients on CAM with drug-induced liver injury(AH-CAM,n=27)and compared them to a control group(classical AH,n=29)on standard of *** without liver biopsy evaluation and other causes for liver dis-ease were *** of the CAMs(n=42)from pa-tients were retrieved and assessed for chemical and ***:All were males,and significantly worse clinical pre-sentation,biochemical severity,and liver disease scores were notable in patients with *** Ayurve-dic-polyherbal formulations were the most commonly used *** liver histology,varying grades of severe-necrosis,severe hepatocellular,canalicular,cholangiolar cholestasis with predominant lymphocytic-portal-inflammation and varying grades of interface-hepatitis were noted in *** of CAMs revealed presence of heavy metals up to 100,000 times above detectable range and adulter-ants,such as antibiotics,chemotherapy agents,nonsteroi-dal anti-inflammatory drugs,alcohols,antidepressants,anxiolytics,and recreational *** follow up,a signifi-cantly higher number of patients with AH on CAM died at end of 1,3-and-6-months compared to controls(37%vs.83%,29%vs.62%,18%vs.52%respectively;plusions:Patients with AH and CAM-related drug-induced liver injury have extremely poor short-term survival in the absence of liver transplantation compared to those patients with AH on evidence-based *** transplant referral and educating on and curbing of CAM use in severe liver disease through strict monitoring of unregulated traditional health practices can help ease the burden of liver-related death.

Biobrane^(TM) for burns of the pubic region: minimizing dressing changes

Background: The pubic region is often involved in accidental hot water or soup-spill burns. Most of these wounds are superficial partial thickness burns. Due to their proximity to the urinary system, as well as vaginal and anal openings,these burns are easily contaminated. Daily dressings are routinely prescribed as the sole treatment. The cumbersome dressing process is uncomfortable and embarrassing for patients. Biobrane^(TM) is a bilayered biosynthetic dressing. Its coverage of superficial partial thickness burns promotes wound healing and allows one-time *** presentations: We report two patients who suffered superficial dermal burns over their pubic region. One patient had 23% total body surface area(TBSA) burns over her lower abdomen, both thighs and pubic region. The second patient had 10% TBSA burns that involved her perineum and the medial sides of both thighs and *** were managed with the standard resuscitation protocol in the initial phase. Their burn injuries were managed by shaving, Foley catheterization and Biobrane^(TM) coverage. Their wounds healed uneventfully without *** epithelization was achieved by post-operative day seven. Both patients consented to medical photography and academic ***: Shaving and catheterization improved the hygiene of the burns of the pubic area. The Biobrane^(TM)method circumvents the need of regular dressing changes, eliminating the pain due to dressing changes and preserving patient dignity.

The novel coronavirus-related coronavirus disease 2019(COVID-19)pandemic has been relentless in disrupting and overwhelming healthcare the world *** outcomes of COVID-19 in patients with chronic comorbidities,especially in those with metabolic syndrome,are well *** liver disease and cirrhosis patients are a special subgroup,among whom the management of COVID-19 is *** the pathophysiology of COVID-19 in patients with cirrhosis and portal hypertension improves our identification of at-risk patients for disease progression that will further help compartmentalize generalized and specialized treatment options in this special patient *** this exhaustive review,we critically review the impact of COVID-19 on the liver and in chronic liver disease and cirrhosis *** further discuss common features associated with the pathophysiology of COVID-19 and cirrhosis,based on the renin-angiotensin system and deliberate current literature on guidelines for the treatment of COVID-19 and extrapolate the same to the cirrhosis population to provide a concise and stepwise,evidence-based management for cirrhosis patients with severe and critical *** are no specific management guidelines for cirrhosis patients with COVID-19 and current recommendations for treatment are as per guidelines for general ***,specific issues like avoiding corticosteroids in decompensated patients with variceal bleeding,suspected sepsis,high grade hepatic encephalopathy and acute kidney injury,use of early mechanical ventilation strategies in those with severe ascites and hepatopulmonary syndrome,avoidance of remdesivir in advanced liver disease,and application of liver-specific severity scores for prognostication and identification of futility need to be highlighted.

A Review of Methods to Control Bleeding from life-Threatening Traumatic Wounds

The large selection of techniques and modalities for field treatment of the bleeding wound may appear as a potpourri of arbitrarily introduced devices. Nevertheless, if we analyze the various offered solutions systematically in an orderly approach, they can be differentiated into set categories. It is necessary to recognize the pros and cons of each available product in order to decide when and where each is the most applicable for any given clinical scenario. Identification, development and evaluation of new products and technologies capable of stopping life threatening hemorrhage are still ongoing. Despite paucity of research on this topic recently more and more information has accumulated to assist in supporting those techniques of superior efficacy. The objective of this review is to guide the reader through the diverse products according to mechanism of action and expose the important literature relevant to this topic. Justification is found for use of each of the categories: tourniquets, hemostatic agents and compression dressings however close scrutiny is needed to select the most appropriate choice in each group. Hopefully, continuing appraisal of these devices together with the growing experience of trauma teams and combat medics can assist to establish the superior products in each class for the benefit of all casualties and trauma victims.
