Evaluation of Left Ventricular systolic Function after Pulmonary Valve Replacement Using Cardiovascular Magnetic Resonance Imaging

Following reparative surgery for tetralogy of Fallot or critical pulmonary stenosis(Ps),patients frequently present with severe right ventricular(RV)volume overload due to pulmonary regurgitation,resulting in decreased RV *** pulmonary valve replacement(PVR)is known to improve RV function,but changes in left ventricular(LV)function after PVR have rarely been *** sought to determine the midterm results regarding LV systolic function after PVR using cardiac MRI in 40 consecutive patients with repaired TOF(31 patients)or Ps(9 patients)with an age of 29±9 years who underwent PVR from 2006 to 2011 at a single *** MRI RV and LV volumes before and after PVR were ***,clinical variables,cardiopulmonary bypass duration,and medications were *** ejection fraction(LVEF)increased from(54±8)to(57±6)%(P=0.02).Before PVR,26 patients had depressed LVEF of(49±5)%(range 36– 54%).In this group,LVEF increased by(7±7)%(Ps correlated with increased LVEF after PVR(regression coefficient-0.7,R 2=0.59,Ps,medications,prior pregnancies,and cardiopulmonary bypass duration had no effect on LVEF after *** increase in LVEF was most significant in patients with low pre-PVR LVEF.

Genetically Targeted Fractionated Chemotherapy

Complex and late stage cancer patients are in need of novel methods of selecting and administering chemotherapy particularly for those patients who are refractory to current treatment methods. The use of biomarkers to enhance decision making with regard to the molecular profile of a person’s cancer is becoming more important in the practice of oncology. The standard for the last several decades is to elect chemotherapeutic agents based on staging and histological identification of the primary cancer site alone versus utilizing the genetic and molecular profile information along with histological primary cancer site and staging to select chemotherapy regimens. Cancers are caused by mutations that occur within cells and therefore selecting treatment based on mutations and not primary cancer site alone can provide advantages that may have gone overlooked. As time progresses, more biomarkers continue to be discovered which can lead to more targets for drugs either currently on the market or clinical trials. In addition to advancements made in the progression of cancer treatment with utilizing molecular profiles effectively, there are other therapeutic strategies that have been postulated as advanced effective ways to administer chemotherapy. These strategies provide chemotherapy to patients while fasting, giving insulin or other biological response modifiers adjunctively prior to chemotherapy for enhanced targeting, and giving chemotherapy in micro-doses to allow for increased frequency of administration and the utilization of multiple targeted chemotherapeutic agents concurrently. In this paper we will discuss these topics and explain their benefits in addition to the evidence that supports these treatments. A review on biomarkers and cancer cell metabolism is discussed as it relates to providing a framework for what constitutes a biomarker in addition to what metabolic processes are related to fasting and administering insulin with chemotherapy. Deepwater Horizon Oil spill as a Case study for Interdisciplinary Cooperation within Developmental Biology, Environmental sciences and Physiology

The Deepwater Horizon Oil spill in the UsA’s Gulf of Mexico created a high degree of exposure of marine organisms to toxic polyaromatic hydrocarbons (PAHs) present in crude oil. To determine the ecological and physiological effects of crude oil on the Gulf of Mexico ecosystem, the Gulf of Mexico Research Initiative created several research consortia to address overreaching questions concerning the biological impacts of the ecology of the Gulf of Mexico that would otherwise be beyond the capabilities of an individual investigator or a small group. One of these consortia, highlighted in this article, is the RECOVER Consortium, which brings together physiologists, developmental biologists, toxicologists and other life scientists to focus on the multifaceted physiological effects of PAHs, especially as they pertain to cardiovascular and metabolic physiology of economically important fish species. Using the Recover Consortium’s interdisciplinary approach to revealing the biological impacts of the Deepwater Horizon Oil spill as a case study, we make the argument for interdisciplinary teams that bring together scientists with different specialties as an efficient way—and perhaps the only way—to unravel highly complex biological effects of marine oil spills.

Multiple Actions of Curcumin Including Anticancer, Anti-Inflammatory, Antimicrobial and Enhancement via Cyclodextrin

Curcumin is a naturally occurring phytotherapeutic that has broad uses including the treatment of cancer and as a potent antimicrobial agent. Its dynamic ability to treat very different kinds of disease has spawned a significant increase in inquiry on how curcumin accomplishes these feats. Thousands of peer reviewed papers were published in the past few years regarding the various functions of curcumin. Unlike most pharmaceuticals, curcumin has a multi-range of different targets that it interacts with. since curcumin’s bioavailabilty is limited, methods of increasing curcumin’s bioavailabilty are discussed. Mechanisms of action regarding curcumin’s ability to effect cancer are discussed including upstream and downstream mechanisms starting with epigenetics to its effect on signal transduction pathways and apoptosis. Curcumin also works adjuvantly with chemotherapy to reduce resistance and enhance the mechanism of certain chemotherapeutic agents. Curcumin’s antibacterial, antifungal, and antiviral capability is also discussed with regard to curcumin’s dynamic ability to treat infections. Curcumin use in cancer adjunctive care and its anti-infectious capabilities make it a unique phytotherapeutic agent with promise.

Population-based frequency of surfactant dysfunction mutations in a native Chinese cohort

Background: Rare mutations in surfactant-associatedgenes contribute to neonatal respiratory distress *** frequency of mutations in these genes in the Chinesepopulation is ***: We obtained blood spots from the GuangxiNeonatal screening Center in Nanning, China thatincluded Han (n=443) and Zhuang (n=313) ethnic *** resequenced all exons of the surfactant proteins-B(sFTPB), -C (sFTPC), and the ATP-binding cassettemember A3 (ABCA3) genes and compared the frequenciesof 5 common and all rare ***: We found minor differences in thefrequencies of the common variants in the Han andZhuang cohorts. We did not find any rare mutations insFTPB or sFTPC, but we found three ABCA3 mutationsin the Han [minor allele frequency (MAF)=0.003] and 7 inthe Zhuang (MAF=0.011) cohorts (P=0.10). The ABCA3mutations were unique to each cohort;five were *** collapsed carrier rate of rare ABCA3 mutations inthe Han and Zhuang populations combined was 1.3%,which is signifi cantly lower than that in the United states(Psions: The population-based frequency ofmutations in ABCA3 in south China newborns is signifi cantlylower than that in United states. Novel Approach to Chemotherapy and Administration selection with Metronomic/Fractionated Dosing

Metronomic dosing of chemotherapy was introduced in the early 2000s and has since gained recognition as a potential game changer in the manner of which chemotherapy can be administered. There are several known candidates for metronomic dosing of chemotherapy with the potential for many more to be elucidated in the future. Minimized overall side effects, longer durations of treatment, potential minimization of multidrug resistance (MDR) mutations, potential less refractory responses, and the potential to safely use more than one chemotherapy treatments also make metronomic dosing of chemotherapy attractive. Metronomic dosing reduces common side effects and has the potential to reduce neutropenia, lymphocytopenia, and cognitive changes associated with maximum tolerated dosages (MTD). Methods of enhancing chemotherapy including fasting and administration of insulin are also discussed. Metronomic dosing combined with a patient’s molecular profile derived from biomarkers is particularly exciting. It holds significant potential with regard to administrating the most relevant chemotherapies and offers maximal beneficial results.

Dear Editor,Malignant tumours of the hand are relatively ***,defined as metastatic bone lesions of the hand or feet are exceedingly rare,with a reported incidence rate of between 0.007%and 0.2%of all metastatic lesions[1].Acrometastasis of metastatic renal cell carcinoma(mRCC)accounts for only 10%e12%of the reported cases with majority of cases originating from primary lung *** presentation poses a diagnostic and management dilemma and is usually delayed as the symptoms and signs are similar to infective or benign *** the prognosis is grim[2,3].In this rarest of rare cases,we report a patient of renal cell carcinoma(RCC)with metastasis to the right shoulder and acrometastasis of the right *** highlight the multidisciplinary team approach utilized and the management by multimodal therapy to achieve the best palliative outcome.

Role of cytokines in inflammatory bowel disease

Inflammatory bowel disease (IBD), which includes Crohn’s disease (CD) and ulcerative colitis (UC), rep- resents a group of chronic disorders characterized by inflammation of the gastrointestinal tract, typically with a relapsing and remitting clinical course. Mucosal mac- rophages play an important role in the mucosal im- mune system, and an increase in the number of newly recruited monocytes and activated macrophages has been noted in the inflamed gut of patients with IBD. Activated macrophages are thought to be major con- tributors to the production of inflammatory cytokines in the gut, and imbalance of cytokines is contributing to the pathogenesis of IBD. The intestinal inflammation in IBD is controlled by a complex interplay of innate and adaptive immune mechanisms. Cytokines play a key role in IBD that determine T cell differentiation of Th1, Th2, T regulatory and newly described Th17 cells. Cytokines levels in time and space orchestrate the development, recurrence and exacerbation of the inflammatory process in IBD. Therefore, several cyto- kine therapies have been developed and tested for the treatment of IBD patients.
