Epidemiological Profile and Therapeutics Results of Bladder Carcinoma in Women

Introduction: The incidence of bladder cancer in women has steadily increased in recent years. The objective of our work is to study the epidemiological profile of bladder cancer in women in our population and the factors favoring its occurrence. Methods: This is a retrospective study carried out at the Mohamed Center VI for cancer treatment, involving 27 women during a two-year period from January 2019 to December 2020. We studied the epidemiological characteristics in these patients, the risk factors and the revealing symptoms. Results: The average age of patients was 67.5 years old and 66.66% of the patients were over 65 years old. One patient had in her antecedents a tumor of the upper excretory tract and 2 patients had an associated renal tumor. The notion of smoking was found in 3 patients. The time to treatment was less than 6 months in 44.4% of cases, between 6 and 12 months in 18.5% of cases, and more than a year in 37% of cases. The symptoms were marked by hematuria in 96.3% of cases, and irritative disorders in 81.4% of cases. In our series;the tumor was unifocal in 77.77% of the cases, and multifocal in the remainder of the cases. Urethrohydronephrosis was found in 74% of cases. Histologically, it was an urothelial carcinoma in 81.48% of cases. The tumor was locally advanced in 11.11% of cases, and metastatic in 11.11% of cases. Lymph node involvement was found in 33.33% of cases. 48.1% of cases underwent surgery. The operative procedure consisted of a partial cystectomy in 46.1% of cases, and only 18.5% was suitable for trimodal treatment. Conclusion: Few studies have addressed the epidemiology of bladder cancer in women due to the low incidence, but it is currently increasing steadily in recent years. These modifications are explained by the change in social habits in women.

Understanding the effects of prostate cancer on quality of life of affected patients is essential for clinical research and also for optimal management and *** main objective of this study was to evaluate the psychometric properties of a Moroccan dialect version of the quality of life questionnaire(EORTC QLQ-PR25).The study was conducted among patients with prostate cancer in oncology department of the Ibn Rochd University Hospital Mohammed VI Center for cancers treatment in Casablanca during the period of study(from January to December 2017).Statistical analysis data was done using R *** consistency was tested using Cronbach’s alpha.A total of 82 subjects were included,the mean age of patients was 69.6 years with a standard deviation of 7.9 *** majority of them were illiterate,with 59.8%,and urban people about 60%.Good reliability was revealed for the Moroccan dialect version with Cronbach’s alpha coefficient ranging from 0.17 for“sexual function”dimension to 0.83 for“urinary symptoms”*** of the scale was good with intra-class correlation coefficients ranging from 0.69 for“sexual function”dimension to 0.87 for“urinary symptoms”*** reproducibility was good with intra-class correlation coefficients ranging from 0.69 for“sexual function”dimension to 0.87 for“urinary symptoms”*** version obtained showed good reliability for the dimension“urinary symptoms”and acceptable for other dimensions except for“sexual function”which was not satisfactory.

Association between Overexpression of Her-2 and Other Clinicopathologic Prognostic Factors in Breast Cancer in Morocco

Background: Breast cancer is the leading cancer among women. The management depends largely on the knowledge of prognostic factors. The aim of our study is to evaluate the overexpression of Her-2/neu in patients with invasive breast carcinoma in Morocco, and to study the association between this overexpression and otherprognostic factors of breast cancer, such as age, tumor size, tumor grade, hormone receptors, vascular space invasion and lymph node status. Methods: This is a transversal study in which we included patients with invasive breast carcinomas treated at University Hospital Ibn Rochd of Casablanca in radiotherapy-oncology departement between January 2008 and December 2010, having had surgery first. Univariate analysis (Chi 2 test and Student’s test) and multivariate (logistic regression) were performed to study this association. Results: The overexpression of Her-2/neu was found in 29.2% of cases. In univariate analysis, Her-2 positive status was significantly correlated with high-grade tumors and large, to a lymph node, vascular space invasion and negative ER/PR. Age alone does not appear as a factor associated with positive Her-2/neu status. In multivariate analysis, the overexpression of Her 2 was significantly associated only with vascular space invasion, high tumor grade and the negativity of estrogen receptors. Conclusion: In our series, only the high grade, the vascular space invasion and the negative hormone receptors are independent factors associated with overexpression of Her 2/neu. Age does not appear as a factor associated with positive Her-2/neu status.

Management of Uterine Sarcomas

Uterine sarcomas are rare malignant tumors characterized by a great clinical and histopathological diversity. The aim of this work is to analyze the difficulties of diagnosis, therapeutic and prognosis posed by these tumors. Thirty-seven patients with uterine sarcoma, collected in the service Radiotherapy and Oncology, University Ibn Rochd of Casablanca between January 2000 and December 2007 were included in this study retrospective. The average patient age was 50 years (17-76). The bleeding was present in all patients, isolated in 54% of cases associated with pelvic pain in 24.6% and a mass abdomino-pelvic in seven patients. The average time of evolution was 10 months. The main histological type was found leiomyosarcoma. Twenty four patients in our series underwent total hysterectomy without annexial conservation. The surgery R0 was obtained in 43% of cases. The sarcomas were classified as stage IV in 51.4%. Adjuvant radiotherapy was indicated in 13 patients. After a mean of 20 months, half of patients evaluable presented a local relapse and/or metastatic, the third of cases were tumor progression while complete remission was maintained in 18.5% of cases. Uterine sarcomas are rare malignant mesenchymal tumor, which often occur in women after menopause. The main prognostic factors are hormonal status of the patient, stage clinical, histological type, histological grade and quality surgical excision. The management of uterine sarcomas is multidisciplinary, based mainly on surgery remains the only means of cure. Adjuvant radiotherapy allows decreased risk of local recurrence, with no impact on survival achieved at best 30% at 5 years. The role of chemotherapy remains confirm.

Monophasic Synovialosarcoma of the Kidney: About a Case and Literature Review

Introduction: Primary renal synovialosarcoma (SVS) is a rare and aggressive disease in which the metastatic potential is very high. Its diagnosis is based on morphological and molecular studies. No therapeutic consensus has yet been established for this histological variant of renal cancer. We describe our experience with a patient followed at the Mohamed IV Center for cancer treatment for primary monophasic synovialosarcoma of the kidney. Case Presentation: In October 2020, a 58-year-old patient with hypertension under treatment, consulted for right back pain without hematuria or calculus emission. The initial examination found a slight right lumbar tenderness. Thoraco-abdominal computed tomography revealed a cystic lesion of the superior pole of the right kidney without secondary lesions. Surgical treatment consisted of an enlarged right total nephrectomy using a bi-subcostal approach. The post-operative consequences were simple. Histologically, it was a poorly differentiated cystic malignant tumor process measuring 17 cm long axis. The limits were healthy as well as the adrenal. Immunohistology made it possible to retain a monophasic synovialosarcoma. However, the patient did not receive genetic analysis. Given the poor prognosis of these tumors, treatment with adjuvant che

Background and Aim: Primary testicular lymphoma (PTL) is a rare form of extranodal non-Hodgkin’s lymphoma. It represents for 1% - 2% of non-Hodgkin’s lymphoma, and mostly affects the elderly. We describe an interesting case of PTL managed by a combined multimodal approach with a review of the literature. Case Presentation: Patient aged 56 years, consulted for an increase in the volume of the right testicle without associated pain, all evolving in the context of a slight decline in general condition. Clinical examination revealed a large painless mass in the right scrotal bursa. A scrotal ultrasound showed a right intra-testicular mass. The patient had undergone inguinal orchiectomy. Pathological analysis showed diffuse large B-cell lymphoma of the testis. Whole-body 18-fluorodeoxyglucose positron emission tomography (18-FDG-PET-CT) showed no suspicious hypermetabolism. Lumbar puncture did not reveal malignant cells in the cerebrospinal fluid (CSF). The patient then received 6 cycles of chemotherapy according to the R-CHOP protocol (Rituximab, Cyclophosphamide, Doxorubicin, Vincristine, and Prednisone) and 2 cycles of intrathecal methotrexate. After chemotherapy, scrotal radiotherapy at a dose of 30 Gy was delivered. The evolution was marked by the death of the patient six months after the end of the scrotal radiotherapy following a diffuse lymph node relapse with a profound alteration of the general state. Conclusion: The treatment depends imperatively on the stage of the disease. The therapeutic approach is multimodal and combined based on orchiectomy, systemic and intrathecal treatment and scrotal radiotherapy. PTL is an aggressive malignant with a poor prognosis. Randomized trials are needed to define a better therapeutic strategy.
