This review highlights recent advances in pathophysiology and treatment for uremic pruritus, especially focusing on various interventions. Pruritus in patients on hemodialysis (HD) and peritoneal dialysis (PD) still remains an unresolved issue. Recently, the efficacy and safety of nalfurafine hydrochloride have been reported in Japan, and at present, more than thirty thousand patients receive this new drug. In comparison with the efficacy of this new drug, acupuncture, a form of traditional Japanese therapy, has been validated for relief of symptoms of pruritus. In this review, various interventions for relief of symptoms of pruritus as well as recent studies on its pathophysiology will be introduced. This review will be helpful for treatment of pruritus in patients on dialyzed therapy in clinical practice.

Trends in Radiology-Related Medical Lawsuits Identified by a Legal Database Search

Background: The number of medical lawsuits has been increasing in Japan. This research aimed to clarify trends in radiology-related cases based on a database search of lawsuits and to consider how to avoid the problems that lead to these kinds of lawsuits. Decrease of radiology-related medical lawsuits makes radiologists work more comfortable. Materials and Methods: The Japanese court case search system “***”, a comprehensive database that includes 29,000 laws and 210,000 precedents, was used to search for relevant lawsuits. The primary (α) keyword was related to radiology. A search was conducted as “α” × “medical” × “compensation for damages” for the period from 1 January 1965 through 30 September 2013. Basic information on 3383 extracted lawsuits was examined. Of these, 35 cases directly related to radiologists were selected, and the judgments or outcomes were investigated. All cases were classified by modality (type of treatment), whether damages were awarded in each modality, and whether the litigation issues were related to procedure, informed consent (IC), diagnosis, or interpretation. Court judgments were analyzed to determine whether a causal relationship was established and how that affected the outcome. Results: The number of lawsuits gradually increased in the 1980s and 1990s and then began to decrease slightly in the 2000s. Interventional radiology (IVR) had the greatest number of cases and greatest number where compensation was awarded. No characteristic trends were identified with regard to litigation issues. There was a tendency to reject cases where no causal relationship was recognized and the treatment was considered appropriate. Conclusion: Mistakes in IVR procedures, lesions overlooked during image interpretation, and misdiagnoses were the main causes of litigation. Study on structural and compositional transitions of coal ash by using NMR

Structural and compositional transitions of Datong coal ash and its CaCO3 additional effects were carefully exam- ined at a temperature range of 300 to 1 600℃ by using XRD and solid state NMR. The quantitative estimation of amorphous structures of ashes can be successfully obtained through the analyses of solid state NMR spectra. Viscosity of molten ash and its changes with CaCO3 addition were also evaluated up to 1 700 ℃ by using a rotary type viscometer. Glasses with poor crystalline and amorphous phase were continuously formed through the eutectic reaction of silica above fusing temperature （FT〉1 500 ℃） that caused broadening and shift of 29Si and 27A1 peaks in NMR results. With the additional amount of CaCO3 increasing, the peaks shifted to downfield obviously; the fraction of Si（OA1）0（OSi）4 decreased, while that of Si（OA1）l（OSi）l at 84.3 x 10-6 increased apparently. These transitions indicated the destruction of large alumina-silicate framework into small segments by the addition of Ca ion.

Malignant ascites affects approximately 10% of patients with gastric cancer(gC), and poses significant difficulties for both patients and clinicians. In addition to the dismal general condition of affected patients and the diversity of associated complications such as jaundice and ileus, problems in assessing scattered tumors have hampered the expansion of clinical trials for this condition. However, the accumulation of reported studies is starting to indicate that the weak response to treatment in g C patients with malignant ascites is more relevant to their poor prognosis rather than to the ascites volume at diagnosis. Therefore, precise assessment of initial state of ascites, repetitive evaluation of treatment efficacy, selection of suitable treatment, and swift transition to other treatment options as needed are paramount to maximizing patient benefit. Accurately determining ascites volume is the crucial first step in clinically treating a patient with malignant ascites. Ultrasonography is commonly used to identify the existence of ascites, and several methods have been proposed to estimate ascites volume. Reportedly, the sum of the depth of ascites at five points(named "five-point method") on three panels of computed tomography images is well correlated to the actual ascites volume and/or abdominal girth. This method is already suited to repetitive assessment due to its convenience compared to the conventional volume rendering method. Meanwhile, a new concept, "Clinical Benefit Response in g C(CBR-GC)", was recently introduced to measure the efficacy of chemotherapy for malignant ascites of g C. CBR-GC is a simple and reliable patient-oriented evaluation system based on changes in performance status and ascites, and is expected to become an important clinical endpoint in future clinical trials. Wind Force Coefficients for Designing Porous Canopy Roofs

Wind force coefficients for designing porous canopy roofs have been investigated based on a series of wind tunnel experiments. Gable, troughed and mono-sloped roofs were tested. The roof models were made of 0.5 mm thick perforated duralumin plates, the porosity of which was changed from 0 to about 0.4. Overall aerodynamic forces and moments acting on the roof model were measured in a turbulent boundary layer with a six-component force balance for various wind directions. The results indicate that the wind loads on canopy roofs generally decrease with an increase in porosity of the roof. Assuming that the roof is rigid and supported by the four corner columns with no walls, the axial forces induced in the columns are regarded as the most important load effect for discussing the design wind loads. Two loading patterns causing the maximum tension and compression in the columns are considered. Based on a combination of the lift and moment coefficients, the design wind force coefficients on the windward and leeward halves of the roof are presented for the two loading patterns as a function of the roof pitch and porosity. The effect of porosity is taken into account as a reduction factor of the wind loads.
