AIM To build a regional database of chronic patients to define the clinical epidemiology of hepatitis B virus(HBV)-infected patients in the Tuscan public health care *** This study used a cross-sectional cohort design. We evaluated chronic viral hepatitis patients with HBV referred to the outpatient services of 16 hospital units. Information in the case report forms included main demographic data, blood chemistry data, viral hepatitis markers, instrumental evaluations, and eligibility for treatment or ongoing therapy and liver transplantation. RESULTS Of 4015 chronic viral hepatitis patients, 1096(27.3%) were HBV infected. The case report form was correctly completed for only 833 patients(64% males, 36% females; mean age 50.1 ± 15.4). Of these HBV-infected patients, 73% were Caucasian, 21% Asian, 4% Central African, 1% North African and 1% American. Stratifying patients by age and nationality, we found that 21.7% of HBV-infected patients were aged < 34 years(only 2.8% were Italian). The most represented routes of transmission were nosocomial/dental procedures(23%), mother-to-child(17%) and sexual transmission(12%). The most represented HBV genotypes were D(72%) and A(14%). Of the patients, 24.7% of patients were HBe Ag positive, and 75.3% were HBe Ag negative. Of the HBV patients 7% were anti-HDV positive. In the whole cohort, 26.9% were cirrhotic(35.8% aged < 45 years), and 47% were eligible for or currently undergoing treatment, of whom 41.9 % were cirrhotic. CONCLUSION Only 27.3% of chronic viral hepatitis patients were HBV infected. Our results provide evidence of HBV infection in people aged < 34 years, especially in the foreign population not protected by vaccination. In our cohort of patients, liver cirrhosis was also found in young adults.

地中海隼(Falco biarmicus feldeggii)是欧洲濒危程度最高的物种之一。其全球数量估计不超过几百对,不均匀分布于西西里至里海的广阔而片断化的地区。最近统计表明,西西里地中海隼在全部种群数量中占很大比例(>25%)。因此,意大利政府提出一项国家行动计划。在这一计划框架下,我们在意大利进行了首次地中海隼种群数量的全国调查(2003～2004年)。本研究的地理范围包括地中海隼的整个繁殖区,即西西里和意大利半岛(n=2909栅格单元,10km×10km)。另外,我们尽量考虑了以往(1993～2001年)区域调查的结果。我们对各个繁殖亚群的大小和分布进行评估,鉴别在景观尺度上与巢址空间分布相关的主要环境特征。在184个栅格单元(全部栅格地图的6.4%)中发现有地中海隼的存在,但同一繁殖期内地中海隼数量估计不超过140～172对(其中70～80对分布于西西里)。大陆繁殖栅格的隔离程度较高,而西西里地区的单元更为密集。海拔高度是意大利单元聚集性的主要影响因素;另外气候、降水量和植被等环境因素对此也有重要影响。本研究表明保护区内的处于繁殖期的地中海隼数量较少(23%～28%),意大利政府的保护措施还不够完善。在适于地中海隼巢址不规则空间聚集的优先性生境保护方面,多个小型、分散分布的特定保育区(SAC)优于大型特定保护区(SPA)。

Dealing with the regional land surfaces heat fluxes over inhomogeneous land surfaces in arid and semi-arid areas is an important but not an easy issue. In this study, one parameterization method based on satellite remote sensing and field observations is proposed and tested for deriving the regional land surface heat fluxes over inhomogeneous landscapes. As a case study, the method is applied to the Dunhuang experimental area and the HEIFE (Heihe River Field Experiment, 1988-1994) area. The Dunhuang area is selected as a basic experimental area for the Chinese National Key Programme for Developing Basic Sciences: Research on the Formation Mechanism and Prediction Theory of Severe Climate Disaster in China (G1998040900, 1999-2003). The four scenes of Landsat TM data used in this study are 3 June 2000, 22 August 2000, and 29 January 2001 for the Dunhuang area and 9 July 1991 for the HEIFE area. The regional distributions of land surface variables, vegetation variables, and heat fluxes over inhomogeneous landscapes in arid and semi-arid areas are obtained in this study.

AIM: To check the safety and efficacy of boceprevir/telaprevir with peginterferon/ribavirin for hepatitis C virus(HCV) genotype 1 in the real-world settings. METHODS: This study was a non-randomized, observational, prospective, multicenter. This study involved 47 centers in Italy. A database was prepared for the homogenous collection of the data, was used by all of the centers for data collection, and was updated continuously. All of the patients enrolled in this study were older than 18 years of age and were diagnosed with chronic infection due to HCV genotype 1. The HCV RNA testing was performed using COBAS-Taq Man2.0(Roche, LLQ 25 IU/m L). RESULTS: All consecutively treated patients were included. Forty-seven centers enrolled 834 patients as follows: Male 64%; median age 57(range 18-78), of whom 18.3% were over 65; mean body mass index 25.6(range 16-39); genotype 1b(79.4%); diagnosis of cirrhosis(38.2%); and fibrosis F3/4(71.2%). The following drugs were used: Telaprevir(66.2%) and PEG-IFN-alpha2a(67.6%). Patients were na?ve(24.4%), relapsers(30.5%), partial responders(14.8%) and null responders(30.3%). Overall, adverse events(AEs) occurred in 617 patients(73.9%) during the treatment. Anemia was the most frequent AE(52.9% of cases), especially in cirrhotic. The therapy was stopped for 14.6% of the patients because of adverse events or virological failure(15%). Sustained virological response was achieved in 62.7% of the cases, but was 43.8% in cirrhotic patients over 65 years of age. CONCLUSION: In everyday practice, triple therapy is safe but has moderate efficacy, especially for patients over 65 years of age, with advanced fibrosis, nonresponders to peginterferon + ribavirin.

Ultrasound detection of abdominal lymphadenomegaly in subjects with hepatitis C virus infection and persistently normal transaminases:a predictive index of liver histology severity

AIM The indications for liver biopsy in anti HCV positive patients with persistently normal alanine aminotransferase levels are not clearly established. Recent studies have correlated the presence of abdominal lymphoadenomegaly with disease severity in patients with chronic hepatitis C. Our study aimed to evaluate the frequency of abdominal lymphoadenomegaly in an anti HCV positive blood donor population with persistently normal alanine aminotransferase and the relationship of abdominal lymphoadenomegaly with the severity of liver changes. METHODS Eighty six anti HCV positive blood donors (58 M, 28 F) with normal alanine aminotransferase were followed up for a median of 31 months (range 12-50). To evaluate the frequency of abdominal lymphoadenomegaly, all patients underwent ultrasound scan. The common parameters of liver function as well as serum HCV RNA levels were determined. Histological changes were evaluated both in a conventional manner and using the numerical scoring systems of Knodell and Desmet. RESULTS Of the 86 donors, 68 (79%) maintained persistently normal alanine aminotransferase levels during follow up, and abdominal lymphoadenomegaly was present in 15 of them (22 0%). The remaining 18 donors (21%) showed rises in alanine aminotransferase above normal levels during the follow up and seven of them (38%) had abdominal lymphoadenomegaly ( P =n.s.). In the subjects with normal alanine aminotransferase, there were no significant differences in the common parameters of liver function and the serum presence of HCV RNA between those with or without abdominal lymphoadenomegaly. Normal liver was found in five patients without abdominal lymphoadenomegaly, but never in patients with abdominal lymphoadenomegaly. Analysis with the Mantel Haenszel test showed a trend toward more serious changes in patients with abdominal lymphoadenomegaly (chi square MH=9 5, P <0 003).
