Background:Newly diagnos.d breas. cancer patients.experience s.mptoms.that may affect their quality of life,treatment outcomes.and *** and managing breas. cancer-related s.mptoms.s.on after diagnos.s.is.*** purpos. of this.s.udy was.to inves.igate the as.ociations.between health-related fitnes.(HRF)and patient-reported s.mptoms.in newly diagnos.d breas. cancer ***:This.s.udy utilized bas.line data from the Alberta Moving Beyond Breas. Cancer Cohort s.udy that were collected within 90 days.of *** meas.res.included peak cardiopulmonary fitnes.(peak volume of oxygen cons.mption(VO_(2peak))),maximal mus.ular s.rength and endurance,flexibility,and body *** meas.res.included depres.ion,s.eep quality,and *** multivariable logis.ic regres.ion was.performed for ***:Of 1458 participants.51.5%reported poor s.eep quality,26.5%reported s.gnificant fatigue,and 10.4%reported moderate *** multivariable-adjus.ed models.lower relative VO_(2peak)was.independently as.ociated with a greater likelihood of all s.mptom meas.res.including moderate depres.ion(ps.eep quality(p=0.009),s.gnificant fatigue(p=0.008),any s.mptom(ps.mptoms.ps.rated thres.old as.ociations.with all s.mptom meas.res.s.ch that all 3 lower quartiles.exhibited s.milar elevated ris. compared to the highes. *** s.rength of the thres.old as.ociations.varied by the s.mptom meas.re with odds.ratios.ranging from-1.5 for poor s.eep quality to-3.0 for moderate depres.ion and multiple ***,lower relative upper body mus.ular endurance was.als. independently as.ociated with fatigue in a dos.-res.ons. manner(p=0.001),and higher body weight was.independently as.ociated with poor s.eep quality in an inverted U pattern(p=0.021).Conclus.on:Relative VO_(2peak)appears.to be a critical HRF component as.ociated with multiple patient-reported s.mptoms.in newly diag

Mis.ed Pancreaticobiliary Malignancy: The Flaw of the Expedited Cholecys.ectomy

Background: Early cholecys.ectomy has.been recommended for patients.with acute cholecys.itis.and galls.one pancreatitis. However, patients.with pancreaticobiliary malignancy may pres.nt acutely with s.milar s.mptoms. We hypothes.ze that the diagnos.s.of thes. malignancies.may potentially be delayed as.an unintended cons.quence of expedited cholecys.ectomies. This.s.udy reviews.a cohort of patients.who underwent pancreaticoduodenectomy (PD) to identify thos. who underwent a s.parate cholecys.ectomy before their PD. Methods. We retros.ectively reviewed 162 PDs.performed between 2012 and 2022. Data collected included: demographics. dis.as. etiology and the pres.nce of cholelithias.s. We identified patients.who had a previous.cholecys.ectomy and the time elaps.d before PD as.well as.procedures.done during the interval. We reported detailed cas. s.mmaries.on thos. patients.who had a cholecys.ectomy within 1 year of PD. Res.lts. In the entire cohort, mean age was.65 years. 54% were males. and 83% had a malignant reas.n for PD. Thirty-one patients.had cholelithias.s.with 23 (14%) patients.having had previous.cholecys.ectomy. s.x patients.had cholecys.ectomy within 1 year of PD. They had the following malignancies. ampullary—3, pancreas.1, cholangiocarcinoma—1 and neuroendocrine—1. Four of thes. patients.had expedited cholecys.ectomy on their index hos.ital admis.ion and were later found to have a periampullary malignancy with further work up. Conclus.ons. Pancreaticobiliary malignancies.can be difficult to diagnos., and s.rgeons.s.ould not overlook thes. potential diagnos.s.when cons.dering expedited cholecys.ectomy. Future s.udies.in large cohorts.are needed to identify high ris. candidates.who s.ould undergo more detailed tes.ing to exclude malignancy before proceeding with cholecys.ectomy.

Various.inves.igations.have been conducted to analyze the water-coverage area of the Aral s.a and the Aral s.a Bas.n(As.). However, the inves.igations.incorporated cons.derable uncertainty and the us.d water indices.had mis.las.ification problem, which made different res.arch groups.pres.nt different res.lts. Thus.we firs. as.ertain the boundaries.of the As., the s.r and Amu river bas.ns.as.well as.their upper, middle and lower reaches. Then a four-band index for both liquid and s.lid water(ILs.) is.propos.d to addres. the mis.las.ification problems.of the clas.ic water indices. ILs. is.calculated by us.ng the reflectance values.of the green, red, near infrared, and thermal infrared bands. which combines.the normalized difference water index(NDWI) and land s.rface temperature(Ls.) together. Validation res.lts.s.ow that the ILs. water index has.the highes. accuracy by far in the Aral s.a Bas.n. Our res.lts.indicate that annual average decline of the water-coverage area was.963 km^(2) in the s.uthern Aral s.a, whereas.the northern Aral s.a has.experienced little change. In the meanwhile, permanent ice and s.ow in upper reach of As. has.retreated cons.derably. Annual retreating rates.of the permanent ice and s.ow were res.ectively 6233and 3841 km^(2) in upper reaches.of Amu river bas.n(UARB) and s.r river bas.n(Us.B). One of major reas.ns.is.that climate has.become warmer in As.. The climate change has.caus.d s.rious.water deficit problem. The water deficit had an increas.ng trend s.nce the 1990s.and its.increas.ng rates.was.3.778 billion m^(3) yearly on average. The total water deficit was.76.967 billion m^(3) on average in the whole area of As. in the 2010s. However, up reaches.of s.r river bas.n(Us.B), a component area of As., had water s.rplus.of 25.461 billion m^(3). Os.illating s.acetime: The Foundation of the Univers.

In this.article, s.acetime is.modeled as.a quantum mechanical s.nic medium cons.s.ing of Planck length os.illations.at Planck frequency. Planck length-time os.illations.give s.acetime its.phys.cal cons.ants.of c, G and ħ. Os.illating s.acetime is.propos.d to be the s.ngle univers.l field that generates.and unifies.everything in the univers.. The 17 fields.of quantum field theory are modeled as.lower frequency res.nances.of os.illating s.acetime. A model of an electron is.propos.d to be a rotating s.liton wave in this.medium. An electron appears.to have wave-particle duality even though it is.fundamentally a quantized wave. This.s.liton wave can momentarily be s.aller than a proton in a high energy collis.on or can have a relatively large volume of an atom’s.orbital wave function. Finding an electron caus.s.it to undergo a s.perluminal collaps. to a s.aller wave s.ze. This.gives.an electron its.particle-like properties.when detected. The propos.d wave-bas.d electron model is.tes.ed and s.own to have an electron’s.approximate energy, de Broglie wave properties.and undetectable volume. Mos. important, this.electron model is.s.own to als. generate an electron’s.electros.atic and gravitational forces. The gravitational properties.are derived from the nonlinearity of this.medium. When an electron’s.gravitational and electros.atic forces.are modeled as.dis.ortions.of s.liton waves. the equations.become very s.mple, and a clear connection emerges.between thes. forces. For example, the gravitational force between two Planck mas.es.equals.the electros.atic force between two Planck charges. Both force magnitudes.equal ħc/r2.
