BACKGROUND Thymic epithelial carcinomas are rare and have a poor prognosis.Treatment of thymic epithelial carcinoma is multimodal and includes surgery,post-operative radiation therapy,adjuvant and neoadjuvant chemotherapy,or exclusive chemotherapy based on disease resectability.However,there is currently no standard treatment regimen for metastatic and recurrent thymic carcinoma.CASE SUMMARY A 45-year-old Caucasian male,with no past medical history,presented with hepatalgia and a cervical mass.A computed tomography(CT)scan showed multiple suspect lesions in the lungs,liver,and anterior mediastinum associated with mediastinal and cervical adenopathy.CT-guided percutaneous biopsies of the liver lesions and anterior mediastinal mass were performed,confirming the histopathology of thymic epithelial carcinoma.Management consisted of several chemotherapy regimens and radiation therapy,administered between April 2016 and December 2018.The patient achieved complete metabolic response.Fluorodeoxyglucose positron emission tomography/CT performed in June 2019 showed disease relapse,with reappearance of a large hypermetabolic hepatic mass and involvement of mediastinal and axillary lymph nodes.Intravenous pembrolizumab(200 mg,every 3 wk)was administered after two prior systemic therapies.The patient’s response to treatment was last documented on March 5,2020.CONCLUSION Pembrolizumab was successful in treatment of a patient with programmed deathligand 1-negative metastatic thymic carcinoma,pretreated with chemotherapy.

Most current seismic design includes the nonlinear response of a structure through a response reduction factor （R）. This allows the designer to use a linear elastic force-based approach while accounting for nonlinear behavior and deformation limits. In fact, the response reduction factor is used in modem seismic codes to scale down the elastic response of a structure. This study focuses on estimating the actual ＇R＇ value for engineered design/construction of reinforced concrete （RC） buildings in Kathmandu valley. The ductility and overstrength of representative RC buildings in Kathmandu are investigated. Nonlinear pushover analysis was performed on structural models in order to evaluate the seismic performance of buildings. Twelve representative engineered irregular buildings with a variety of characteristics located in the Kathmandu valley were selected and studied. Furthermore, the effects of overstrength on the ductility factor, beam column capacity ratio on the building ductility, and load path on the response reduction factor, are examined. Finally, the results are further analyzed and compared with different structural parameters of the buildings.

In this paper, we introduce a new concept of land-surface state representation for southern South America, which is based on ＂functional＂ attributes of vegetation, and implement a new land-cover （Ecosystem Functional Type, hereafter EFT） dataset in the Weather and Research Forecasting （WRF） model. We found that the EFT data enabled us to deal with functional attributes of vegetation and time-variant features more easily than the default land-cover data in the WRF. In order to explore the usefulness of the EFT data in simulations of surface and atmospheric variables, numerical simulations of the WRF model, using both the US Geological Survey （USGS） and the EFT data, were conducted over the La Plata Basin in South America for the austral spring of 1998 and compared with observations. Results showed that the model simulations were sensitive to the lower boundary conditions and that the use of the EFT data improved the climate simulation of 2-m temperature and precipitation, implying the need for this type of information to be included in numerical climate models.

This study investigated changes in the complexity(magnitude and structure of variability)of the collective behaviours of association football teams during competitive performance.Raw positional data from an entire competitive match between two professional teams were obtained with the ProZone^(?) tracking system.Five compound positional variables were used to investigate the collective patterns of performance of each team including:surface area,stretch index,team length,team width,and geometrical centre.Analyses involve the coefficient of variation(%CV) and approximate entropy(ApEn),as well as the linear association between both parameters.Collective measures successfully captured the idiosyncratic behaviours of each team and their variations across the six time periods of the match.Key events such as goals scored and game breaks(such as half time and full time) seemed to influence the collective patterns of performance.While ApEn values significantly decreased during each half,the%CV increased.Teams seem to become more regular and predictable,but with increased magnitudes of variation in their organisational shape over the natural course of a match.

In this paper, the effects of land cover changes on the climate of the La Plata Basin in southern South America are investigated using the Weather and Research Forecasting （WRF） Model configured on a 30/10- km two-way interactive nested grid. To assess the regional climate changes resulting from land surface changes, the standard land cover types are replaced by time-varying Ecosystem Functional Types （EFTs）, which is a newly devised land-cover classification that characterizes the spatial and interannual variability of surface vegetation dynamics. These variations indicate that natural and anthropogenic activities have caused changes in the surface physical parameters of the basin, such as albedo and roughness length, that contributed to regional climate changes. EFTs are obtained from functional attributes of vegetation computed from properties of the Normalized Difference Vegetation Index （NDVI） to represent patches of the land surface with homogeneous energy and gas exchanges with the atmosphere. Four simulations are conducted, each experimental period ranging from September to November in two contrasting years, 1988 and 1998. The influence of an identical EFT change on the surface heat fluxes, 2-m temperature and humidity, 10-m winds, convective instabilities and large-scale moisture fluxes and precipitation are explored for 1988 （a dry year） and 1998 （a wet year）. Results show that the surface and atmospheric climate has a larger response to the same EFT changes in a dry year for 2-m temperature and 10-m wind; the response is larger in a wet year for 2-m water vapor mixing ratio, convective available potential energy, vertically integrated moisture fluxes and surface precipitation. For EFTs with high productivity and a weak seasonal cycle, the near- surface temperature during the spring of 1988 and 1998 increased by as much as I^C in the central and western portions of La Plata Basin. The disposal of mining tailings has increasingly focused on the use of dry stacks.These structures offer more security since they use filtered and compacted material.Because of the construction method and the heights achieved,the material that compounds the structure can be subjected to different stress paths along the failure plane.The theoretical framework considered in the design of these structures generally is the critical state soil mechanics(CSSM).However,the data in the literature concerning the uniqueness of critical state line(CSL)is still controversial as the soil is subjected to different stress paths.With respect to tailings,this question is even more restricted.This paper studies two tailings with different gradings due to the beneficial processes over extension and compression paths.A series of drained and undrained triaxial tests was conducted over a range of initial densities and stress levels.In the q-p'plane,different critical stress ratio(M)values were obtained for compression and extension stress paths.However,the critical state friction angle is very similar with a slightly higher critical state friction angle for extension tests.Curved stress path dependent CSLs were obtained in the n-lnp0 plane with the extension tests below the CSL defined in compression.Regarding the fines content,the studied tailings presented very similar M and critical state friction angle values.However,the fines content af-fects the volumetric behavior of the studied tailings and the CSLs on the n-lnp0 plane shift downwards with the increasing fines content for compression and extension tests.In relation to dilatancy analysis,the fines content did not present an evident influence on the dilatancy of the materials.However,different values of mean stress ratio N were obtained between compression and extension tests and can corroborate the existence of non-unique CSLs for these materials.

Multi-fluid modeling of density segregation in a dense binary fluidized bed

This paper presents simulation results of the density segregation in a dense binary gas fluidized bed using a multi-fluid model from Chao et al. （2011）, The segregation behavior of two types of particles with approximately same particle diameters and different particle densities was studied and validated using the experimental data from Formisani et al, （2008）. Some detailed information regarding the gas, particle velocity profiles, the distributions of the particle volume fractions and the flotsam-to-total par- ticle volume fraction ratios is presented. The simulation results show that the simulated axial average flotsam-to-total particle volume fraction ratio distribution agrees reasonably with the experimental data of Formisani et al. （2008）. The binary particle velocities are closely coupled though the segregation exists, The segregation behavior and the particle velocity profiles are superficial gas velocity dependent. The number and distribution of particle velocity vortices change dramatically with superficial gas velocity： at a comparatively low superficial gas velocity, the particles mainly segregate axially, and at a comparatively high superficial gas velocity, the particles segregate both axially and radially.

Nutritional supplements use in high-performance athletes is related with lower nutritional inadequacy from food

Background: The use of nutritional supplements(NS) among athletes is widespread. However, little is known about the relationship between nutritional adequacy and NS usage. The aims of this study were to evaluate the NS usage and to compare the nutritional intake from food and prevalence of micronutrient inadequacy(PMI) between NS users and non-users.Methods: Portuguese athletes from 13 sports completed an NS usage questionnaire and a semi-quantitative food-frequency questionnaire assessing information over the previous 12 months. The estimated average requirement cut-point method was used to calculate PMI. General linear models were used to compare nutritional intake and NS usage. Chi-squared tests and logistic regression were performed to study, respectively,relationships and associations between PMI and NS usage.Results: From the 244 athletes(66% males, 13–37 years), 64% reported NS usage. After adjustment, NS users showed a higher intake from food(p < 0.05), for at least 1 gender, for energy, and for 7 of the 17 studied nutrients. The highest PMI were seen for vitamins D and E, calcium, folate,and magnesium. After adjustment, NS users, irrespective of gender, reported lower PMI for calcium(OR = 0.28, 95%CI: 0.12–0.65), and female users for magnesium(OR = 0.06, 95%CI: 0.00–0.98).Conclusion: Athletes using NS reported a higher nutritional intake from food, and a lower PMI for several nutrients. Perhaps, those who were taking NS were probably the ones who would least benefit from it.
