Comparative study of outcomes following laparoscopic Roux-en-Y gastric bypass and sleeve gastrectomy in morbidly obese patients: A case control study

AIM To compare the impact of laparoscopic Roux-en-Y gastric bypass(LRYGB) and laparoscopic sleeve gastrectomy(LSG) on weight loss and obesity related comorbidities over two year follow-up via case control study *** Forty patients undergoing LRYGB, who completed their two year follow-up were matched with 40 patients undergoing LSG for age, gender, body mass index and presence of type 2 diabetes mellitus(T2DM). Data of these patients was retrospectively reviewed to compare the outcome in terms of weight loss and improvement in comorbidities, i.e., T2 DM, hypertension(HTN), obstructive sleep apnea syndrome(OSAS), hypothyroidism and gastroesophageal reflux disease(GERD).RESULTS percentage excess weight loss(EWL%) was similar in LRYGB and LSG groups at one year follow-up(70.5% vs 66.5%, p = 0.36) while it was significantly greater for LRYGB group after two years as compared to LSG group(76.5% vs 67.9%, p = 0.04). The complication rate after LRYGB and LSG was similar(10% vs 7.5%,p = 0.99). The median duration of T2 DM and mean number of oral hypoglycemic agents were higher in LRYGB group than LSG group(7 years vs 5 years and 2.2 vs 1.8 respectively, p provement in T2 DM, HTN, OSAS and hypothyroidism. However, GERD resolved in all patients undergoing LRYGB while it resolved in only 50% cases with LSG. Eight point three percent patients developed new-onset GERD after *** LRYGB has better outcomes in terms of weight loss two years after surgery as compared to LSG. The impact of LRYGB and LSG on T2 DM, HTN, OSAS and hypothyroidism is similar. However, LRYGB has significant resolution of GERD as compared to LSG.

RESUME Major phases in the CaO - A12O3, - SiO2 - SO3 system, normally formed are C4 A3S. C12 A7, CA, C2S and CS. Within this system to obtain C4 A3S as the major phase along with C2S, raw mixes were designed and sintered In the temperature range of 1250-1350℃. The clinkers obtained were studied by microscopic and X-Ray differaction techniques. Both these studies confirmed the presence of C4 A3S as a major phase with dicalcium silicate (C2S). properties of cement produced from these clinkers have been studied earlier. The present study involves the effect of uncombined lime in the clinkers at different levels and the influence of gypsum incorporated in the cement during cement grinding on hydraulic behaviour of the resultant cement. The properties of self levelling grouting composition made from such clinker is also reported.

Objective: To study the prevalence, type, and features of pattern dystrophy in patients with pseudoxanthoma elasticum (pXE). Methods: Asearch of the photograp hic records at theVanderbilt Eye Institute using the “angioid streaks and pseudoxanthoma elasticum”yielded 28 names. Of the 23 subjects meeting the p atient selection criteria, 22 were confirmed to have a positive diagnosis for pX E after reviewing themedical history information. The diagnosis was confirmed by the constellation of fundus findings in all 22 subjects, by a clinical examinat ion of the skin in 9, and by a skin biopsy specimen in 1. Results: pattern dystr ophy was present in 16 patients (27 eyes) of those with pXE. Fourteen patients ( 23 eyes) had fundus pulverulentus, 3 patients (5 eyes) had butterfly-shaped dys trophy,and 1 patient (2 eyes) each had fundus flavimaculatus and reticular dystr ophy. One eye of one patient developed solitary vitelliform pattern dystrophy du ring follow-up. Two patients showed progression from one pattern into another d uring follow-up. Another patient, who at first showed no evidence of pattern dy strophy in either eye, developed fundus pulverulentus in both eyes 5 years later . One patient had simultaneous evidence of 2 types: butterfly and fundus flavima culatus pattern in each eye. Angioid streaks were seen in each eye of all patien ts. peau d’orange was noted in 18 patients, optic nerve drusen in 5, and retina l crystalline bodies in 9. Choroidal neovascular membrane was present in 15 pati ents. Conclusions: All 5 varieties of pattern dystrophy, 2 ofwhichwere not previ ously associatedwith pXE, were seen in patients with pXE. Fluorescein angiogram was useful in delineating the type and extent of pattern dystrophy.

The bio-mass(Food, Fodder and Forestry, etc. ) in a habitat has beencontinuously depleting due to rapid industrialization and over-population leadingto various kinds of undesirable effects on our environment and on land masscausing deep concern to planners and management experts. In this paper, therefore, a mathematical model for degradation of bio-massis presented to study the effects of industrialization and associated growth ofpopulation, It is shown that with the increase in the levels of these factors,the bio-mass resource may not last long. It has also been pointed out mathematicallythat the level of the biomass can be maintained at desired equilibrium level bya suitable afforestation programme.

Heterotopic ossification(HO)is a debilitating condition characterized by the pathologic formation of ectopic *** occurs commonly following orthopedic surgeries,burns,and neurologic *** surgical excision may provide palliation,the procedure is often burdened with significant intra-operative blood loss due to a more robust contribution of blood supply to the pathologic bone than to native *** on these clinical observations,we set out to examine the role of vascular signaling in *** endothelial growth factor A(VEGFA)has previously been shown to be a crucial pro-angiogenic and pro-osteogenic cue during normal bone development and *** findings,using a validated mouse model of HO,demonstrate that HO lesions are highly vascular,and that VEGFA is critical to ectopic bone formation,despite lacking a contribution of endothelial cells within the developing anlagen.

Relationship between acrosin activity of human spermatozoa and oxidative stress

Aim: To study the association between seminal oxidative stress and human sperm acrosin activity. Methods: It is a prospective study consisting of 30 infertile men and 12 fertile normozoospermic volunteers. A full history, clinical examination and scrotal ultrasound were done to exclude other related factors such as smoking and varicocele. presence of white blood cells (WBCs) in semen samples was evaluated by peroxidase staining. Lipid peroxidation in spermatozoa was induced after incubating with ferrous sulphate (4 mmol/L) and sodium ascorbate (20 mmol/L). Induced peroxidation of spermatozoa was assessed by determining the production of thiobarbituric acid reactive substances (TBARS). Acrosin activity was measured using the gelatinolysis technique. The halo diameters around the sperm heads and the percentages of spermatozoa showing halo formation were evaluated. An acrosin activity index was calculated by multiplying the halo diameter by the halo formation rate. Results: A significant difference was observed in acrosin activity parameters and TBARS levels between samples with WBCs (>1×106/mL of ejaculate) and those without. This difference was also noted between the normozoospermic and the oligoasthenoteratozoospermic semen samples. The TBARS production by spermatozoa had a significant negative correlation with the acrosin activity index (r = -0.89, p presence of oxidative stress in an individual with leukocytospermia and/or abnormal semen parameters is associated with impaired sperm function as measured by its acrosin activity.

Involvement of adenosine and standardization of aqueous extract of garlic(Allium sativum Linn.)on cardioprotective and cardiodepressant properties in ischemic preconditioning and myocardial ischemia-reperfusion induced cardiac injury

The present study investigated the effect of garlic （Allium sativum Linn.） aqueous extracts on ischemic pre- conditioning and ischemia-reperfusion induced cardiac injury, as well as adenosine involvement in ischemic pre- conditioning and garlic extract induced cardioprotection. A model of ischemia-reperfusion injury was established using Langendorff apparatus. Aqueous extract of garlic dose was standardized （0.5%, 0.4%, 0.3%, 0.2%, 0.1%, 0.07%, 0.05%, 0.03%, 0,01%）, and the 0.05% dose was found to be the most effective. Higher doses （more than 0.05%） were highly toxic, causing arrhythmia and cardiodepression, whereas the lower doses were ineffective. Garlic exaggerated the cardioprotective effect of ischemic preconditioning. The cardioprotective effect of ischemic preconditioning and garlic cardioprotection was significantly attenuated by theophylline （1,000 ~tmol/L） and 8-SpT （10 mg/kg, i.p.） and expressed by increased myocardial infarct size, increased LDH level, and reduced nitrite and adenosine levels. These findings suggest that adenosine is involved in the pharmacological and molecular mechanism of garlic induced cardioprotection and mediated by the modulation of nitric oxide.

peanut （Arachis hypogaea; 2n = 4x = 40） is a nutritious food and a good source of vitamins, minerals, and healthy fats. Expansion of genetic and genomic resources for genetic enhancement of cultivated peanut has gained momentum from the sequenced genomes of the diploid ancestors of cultivated peanut. To facil- itate high-throughput genotyping of Arachis species, 20 genotypes were re-sequenced and genome-wide single nucleotide poiymorphisms （SNps） were selected to develop a large-scale SNp genotyping array. For flexibility in genotyping applications, SNps polymorphic between tetraploid and diploid species were included for use in cultivated and interspecific populations. A set of 384 accessions was used to test the array resulting in 54 564 markers that produced high-quality polymorphic clusters between diploid species, 47 116 polymorphic markers between cultivated and interspecific hybrids, and 15 897 polymorphic markers within A. hypogaea germplasm. An additional 1193 markers were identified that illuminated genomic re- gions exhibiting tetrasomic recombination. Furthermore, a set of elite cultivars that make up the pedigree of US runner germplasm were genotyped and used to identify genomic regions that have undergone pos- itive selection. These observations provide key insights on the inclusion of new genetic diversity in culti- vated peanut and will inform the development of high-resolution mapping populations. Due to its efficiency, scope, and flexibility, the newly developed SNp array will be very useful for further genetic and breeding applications in Arachis.
