The use of bilateral internal thoracic arteries to treat coronary artery disease is very less despite of improved long-term survival. In this case report, a patient presented with TVD has been managed with anaortic off-pump complete arterial revascularization (OPCABG) by using composite LIMA RIMA Y Sequential 5 grafts to achieve complete arterial revascularization successfully. This type of operative procedure is technically difficult in a resource poor country like Bangladesh. However, this has been done by a group of young cardiac surgeon. During follow up,yle="font-family:""> the patient is in a good condition.

Several different pharmacological effects have been described for Nigella sativa(Siah-Daneh) ,including an anti-inflammatory *** the present study,the effect of the extract of *** on lung pathology and blood interleukin-4(IL-4) and interferon-γ(IFN-γ) of sensitized guinea pigs was *** groups(n=8 for each group) of guinea pigs sensitized to ovalbumin(OA) were given drinking water alone,and drinking water containing low and high concentrations of the plant extract,*** animals of the control group(n=8) were treated with saline instead of OA and were given drinking *** pathological changes of the lung,including infiltration of eosinophils and lymphocytes,local epithelial necrosis,the presence of oedema,thickening of the basement membrane,smooth muscle layer hypertrophy,mucosal secretion,and the presence of mucosal plug,and blood IL-4 and IFN-γof sensi-tized guinea pigs were *** lungs of the sensitized group showed significant pathological changes(Py) .Treatment of sensitized animals with the extract led to a significant decrease in pathological changes of the lung(P<0.01 to P<0.001) ,except for the oedema in the sensitized group treated with low concentration of the extract,but an increased IFN-γ.These results confirm a preventive effect of *** extract on lung inflammation of sensitized guinea pigs.

Carotid endarterectomy is a well-established treatment for preventing stroke in selected patients. Although there is debate over whether patch angioplasty or primary closure should be used to reconstruct the bifurcation after carotid endarterectomy, there is growing evidence in the literature in favor of patch angioplasty. When compared to primary closure, patch angioplasty during conventional carotid endarterectomy is suggested to lower the incidence of restenosis and recurrent ipsilateral stroke. Various materials have been used as a patch in this procedure, including the saphenous vein, synthetic patches, or less frequently, an internal jugular vein patch where extensive narrowing of the internal carotid artery is evident. In our case, we used an internal jugular vein graft after inadvertent severing the internal carotid artery (ICA) during carotid endarterectomy after the failure of reconstruction with a saphenous vein patch. We also encountered immediate postoperative reactionary hemorrhage following anesthetic reversal, necessitating an urgent re-exploration. The purpose of this case report is neither an attempt to suggest all patients need angioplasty nor to state that an internal jugular vein patch or graft is superior to synthetic material or saphenous veins;rather, it is an attempt to emphasize a potentially effective rescue way to reconstruct inadvertent extensive vascular injury during carotid endarterectomy.

This work developed models to identify optimal spatial distribution of emergency evacuation centers(EECs)such as schools,colleges,hospitals,and fire stations to improve flood emergency planning in the Sylhet region of northeastern *** use of location-allocation models(LAMs)for evacuation in regard to flood victims is essential to minimize disaster *** the first step,flood susceptibility maps were developed using machine learning models(MLMs),including:Levenberg-Marquardt back propagation(LM-BP)neural network and decision trees(DT)and multi-criteria decision making(MCDM)*** of the MLMs and MCDM techniques were assessed considering the area under the receiver operating characteristic(AUROC)*** approaches in a geographic information system(GIS)for four well-known LAM problems affecting emergency rescue time are proposed:maximal covering location problem(MCLP),the maximize attendance(MA),p-median problem(PMP),and the location set covering problem(LSCP).The results showed that existing EECs were not optimally distributed,and that some areas were not adequately served by EECs(i.e.,not all demand points could be reached within a 60-min travel time).We concluded that the proposed models can be used to improve planning of the distribution of EECs,and that application of the models could contribute to reducing human casualties,property losses,and improve emergency operation.

The sol-gel auto-combustion method was adopted to synthesize nanomaterials of single-phase X-type hexagonal ferrites with the composition of Sr;Gd;Ni;Fe;Cd;O;（x=0.00,0.02,0.04,0.06,0.08,0.10 and y = 0,0.1,0.2,0.3,0.4,0.5）.The structural properties were carried out by XRD analysis and the lattice parameters show variation with the doping of *** average particle size measured by TEM was in the range of 8-10 nm which is beneficial in obtaining suitable signal-to-noise ratio in recording media and biomedical *** room temperature resistivity enhanced with the increase of the dopant *** increase in resistivity indicates that the synthesized materials can be considered good for the formation of the multilayer chip inductors（MLCIs） as well as for the reduction of eddy current *** dielectric constant decreased with the increase in the frequency which is the general reported trend of the hexagonal ferrites and can be explained on the basis of koop’s theory and Maxwell-Wagner *** abnormal dielectric behavior indicates the formation of small polarons in the *** maximum value of tangent loss at low frequencies reflects the application of these materials in medium frequency devices（MF）.

Bangladesh experiences frequent hydro-climatic disasters such as *** disasters are believed to be associated with land use changes and climate ***,identifying the factors that lead to flooding is *** study mapped flood susceptibility in the northeast region of Bangladesh using Bayesian regularization back propagation(BRBP)neural network,classification and regression trees(CART),a statistical model(STM)using the evidence belief function(EBF),and their ensemble models(EMs)for three time periods(2000,2014,and 2017).The accuracy of machine learning algorithms(MLAs),STM,and EMs were assessed by considering the area under the curve—receiver operating characteristic(AUC-ROC).Evaluation of the accuracy levels of the aforementioned algorithms revealed that EM4(BRBP-CART-EBF)outperformed(AUC>90%)standalone and other ensemble models for the three time periods ***,this study investigated the relationships among land cover change(LCC),population growth(PG),road density(RD),and relative change of flooding(RCF)areas for the period between 2000 and *** results showed that areas with very high susceptibility to flooding increased by 19.72%between 2000 and 2017,while the PG rate increased by 51.68%over the same *** Pearson correlation coefficient for RCF and RD was calculated to be *** findings highlight the significant association between floods and causative *** study findings could be valuable to policymakers and resource managers as they can lead to improvements in flood management and reduction in flood damage and risks.

Gastric cancer(GC)is one of the most common cancers and ranks the third in cancer mortality all over the *** goal of this study was to identify potential hub-genes,highlighting their functions,signaling pathways,and candidate drugs for the treatment of GC *** used publicly available next generation sequencing(NGS)data to identify differentially expressed(DE)*** top DE genes were mapped to STRING database to construct the protein-protein interaction(PPI)network and top hub genes were selected for further *** found a total of 1555 DE genes with 870 upregulated and 685 downregulated genes in *** selected the top 400(200 upregulated and 200 downregulated)genes to construct a PPI network and extracted the top 15 hub *** gene ontology(GO)term and kyoto encyclopedia of genes and genomes(kEGG)pathway enrichment analyses of the 15 hub genes exposed some important functions and signaling pathways that were significantly associated with GC *** survival analysis of the hub genes disclosed that the lower expressions of the three hub genes CDH2,COL4A1,and COL5A2 were associated with better survival of GC *** three genes might be the candidate biomarkers for the diagnosis and treatment of ***,we considered 3 key proteins(genomic biomarkers)(COL4A1,CDH2,and CO5A2)as the drug target proteins(receptors),performed their docking analysis with the 102 meta-drug agents,and found Everolimus,Docetaxel,Lanreotide,Venetoclax,Temsirolimus,and Nilotinib as the top ranked 6 candidate drugs with respect to our proposed target proteins for the treatment against GC ***,the proposed drugs might play vital role for the treatment against GC patients.

Effect of diluted povidone iodine in adenoviral keratoconjunctivitis on the rate of subepithelial corneal infiltrates

AIM: To evaluate the clinical characteristics of adenoviral keratoconjunctivitis, the management modalities, as well as the incidence of subepithelial corneal infiltrates(SEI).METHODS: Patients with characteristic clinical symptoms and signs, who presented to our clinic within the first week of symptoms and received the diagnosis of adenoviral keratoconjunctivitis between January 2013 and April 2016, were included in the study. A total of 211 patients were included in the study. Patients were evaluated for the incidence of clinical signs, late complications, management preferences, and the effect of diluted povidone-iodine(d-PVP-I) 2%. RESULTS: Patients’ mean age was 33.03±14.76 y. We observed an increase in the number of cases according to the years. At presentation and/or early follow-up, the clinical signs were conjunctival hyperemia(100%), conjunctival follicules(79.1%), edema of the eyelids(39.3%), chemosis(16.1%), pseudomembrane formation(16.6%), and corneal epitheliopathy(29.9%). During late follow-up 13.3% patients developed conjunctival subepithelial fibrosis, and 39.8% developed SEI. A significant decrease in the incidence of SEI development was observed in patients who used d-PVP-I 2%(P=0.032;33.3% vs 45.9%, respectively in patients who received d-PVP-I 2% and who did not).CONCLUSION: Adenoviral keratoconjunctivitis has a tremendous effect on patient’s comfort and abilities in short-term. Additionally, almost half of the patients develop visual problems related to SEI. According to our clinical experience, using d-PVP-I 2% in the first days of adenoviral keratoconjunctivitis might be helpful in reducing the risk of SEI as a complication.
