Introduction: Coblation® is a low temperature-controlled tissue ablation technique widely used in human surgery. Its development in otorhinolaryngology has allowed a spectacular advance in the management of diseases of the nasal and oral cavities. The objective of our work was to study the indications for radiofrequency in ENT, to describe the technical aspects and to evaluate the results of his practice at the HMO. Patients and Methods: This is a retrospective study carried out in the ENT and Head and Neck Surgery department of the Ouakam Military Hospital (MHO) over a period of 3 years (from January 2015 to the end of December 2017), involving 37 patients who received radiofrequency treatment in the ENT sphere. Results: The study was carried out on 37 patients. The sex ratio of 0.19. The mean age of the patients was 33.3 years. The indications were dominated by chronic caseous cryptic tonsillitis with or without halitosis with 24 cases (64.9%) followed by recurrent tonsillitis in 5 cases (13.5%) then we have obstructive hypertrophy of the palatal tonsils with syndrome sleep apnea (OSAS) in 4 cases (10.8%) followed by nasal obstruction (hypertrophy of the inferior turbinates) and hypertrophy of the soft palate and uvula in 2 cases (5.4%). The complication rate was 8.1% and dominated by transient dysgeusia in 1 case or 2.7%, uvula edema in 1 case or 2.7% and bleeding in 1 case or 2.7%. The postoperative follow-up was done one week, 1 month, 3 months, 6 months and 12 months after radiofrequency. Volumetric reduction begins to set in from 1 month (62.5%) and becomes maximum in 3 months (74.3%). Conclusion: Coblation® is a controlled tissue ablation technique at low temperature, patented by ArthroCare® Corporation in 1997. Very widespread in human surgery. Natural convection heat transfer in open or closed cavities takes place in different engineering areas. The hemispherical cavity is a part of basic geometries although it is not widely studied. The present paper reports the numerical study of natural convection in a closed hemispherical annulus delimited by two vertically eccentric hemispheres filled with Newtonian fluid (air in this case with Pr = 0.7) is conducted. The inner hemisphere is heated by a heat flux of constant density and the outer one is maintained isothermal. Based on the Boussinesq assumptions, the governing equations are numerically studied using unsteady natural convection formulated with vorticity and stream-function variables. These equations are written by using bispherical coordinates system and solved by using a finite difference method. The effect of the control parameters such as the Rayleigh number (103 ≤ Ra ≤ 106) or the eccentricity (e = ±0.2, ±0.5, 0) in the dynamic and thermal behaviours of the fluid is investigated.

Introduction: Childhood heart disease consists of congenital and acquired heart disease. The aim of this study was to describe the epidemiological profile and the clinical and aetiological characteristics of childhood heart disease at the H?pital de la Paix in Ziguinchor. Method: This was a retrospective study from 1 January 2016 to 1 June 2020 in the cardiology and paediatrics departments of the H?pital de la Paix in Ziguinchor. We included in the study all patients aged 0 - 18 years, hospitalised with heart disease confirmed by cardiac ultrasound. Results: In total, we included 57 patients in the study. The hospital prevalence was 1.6%. The average age was 67.85 ± 65 months. The predominance was male with a sex ratio of 0.96. The majority of mothers (50%) were between 15 and 30 years old. In our study, 8.85% of patients had a history of previous angina. Trisomy 21 was present in 12.35% of our patients. Severe acute malnutrition was found in 21.05% of cases. Lower limb oedema (82.4%) and dyspnoea (59.6%) were the most frequent clinical signs. On cardiac ultrasound, 52.6% of patients had congenital heart disease and 47.4% of patients had acquired heart disease. Ventricular septal defect (33.33%) was the most common congenital heart disease. Among the acquired heart diseases, mitral insufficiency predominated with 70.8% of cases. Infective endocarditis was the main complication observed. Management was mainly drug-based with diuretics and ACE inhibitors. No patient had undergone surgery. The in-hospital evolution was favourable in 85.9% of cases and the mortality rate was 10.5%. Conclusion: Our study showed that congenital heart disease was more frequent than acquired heart disease and that management, particularly surgery, was lacking.

目的:中医学的发展最早可追溯到远古时期,其理论成果、临床实践可见一斑。相火作为早期的理论产物被历代各大名家不断丰富之下,相火及其相关理论有着更为深刻、广阔的内涵。就在各位医家扩充的过程,百家争鸣,各自赋予其具有个人的特色的理论和涵义。因此,本课题的目的就是通过对相火的相关文献进行探讨,对相火的源流进行系统的梳理,总结归纳相火的根源、流支以及其相关理论的发展与运用,以期能更好地指导临床实践。方法:本文以时间为线索,以古今关于相火、其相关理论以及与临床各科的联系的各类文献为研究主体,采用历史学研究方法、发生学研究方法、文献学研究方法以及逻辑学研究方法,进行系统地整理、分析、归纳、总结。从“相火”的出现,再到代表医家对其的见解,从共性到个性,从联系到区别,试图对相火进行探讨,总结相火的演变与传承之路,进而对其源流有更为系统、深刻的认识与了解,对临床有着更进一步的指导意义。结果:1.相火之“源”:“相火”最早见于《黄帝内经》,“相火理论”始于金代医家刘完素。2.相火之“流”:金元时期为相火理论的鼎盛时期,朱丹溪从理学角度论述相火的生理与病理变化,此时相火理论体系已基本完善,张景岳认为君相皆为人体正气,相火源于命门。明清时期李时珍与赵献可等人对朱丹溪的相火理论进行发展,对临床应用进行补充说明。3.相火理论的核心内涵:相火由元气化生,由命门所主,寄居于肝肾,以心包络为充,以三焦为通道,起到温养全身脏腑、四肢百骸、经络诸窍的作用,促进推动人体生命的正常有序进行。4.相火理论的应用规律:相火理论用于疾病论治可分为外感相火疾病与内伤相火疾病两大类进行规律探讨。相火外感疾病多用正治之法,相火内伤疾病多用从治之法。结论:中医相火理论历经各朝各代医家,通过其临床实践、理论推理、归纳总结,形成了较为成熟的相火理论体系。本课题以时间为轴,一一呈现相火的起源、相火理论的发生、发展以及形成,并与临床实践相结合,展示相火理论子在临床各科的应用及其规律。初步较为系统地构建相火理论的框架体系,充分展现了相火理论的发展的深层规律。通过课题研究,对于探索中医相火理论的“源”与“流”,进一步完善了中医基础理论的基本体系,并且对于临床实践的指导有着更深刻的意义。

In this study, a method for determining the intrinsic recombination velocity at the junction of a silicon solar cell is presented. The expression of intrinsic recombination velocity at the junction was established under irradiation in frequency modulation. Based on this expression, an electrical model of the intrinsic recombination velocity at the junction is presented.

Aim: We endeavored to describe the epidemiological profile of prostatic cancer, and to evaluate its diagnostic and therapeutic aspects. Method: We conducted a descriptive retrospective study on the conformational radiotherapy of prostatic adenocarcinoma at the Dalal Jamm University Hospital, Senegal from June 2018 to December 2019. We included 62 consecutive patients. The average age of the patients was 68.9 years. The average consultation time was 7.6 months. Results: Symptom manifestation and high PSA triggered the diagnosis of cancer in 74.2% and 25.8% of patients, respectively. Symptoms: pollakiuria (72.3%), bone pain (8%) and erectile dysfunction (4.8%). The digital rectal examination: normal (33.8%), nodular (30.6%), induration (24.1%), and shielding (11.3%). The mean PSA level was 90.6 ng/ml. Histology: adenocarcinoma was the most common (98.6%). The Gleason score: 7 (19.4%). The majority of patients were in the high-risk group (70.9%) and 7 patients (11.2%) were metastatic at diagnosis. Therapy: first radical prostatectomy (20.9%), first-line curative radiotherapy (67.8%), adjuvant (21%) and palliative radiotherapy in 7 patients (11.2%): patients having received palliative radiotherapy had an estimated decline in symptoms of 80%. Hormone therapy was performed in 88.8% of patients, with average duration of 12.5 months. After a mean follow-up of 15 months, 59 patients were alive, including 45 cases (81.8%) in complete remission;3 patients with metastasis at the first visit (having received palliative radiotherapy) died. The aim of this study is to show the influence of the wavelength on the electrical parameters of vertical parallel junction silicon solar cell by use of impedance spectroscopy technique. The Bode diagrams of the diffusion capacitance are shown for different wavelengths while the solar cell operates under both open circuit and short circuit conditions. The Nyquist diagram of the diffusion capacitance has shown the extension of the space charge region for various wavelengths.

Introduction: Strokes are becoming more common in Africa, they are often accompanied by depressive symptoms, hence the need to investigate the factors associated with it for better management and prevention. Methodology: We conducted a prospective cross-sectional study amongst patients hospitalized with stroke at the Neurology/Neuroscience Clinic of the FANN UNHC (Dakar) in 2016. After a clinical diagnosis, patients are subjected to socio-demographic questionnaire and were administered with modified Rankin and MADRS scores. Results: Among the patients, 33.7% had Post-Stroke Depression, most were aged 50 to 70 years, male and married. History of stroke and depression was strongly associated with Post-Stroke Depression, while functional or cognitive impairment caused by stroke contributed to the development of Post Stroke depression. The type of ischemic or haemorrhagic stroke did not appear to affect the occurrence of the Post-Stroke Depression, but Post-Stroke Depression was more frequent in subjects with a left hemispheric lesion 47.8% (p Discussion: The occurrence of a Post-Stroke Depression is a factor of poor prognosis, early diagnosis and holistic care lead to improved quality of life.

Rhabdomyosarcoma (RMS) belongs to the soft tissue sarcomas that have striated muscle differentiation. It is most common in young children under 5 years of age and adolescents. The definite diagnosis of rhabdomyosarcoma is based on histology and the positivity of striated muscle markers in immunohistochemistry. Our objective was to describe the prognostic characteristics of RMS diagnosed in Dakar. MATERIAL AND METHODS: This was a retrospective and descriptive study spread over nine (09) years from 1 January 2011 to 31 December 2019. It was conducted from the histological report archives of the anatomy and pathology laboratories of Dakar. Paraffin blocks were reread and immunohistochemically studied by manual method. Antidesmin and antimyogenin antibodies were used. RESULTS: We collected 44 patients with rhabdomyosarcoma out of 228 cases of soft tissue cancers, i.e. 19.29% of all malignant soft tissue tumours. The mean age of the patients was 25.41 ± 23.95 (standard deviation) years with a median age of 16 years and extremes of 6 months and 81 years. Patients were less than or equal to 17 years of age in 61.4% of cases. A clear male predominance was observed with 29 men (65.9%) against 15 women (34.1%). Rhabdomyosarcomas were located in the limbs in 34.1% of cases, in the head and neck in 29.5% of cases and in the genitourinary tract in 25% of cases. The average tumour size was 7.45 cm ± 4.64 (standard deviation) with a minimum of 2 cm and a maximum of 17 cm. It was greater than or equal to 5 cm in 15 patients (68.2%). Embryonal rhabdomyosarcoma (ERMS) was the most frequent histological type with 34 cases (77.3%), followed by pleomorphic rhabdomyosarcoma 7 cases (15.9%) and alveolar rhabdomyosarcoma 3 cases (6.8%). The histological subtypes of ERMS consisted of conventional ERMS (91.18%);botryoid RMS (5.88%) and spindle cell RMS (2.94%).
