Can we predict the incidence of high-grade Clavien-Dindo complications in patients with forgotten encrusted stents undergoing endourologic management?

Objective:Ureteral stents are customarily inserted to facilitate urinary drainage,but they come with their own glitches of being forgotten and/or encrusted leading to serious *** present study aimed to report the complications in patients with forgotten and encrusted stents according to the Clavien-Dindo system specific to urological procedures and identify the factors leading to high-grade(Clavien-Dindo Grade 4A or above)***:The hospital records of patients with forgotten encrusted double-J stents over a period of 8 years were *** parameters recorded included patient demographics,indwelling time,need for percutaneous nephrostomy,hemodialysis,urine culture,blood culture,total blood counts,serum creatinine,radiologic findings,management techniques,number of surgical interventions,modified Clavien-Dindo complications,follow-up,and mortality,if ***:Forty patients were included in the *** median age was 52(range 6-85)*** the total,25(62.5%)patients had a“significant”stent load;31(77.5%)had renal failure or acute kidney injury on presentation;19(47.5%)patients had sepsis at *** the patients presented with sepsis,11(57.9%)patients demonstrated a positive urine culture;and 7/11(63.6%)patients exhibited pan-resistant *** out of 40(30.0%)patients in our series developed high-grade Clavien-Dindo *** univariate analysis,sepsis at presentation(p=0.007),stent load(p=0.031),diabetes(p=0.023),positive urine culture(p=0.007),and stent indwelling time of more than 1 year(p=0.031)were found to be *** multivariate logistic regression analysis,sepsis at presentation(p=0.017)and positive urine culture(p=0.016)were significant predictors for high-grade ***:It is prudent to identify specific risk factors,namely sepsis at presentation and positive urine culture to triage and optimize these patients before surgical management.

The use of clay minerals in the synthesis of nanosized noble metal particles to give increased catalytic activity was investigated. Nanosized platinum and ruthenium catalysts intercalated in clay (montmorillonite/hectorite) were synthesised and their catalytic activity was evaluated for the selective hydrogenation of three different α,β-unsaturated aldehydes, namely, crotonaldehyde, cinnamaldehyde, and citral, in a gas phase microreactor. The metal nano-sol was prepared by the chemical reduction of its precursor by the micellar technique in the presence of cetyl trimethyl ammonium bromide (CTAB), and the micelle stabilized metal particles were intercalated in the clay mineral by ion exchange. TEM micrographs of the catalyst particles showed that the metal particles were in the nanometre range. The average diameters of the particles were 1-25 nm. The effects of temperature, metal loading, and hydrogen flow rate on the catalytic activity and selectivity for α,β- unsaturated alcohol were studied. The results were correlated with the structural properties of the catalysts. The products formed in each reaction over the different catalysts showed that the catalysts were very active for hydrogenation, and the selectivity for the desired product, namely, α,β-unsaturated alcohol, was good. The metal catalysts intercalated in montmorillonite showed better selectivity than that in hectorite because of its higher acidity. Increased selectivity for α,β-unsaturated alcohol was observed with increased flow rate of hydrogen.

ODFF: Optimized Dual Fuzzy Flow Controller Based Voltage Sag Compensation for SMES-Based DVR in Power Quality Applications

The booming electronics itself carries an impact on power quality. Superconducting Magnetic Energy Storage (SMES) is proposed to enhance power quality in three-phase systems under various loads. This paper aimed to compensate the voltage sags under various faults in the grid systems. The SMES is selected as an energy storage unit to improve the capability of voltage sag compensation. Optimized Dual Fuzzy Flow (ODFF) logic controller is designed to prevent the voltage sag time during excessive phase voltage variation. Hence the proposed controller strategy reduces the total harmonic distortion during various fault conditions. To regulate the contribution of active power, the least possible value is improved using ODFF. The depth of voltage sags compensation is achieved by the over modulation and an iterative loop is designed in the control block. While protecting sensitive loads from voltage disturbances, and sags initiated by the power system, the proposed configuration is advantageous for an industrial implementation. It is found that the proposed method can result in more than 50% additional sag support time when compared with the previous methods such as PI and PSO. Utilizing MATLAB Simulink, compensation of sag and minimization of THD is established, and the simulation tests are performed to evaluate the performance of the proposed control method.

Modeling and Implementation of AC Electrical Capacitance Tomography

Electrical Capacitance Tomography (ECT) determines the dielectric permittivity of the interior object depending on the measurements of exterior capacitance. Generally, the electrodes are placed outside the PVC cylinder where the medium to be imaged is present;but in ECT using inter-electrode capacitance measurements can be achieved by placing inside of the dielectric medium. In the proposed ECT system, the ECT sensor is modeled using ANSYS software and the model is implemented in real ECT system. For each step of measurement, a stable AC signal is applied to a pair of electrodes that form a capacitor. The novel system is to measure the capacitance range variation in picofarad and the corresponding voltage ranges from 1 volt to 4 volts. The switching speed of all combinational electrodes is implemented using embedded system to achieve higher speed performance of AC ECT system which eliminates the drift and stray capacitance error. This is yielding the original image of unknown multiphase medium inside the electrodes using Lab VIEW. This paper investigates several advantages such as improved overall system performance;simple structure, avoids stray capacitance effect, reduces the drift problem and achieves high signal to noise ratio.

Optimization Studies in Simultaneous Saccharification and Fermentation of Wheat Bran Flour into Ethanol

The effects of process variables in Simultaneous Saccharification and Fermentation (SSF) of wheat bran flour were studied in bulk fermentation using a coculture of Aspergillus niger - Kluveromyces marxianus. The effect of substrate density, pH, temperature, and enzyme concentration on wheat bran was predicted by designing experiments in which a single parameter is varied keeping other variables at a constant level. The above parameters were optimized for a batch culture in a fermentor. Optimal values for substrate concentration, pH, temperature, and enzyme concentration during processing were 200 g/l, 5.5, 65°C, and 7.5 IU, respectively. In pre-treatment experiments, the concentration of enzymes and the pre-treatment temperature are highly correlated. The influence of pH, temperature, and substrate density on ethanol production was investigated. Temperature pH was determined as optimal, 32°C and 5.5, respectively. After 48 hours of fermentation at optimum pH, a solution of wheat bran containing a maximum of 6% starch produces a maximum of 22.9 g/l ethanol.

Design of an Efficient Binary Vedic Multiplier for High Speed Applications Using Vedic Mathematics with Bit Reduction Technique

Vedic mathematics is the system of mathematics followed in ancient Indian and it is applied in various mathematical branches. The word “Vedic” represents the storehouse of all knowledge. Because using Vedic Mathematics, the arithmetical problems are solved easily. The mathematical algorithms are formed from 16 sutras and 13 up-sutras. But there are some limitations in each sutra. Here, two sutras Nikhilam sutra and Karatsuba algorithm are considered. In this research paper, a novel algorithm for binary multiplication based on Vedic mathematics is designed using bit reduction technique. Though Nikhilam sutra is used for multiplication, it is not used in all applications. Because it is special in multiplication. The remainder is derived from this sutra by reducing the remainder bit size to N-2 bit. Here, the number of bits of the remainder is constantly maintained as N-2 bits. By using Karatsuba algorithm, the overall structure of the multiplier is designed. Unlike the conventional Karatsuba algorithm, the proposed algorithm requires only one multiplier with N-2 bits only. The speed of the proposed algorithm is improved with balancing the area and the power. Even though there is a deviation in lower order bits, this method shows larger difference in higher bit lengths.

Interfacial engineering of two-dimensional(2D)monometallic phosphides enables remarkable structural and electrochemical properties in energy storage ***,2D nanosheets(NSs)of FeP_(2)/Co_(2) P were grown on Ni-foam(FCP)using a solution-based and phosphorization approach to be used as freestanding for high-performance energy storage *** effective phosphorization strategy is successfully de-veloped to improve the overall crystalline phase,tailor the morphology,and boost the electrochemical performances of *** FCP NSs electrode exhibits a battery-type redox behavior with a maxi-mum high areal capacity of 1.96 C cm^(-2) at 4 mA cm^(-2) in 6 M KOH aqueous electrolyte compared to the other *** superior electrochemical performance was achieved by increasing the electroac-tive sites and high conductivity via surface tailoring and fast redox ***,a supercapattery was assembled utilizing FCP and activated carbon(AC)electrodes and it revealed maximum specific en-ergy(E_(s))and specific power(P_(s))of 41.2 Wh kg^(-1) and 7578 W kg^(-1) with good cycling stability of 91%after 10,000 cycles at 5 A g^(-1).Eventually,the supercapattery has been explored in practical applications by lighting up light-emitting diodes(LEDs),representing the real-time performance of superior energy storage devices.

Structure-Function Analysis of Arabidopsis thaliana Histidine Kinase AHK5 Bound to Its Cognate Phosphotransfer Protein AHP1

The multi-step phosphorelay （MSP） system defines a key signal transduction pathway in plants and many eukaryotes. In this system, external stimuli first lead to the activation of a histidine kinase, followed by transfer of a phosphoryl group from the receiver domain of the kinase （HKRD） to downstream, cytosolic phosphotransfer proteins （HPs）. In order to establish the determinants of specificity for this signaling relay system, we have solved the first crystal structure of a plant HKRD, AHK5RD, in complex with one of its cognate HPs, AHP1. AHP1 binds AHK5RD via a prominent hydrogen bond docking ridge and a hydrophobic patch. These features are conserved among all AHP proteins, but differ significantly from other structurally characterized prokaryotic and eukaryotic HPs. Surface plasmon resonance experiments show that AHK5RD binds to AHP1-3 with similar, micromolar affinity, consistent with the transient nature of this signaling complex. Our correlation of structural and functional data provide the first insight, at the atomic level as well as with quantitative affinity data, into the molecular recognition events governing the MSP in plants.
