Impact of adjuvant chemotherapy delay on survival in cancer breast patients

Objective： The proper time to commence adjuvant chemotherapy after primary surgery for breast cancer is unknown. It is usually prescribed within 2-3 months after definitive surgery. The aim of this retrospective study was to assess the impact of adjuvant chemotherapy （CT） delay beyond 3 weeks （ 21 days） in premenopausal patients with ER-absent tumors being treated for early stages breast cancer on overall survival （OS） and disease-free survival （DFS）. methods： This retrospective study was conducted through revision of medical records of premenopausal patients diagnosed with early stage ｜-｜IIA breast cancer and ER-absent tumors who received adjuvant CT after definitive surgery at the Department of Clinical Oncology, Ain-Shams University Hospitals. Results： Between 2005 and 2008, 105 patients were retrospectively analyzed and included. Patients were divided into 2 groups： Group A including 48 patients who started adjuvant CT 〈 21 days of surgery and group B which included 57 patients who had CT delay 〉 21 days. Both groups were matched demographically. Comparisons of overall survival, and disease-free survival between group A and group B patients all favored group A. At 5-year the OS rates were 87% and 73% for groups A and B respectively （P = 0.001）, while DFS rates were 85% and 64% in groups A and B respectively （P = 0.001）. Analysis of other prognostic factors （age, T, N, grade, HER2 status, surgery type, CT type, local radiotherapy received） were analyzed. Only nodal status predicted for worse DFS （P = 0.05） and OS （P = 0.006）. Conclusion： Delay in initiating adjuvant chemotherapy for early stage breast cancer patients with ER-absent tumors was associated with a decrease in both OS and DFS rates.

Protectivity of Freeze Dried Inactivated Rift Valley Fever Vaccine

Background: Vaccinations for animals are crucial for food production, animal welfare, public health, and animal health. They are an affordable way to stop animal sickness, increase food production efficiency, and lessen or stop the spread of zoonotic diseases to humans. Animal vaccines that are both safe and efficacious are vital to modern culture. The vaccine should induce a strong, protective and prolonged immune response against the antigenic factor. In order to achieve these goals, novel vaccination techniques and an efficient adjuvant are required to render the vaccine immunogenically protective and trigger a strong immune response. Aim: Our study aims to promote and enhance the immunogenicity against RVF virus disease through lyophilized inactivated RVF vaccine through induction of early cellular, high and prolonged humeral immunity in vaccinated animals using cabopol as stabilizer and Saponin or normal saline as a diluent at time of vaccination. moreover, manufacturing of these vaccines is easy to be done. Results: The gained results revealed that RVF freeze-dried vaccine with Carbopol that reconstituted using Saponin elicited better immune response than that reconstituted using normal saline (NaCl). The cell mediated immune response as represented by lymphocyte blastogenesis and phagocytic activity were markedly increased with high levels when we used Saponin as a diluent than that in group vaccinated with vaccine diluted with NaCl, on the other side the humeral immune response in group vaccinated using the Saponin as diluent is more detected and stayed within the protective level till the end of 11th month post vaccination (1.5 TCID50) while the immune response induced after using normal saline as a diluent stayed within the protective level till the end of 10th month post vaccination (1.8 TCID50). Conclusion: The use of Saponin as a diluent for reconstitution of the freeze dried RVF vaccine is preferable than the

Synthesis of zinc oxide nanoparticles (Zn-NPs) via green method is an outstanding alternative to conventional/regular methods; however， the safety or toxicity of the biosynthesized ZnO-NPs in vivo is not fully explored. This study was conducted to evaluate the protective efficiency of cinnamaldehyde-loaded chitosan nanoparticles (Cin@CSNPs) against oxidative damage and genotoxicity of zinc oxide nanoparticles (ZnO-NPs) in mice. ZnO-NPs were biosynthesized using the extract of fresh leaves of mentha pulegium L. Cin was extracted from cinnamon essential oil， and was loaded into chitosan nanoparticle (Cin@CSNPs). Both ZnO-NPs， Cin@CSNPs and CSNPs were characterized. The in vitro release of Cin@CSNPs was determined. In the biological study， 6 groups of male Balb/c mice were treated by gavage for 3 weeks as follows， control group， the group received ZnO-NPs (25 mg/kg b.w)， the groups received Cin@CSNPs at low dose (50 mg/kg b.w) or high dose (100 mg/kg b.w)， and the groups received ZnO-NPs plus Cin@CSNPs at the two tested doses. Blood and tissue samples were collected for different biochemical， genetical and histological studies. The particle size of ZnO-NPs， CSNPs， and Cin@CSNPs were (20.78 ± 2.6)， (170.0 ± 3.7)， and (218.23 ± 2.9) nm， and ξ-potential were (32.7 ± 4.6)， (8.32 ± 0.27) and (4.8 ± 0.21)， respectively. ZnO-NPs disturbed the biochemical and oxidative stress indices， AFP， CEA， TNF-α， chromosomal aberrations in somatic and germ cells， and sperm abnormality along with severe pathological changes in the hepatic， renal， and testicular tissues. Cin@CSNPs improved significantly all the parameters tested and the histological picture in a dose-dependent. Therefore， the biosynthesized ZnO-NPs exhibit oxidative damage and genotoxicity， and Cin@CSNPs have potential protective effects against the risks of ZnO-NPs and may be a promising tool to overcome the challenges of using Cin in food and pharmaceuticals applications.

Hygrothermal Properties of a Composite Based on Clay Soils and Diatomite

The purpose of this paper is to make a contribution to the use of diatomite as a mineral additive in the composition of compressed earth blocks. The aim is to study the influence of diatomite on the hygrothermal behaviour of composites based on clay soils. For this reason, two clay soils with different physicochemical and mineralogical compositions were incorporated with diatomite at percentages ranging from 5% to 50% with a step of 5 to produce compressed earth blocks. After assessing the hydric and thermal characteristics of the composites, it was found that the incorporation of diatomite into the clay matrix favours the absorption of water by capillary action for all the composites. The diatomite-amended blocks subjected to the rain erosion test were less eroded than the unamended blocks. In addition, BYD composites were found to be more resistant than BTD composites, due to the high percentage of clay in T soil. The thermal conductivity of the latter decreases respectively from 0.72 to 0.29 W/m∙K for BTD composites and from 0.52 to 0.21 W/m∙K for BYD composites. This reduction proves the thermal insulating properties of diatomite. Despite the high capillary absorption capacity of these composites, they have good thermal properties, enabling them to be used in the construction of buildings for improved indoor thermal comfort.

Transient elastography and diffusion-weighted magnetic resonance imaging for assessment of liver fibrosis in children with chronic hepatitis C

BACKGROUND Chronic hepatitis C(CHC)is a health burden with consequent morbidity and *** biopsy is the gold standard for evaluating fibrosis and assessing disease severity and prognostic purposes *** altern-atives for liver biopsy such as transient elastography(TE)and diffusion-weighted magnetic resonance imaging(DW-mRI)are critical *** To evaluate TE and DW-mRI as noninvasive tools for predicting liver fibrosis in children with *** This prospective cross-sectional study initially recruited 100 children with CHC virus ***-four children completed the full set of investigations including liver stiffness measurement(LSm)using TE and measurement of apparent diffusion coefficient(ADC)of the liver and spleen using *** biopsies were evaluated for fibrosis using Ishak scoring *** and liver and spleen ADC were compared in different fibrosis stages and correlation analysis was performed with histopathological findings and other laboratory *** most patients had moderate fibrosis(73.5%)while 26.5%had mild *** had severe fibrosis or *** majority(68.8%)had mild activity,while only 7.8%had moderate *** scores had a significant direct correlation with LSm(P=0.008)and were negatively correlated with both liver and spleen ADC but with no statistical significance(P=0.086 and P=0.145,respectively).Similarly,histopatho-logical activity correlated significantly with LSm(P=0.002)but not with liver or spleen ADC(P=0.84 and 0.98 respectively).LSm and liver ADC were able to significantly discriminate F3 from lower fibrosis stages(area under the curve=0.700 and 0.747,respectively)with a better performance of liver *** TE and liver ADC were helpful in predicting significant fibrosis in children with chronic hepatitis C virus infection with a better performance of liver ADC.

Central retinal artery resistive index and optical coherence tomography in assessment of glaucoma progression

AIm: To assess the relation between central retinal artery(CRA) resistive index(RI) and retinal nerve fiber thickness measured by optical coherence tomography(OCT) in assessment of disease progress in cases of open angle ***: Twenty-three patients with diagnosed open angle glaucoma were included in this study. They were examined by colored duplex ultrasonography of CRA with estimation of RI of CRA and ophthalmic artery(OA)with estimation of CRA/OA RI ratio as well as OCT measurement of the average retinal nerve fiber layer(RNFL) thickness in order to assess the disease ***: There was strong inverse relation between the increased RI in CRA as well as the increased CRA/OA RI ratio and the decrease in average RNFL thickness in cases of open angle ***: Assessment of CRA RI can indirectly assess the vascular changes associated with glaucoma and can assess the degree of retinal atrophy helping in evaluating prognosis thus guiding the choice of treatment.

The differential cross sections of elastic and inelastic scattering of ~3He ions on ^（13）C and ^（14）C have been studied at an energy of 37.9 meV with a double folding model based on m3Y-Reid effective nucleon-nucleon *** resulted parameters have been used for the standard Distorted Wave Born Approximation calculations of angular distributions corresponding to different excitations levels of ^（13）C and ^（14）C and deformation parameters have been deduced.

Barrett's esophagus:Prevalence and risk factors in patients with chronic GERD in Upper Egypt

AIm： To determine the prevalence and possible risk factors of Barrett＇s esophagus （BE） in patients with chronic gastroesophageal reflux disease （GERD） in EI minya and Assuit, Upper Egypt. mETHODS： One thousand consecutive patients with chronic GERD symptoms were included in the study over 2 years. They were subjected to history taking including a questionnaire for GERD symptoms, clinical examination and upper digestive tract endoscopy. Endoscopic signs suggestive of columnar-lined esophagus （CLE） were defined as mucosal tongues or an upward shift of the squamocolumnar junction. BF was diagnosed by pathological examination when specialized intestinal metaplasia was detected histologically in suspected CLE. pH was monitored in 40 patients. RESULTS： BE was present in 7.3% of patients with chronic GERD symptoms, with a mean age of 48.3 ± 8.2 years, which was significantly higher than patients with GERD without BE （37.4 ± 13.6 years）. Adenocarcinoma was detected in eight cases （0.8%）, six of them in BE patients. There was no significant difference between patients with BE and GERD regarding sex, smoking, alcohol consumption or symptoms of GERD. Patients with BE had significantly longer esophageal acid exposure time in the supine position, measured by pH monitoring. CONCLUSION： The prevalence of BE in patients with GERD who were referred for endoscopy was 7.3%. BE seems to be associated with older age and more in patients with nocturnal gastroesophageal reflux.

A 56-day feeding period was performed to investigate the possible impacts of dietary nano curcumin(0,50,100,150,and 200 ppm)on the growth,nutrient utilization,non-specific immune parameters,antioxidants in Nile tilapia under chronic low temperature(21.02±0.11◦C).Fishes(n=225;Initial weight=4.39±0.08 g/fish)were randomly stocked at 15 fish/tank for five experimental groups in *** low-temperature circumstances,dietary curcumin in nano form showed no notable alteration in growth variable,nutrient efficiency,digestive enzymes efficiency,biometric indices,survival rates,and hematological ***,the serum of fishes with nano curcumin diets under low-temperature stress displayed higher total protein as well as lower glucose,cortisol,and total cholesterol compared with the control ***,fish fed nano curcumin diets displayed higher lysozyme and bactericidal activities compared to the control group and the best performance was found at dietary nano curcumin level of≥100 ***,groups fed the basal diet demonstrated the poorest antioxidant capacity,and the best superoxide dismutase(SOD)and glutathione peroxidase(GPx)existed in fish with nano curcumin diets while the best catalase(CAT)efficiency occurred at higher nano curcumin levels≥100 *** addition,higher counts of intestinal microbiota in terms of total bacterial count(TBC),total yeast and molds count(TYmC),and coliform were noticed in fish consumed the basal diet compared to groups fed on nano curcumin *** conclusion,incorporating nano curcumin at a level of≥100 mg/kg diet(particularly at 150 mg/kg)improved a non-specific immune response,antioxidant,and healthier gastrointestinal microbiota in Nile tilapia under chronic low-temperature stress.
