New-onset diabetes after kidney transplantation:Incidence and associated factors

AIM To determine the incidence and associated factors of new-onset diabetes after transplantation(NODAT) in a Portuguese central hospital. METHODS This single-center retrospective study involved consecutive adult nondiabetic transplant recipients, who had undergone kidney transplantation between January 2012 and March 2016. NODAT was diagnosed according to the criteria of the American Diabetes Association. Data were collected from an institutional database of the Nephrology and Kidney Transplantation Department(Santa Maria Hospital, lisbon, Portugal) and augmented with data of laboratorial parameters collected from the corresponding patient electronic medical records. Exclusion criteria were preexisting diabetes mellitus, missing information and follow-up period of less than 12 mo. Data on demographic and clinical characteristics as well as anthropometric and laboratorial parameters were also collected. Patients were divided into two groups: With and without NODAT-for statistical *** A total of 156 patients received kidney transplantduring the study period, 125 of who were included in our analysis. NODAT was identified in 27.2% of the patients(n = 34; 53% female; mean age: 49.5 ± 10.8 years; median follow-up: 36.4 ± 2.5 mo). The incidence in the first year was 24.8%. The median time to diagnosis was 3.68 ± 5.7 mo after transplantation, and 76.5% of the patients developed NODAT in the first 3 mo. In the group that did not develop NODAT(n = 91), 47% were female, with mean age of 46.4 ± 13.5 years and median follow-up of 35.5 ± 1.6 mo. In the NODAT group, the pretransplant fasting plasma glucose(FPG) levels were significantly higher [101(96.1-105.7) mg/d l vs 92(91.4-95.8) mg/d l, P = 0.007] and pretransplant impaired fasting glucose(IFG) was significantly more frequent(51.5% vs 27.7%, P = 0.01). Higher pretransplant FPG levels and pretransplant IFG were found to be predictive risk factors for NODAT development [odds ratio(OR): 1.059, P = 0.003; OR: 2.77

In human immunodef iciency virus(HIV)-infected people kidney disease is as an important cause of morbidity and mortality. Clinical features of kidney damage in HIV-infected patients range from asymptomatic microalbuminuria to nephrotic syndrome. The lack of specif ic clinical features despite the presence of heavy proteinuria may mask the renal involvement. Indeed, it is important in HIV patients to monitor renal function to early discover a possible kidney injury. After the introduction of antiretroviral therapy, mortality and morbidity associated to HIV-infection have shown a substantial reduction, although a variety of side effects for longterm use of highly active antiretroviral therapy, including renal toxicity, has emerged. Among more than 20 currently available antiretroviral agents, many of them can occasionally cause reversible or irreversible nephrotoxicity. At now, three antiretroviral agents, i.e., indinavir, atazanavir and tenofovir disoproxil fumarate have a well established association with direct nephrotoxicity. This review focuses on major causes of proteinuria and other pathological f indings related to kidney disease in HIV-infected children and adolescents.

Yerba mate(Ilex paraguariensis ***.-Hil.)is a species of great economic,social and environmental importance for the southern regions of Brazil,Uruguay and *** the most diverse products are obtained from mate leaves,including mate *** objective of this study was to establish shoot meristem cultures(meristematic dome and a few primordia)of elite clones and identify the endophytic bacteria present in the *** tested the effect of clones(F1,F2,A03 and A07),culture media(MS,1/2MS,1/4MS and WPM),cytokinins(kinetin,BA and 2iP),activated charcoal(1,2 and 3 g l^-1),and disinfecting agent(sodium hypochlorite and mercuric chloride)on in vitro establishment.F1 and F2 clones were the most responsive for shoot meristem in vitro *** medium supplemented with 8.8 lM 2iP,0.2 lM NAA and 3 g l^-1 activated charcoal was the most suitable for the in vitro establishment of the F1 *** phytotoxic effect of the disinfectant was observed and some meristems *** isolated endophytic bacterium was identified for the first time in yerba mate as Agrobacterium *** conclude,we were able to establish in vitro culture of yerba mate using meristems as explants but the tissues were not free of endophytic microorganisms which could interfere with explant response.

Objective： HPP （Hypophosphatasia） is a congenital disease characterized by a deficiency of tissue-specific AlP （alkaline phosphatase）, which causes the generation of abnormal bone and tooth tissue. The clinical manifestations are variable, from neonatal forms with high mortality to milder adult forms with fractures and osteomalacia. The present study reports a clinical case of HPP, with emphasis on the physical characteristics and laboratory tests. The patient was attended at the medical genetics outpatient clinic of the Association of Parents and Friends of the Exceptional of Manaus （Associa？？o de Pais e Amigos dos Excepcionais-APAE/Manaus）. Methodology： For the case report, anamnesis, peripheral blood G-band karyotyping, Vitamin and AlP dosage and resonance examination were performed. Results： A.M.M.S, 9 years, non-consanguineous parents, no bone disease in the family, with one sister from the mother. Birth weight AGA without intercurrence. The patient presented a slight delay in psycho-motor development and was diagnosed with craniostenosis through CNS imaging examination. The physical examination, in addition to a short stature, showed dolichocephaly, frontal bossing, curved tibias and great dental compromise with many early cavities and loss of teeth. laboratory evaluation： AlP dosage： 63/l （N = 69-325）; Vitamin B6 dosage： 27.4 （5.2-34.1）. The mother and the younger sister also had an AlP result below that expected for the age. Conclusions： The clinical phenotype and laboratory diagnosis of the patient were compatible with ICD Q78 HPP.

Green macroalgae,e.g.,Ulva lactuca,are valuable bioactive sources of nutrients;but algae recalcitrant cell walls,composed of a complex cross-linked matrix of polysaccharides,can compromise their utilization as feedstuffs for monogastric *** study aimed to evaluate the ability of pre-selected Carbohydrate-Active enZymes(CAZymes)and sulfatases to degrade *** cell walls and release nutritive compounds.A databank of 199 recombinant CAZymes and sulfatases was tested in vitro for their action towards *** cell wall *** enzymes were incubated with the macroalga,either alone or in combination,to release reducing sugars and decrease fluorescence intensity of Calcofluor White stained cell *** individual action of a polysaccharide lyase family 25(Pl25),an ulvan lyase,was shown to be the most efficient in cell wall *** ulvan lyase treatment,in triplicate measures,promoted the release of 4.54 g/l(Pligosaccharides release of 11.4 and 11.2 mmol/100 g of dried alga(Ply,and a decrease of 41.7%(Pll wall fluorescence,in comparison to *** ability of ulvan lyase treatment to promote the release of nutritional compounds from alga biomass was also evaluated.A release of some monounsaturated fatty acids was observed,particularly the health beneficial 18:1c9(Please of total fatty acids(P>0.05),proteins(P?0.861)or pigments(P>0.05)was *** results highlight the capacity of a single recombinant ulvan lyase(Pl25 family)to incompletely disrupt *** cell *** enzyme could enhance the bioaccessibility of *** bioactive products with promising utilization in the feed industry.

Paperboard is an environment-friendly multi-layer material widely used for packaging applications. However, for food packaging paperboard lacks essential barrier properties towards oxygen and water vapor. Conventional solutions to enhance these barrier properties (e.g. paperboard film coating with synthetic polymers) require special manufacturing facilities and difficult the end-of-life disposal and recycling of the paperboard. Paperboard coating with silica-based formulations is an eco-friendly alternative hereby disclosed. Silica-nanocoatings were prepared by sol-gel synthesis, with or without the addition of Zn(2)-Al-NO3 layered double hydroxides (lDHs), and applied on the surface (ca 2 g/m2) of industrial paperboard samples by a roll-to-roll technique. The physicochemical features of silica-nanocoatings were studied by FTIR-ATR, SEM/EDS, XRD analysis and surface energy measurements. The barrier properties of uncoated and silica-coated paperboard were accessed by water vapor transmission rate (WVTR) and oxygen permeability (Jo2) measurements. The best barrier results were obtained for paperboard coated with a mixture of tetraethoxysilane (TEOS) and 3-aminopropyltriethoxysilane (APTES), with and without the incorporation of lDHs.
