Endometrioid adenocarcinoma arising from abdominal wall endometriosis in the appendectomy scar: A case report

We report a very rare case of endometrioid adenocarcinoma arising from abdominal wall endometriosis in the appendectomy scar. A 47-year-old woman visited the surgical department, since she had a gradually growing and painful tumor both in an appendectomy scar and at an umbilical site. She underwent appendectomy at age 18 years, and noticed the tumor at age 22 years. Partial tumor resection was performed in that department, and the pathology revealed endometrioid adenocarcinoma. She was referred to our department for radical therapy. Tumors in the both sites were dissected together with some swelling lymph nodes in our department. A pathological diagnosis of the tumor in the umbilical site showed only benign endometriosis. In contrast, the tumor in the appendectomy scar showed benign endometriosis, atypical endometriosis and well differentiated endometrioid adenocarcinoma. Resected lymph nodes also contained endometrioid adenocarcinoma, and were diagnosed as metastases. It was concluded that the endometrioid adenocarcinoma in the tumor of the appendectomy scar was a malignant transformation arising from abdominal wall endometriosis from the pathological findings. Since the operation, adjuvant and maintenance chemotherapy with paclitaxel and carboplatin had been administered for 3 years. She is free of disease 3.5 years after the operation.

Objective: To compare the result of pancreatogastrostomy (PG) with Roux-en-Y reconstruction versus pancreatojejunostomy (PJ) with conventional Traverso-type reconstruction following pylorus-preserving pancreatectomy (PP- PD) in a retrospective study. PPPD has been accepted as a radical surgical treatment for malignant periampullary neoplasms. However, the reconstruction of this surgery is technically complicated. The leakage of pancreatic juice and the delayed gastric emptying are the major complications of PPPD and may be fatal. To solve these problems, we have performed PG with Roux-en-Y anastomosis. There are several techniques proposed the reconstruction after PPPD, however there have been no previous study describing the result of PG with Roux- en-Y anastomosis. Since 2002 we have perfor- med 32 cases of PPPD. They were divided into two groups according to the reconstruction procedures: PG with Roux-en-Y reconstruction (group PG-RY) (17 cases) and pancreatojejunostomy with Traverso-type reconstruction (group PJ-T) (15 cases). Results: Patient age, gender, and underlying disease were comparable among the groups. Two groups showed no differences in intraoperative bleeding amount, and the time of surgery. The occurrence of the pancreatic leakage was significantly reduced in the group PG-RY compared with the group PJ-T. There was no case of the delayed gastric emptying in the group PGRY. Conclusions: Pancreatogastrosotmy with Roux-en-Y anastomosis can reduce the occurrence of the pancreatic leakage and delayed gastric emptying following pylorus-preserving pancreatoduodenectomy.

Recently, much interest is focused on the studies for development of new processes using the unique behavior in supercritical region of carbon dioxide (CO2). The large heat effect of binary mixtures containing supercritical CO2 is expected to the applications in heat pump and other new energy-saving processes In our previous papers, we have measured the excess molar enthalpies (HE) for several binary systems at high pressure containing supercritical CO2 with a flow isothermal microcalorimeter (IMC).

The aim of this study was to prepare diphenhydramine hydrochloride (DPH)-loaded orally fast-disintegrating mini-tablets (OFDMTs) containing either L-aspartic acid (Asp) or L-glutamic acid (Glu) as bitterness-suppressant, to characterize the prepared tablets and to evaluate their bitterness under conditions mimicking those of the oral cavity. The preparation of five formulation batches of the OFDMTs involved mixing DPH, with or without two different concentrations of Asp or Glu, and a premix containing a disintegrating agent. When all ingredients were well mixed, the mixture was directly compacted to form small (4 mm diameter) DPH-loaded OFDMTs. There were only small differences between the tablets with respect to mass, diameter, width and hardness. The disintegration times of the five formulation batches of DPH-loaded OFDMTs were measured using the OD-mate, a disintegration test apparatus in which conditions resemble those of the oral cavity. The disintegration times were all within 10 s of exposure to a medium representing the inside of the oral cavity. Rapid release profiles were observed for DPH, Asp and Glu in these dissolution tests. The taste sensor outputs of samples taken at different times (5 - 30 s) from the dissolution test solutions of the four DPH-loaded OFDMTs containing Asp or Glu were significantly inhibited compared with those of control DPH-loaded OFDMT. These results suggest that the inclusion of Asp or Glu in DPH-loaded OFDMTs is sufficient to mask bitterness in the oral cavity for the first 30 s after the tablet is placed in the mouth. It is anticipated that swallowing will have taken place within 30 s.

The Relationship between Assessment Skills and Nursing Knowledge

This research aimed to clarify the relationship between nursing students’ levels of structural knowledge and assessment skills. Participants were 17 third-year university students majoring in nursing who participated individually in the experiments. The experiments included a nursing-scene task, free-recall task, and sorting task. Experiments were conducted before and after their clinical practice. Each student’s level of structural knowledge was calculated from the free-recall and sorting task responses, and each student’s assessment skill was calculated from the nursing-scene task responses. Levels of structural knowledge were significantly higher in post-examination compared with pre-examination (p p n = 5) had significantly higher scores for their conclusions and reasons than the low-structured group (n = 5) (p p < 0.10). Well-structured knowledge of students in the high-structured group seemed to enable them to acquire and activate highly related information during assessment. Students in the high-structured group apparently made assessments not only by using information given but also by over viewing information comprehensively.

Availability of Circulating MicroRNAs as a Biomarker for Early Diagnosis of Diffuse Large B-Cell Lymphoma

Background: MicroRNA (miRNA) regulates post-transcriptional gene expression through binding to complementary sites of target messenger RNA, including that from oncogenes or tumor suppressor genes. This study planned to pursue the possibility that circulating miRNA could be used for the early diagnosis of diffuse large B-cell lymphoma (DLBCL). Materials and Methods: Expression levels of miRNA obtained from serum, exosome-enriched serum, and formalin-fixed paraffin-embedded (FFPE) tissue were evaluated. Samples were collected from patients with newly diagnosed DLBCL (n = 33) or healthy volunteers (n = 22). Based on the results of previous reports, ten miRNAs were selected and expression levels were analyzed by the quantitative real-time PCR. Results: The expression levels of hsa-miR-15a-3p, hsa-miR-21-5p, hsa-miR-181a-5p, and hsa-miR-210-5p differed significantly between DLBCL patients and controls in serum and/or exosome-enriched serum, but not in FFPE tissue. In contrast, expression levels of hsa-miR-155-5p in FFPE tissue were significantly higher in DLBCL patients, as previously reported. Conclusion: We confirmed that some miRNAs were differentially expressed in serum from DLBCL patients as previously reported. Measurement of these miRNA in exosome-enriched serum did not improve the accuracy in the differential diagnosis of DLBCL. In addition, these miRNAs seem to be produced outside of lymphoma tissue.

Background: The objective of this study was to determine the levels of tumor necrosis factor-alpha (TNF-α) and receptor activator of NF-kB ligand (RANKL) in the gingival crevicular fluid (GCF) in patients with severe root resorption after orthodontic treatment. Materials and Methods: Ten patients who had been receiving orthodontic treatment (5-control subjects and 5-severe root resorption subjects) participated in this study. GCF was collected from all patients. Subjects with severe root resorption (>1/3 of the original root length) were identified. Control group subjects with no loss of the root structure undergoing orthodontic treatment were also identified. The GCF was collected non-invasively from the mesial and distal sides of each of the upper central and lateral incisors using filter paper strips. The eluted GCF was used for a Western blot analysis with Antibodies against TNF-α and soluble RANKL (sRANKL). Ten male 6-week-old Wistar rats were subjected to orthodontic force of 50 g to induce a mesially tipping movement of the upper first molars for 7 days. The expression levels of TNF-α and RANKL proteins were determined in periodontal ligament (PDL) by immunohistochemical analysis. Results: The Western blot analysis showed that the TNF-α and sRANKL expressions were significantly higher in the severe root resorption group than in the control group. In the experimental tooth movement in vivo, resorption lacunae with multinucleated cells were observed in 50 g group. The immunoreactivity for TNF-α and RANKL was detected in PDL tissue subjected to the orthodontic force on day 7. Conclusion: These results suggest that TNF-α and RANKL play important roles in inducing or facilitating the development of orthodontically-induced inflammatory root resorption (OIIRR).
