目的:研究频域光学相干断层成像(spectral domain optical coherence tomography,SD-OCT)定量和定性检测指标在接受玻璃体腔内注射贝伐单抗的年龄相关性黄斑变性(agerelated macular degeneration,AMD)患者人群中的变化,以评估这些指标是否可以用于预测治疗后视力情况。方法:回顾性分析66眼61例未进行过AMD相关治疗的患者接受至少3mo玻璃体腔内注射贝伐单抗治疗的情况。治疗前后SD-OCT定量检测指标[中央视网膜厚度(central foveal thickness,CFT),外界膜(external limiting membrane,ELM)和椭圆区(ellipsoid zone,EZ)长度]和定性检测指标进行分析和比较。同时,分析这些指标和治疗前后的视力的相关性。结果:平均视力(Log MAR)、CFT、ELM和EZ长度治疗前为0.62±0.41、419.3±110.0μm、378.2±377.2μm和156.4±253.7μm,治疗后为0.53±0.44、325.8±117.9μm、547.1±421.5μm和173.1±207.1μm。治疗前视力和CFT(rs=0.27)、ELM长度(rs=-0.30)及ELM断裂(rs=0.43)有相关性。治疗后视力同样和治疗后ELM长度相关(rs=-0.40)。治疗后视力和治疗前视力(rs=0.66)、ELM长度(rs=-0.35)和ELM断裂(rs=0.46)相关。结论:研究显示:治疗前视力、ELM长度和ELM断裂可以用于预测治疗后视力。

目的:通过频域光学相干断层扫描(spectral-domain optical coherence tomography,SD-OCT)观测特发性黄斑裂孔患者对侧眼中心凹的形态结构变化。方法:将50例50眼研究对象分为两组:组1为25例特发性黄斑裂孔患者,组2为25位健康人(对照组)。使用RTVue 100(Optovue,Inc.,Fremont,CA,USA)的"交叉十字"SD-OCT扫描模式,对所有研究对象进行黄斑中心凹广域B型方向扫描。通过卡尺工具测量二维横断面图上中心凹的形态面积。结果:组1中心凹形态面积为0.159±0.03mm2,组2为0.079±0.01mm2。两组比较,差异有统计学意义(p<0.001)。组1中有1例患者对侧眼产生黄斑裂孔。结论:同健康人相比,特发性黄斑裂孔患者的对侧眼中心凹形态有增大趋势。该发现结合其他的一些玻璃体黄斑的致病因素,可能导致特发性黄斑裂孔。

Land use and land cover (LULC) changes strongly affect local hydrology and sediment *** current study focused on a basin in the Brazilian Amazon and had the following three objectives:(1) to perform an effective diagnosis of flow and sediment yield,(2) to evaluate the impacts of LULC changes over the last 40 years on the hydro-sedimentological variables,and (3) to investigate the impacts of the possible trends or breaking points in the flow,surface runoff,and sediment yield *** Soil and Water Assessment Tool (SWAT) model validation showed that the simulated results were consistent with the data measured in the dynamic reproduction of flow ***,changes in LULC altered surface runoff,sediment yield,and flow according to the Mann-Kendall and pettitt non-parametric *** was also observed that the sub-basins in which pastureland is predominant are more susceptible to increased surface runoff and sediment *** to trend and homogeneity tests,these sub-basins had break points in the time series of these *** the other hand,in the sub-basins whose land cover is predominantly forest,the time series is homogeneous and *** current study highlighted the impacts that 40 years of LULC changes in an Amazonian basin had on hydro-sedimentological variables,which,in turn,can play an important role in changing the hydrological cycle,consequently these changes can also impact diverse human activities in the region,such as agribusiness,livestock,energy production,food security,and public water ***,it is concluded that the SWAT model is a good estimator of hydro-sedimentological processes in Amazonian basins and can be used by decision makers in the management of water and environmental resources.

Background: Health education has proven to be an effective strategy to enable people with diabetes mellitus to manage this condition. However, few studies in Brazilian population samples have been conducted to evaluate the effects of educational programs on diabetes knowledge and self-care. Aim: To evaluate the knowledge about diabetes mellitus and the performance of self-care activities, before and after participation in an educational program. Methods: This is an intervention study, with a quantitative approach, in a single comparison group, for the analysis of “before and after” results related to an educational program focused on self-care and concurrent physical training. The study interventions consisted of 42 exercise sessions, as well as individual educational meetings, according to the needs of each participant and through nursing consultations, using educational material prepared from the literature. The sample was initially composed of 33 adults with diabetes mellitus, but 18 completed the study. For the assessment of knowledge and self-care activities, the revised Brazilian versions of the Diabetes Knowledge Scale and the Diabetes Self-Care Activity Questionnaire were used, respectively, in the two moments of the study, that is, prior to the first educational meeting and immediately before the first physical training session, and after the last educational meeting, which ran parallel to the 42nd physical training session, making a six-month interval between the two assessment moments for each participant. Results: The analysis of knowledge about diabetes showed significant improvement after the educational program and, as for self-care, there was clinical improvement in all dimensions, but only the dimension “general diet” obtained statistically significant improvement. Objective:To investigate the isolation of enterobacteria associated with Macrobrachium amazonicum(***) farming and evaluate the in vitro antimicrobial susceptibility of Vibrio ***:Strains were isolated from female *** prawns and environmental and hatchery *** assays were used to identify bacterial genera and those belonging to the genus Vibrio were submitted to further analyses for species identification,through Vitek 2 automated system and *** test was performed according to Clinical Laboratory Standards ***:The following genera of enlerobacteria were recovered:Enterobacter(n=11),Citrobacter(n=10),proteus(n=2),Serratia(n=2),Kluyvera(n=2),providencia(n=2),Cedecea(n=1),Escherichia(n=1).Edwardsiella(n=1) and BulliauxeUa(n=1).As for Vibrio,three species were identified:Vibrio cholerae non-O1/non-O139(n=4).Vibrio vulnificus(***)(n=1) and Vibrio mimicus(n=1).Vibrio *** minimum inhibitory concentrations values within the susceptibility range established by Clinical Laboratory Standards Institute for almost all antibiotics,except for ***,which presented intermediate profile to ***:Enterobacteria do not seem to be the most important pathogens associated with *** farming,whereas the recovery of Vibrio *** larviculture,with emphasis on Vibrio cholerae and ***,deserves special attention due to their role as potentially zoonotic aquaculture-associated ***,the intermediate susceptibility of *** to ampicillin reflects the importance of monitoring drug use in prawn farming.

The utility of programmed ventricular stimulation to predict future arrhythmic events in patients with cardiac sarcoidosis is unknown. Similarly, the long-term benefit of implantable cardioverter-defibrillators(ICDs) in cardiac sarcoidosis has not been established. Thirty-two consecutive patients with cardiac sarcoidosis underwent programmed ventricular stimulation. patients with spontaneous or inducible sustained ventricular arrhythmias(n=12) underwent ICD insertion. All study patients were followed for the combined arrhythmic event end point of appropriate ICD therapies or sudden death. Mean length of follow-up to sustained ventricular arrhythmia or sudden death was 32±30 months. Five of 6 patients(83%) with spontaneous sustained ventricular arrhythmias and 4 of 6 patients(67%) without spontaneous but with inducible sustained ventricular arrhythmias received appropriate ICD therapy. Two of 20 patients(10%) with neither spontaneous nor inducible sustained ventricular arrhythmias experienced sustained ventricular arrhythmias or sudden death. programmed ventricular stimulation predicted subsequent arrhythmic events in the entire population(relative hazard 4.47, 95%confidence interval[CI] 1.30 to 15.39) and in patients who presented without spontaneous sustained ventricular arrhythmias(relative hazard 6.97, 95%CI 1.27 to 38.27). No patient with an ICD died of a primary arrhythmic event. In patients with spontaneous or inducible sustained ventricular arrhythmias, mean survival from first appropriate ICD therapy to death or cardiac transplant was 60±46 months, with only 2 patients dying or reaching transplant at study end. In conclusion, programmed ventricular stimulation identifies patients with cardiac sarcoidosis at high risk for future arrhythmic events. ICDs effectively terminate life-threatening arrhythmias in high-risk patients, with significant survival after first appropriate therapy.

Visceral leishmaniasis(VL) or kala-azar is a zoonosis caused by intracellular protozoa of the Leishmania genus and is transmitted to humans by the bite of phlebotomine sandflies. It particularly affects cells in the phagocytic mononuclear system, accompanied by disturbances of cellular and humoral immunity. VL is potentially fatal and is characterized by fever, hepatosplenomegaly, diarrhea, epistaxis, jaundice, anemia, leucopenia, thrombocytopenia, hypoalbuminemia and hyperglobulinemia. Diagnostic suspicion is based on epidemiological, clinical and laboratory data and isconfirmed by detecting the parasite in infected tissue. Splenic aspiration is the most sensitive method, followed by bone marrow aspiration(BMA) by sternal puncture, liver biopsy and lymph node aspiration; but, due to safety concerns, BMA is the most recommended method. VL is included as a target disease by players in drug research and development. Severe liver dysfunction associated with VL is uncommon. We report two VL cases in pregnant women from Bauru, Sao paulo state, Brazil, considered an endemic area. The first of them developed hepatic failure due to fulminant hepatitis. In both cases, BMA was unable to find the protozoan; thus, liver biopsy was the only means of making the diagnosis.
