Risk factors for liver fibrosis among human immunodeficiency virus monoinfected patients using the FIB4 index in Morocco

AIM: To study the prevalence and risk factors of significant hepatic fibrosis in Moroccan human immunodeficiency virus(HIV) monoinfected ***: We conducted a cross-sectional study among HIV monoinfected patients(negative for hepatitis B surface antigen and hepatitis C antibody). Clinical and laboratory data were collected from the data base of the Infectious Diseases Unit in Ibn Rochd Hospital Center [age, gender, duration of HIV infection, CD4 T lymphocyte count, HIV viral load, glycemia and current or prior use of antiretroviral and antiretroviral therapy(ART) duration]. The primary outcome was a FIB4score > 1.45. Multivariable logistic regression identifiedindependent risk factors for FIB4 > ***: A FIB4 score > 1.45 was identified in 96among 619(15.5%). HIV monoinfected patients followed up between September 1990 and September2012. Multivariate analysis showed that only a viral load > 75(OR = 2.23, 95%CI: 1.36-3.67), CD4 > 200cells/mm3(OR = 0.39, 95%CI: 0.21-0.72) and age at FIB4 index calculation(OR = 1.10, 95%CI: 1.07-1.13)were independently associated with the occurrence of FIB4 index(> 1.45). Gender, duration of HIV infection,glycemia, use of antiretroviral therapy and ART duration were not associated with significant fibrosis by ***: FIB4 score > 1.45 was found in 15.5%of Moroccan HIV monoinfected patients. Age, HIV viremia > 75 copies/mL and CD4 count > 200 cells/mm3are associated with liver fibrosis. Further studies are needed to explore mechanisms for fibrosis in HIV monoinfected patients.

Pantoea SPP is a gram-negative bacillus, which usually colonizes plants, soil and water. This pathogen very ra.ely causes neonatal sepsis. The most common infections caused by Pantoea SPP are septic arthritis or synovitis, meningitis frequently complicated by brain abscess, upper respiratory infections, and peritonitis. We present the case of a premature infant who presented neonatal respiratory distress and whose evolution was complicated by the occurrence of a nosocomial infection for which a blood culture was performed isolating the germ Pantoea SPP. The patient’s management was initially centered on non-invasive ventilation with antibiotherapy based on carbapenem and aminoglycoside. Due to the clinical and biological worsening, the neonate was intubated and sedated and put on colymicin. The evolution was unfavorable marked by a death at 16th days of life. Considering the high pathogenicity of this germ and its multi-resistance to antibiotics, it is crucial to know the clinical spectrum of Panteoa SPP infections in neonatal intensive care units, in order to palliate the fulminant evolution of multifocal attacks due to this germ.

Contribution to the Study of Diabetic Kidney Disease in a Sub-Saharan Environment: An Example of the Aristide Le Dantec University Hospital in Dakar

Introduction: Diabetic kidney disease (DKD) is a leading cause of chronic kidney disease and dialysis admission. Few studies are available in Sub-Saharan Africa. The objective of this work was to study the epidemiological, clinical, diagnostic and therapeutic characteristics of DKD in our context. Patients and Methods: We conducted an observational, exhaustive and retrospective study focusing on diabetic patients seen in consultation or hospitalized in the Nephrology Department of at the Aristide Le Dantec University Hospital in Dakar during a period of 5 years from January 1, 2017 to December 31, 2021. Results: Of 4735 patients seen during the study period, 491 had DKD, i.e. a hospital prevalence of 10.36%. The average age was 59.1 ± 11.4 years with a sex ratio of 0.95. Type 2 diabetes predominated with 93.4%. The average duration of diabetes was 11.5 ± 7.6 years. Diabetes was associated with high blood pressure in 78.81% of cases, dyslipidemia in 23.2% of cases, active smoking in 6.7% of cases and obesity in 1.6% of cases. Renal failure was the main reason for referral 72.3%. One hundred and forty-eight patients (30.1%) had uncontrolled diabetes. Macroalbuminuria was found in 64.8% and microalbuminuria in 18.7% of cases. One hundred and eighty-five patients (37.7%) were in Stage V of kidney disease and 137 patients were in Stage III (18.1% in Stage IIIb and 9.8% in Stage IIIa). Diabetic nephropathy was the main etiology at 61.30%. Nephropathy was mixed (diabetic and hypertensive) in 18.12 cases. Renin-angiotensin-aldosterone system (RAAS) blockers were prescribed in 83.5% of patients. Conclusion: The different etiologies encountered during the study show the diversity of diabetic kidney disease. Diabetic nephropathy is not the only kidney damage that can occur in diabetics in our context.

The studied area at Wadi Nugrus, Southeastern Desert, Egypt, is located between lat. 24°41'00'' and 24°41'35''N and long. 34°36'47'' and 34°37'09''E. The rock types are represented by layered metagabbros, biotite schists, gneisses, granodiorites, and Muscovite granites. The muscovite granite of Wadi Nugrus, is small exposure in size (~5.0 km2), emplaced along NW-SE trend, with about 0.5 - 4.5 Km in length and 100 - 250 m in width and intruding the biotite schists. The studied muscovite granite is composed mainly of plagioclases, potash feldspars, quartz, biotite and muscovite. The recorded minerals assemblage can be classified into secondary U-minerals (uranophane and meta-autunite), U-bearing minerals (uranothorite and columbite) and accessory minerals (zircon, flourite, allanite, zinnwaldite and hematite). The U/eU is more unity where, the measured chemical uranium is higher than the measured equivalent uranium in the most analyzed samples especially in trenches, which reflect disequilibrium state. The activity concentrations range from 251.72 to 1096.2 Bq·kg-1 for 232Th, from 494 to 2593.5 Bq·kg-1 for 226Ra, and from 1314.6 to 1846.7 Bq·kg-1 for 40K. The obtained radiological data show that the average internal and external hazard indices are 9.11 and 5.78, respectively which are more than unity and highly exceeding the permissible limits (International Commission on Radiation Protection, ICRP). The internal and external hazards are mainly due to 226Ra nuclei while the absorbed dose rate (nGy/h) is related to 232Th nuclei. The contributions of the three nuclei in the total absorbed dose rates and internal and external hazardous, for 226Ra, it contributes by 72% in Hin, 57% in Hex and 55% of DR, for 232Th it contributes by 24% in Hin, 37% in Hex and 36.9% of DR, while f

Nano-composite silica-phosphate system (SiO2- P2O5) co-doped with Al2O3 as a host material and prepared by sol gel technique in two forms monolith and thin film using multilayer spin-coating method were activated by Er3+ and Yb3+ ions as in the formula;[SiO2: 11P2O5: 3 Al2O3: (1.2) Er: (1.2, 1.8 and 3) Yb]. The prepared samples have been synthesized using tetra-ethyl-orthosilicate (TEOS) and triethyl-phosphate (TEP) as precursor sources of silica and phospho-rus oxides. The microstructure and crystallinity of the prepared samples will be characterized by using x-ray diffraction (XRD) which, imply that the crystallite sizes of [SiO2: 11P2O5: 3 Al2O3: (1.2) Er: (1.2) Yb] at 500oC for both thin film and monolith forms of the prepared samples were found to be equal to 35 and 33 nm, respectively. The refractive index will be evaluated by measuring the normal transmission and specular reflection of the prepared samples. The photolu-minescence properties have analyzed in the visible wa.elength range between 500 and 800 nm as a function of sample composition.

Low-Level Laser Therapy versus Four Layers Compression Technique in the Treatment of Venous Ulcers

Objective: The aim of the study is to evaluate the effect of low-level laser therapy in comparison to compression bandage therapy in the treatment of venous ulcers. Subjects & Methods: This study was carried out in the period from March 2013 to March 2014. The study included 40 adult patients with a diagnosis of venous ulcer classified as C6 according to the clinical classification of CEAP classification. The patients were divided into 2 groups: the first one included 20 patients treated by low-level laser therapy at the Rheumatology and Rehabilitation department, and the second group included 20 patients in whom we use four layers compression bandaging at the vascular surgery department. Results: 32 ulcers were treated in group I and 35 ulcers were treated in group II. The patients of group I were 7 (35%) females and 13 (65%) males, their age range were (24 - 56) years. The patients of group II were 11 (55%) males and 9 (45%) females;their age ranges were (32 - 50) years. Measurements of the ulcers size by (cm2) were taken at the beginning of therapy, at 1 month, at 2 months and at 3 months later on. The calculation of the area of the ulcer was done by using the graph papers to document the ulcer’s perpendicular linear dimensions. According to the size of the ulcer, some ulcers heal within 1 month (15.6%) ulcers in group I, and (28.5%) in group II. Some ulcers heal within 2 months (28%) in group I, and (37%) in group II. The remaining ulcers heal within 3 months or more which are (56%) ulcers in group I, and (34.2%) ulcers in group II. The recurrence rate of chronic venous leg ulcer in compression bandage technique used in group II was the least one as the total number of recurrent cases were 5 (25%), followed by laser therapy used in group I. Conclusion: There was no significant efficacy of the low-level laser therapy over the four layers compression technique in the management of chronic venous ulcers.

In one trial 360 gilthead bream fingerlings received two different feed mixtures containing two different levels of protein (diet A 400 and B 360 g?kg?1). The average initial live weight was about 2.2 ± 0.1 g fish?1 in each replicate of all treatments. Sixty fish per tank were stocked randomly in 6 fiberglass tanks (1 m3 each). Fish were fed with experimental diets at rate 8% of the body weight daily. The Amino Acids (AA) content was analyzed in the diets, whole fish and muscle at the end of the growth period (60 days). The results indicated that at the end of experiment gilthead bream fingerlings had reached an average live weight 18.5 and 16.2 g fish?1 for fish fed diet A and B respectively. Also the increase of protein levels in the feed to 40% caused a significant increase in the content of Indispensable Amino Acid (IAA) and dispensable amino acid (as %v of whole fish and muscle of wet weight). Two methods were subsequently used to estimate the quantitative IAA requirements of gilthead bream fry based on the hypothesis that (a) the dietary requirement pattern of IAA reflected the tissue pattern and (b) the rate of daily deposition in the fish could be equated with the dietary requirement as percentage of 100 g?1 diet. The quantitative IAA requirement (%) of gilthead bream (Sparus aurata) diets was as follow: Lysine 2.27, Methionine 1.09, Therionine 0.61, Leucine 2.4, Isoleucine 0.47, Histidine1.6, Arginine 1.39, Phenylalanine 1.43, Valine 1.22 and Tryptophane 0.42.

Stochastic partial differential equations (SPDEs) describe the dynamics of stochastic processes depending on space-time continuum. These equations have been widely used to model many applications in engineering and mathematical sciences. In this paper we use three finite difference schemes in order to approximate the solution of stochastic parabolic partial differential equations. The conditions of the mean square convergence of the numerical solution are studied. Some case studies are discussed.
