Objective:This research study explores the perceptions of gerontological nursing competencies,attitudes toward older individuals,and the willingness to provide care for the elderly among internship nursing *** and Methods:A convenience sample of 350 internship nursing students from Cairo University Faculty of Nursing participated in the *** were collected using a set of comprehensive tools,including personal data and work experience assessment,the Hartford geriatric nurse competency tool,Kogan’s attitudes toward old people scale,and the modified elderly patient care ***:Descriptive analysis revealed balanced gender representation,with 60%identifying as *** percent of participants reported prior experience in gerontological ***-assessed competency scores indicated moderate proficiency in communication,physiological changes,and functional status *** such as pain management and restraint use demonstrated potential gaps in self-perceived *** toward older individuals were predominantly positive,and the willingness to provide care showed positive tendencies,with a minor degree of hesitation ***:The findings underscore the need for targeted interventions in nursing education to address competency gaps,enhance positive attitudes,and alleviate potential hesitations in caregiving for older *** the elderly population continues to grow,nursing education programs must prepare future practitioners to deliver comprehensive and compassionate care tailored to the unique needs of older adults.

Over the recent past few years, there is a huge innovation in plastic surgery and orthopedic surgery through implantation of new techniques, which enabled a great level of success in hand salvage. Conditions such as trauma, tumor, sepsis, or vascular disease, may necessitate hand salvage. The most frequent argument among surgeon from different subspecialties (orthopedics, plastics, trauma, and vascular surgery) are characterized by in what way each one can do his own part of the salvage operation, be it bony fixation, revascularization, or soft-tissue coverage, but none of them is sure whether it should be endeavored. What is necessary in such clinical situations is an interdisciplinary team attitude led by individual or groups of clinicians who are conversant not only with their identifiable subspecialized skills but also with those of their coworkers and the consequences accompanying the joined efforts at hand salvage. The perception of orthoplastic surgery is based on such an indication, where the integrated skills and techniques of the orthopedic surgeon and reconstructive microsurgeon are performed in recital to direct efforts concerning hand salvage or choose against it when it is not designated. The current article reviews the roles of orthopedic and plastic surgery and how this team can deal with the existing techniques to improve outcomes in hand salvage surgery.

WSNs are designed to efficiently collect data and monitor environments, among other applications. This article describes the concept and realization of an Active Security System for security management of warehousing of chemical substances using WSNs. We present an approach to modeling and simulating cooperation between intelligent products that are equipped with a platform of sensor networks and ambient communication capabilities to increase their security, in a context of ambient intelligence of a deposit for chemical substances. Behavior evolution of every intelligent product is modeled by hierarchical Petri Nets. The simulation of the model is implemented in the Castalia-OMNET++ Tools language.

Acute Kidney Injury with Rhabdomyolysis: 25 Years Experience from a Tertiary Care Center

Objective: To describe patients presenting with acute kidney injury after rhabdomyolysis at a tertiary renal care center in Pakistan. Patients and Methods: An observational cohort of patients identified as having acute kidney injury (AKI) with rhabdomyolysis, which was diagnosed by rise in creatinine phosphokinase (CK) and lactate dehydrogenase (LDH) more than 4 times the reference range whereas AKI was defined according to RIFLE criteria. On ultrasonography, all patients had normal size non obstructed kidneys, and no other co morbid. Results: Between January1990 to December 2014, 334 patients with rhabdomyolysis and AKI registered to this hospital. Mean age was 28.22 ± 11.22 years with M:F ratio of 3.33:1. Mean values of CK and LDH were 597,749.790 ± 180,461.360 and 4077.026 ± 5050.704 U/L with reference range of 26 - 174 U/L and 91 - 180 U/L respectively. We divided the study population into 4 groups over timeline. Rhabdomyolysis etiology was divided in 3 groups;1) traumatic, 2) non-traumatic exertional, and 3) non-traumatic non-exertional. In the last group, which spans from 2010-2014, we treated many cases with toxic rhabdomyolysis and main toxin was paraphenylenediamine (PPD). The other causes showed more or less same prevalence over two and a half decade, except non-traymatic exertional which has decreased during last 5 years without any explainable cause. Renal replacement therapy (RRT) was required on arrival in 94% cases. Complete renal recovery was observed in 70%, while 15.86% died and 10% were lost during recovery phase. A small number 2.69% left against medical advice during acute phase of illness and 0.8% developed chronic kidney disease (CKD). Conclusion: The common clinical conditions found associated with rhabdomyolysis and AKI includes trauma, immobilization, sepsis, overexertion, and drugs and toxins. In recent years, we have seen many young patients with PPD poisoning;we have found good renal recovery in patients who survived initial 2 - 3 weeks.

A Novel Low-Complexity Low-Latency Power Efficient Collision Detection Algorithm for Wireless Sensor Networks

Collision detection mechanisms in Wireless Sensor Networks (WSNs) have largely been revolving around direct demodulation and decoding of received packets and deciding on a collision based on some form of a frame error detection mechanism, such as a CRC check. The obvious drawback of full detection of a received packet is the need to expend a significant amount of energy and processing complexity in order to fully decode a packet, only to discover the packet is illegible due to a collision. In this paper, we propose a suite of novel, yet simple and power-efficient algorithms to detect a collision without the need for full-decoding of the received packet. Our novel algorithms aim at detecting collision through fast examination of the signal statistics of a short snippet of the received packet via a relatively small number of computations over a small number of received IQ samples. Hence, the proposed algorithms operate directly at the output of the receiver's analog-to-digital converter and eliminate the need to pass the signal through the entire. In addition, we present a complexity and power-saving comparison between our novel algorithms and conventional full-decoding (for select coding schemes) to demonstrate the significant power and complexity saving advantage of our algorithms.

Endoscopic Ultrasound-Guided Fine Needle Aspiration Cytology of Gastrointestinal Tract Tumors and Related Organs: Predictive Factors for Accurate Cytopathological Diagnosis

Background: Endoscopic ultrasonography (EUS) is well-established, highly accurate clinical diagnostic test for detection and staging of gastrointestinal tract lesions and related organs. The addition of endoscopic guided fine needle aspiration cytology (EUS-FNAC) has improved the performance characteristics of EUS. Aim: Evaluate the validity of EUS-FNAC in diagnosis of gastrointestinal tract lesions and related organs and assess predictive factors for an accurate EUS-FNAC diagnosis. Methods: Our study included cytological sampling from one hundred sixty-six lesions obtained from gastrointestinal tract and related organs. Factors affecting EUS-FNA accuracy were analyzed. The histopathological results or clinical follow-up were used as the gold standard method. Results: Samples were obtained from: pancreatic masses (n = 80), gastric masses (n = 34), lymph nodes (n = 22), hepatobiliary masses (n = 18) and rectal masses (n = 2). Statistical analysis of sensitivity, specificity, Positive Predictive Value, Negative Predictive Value, and diagnostic accuracy of EUS alone was 80.4%, 51.6%, 75.8%, 76.2% and 75.9% respectively. The sensitivity, specificity, positive predictive value and negative predictive value of EUS-FNAC are 78.8%, 96.8%, 97.6% and 73.2% respectively. Addition of FNAC to EUS improved diagnostic accuracy to 85.5%. Positive statistical association was present between cytological adequacy and adequate cell block preparations, larger lesion size, presence of rapid on site evaluation (ROSE) and obtaining at least two passes from target lesion. Conclusions: EUS is valuable diagnostic and cost effective tool for gastrointestinal tract lesions and related organs when combined with FNAC.

Heuristic and metaheuristic techniques are used for solving computationally hard optimization problems. Local search is a heuristic technique while Ant colony optimization (ACO), inspired by the ants' foraging behavior, is one of the most recent metaheuristic technique. These techniques are used for solving optimization problems. Multiple-Input Multiple-Output (MIMO) detection problem is an NP-hard combinatorial optimization problem. We present heuristic and metaheuristic approaches for symbol detection in multi-input multi-output (MIMO) system. Since symbol detection is an NP-hard problem so ACO is particularly attractive as ACO algorithms are one of the most successful strands of swarm intelligence and are suitable for applications where low complexity and fast convergence is of absolute importance. Maximum Likelihood (ML) detector gives optimal results but it uses exhaustive search technique. We show that 1-Opt and ACO based detector can give near-optimal bit error rate (BER) at much lower complexity levels. Comparison of ACO with another nature inspired technique, Particle Swarm Optimization (PSO) is also discussed. The simulation results suggest that the proposed detectors give an acceptable performance complexity trade-off in comparison with ML and VBLAST detectors.

Objective: The aim of this study was to assess school indoor exposure to microbial products and prevalence of asthma and allergies in rural and urban ***: This study was carried on a rural and an urban *** endotoxin level was measured in multiple samples of the ambient indoor air dust collected on special aseptic filter papers from the two *** two hundred children history taking,clinical examination,allergen skin prick test and basic pulmonary function test were ***: Environmental endotoxin levels showed significantly higher mean values(P<0.01) in rural school(3 EU/mg) as compared to the urban school(0.1 EU/mg) with(OR=5.163;95% CI: 0.95-28).History of allergic symptoms was significantly more in urban than rural students(P=0.01).Mean values of pulmonary function parameters were significantly lower values in urban students compared to rural *** prick test results showed significant reactions to all tested allergens in urban children compared to rural children(P<0.05).Conclusion: There is an inverse association between environmental exposure to endotoxins and susceptibility for allergic manifestations in school children.

Background:?Perforated peptic ulcer is a common surgical emergency. The classic treatment is the mid-line laparotomy. However, laparoscopic treatment has been shown to be reliable. Few studies have evaluated its overall utility. The aim of this study is to assess the efficacy of laparoscopic repair of perforated duodenal ulcer.?Patients and Methods:?This study included 50 patients presented by perforated peptic ulcer between July 2009 and August 2014. They were submitted to laparoscopic omental patch repair with thorough peritoneal wash. Patients’ demographics, diagnostic techniques, management and outcome were evaluated. Results:?The mean age was 38.6 years with male to female ratio being 1.6:1. The perforation was diagnosed by plain X-ray abdomen in erect position in 43 patients and by abdominal CT scan in 7 patients. The laparoscopic repair of the perforation was successful in 48 patients while in 2 patients mid-line laparotomy was needed for proper control of the severe intra-abdominal sepsis. Post-operatively, all patients tolerated soft diet on the 3rd?post-operative day and full diet on the 4th?post-operative day. The mean duration of hospital stay was 4.5 days. Two patients developed post-operative intra-abdominal collection that was treated by ultrasound guided drainage, three patients developed umbilical port site wound infection while only two patients developed leakage, one of them reoperated after failed conservative surgery. No mortality was encountered in the study. Conclusion:?Laparoscopic repair of perforated peptic ulcer is a safe and reliable technique with accepted morbidity and mortality rates with all the advantages of the minimally invasive surgery.
