Assessment of Radon Concentrations inside Residential Buildings and Estimation of the Dose in the City of Kaya, Burkina Faso

Colorless, incessant radon gas is notably the second most important cause of lung cancer after smoking in smokers and the first cause in non-smokers. Having little effect in the atmosphere, radon gas accumulates in confined spaces. Therefore, the determination of radon concentrations inside residential buildings is very important to improve the health of the inhabitants. The objective of this research is to measure the concentration of radon in indoor air in residential areas of the city of Kaya and to assess the absorbed dose, the effective dose and the relative risk of lung cancer. In this study, the CORENTIUM AIR THINGS digital radon detector is used to determine the radon concentration in twenty-one houses in Kaya. The CORENTIUM AIR THINGS digital radon detector has been placed in each residential building for a minimum period of one week and the concentration values are read every 24 hours. This research revealed that the average concentration of radon was 28.47 Bq/m3 in the residential areas of Kaya. The radon concentration in a house has been found to exceed 100 Bq/m3, which is the level authorized by the World Health Organization (WHO). In the long term, the absorbed dose varies from 0.118 mSv to 4.975 mSv and the effective dose is between 0.229 mSv and 12.002 mSv. In the short term, the absorbed dose varies between 0.095 mSv to 5.001 mSv and the effective dose is between 0.283 mSv to 11.935 mSv. The mean lung cancer relative risk (CPRR) from indoor exposure was 1.026. There is a need to raise awareness among the population of the city of Kaya on this issue and to take measures to reduce radon in homes when the concentrations are above the limit recommended by the WHO.

Phytosanitary Practices and Pesticide Levels in Fresh and Dried Mangoes Produced in Burkina Faso

Pesticides are used to control fruit flies and other potential crop pests, particularly mango in Burkina Faso. Pesticides are a potential health hazard for users and consumers and are a major constraint to the export and commercialization of mangoes. The objective of this study was to evaluate phytosanitary practices in orchards and pesticide residue levels in four varieties of fresh and dried mangoes (Amelie, Brooks, Kents and Lippens) produced in four localities in Burkina Faso in order to determine their health impact on consumers. Surveys on pesticide use and monitoring of phytosanitary practices were carried out among 16 orchard owners in the localities of Bobo-Dioulasso, Orodara, Toussiana and Banfora, a high mango production area. Also, a total of 120 samples, including 60 samples of fresh mangoes and dried 60 samples of dried mango were collected. A multi-residue method was developed to detect pesticide levels in the collected samples by gas chromatography with a micro-electron capture detector. The study revealed the use of unauthorized pesticides, often specifically for other crops, and a monitoring trapping network in the orchards against insects. 34 chemical pesticides were detected in 120 samples of all mango varieties collected. Pesticide residues were detected in 72% of the samples, and of the positive samples, 48.52% contained concentrations above the maximum residue limit permissibility. Organochlorine pesticide residues were present in fresh mangoes (60.5%) and a total of fifteen active compounds were detected in dried mangoes. The very poor management and use of pesticides found in these orchards could pose a threat to the productivity of natural ecosystems and the health of producers, processors and consumers. Awareness raising and training of producers on the knowledge of the risks linked to the use of pesticides and good practices are necessary to preserve the health of all.

Epidemiology and Morphological Hallmark in Symptomatic Spondylotic Beninese Patients

Spondylolisthesis is a well-known neurosurgical condition, in relation with various factors. The main objective was to report the morphological and radiological hallmark of patients presenting with symptomatic spondylolisthesis. This is a prospective, longitudinal, descriptive and analytical study： 1 st April to 30th September 2014. Twenty nine patients were followed up, with female ascendance. The average age is 50 to 69 years. The most frequent level was LA-L5 and in most cases, only one level was involved. Lesions of Grade I were commonest （79.0%）. The average values of the SS （sacral slope） and PI （pelvic incidence） was respectively 36.4 ± 6.9° （17.3° to 49.6°） with a median of 37.8° and 64.9 ±9.2° （42.98° to 87.47°） with a median of 64.6°. LL （lumbar lordosis）, had a mean value of 43.8 ±10.8° （26.2° to 67.0°） with a median of 43.1°. DH （disc height） was 5 to 10 mm in most cases. A report was made with the morphology of the basin of the African woman. Our results are consistent for the most part with the literature data. This present study illustrates a female to male ration of 3. According to BMI （body mass index） assessment, 69% of patients presented with overweight. The main radiologic parameters are compromised with increasing sacral slop and PI and reduction of DH and LL. Findings are characteristic in female population.

Evaluation of Radon Concentration and the Health Risk in the Offices of the Institute of Science and Technology of the “École Normale Supérieure”, Burkina Faso

The second most important cause of lung cancer after smoking is radon gas. Thus, the determination of indoor radon concentrations in residential buildings and workplaces is an important public health concern. The purpose of this research was to measure the concentration of radon gas in the offices of the Institute of Science and Technology and to evaluate the effective dose in the lungs and the risk of cancer. This study used Corentium’s AIR THINGS digital radon detector to determine the radon concentration in sixteen (16) offices. The digital radon detector air Things of Corentium was placed in each office for a minimum period of one week and the concentration values were recorded every 24 hours. The values recorded in each office were the short-term average and the long-term average during seven days of measurement. The short-term radon concentrations vary between 5.286 Bq/m3 and 192.714 Bq/m3 with an average of 48.01 Bq/m3 and those in the long-term were between 6.143 Bq/m3 and 172.571 Bq/m3 with an average of 52.46 Bq/m3. The measurements in office N°6 and 13 were above the lower limit of 100 Bq/m3 proposed by the WHO. The short-term and long-term effective doses in the lungs for offices N°6 and 13 were above the “normal” background level of 1.1 mSv/year proposed by UNSCEAR-2000. The short-term effective dose in the lungs for office N°6 was above the lower limit of 3 mSv per the ICRP-23 recommendation. The average number of lung cancer cases per year per million people was 15.

Sacrospinous fixation (SSF) or Richter’s intervention (RI) aims to treat genital prolapse by securing the posterior vaginal wall to the small sacrospinous ligament. It is performed by low approach and includes a dissection of the pararectal space, visual exposure of the sacrospinous ligament and a needle with strait needle holder with nonabsorbable threads. It is often associated with a more complex corrective procedure, including cystocele cure, vaginal hysterectomy and posterior myorrhaphy. The objective of this study is to report the results of SSF in the gynecology department of Hopital du Mali. A descriptive study was conducted from September 2014 to September 2015 concerning 37 patients operated on for uterine prolapse (UP). All patients with grade III UP were included in our study in whom a unilateral hysterectomy (UH) and sacrospinous fixation (SSF) were performed. All the patients were scheduled. Preoperatively they had benefited from an assessment and a pre-anesthetic consultation. Hospitalization of at least 24 hours prior to the operation was required. Postoperative follow-up was two years with a physical examination at 3 months, 9 months and 15 months, and phone calls between physical consultations. During the study period, we performed 37 RIs. The mean age of the patients was 48 years with extremes of 41 to 73 years. The large multiparity was found out in 35 cases (94.59%), the pauciparous were two with 3 deliveries for each. Long labor of more than 18 hours was found out in 9 patients (24.32%) and home delivery in 13 cases (35.13%). The duration of the occurrence of prolapse was at least two years and 35 patients were going through menopause. The type of anesthesia used for the surgery was spinal anesthesia for all patients. The average duration of the operation was 90 minutes. Complications occurred in three patients or 8.10% of cases, two cases of acute urine retention and one case of hematoma of the para-rectal space. Infectious Complications after Cesarean Delivery: Trends of Incidences, Risk Factors, and Prognosis in a Third Health Level Center in Africa. Case of Gabriel TouréTeaching Hospital

Post cesarean infections are the main sources of fever in the postpartum. We have undertaken this study in an African health setting where conditions of working are different from those in developed countries. Objectives: The objectives of this survey were to appreciate incidences, risk factors and prognosis of post cesarean infections. Method: We conducted a randomized historical cohort study in the department of gynecology and obstetrics of Gabriel Touré teaching hospital from 2010 to 2015. Data have been analyzed using X2 or Fisher test according their application conditions, p value Results: From 2010 to 2015 we performed 15,963 deliveries within 5263 cesareans sections (32.97%). According to all the deliveries, the global frequency of infection fluctuates from 1.5% in 2010 to 2.1% in 2015. The main risks that influenced the occurrence of post cesarean infections were: the context of cesarean section (RR = 2.05;CI95% (1.35 - 3.11);p 0.01), the prolonged labor (RR = 1.38;CI95% (1.05 - 1.81);p 0.01), the length of cesarean (RR = 3.00;CI95% (1.89 - 4.90);p 0.01), and genital bleeding (RR = 1.50;CI95% (1.10 - 1.90);p 0.01). The complications reported were endometritis (43.55%), wound infection (18.11%), breast infection (32.05%), puerperal psychosis (9 cases). We recorded six cases of pelviperitonitis and three cases of sepsis. Four cases of maternal death due to septic shock have been recorded (1.43%). Conclusion: Post cesarean infections constitute a real problem of public health in developing countries. In our survey, the main factors of infections have been context of cesarean, prolonged labor and length of cesarean. Endometritis, breast infection and wound infection are the major complications after cesarean section. The respect of protocol of the management of patients should permit to prevent this deadly complication.

Maternal Deaths in Patients Evacuated to the Fousseyni Daou Hospital in Kayes over a Decade

Introduction: Evacuation refers to the rapid transfer of a patient in an emergency, from one health center to another more equipped and better specialized. The objective of this study was to study maternal mortality in patients evacuated to the gynecology and obstetrics department at the Fousseyni Daou Hospital in Kayes over a period of 10 years. Materials and Methods: This was a descriptive, cross-sectional, retrospective study over nine years from January 1, 2011 to December 31, 2019 and prospective over one year from January 1, 2020 to December 31, 2020 involving all patients or parturients evacuated for obstetrical causes and died in the gynecology-obstetrics department of the Fousseyni Daou Hospital. Confidentiality and anonymity were respected. The processing and analysis of the statistical data was carried out using SPSS 20.0 software. Results: during our study period we identified 38,854 obstetric admissions including 6758 evacuations or 17.4%, among the 6758 cases of obstetric evacuations 284 died, a frequency of 4.2%. The maternal death audit committee of the Fousseyni Daou hospital in Kayes audited 101/284 cases (files) or 35.5% of which 64 maternal deaths (63.4%) were considered inevitable. In our study the 20 - 29 age group was more represented with 38%. 85% of the deceased patients lived in rural areas. In our series, 63.7% of the deceased patients had not had antenatal consultations (NPC). Eclampsia accounted for 26.8% of admissions diagnoses and 30.9% of causes of death. Seventy-two (72%) of deaths occurred postpartum, 15% perpartum and 13% prepartum. Direct obstetric causes were predominant at 65.1%. Conclusion: Indicators of maternal deaths among evacuated patients remain poor in our work. Maternal deaths were driven by socio-economic and cultural factors, but also by factors related to the health system.

Objective: The purpose of this study was to assess the risk factors associated with seat presentation and to determine the maternal-fetal prognosis of delivery in the presentation of the seat. Materials and methods: This was a descriptive, cross-sectional and analytical case-control study with prospective data collection, from January 1, 2018 to December 31, 2019, a 12-month period at Kayes Hospital, Mali. Results: During our study period we recorded 3820 deliveries, including 120 breos;a frequency of 3.14% of all deliveries. The 20 - 35 age group was the most represented in cases 61.7%. Of the cases 66.7% (n-80) were out of school, we note 75.8% (n-182) of out-of-school controls. Primiparaes were the majority among cases 39.2% (n-47), as were 39.1% of the controls (n-118). The cases had a history of caesarean section at 15% (n-18);13, 3% (n-32). The most common mode of presentation was the 70% decomplete seat. Bart’s Syndrome: A Neonatal Observation about a Case Report

Introduction: Bart’s syndrome is a rare neonatal pathology combining congenital skin aplasia affecting the extremities and congenital epidermolysis bullosa, exceptionally described on black skin. Observation: A 2-day-old male newborn was referred for multiple ulcerations of the limbs observed at birth. The clinical examination found an absence of bilateral and symmetrical skin occupying almost all of the two lower limbs with some flaccid bubbles. The vascular network was clearly visible. The rest of the skin coating was normal. The diagnosis of Bart syndrome in connection with epidermolysis bullosa was evoked clinically and despite pediatric and dermatological management, the evolution was rapidly fatal by severe sepsis. Discussion: Bart syndrome corresponds to a clinical picture of congenital skin aplasia associated with congenital epidermolysis bullosa suspected by areas of fragility and sometimes bubbles. All types of congenital epidermolysis bullosa may be associated with this syndrome. The clinical diagnosis is generally easy but the therapeutic management is difficult and the prognosis reserved. Conclusion: Bart syndrome is a curious congenital association of well-defined skin symptoms, the etiopathogeny of which still remains poorly elucidated, hence the difficulty of establishing an antenatal diagnostic strategy or giving appropriate genetic advice.

Introduction: The World Health Organization estimates that each year more than 20 million women suffer from the after-effects of unattended childbirth, of which 800 women die every day as a result of obstetric complications. Objective: Study the socio-epidemiological aspects and prognosis of unassisted deliveries arriving at the Fousseyni Daou De Kayes hospital. Materials and Methods: This was a descriptive, cross-sectional study with prospective collection, case-control that took place at the maternity ward of the Fousseyni Daou hospital in Kayes, from 1 January to 31 December 2021. Results: The frequency of unattended birth was 4.1%. The 30 - 35 age group was the most represented with 32.5%. In our study 58.1% of cases were out of school, 19.4% were primigestes and 39.4% were multiparous. In our series 60.6% of cases had given birth at home and 39.4% along the way. The majority of cases regretted having given birth without the assistance of qualified personnel in 45.6%. Maternal complications were dominated by perineal lesions in 14.4% of cases, haemorrhage of delivery in 25% of cases, severe anaemia in 30% of cases;16.7% of newborns were in poor condition, 13.6% were fresh stillbirths, 15.6% of newborns had obstetric trauma. The maternal death rate on arrival was 3%. Conclusion: The frequency of unassisted childbirth is high in the Kayes region. Its complications can be serious for both the mother and the newborn.
