Comparative study of CEA and CA19-9 in esophageal,gastric and colon cancers individually and in combination(ROC curve analysis)

Objective:To determine the clinical serum levels of carcinoembryonic antigen(CEA) and carbohydrate antigen 19-9(CA19-9),individually and in combination,for the diagnosis of 50 healthy subjects and 150 cases of esophageal,gastric,and colon ***:The sensitivities of the two markers were compared individually and in combination,with specificity set at 100%.Receiver operating characteristic(ROC) curves were ***:Serum CEA levels were significantly higher in cancer patients than in the control *** sensitivity of CEA was determined:in esophageal cancer,sensitivity=28%,negative predictive value(NPV)=61.72%,and AUC=0.742(SE=0.05),with a significance level of P<0.0001;in gastric cancer,sensitivity=30%,NPV=58.82%,and AUC=0.734(SE=0.05),with a significance level of P<0.0001;in colon cancer,sensitivity=74%,NPV=79.36%,and AUC=0.856(SE=0.04),with a significance level of P<*** sensitivity of CA19-9 was also evaluated:in esophageal cancer,sensitivity=18%,NPV=54.94%,and AUC=0.573(SE=0.05),with a significance level of P=*** gastric cancer,sensitivity=42%,NPV=63.29%,and AUC=0.679(SE=0.05),with a significance level of P<*** colon cancer,sensitivity=26%,NPV=57.47%,and AUC=0.580(SE=0.05),with a significance level of P=*** following were the sensitivities of CEA/CA19-9 combined:in esophageal cancer,sensitivity=42%,NPV=63.29%,SE=0.078(95% CI:0.0159-0.322);gastric cancer,sensitivity=58%,NPV=70.42%,SE=0.072(95% CI:-0.0866-0.198);and colon cancer,sensitivity=72%,NPV=78.12%,SE=0.070(95% CI:0.137-0.415).Conclusion:CEA exhibited the highest sensitivity for colon cancer,and CA19-9 exhibited the highest sensitivity for gastric *** analysis indicated an increase in diagnostic sensitivity in esophageal and gastric cancer compared with that in colon cancer.

In this paper, the notions of integral Φ0-stability of ordinary impulsive differential equations are introduced. The definition of integral Φ0-stability depends significantly on the fixed time impulses. Sufficient conditions for integral Φ0-stability are obtained by using comparison principle and piecewise continuous cone valued Lyapunov functions. A new comparison lemma, connecting the solutions of given impulsive differential system to the solution of a vector valued impulsive differential system is also established.

Comparison of encirclage and cryotherapy with argon laser in the management of traumatic cyclodialysis cleft

The study compared anatomical and functional outcome of a novel technique of encirclage and cryotherapy with the conventional argon laser in the management of traumatic cyclodialysis cleft in 19 patients(10: encirclage and 9: argon laser). The postoperative anatomical success was taken as rise in intraocular pressure(IOP) more than 6 mm Hg or the closure of cleft on gonioscopy or ultrasound biomicroscopy(UBM). Visual outcome was measured as change in logMAR visual acuity. Traumatic cataract and hypotony were present in majority. IOP>6 mm Hg was noted in 90% of encirclage group and in 77.70% of argon laser group. Two cases had non-closure of cleft. Encirclage is comparable to argon laser in terms of anatomical and functional success with possible advantage of single procedure and use in cases with failed laser.

Human immunodeficiency virus and nodular regenerative hyperplasia of liver: A systematic review

AIM: To investigate the diagnosis, pathogenesis, natural history, and management of nodular regenerative hyperplasia(NRH) in patients with human immunodeficiency virus(HIV). METHODS: We performed a systematic review of the medical literature regarding NRH in patients with HIV. Inclusion criteria include reports with biopsy proven NRH. We studied the clinical features of NRH, in particular, related to its presenting manifestation and laboratory values. Combinations of the following keywords were implemented: "nodular regenerative hyperplasia", "human immunodeficiency virus", "noncirrhotic portal hypertension", "idiopathic portal hypertension", "cryptogenic liver disease", "highly active antiretroviral therapy" and "didanosine". The bibliographies of these studies were subsequently searched for any additional relevant ***: The clinical presentation of patients with NRH varies from patients being completely asymptomatic to the development of portal hypertension – namely esophageal variceal bleeding and ascites. Liver associated enzymes are generally normal and synthetic function well preserved. There is a strong association between the occurrence of NRH and the use of antiviral therapies such as didanosine. The management of NRH revolves around treating the manifestations of portal hypertension. The prognosis of NRH is generally good since liver function is preserved. A high index of suspicion is required to make a identify NRH. CONCLUSION: The appropriate management of HIVinfected persons with suspected NRH is yet to be outlined. However, NRH is a clinically subtle condition that is difficult to diagnose, and it is important to be able to manage it according to the best available evidence.

Use of intravascular lithotripsy in non-coronary artery lesions

BACkGROUND Intravascular lithotripsy(IVL)is a novel technique increasingly used for plaque modification and endovascular revascularization in patients with severe calcification and peripheral artery ***,much of the available literature on IVL is focused on its use in coronary arteries,with relatively limited data on non-coronary artery *** To analyze the safety and efficacy of current IVL use in non-coronary artery lesions,as reported in case reports and case *** We searched EMBASE,PubMed,and Reference Citation Analysis databases for case reports and case series on IVL use in peripheral artery *** then extracted variables of interest and calculated the mean and proportions of these *** We included 60 patients from 33 case reports/case ***-eight percent of the cases had IVL usage in only one blood vessel,while four had the IVL used in two vessels(2.0%),resulting in 64 Lesions treated with *** mean age of the patients was 73.7(SD 10.9).IVL was successfully used in severe iliofemoral artery stenosis(51.6%),severe innominate,subclavian,and carotid artery stenosis(26.7%combined),and severe mesenteric vessel stenosis(9.4%).Additionally,IVL was successfully used in severe renal(7.8%)and aortic artery(4.7%)*** were complications in 12%of the cases,with dissection being the *** IVL has successfully used in plaque modification and endovascular revascularization in severely calcified and challenging lesions in the iliofemoral,carotid,subclavian,aorta,renal,and mesenteric *** most severe but transient complications were with IVL use in the aortic arch and neck arteries.

The heavy metals present in the environment accumulate in the plants and affect their productivity and *** entering the food chain,metals cause several serious health problems in human beings as well as in other *** acetic acid(IAA)is known to act as a signaling molecule between symbiotic association of metal accumulating plants and plant growth promoting rhizobacteria(PGPR).Present study demonstrated a protective role of IAA against surplus Zinc(Zn)-induced toxicity to Hordeum vulgare *** Zn concentrations suppressed the plant growth,caused a reduction in leaf relative water contents(RWC)and elevated free proline and non-protein thiols(NPT)*** treatment also led to enhanced lipid peroxidation(MDA contents)as well as the activity of ascorbate peroxidase(APX),showing the involvement of antioxidative defense mechanism to reduce Zn induced *** oxidase activity was also observed to increase due to Zn *** pretreatment of *** caryopsis could partly revert the Zn-induced toxicity in seedlings.

Comparative study on effects of dexmedetomidine and dexamethasone on the incidence of postoperative nausea and vomiting in patients undergoing laparoscopic surgery

Objective:To compare the safety and efficacy of dexmedetomidine and dexamethasone for the prevention of postoperative nausea and vomiting(PONV)in patients scheduled for laparoscopic ***:A total of 86 female patients were prospectively administered dexmedetomidine 1μg/kg i.v.(the group A,n=43),and dexamethasone 8 mg i.v.(the group B,n=43).The two groups were compared in treatment response,hemodynamic changes,and Numerical Analog Scale(NAS).Besides,the relation of PONV with patient baseline characteristics in the perioperative period was determined as ***:Patients in group A had lower PONV scores(t=3.1,P<0.002),less needs for rescue anti-emetics(χ2=0.47,P<0.001),and decreased intraoperative heart rate(t=9.72,P<0.001)and mean arterial pressure(t=7.58,P<0.001)compared to that of group *** A reported lower NAS than group B(t=2.66,P<0.001).In addition,we found no relationship between PONV score and rescue anti-emetic requirement,age,or body mass index(P=0.96,P=0.60,P=0.28,respectively).Conclusion:Dexmedetomidine could be used as an effective antiemetic in laparoscopic surgeries,with better efficacy than *** not only can reduce PONV but also is effective in postoperative analgesia.

Complementary and Alternative Medicine Therapies for Chronic Pain

ABSTRACT Pain afflicts over 50 million people in the US, with 30.7% US adults suffering with chronic pain. Despite advances in therapies, many patients will continue to deal with ongoing symptoms that are not fully addressed by the best conventional medicine has to offer them. The patients frequently turn to therapies outside the usual purview of conventional medicine （herbs, acupuncture, meditation, etc.） called complementary and alternative medicine （CAM）. Academic and governmental groups are also starting to incorporate CAM recommendations into chronic pain management strategies. Thus, for any physician who care for patients with chronic pain, having some familiarity with these therapies--including risks and benefits--will be key to helping guide patients in making evidence-based, well informed decisions about whether or not to use such therapies. On the other hand, if a CAM therapy has evidence of both safety and efficacy then not making it available to a patient who is suffering does not meet the need of the patient. We summarize the current evidence of a wide variety of CAM modalities that have potential for helping patients with chronic pain in this article. The triad of chronic pain symptoms, ready access to information on the internet, and growing patient empowerment suggest that CAM therapies will remain a consistent part of the healthcare of patients dealing with chronic pain.
