Background :Non-Hodgkin lymphomas are the first childhood cancer in sub-Saharan Africa. Objective: The purpose of this study was to assess non-Hodgkin lymphomas cases in our setting. Methodology: A retrospective and descriptive study carried out in the pediatric oncology unit of the Gabriel Touré Teaching Hospital Bamako over 10 years from 1st January 2005 to 31th December 2015. Results: We exploited 274 (21.6%) cases of Non-Hodgkin Lymphoma out of 1295 cancer cases registered, the age group 6 - 10 years was the most represented (46.4%);the male sex was predominant with a sex-ratio of 1.8;digestive signs were the most common signs of discovery (44.2%) followed by maxillary swelling (42.7%);the majority of patients (52.9%) consulted between 1 and 3 months after the onset of signs;the malnutrition rate was 39.8%, of which 24.1% were severe cases and 15.7% were moderate rate. Abdominal localization was the most common (43.1%) followed by maxillofacial localization (33.9%). Almost all were Burkitt type cytology (92.7%), the majority (73.4%) were in Murphy stage III. Almost all (96%) had received chemotherapy and the modified LMB 01 protocol was widely used (62.4%). The majority of patients (85%) were chemosensitive at day 7 or after the third cyclophosphamide injection but at the end of induction only 31% were in complete remission. Gastrointestinal toxicity was the most common (37.13%) followed by hematologic toxicity 35.09 %;9.12 % of patients were lost of follow-up and 22.26% died. Tumor progression was the most common cause of death (60.66%) followed by infection (21.31%). Conclusion: In light of these findings, the late diagnosis and the poor management of NHL, as well as the limited ability to primarily treat metabolic complications, explain the high case-fatality rate, hence the important role of early diagnosis and treatment multidisciplinary.

Epidemio-Clinical Characteristics of Perinatal Anoxia and Immediate Outcome of Patients at Hospital Teaching Gabriel Toure of Bamako

Introduction: Neonatal asphyxia (NA) is one of the most likely causes of neuro-developmental abnormalities in children. In Mali it is responsible for half of the early deaths and the third of neonatal mortality. Updated data would help understand and improve intervention strategies to reduce mortality. Objective: It is the study of epidemiological and clinical characteristics, the immediate outcome and the factors associated with newborn (NB) mortality with NA. Material and Methods: This was a prospective cross-sectional study from June 27th to September 3rd 2016 about the NBs admitted for NA in the Hospital Teaching Gabriel Touré of Bamako. The clinical and biological data including the prognosis were collected from the health records of women, the liaison sheets and the medical file. The analysis was done using the software Epi info version 3.5.1. Results: 76 NBs were included which represented 23.45% of hospitalizations. The majority (89.5%, n = 68) were admitted to less than 24 hours of life for NA grade III according to the Sarnat classification (43.4%, n = 33). The average age of mothers was 24.17 ± 5.5 years. Almost half (41.3%, n = 31) were primigravida. The most common obstetrical event was dystocia (64.5%, n = 49). The prognosis was poor in grade III anoxia in our patients (56%) of deaths. Conclusion: The périnatal anoxia (PA) is a major health issue in Mali because of its frequency and severity. Monitoring of pregnancies, delivery assisted by skillful and qualified personnel, mastery of neonatal resuscitation techniques are good means of prevention.

As a natural and environmentally friendly renewable material,Northeast China ash wood(NCAW)(Fraxinus mandshurica Rupr.)was cut by water-jet assisted CO_(2) laser(WACL),the surface quality was evaluated by surface roughness of cut *** surface roughness was measured by three-dimensional(3D)***,the micromorphology of machined surface was observed by scanning electronic microscopy(SEM).Carbon content changes of machined surface were measured by energy dispersive spectrometer(EDS).A relationship between surface roughness and cutting parameters was established using response surface methodology(RSM).It is concluded that the cutting speed,laser power and water pressure played an important role in surface roughness of cut *** surface roughness increased as an increase in laser *** decreased caused by increasing of cutting speed and water *** revealed that the surface quality of NCAW part was improved using the optimized combination of cutting *** established quadratic mathematical model of a good prediction is helpful for matching suitable cutting parameters to obtain expected surface quality.

Performance Study of a Self-Excitation Dual Stator Winding Induction Generator for Renewable Distributed Generation Systems

Renewable power generation is a suitable technology used to deliver energy locally to customers especially in remote regions. Wind energy based on induction generator situates in a foreground position in the total energy produced using renewable sources. In the last few decades, a new self- excitation generator was based on multi-stator induction strongly emerges. This article presents a systematic modelling, a detailed analysis and the performance analysis of self-excitation dual stator winding induction generator (SE-DSWIG). The modelling of the SE-DSWIG was done with taking in account the common mutual leakage inductance between stators and the magnetizing inductance, which played a principal role in the stabilization of the output voltage in the steady state. The generator feeds the end user emulated by an inductive-resistive load. In order to simulate the weather conditions’ variation, a step change of the prime mover speed was applied on the SE-DSWIG. A passive series and shunt compensator was used to mitigate the voltage sag and swell appeared in the power system due to wind variation and the lack of reactive power consumed by the inductive load.

激光切割单板的理论工艺参数求解与试验验证

【目的】利用固体激光器切割单板,获取合理的切割工艺参数,降低切缝宽度以减少锯路损失,保证单板切口断面平整度。【方法】从激光脉冲能量角度建立激光切割单板数学模型,获得激光输出功率(Pb)与激光切割速度(v)之间的关系表达式。以厚度0.3 mm、含水率10%的黑胡桃单板为加工对象,选取2组激光输出功率进行试验验证,每组激光输出功率在6种不同切割速度下切割,理论切割速度在6种试验切割速度范围内,加工后多次测量直线切缝宽度取平均值,在40×10光学显微镜下观察切割后试件切缝宽度及切口断面平整度,找出最优试验结果对应的切割速度,并与计算的理论切割速度进行对比。【结果】第1组激光输出功率(脉冲电压400 V,单脉冲能量96.5 mJ,脉冲频率10 Hz)下,当切割速度为2.5 mm·s^-1时可保证单板切缝被完全切透,形成连续完整的切缝,切缝宽度最窄,平均值为0.544 mm,锯路损失最小,且切口断面较为平整;第2组激光输出功率(脉冲电压500 V,单脉冲能量198.2 mJ,脉冲频率10 Hz)下,当切割速度为5.0和5.5 mm·s^-1时可保证单板切缝被完全切透,切缝宽度最窄,平均值分别为0.69和0.62 mm,且切口断面也较为平整。对比2组试验数据得出,当切割速度与理论切割速度接近时,单板切口连续,切缝宽度最小;当切割速度低于理论切割速度时,切缝宽度逐渐增大;当切割速度高于理论切割速度时,切口容易出现不连续现象。【结论】利用固体激光器切割单板时,切割工艺参数对切缝宽度和切口断面平整度具有直接影响,合理的切割工艺参数对获得较小的切缝宽度至关重要,每个激光器的输出功率对应着最佳切割速度,在实际加工过程中,应保证切割速度与理论切割速度一致或接近,以使切割效果更好。

信息安全在任何系统中都是必不可少的部分,而对于那些在网络和通信中使用的嵌入式系统如机顶盒、无线接入点、电话等来说,信息安全显得尤为重要。各种通过无线电系统提供的信息服务被引入到了众多领域并发挥了重要作用。付费电视系统就是这种控制访问系统的一个例子。这种系统包含了用户保密的个人信息如授权控制消息和私密信息（加密算法和／或密钥）。这些信息可以很容易的被复制,攻击者获得这些加密信息后就可以免费地观看电视。所以,在非安全环境中保障无线嵌入式系统操作的安全性就变得越来越具有挑战性。本文主要研究解决现有付费电视访问控制弱点（私密信息的复制）的方法。这个问题关键在于如何防止从对称密钥算法中解析加密信息的攻击,而这些对称密钥算法通常在智能卡、存储器或集成接收设备（Integrated Receiver Devices, IRD）七实现。为了克服这些漏洞,本文探讨了集成接收设备上的安全漏洞所带来的威胁,并采用一种结合随机验证算法（random check algorithm）的密码系统来构建一个新的基于嵌入式平台的控制访问安全系统（Control Access Secure Scheme, CASS）。为了解决这些问题,本文采用的策略主要包括：将所有的保密信息存储于服务器端（Service Provider, SP）来解决将保密信息存储于设备端带来的问题。这样服务提供商就可以随机地改变密钥并使其在集成接受设备的认证过程中暂时有效,而不会中断服务。 -采用单通信协议（Single Communication Protocol, SCP）保证服务供应端与集成设备接收端的安全通信。这也是在系统启动的认证之后采取的第二层安全机制。通过建立一种多层次的安全机制来提高安全抽象层的架构。这也就提升了系统的安全级别。 -服务供应端发出的随机认证请求（Random Authentication Request, RAR）用来完成对集成接收端的认证。它会在随机的服务器时间内发送,可以解决只在系统启动时进行认证的缺陷。这样,在每次服务器端发送随机认证请求之后,攻击者们都会遇到重新认证的问题。 -所有技术和机制都在嵌入式平台（GM8120）上实现,以便在低功耗条件下提升系统的执行效率和安全级别。此外,认证步骤使用一个加密机制,整个过程采用AES算法进行处理。本文第一个提出基于无线电频率的随机认证请求算法,它可以在服务期间有效地发现所有的非授权IRDs,之前的研究者都没有在这种情况下考虑问题。本文针对抗边际信道攻击进行了改进,这对加强以后的无线嵌入式系统安全性有一定的参考价值,以后该问题必将需要更高级别的数据加密方法。实验结果显示私密信息（如密钥和算法）可以通过SCP和RARs机制成功地进行传输。此外,本文提出的控制访问安全策略使得基于嵌入式系统的付费电视系统的信息安全性得到了加强。

Islanding Detection Method for Multi-Inverter Distributed Generation

Islanding detection is an essential function for safety and reliability in grid-connected distributed generation (DG) systems. Several methods for islanding detection are proposed, but most of them may fail under multi-source configurations, or they may produce important power quality degradation which gets worse with increasing DG penetration. This paper presents an active islanding detection algorithm for Voltage Source Inverter (VSI) based multi-source DG systems. The proposed method is based on the Voltage Positive Feedback (VPF) theory to generate a limited active power perturbation. Theoretical analyses were performed and simulations by MATLAB /Simulink /SimPowerSystems were used to evaluate the algorithm’s performance and its advantages concerning the time response and the effects on power quality, which turned out to be negligible. The algorithm performance was tested under critical conditions: load with unity power factor, load with high quality factor, and load matching DER’s powers.

Hypopharyngeal Cancer: Epidemiological, Clinical and Paraclinical Aspects at the University Hospital Center (CHU) Gabriel Toure in Bamako

Introduction: Hypopharyngeal cancers are ubiquitous cancers. These are very lymphophilic cancers;cervical lymph node metastases are present in 60% to 80% of patients at the time of diagnosis. Alcohol and tobacco have been implicated in their genesis. Objective: To study the epidemiological, clinical and paraclinical aspects of hypopharyngeal cancer in the ENT department of the CHU Gabriel Toure. Patients and Methods: It is a retrospective and prospective quantitative study covering a period of 4 years from June 2014 to July 2018;all patients who presented with hypopharyngeal cancer were included. Results: Thirty-five (35) patients were collected during the study period. The mean age of the patients was 44 ± 14 years with extremes of 16 and 78 years. The sex ratio was 0.47. Dysphagia was the reason for consultation at 100%. The average consultation time was 14 months, with extremes of 3 to 26 months. Smoking intoxication was found in 13 patients. The piriform sinus was interested in 52%. More than half of the patients were received at the advanced stage (T3 - T4). The histology was dominated by the well-differentiated epidermal carcinoma (91%). Conclusion: Hypopharyngeal cancer is common and occurs at all ages. T3 - T4 tumors are bad factors, hence early diagnosis and appropriate management are important for improving the prognosis.
