BACKGROUND Total hip and knee replacements are being performed in increasing numbers in progressively younger patients with higher activity demands. Many such patients have expectations of returning to athletic activity post-operatively yet are not always able to do so and the reasons behind this have not been extensively examined. We hypothesise that any reasons for a failure to return to athletic activity post-operatively are *** To quantify the return to athletic activity following lower limb joint arthroplasty and understand qualitative reasons for altered activity *** A single centre, single surgeon retrospective questionnaire for hip and knee arthroplasty patients under age 60 years, minimum two years post-surgery with exclusion criteria of multiple degenerative joint involvement and multiple medical co-morbidities. Outcomes were validated joint-specific(Oxford hip and knee) and lifestyle questionnaires [short form 12(SF-12) and University of California, Los Angeles(UCLA)] and an activity questionnaire assessing ability participation in athletic activity post-operatively. Statistical analysis was performed on the validated outcome data, including comparison between hip and knee replacements. Frequency tables were produced to quantify the different athletic activities participated in by *** Responses were received from 64 patients(80% response rate). There was a statistically significant improvement in Oxford hip and knee scores following surgery. SF-12 scores also improved for all patients, but no statistically significant difference was seen between joints(P = 0.88). Mean UCLA scores pre-operatively were 7.67 and at two years post-operatively were 7.69, with no statistically significant change(P = 0.91). The calamitous consequences of 2017 Hurricane Maria for the Caribbean island of Dominica highlighted the acute and increasing susceptibility of the region to disasters. Despite increasing international attention to disaster risk reduction, recovery from hazard events can be especially lengthy and difficult for small island developing states. In this article, we build on existing understandings of disaster risk as a physical and social condition, showing that historical processes are fundamental to understanding how conditions of risk emerge and persist over time. We take an integrated approach to analyzing the drivers of risk accumulation, using the example of Dominica, where processes set in motion during the colonial period have shaped the location of people and assets, the degree to which they might be harmed, the societal repercussions of that harm and the prospects for recovery. We focus on the underlying economic vulnerabilities and physical exposure to hazards created by agricultural, economic, and social practices, and successive disaster responses that have constrained recovery. Uncovering these historical drivers and persistent issues, elucidates lessons for pursuing a more resilient development trajectory, including through the promotion of economic restructuring and diversification, and land reform.

Evaluation of Molecular Techniques in Characterization of Deep Terrestrial Biosphere

A suite of molecular methods targeting 16S rRNA genes (i.e., DGGE, clone and high-throughput [HTP] amplicon library sequencing) was used to profile the microbial communities in deep Fennoscandian crystalline bedrock fracture fluids. Variation among bacterial 16S rRNA genes was examined with two commonly used primer pairs: P1/P2 and U968f/U1401r. DGGE using U968f/ U1401r mostly detected β-, γ-proteobacteria and Firmicutes, while P1/P2 primers additionally detected other proteobacterial clades and candidate divisions. However, in combination with clone libraries the U968f/U1401r primers detected a higher bacterial diversity than DGGE alone. HTP amplicon sequencing with P1/P2 revealed an abundance of the DGGE bacterial groups as well as many other bacterial taxa likely representing minor components of these communities. Archaeal diversity was investigated via DGGE or HTP amplicon sequencingusing primers A344F/ 519RP. The majority of archaea detected with HTP amplicon sequencing belonged to uncultured Thermoplasmatales and Pendant 33/DHVE3, 4, 6 groups. DGGE of the same samples detected mostly SAGMEG and Methanosarcinales archaea, but almost none of those were revealed by HTP amplicon sequencing. Overall, our results show that the inferred diversity and composition of microbial communities in deep fracture fluids is highly dependent on analytical technique and that the method should be carefully selected with this in mind.
