Background:Bariatric surgery represents an important treatment option for severely obese patients with nonalcoholic fatty liver disease(NAFLD).However,there remains inadequate data regarding the effects of different bariatric procedures on various NAFLD parameters,especially for histological ***,this meta-analysis aimed to compare the effects of restrictive bariatric procedures and foregut bypass on the metabolic,biochemical,and histological parameters for patients with ***:Medline and Embase were searched for articles relating to bariatric procedures and *** meta-analysis was conducted to compare efficacy of bariatric procedures ***-procedure with subgroup analysis to further compare restrictive against foregut bypass ***:Thirty-one articles involving 3,355 patients who underwent restrictive bariatric procedures(n=1,460)and foregut bypass(n=1,895)were *** foregut bypass(P<0.01)and restrictive procedures(P=0.03)significantly increased odds of fibrosis *** to restrictive procedures,foregut bypass resulted in a borderline non-significant decrease in fibrosis score(P=0.06)and significantly lower steatosis score(P<0.001).For metabolic parameters,foregut bypass significantly lowered body mass index(P=0.003)and low-density lipoprotein(P=0.008)compared to restrictive *** significant differences were observed between both procedures for aspartate aminotransferase(P=0.17)and alkaline phosphatase(P=0.61).However,foregut bypass resulted in significantly lower gamma-glutamyl transferase than restrictive procedures(P=0.01)while restrictive procedures resulted in significantly lower alanine transaminase than foregut bypass(P=0.02).Conclusions:The significant histological and metabolic advantages and comparable improvements in biochemical outcomes support the choice of foregut bypass over restrictive bariatric procedures in NAFLD management.

Background: Late- onset tay- Sachs disease (LOTS) is an adult- onset, autosomal recessive, progressive variant of GM2 gangliosidosis, characterized by involvement of the cerebellum and anterior horn cells. Objective: To determine the range of visual and ocular motor abnormalities in LOTS, as a prelude to evaluating the effectiveness of novel therapies. Methods: Fourteen patients with biochemically confirmed LOTS (8 men; age range 24 to 53 years; disease duration 5 to 30 years) and 10 age- matched control subjects were studied. Snellen visual acuity, contrast sensitivity, color.vision, stereopsis, and visual fields were measured, and optic fundi were photographed. Horizontal and vertical eye movements (search coil) were recorded, and saccades, pursuit, vestibulo- ocular reflex (VOR), vergence, and optokinetic (OK) responses were measured. Results: All patients showed normal visual functions and optic fundi. The main eye movement abnormality concerned saccades, which were “ multistep,” consisting of a series of small saccades and larger movements that showed transient decelerations. Larger saccades ended earlier and more abruptly (greater peak deceleration) in LOTS patients than in control subjects; these changes can be attributed to premature termination of the saccadic pulse. Smooth- pursuit and slow- phase OK gains were reduced, but VOR, vergence, and gaze holding were normal. Conclusions: Patients with late- onset tay- Sachs disease (LOTS) show characteristic abnormalities of saccades but normal afferent visual systems. Hypometria, transient decelerations, and premature termination of saccades suggest disruption of a “ latch circuit” that normally inhibits pontine omnipause neurons, permitting burst neurons to discharge until the eye movement is completed. These measurable abnormalities of saccades provide a means to evaluate the effects of novel treatments for LOTS.

Dear editor,Prostate cancer(PCa)remains a major healthcare burden in men globally[1].Most patients present with localized disease,and treatment is recommended based on risk classification systems like the National Comprehensive Cancer Network(NCCN)[2].However,these methods are imprecise for estimating metastasis-free survival and prostate cancer-specific mortality and thus biomarkers that can predict tumor aggression are needed[3–5].Several studies have since characterized the molecular landscape of localized PCa in White[4,5]and Black/African-American men[6],but data is lacking in Asian *** Chinese Prostate Cancer Genome and Epigenome Atlas(CPGEA)reported on the genomic and epigenomic landscape of 208 PCa of men from China[7].Comparative analyses between the CPGEA cohort and data from The Cancer Genome Atlas(TCGA)revealed higher frequencies of Forkhead box A1(FOXA1)and chromodomain-helicase DNA-binding 1(CHD1)mutations,and lower frequencies of phosphatase and tensin homolog(PTEN)mutations and transmembrane protease serine 2-E26 transformationspecific related gene(TMPRSS2-ERG)fusion in Chinese compared with White men[7].These preliminary findings highlight the presence of race-specific differences in molecular phenotypes of PCa.

地点:乌干达国家结核病治疗中心Mulago医院与协和临床医学研究中心,乌干达,Kampala。目的:比较感染HIV和未感染HIV的结核病人在标准化短程化疗(SCC)期间的痰结核杆菌定量。设计:选择14个感染HIV和22个未感染HIV涂阳成年肺结核病人,比较在标准的4药短程化疗的开始以及治疗期间二者的临床特征以及定量痰菌,定量痰菌是按照定量抗酸杆菌(AFB)涂片,菌落形成单位分析以及在BACTEC放射测量性液体系统培养出现阳性结果的时间这3个指标来测量。结果:肺空洞和肺腺病在两组人群分布不同,前者多见于未感染HIV肺结核病人,而后者多见于感染HIV肺结核病人;两组间开始的人口学特征、临床表现、放射学以及实验室检查结果没有显著性差异。两组间治疗前的痰菌平均数(感染HIV肺结核病人为6.5±0.51 lor.(10)AFB/ml,5.91±0.91 log_(10)cfu/ml以及BACTEC培养出现阳性为1.8±1.7天;未感染HIV肺结核病人为6.32±0.85 log10AFB/ml,5.58±0.68log10cfu/ml以及BACTEC培养出现阳性1.9±1.2天相似。病人通过标准化短程化疗(SCC),两组病人的临床和细菌学反应及其治疗结果无差异。结论:感染HIV和未感染HIV的成年涂阳结核病人在标准化短程化疗开始以及治疗期间的定量痰菌没有显著性差异。

Objective:To evaluate the antidiabetic and hypolipidemie activities of ethanolic leaf extract and Fraction of Melanthera scandens(***) in alloxan-induced diabetic ***:*** leaf extract/fractions(37-111 mg/kg) were administered to alloxan-induced diabetic rats for 14 days and blood glucose levels(BGL) of the diabetic rats were monitored at intervals of 7 hours for acute study and 14 days lor.prolonged *** profiles of the treated diabetic rats were determined after the period of ***:Treatment of alloxan-induced diabetic rats with the extract/fractions caused a significant(Pjustifies its use in ethno medicine and can be exploited in the management of diabetes.
