Ultrasound Role in Diagnosis of Carpal Tunnel Syndrome and Postoperative Evaluation in Endoscopic Carpal Tunnel Release

Carpal tunnel syndrome (CTS) represents about 90% of all entrapment neuropathies and is caused by entrapment of the median nerve while passing in the carpal tunnel. US has revealed equal effectiveness as magnetic resonance imaging (MRi) in diagnosis of CTS;however further attention should be given in case of equivocal results from nerve conduction studies (NCS) and/or before endoscopic surgical intervention. This study was conducted to evaluate the role of US in diagnosis and treatment of CTS which was done by endoscopic release. This is a prospective study including patients having CTS who were treated by endoscopic release between December 2017 and December 2018. All cases were evaluated by electrophysiological studies and US at the preoperative period. US was used to measure the cross sectional area (CSA) of the median nerve. Patients underwent endoscopic carpal tunnel release and were called for follow up after 1, 3, and 6 months for clinical and US assessment of the median nerve. Forty cases were included with mean age of 27.85 years. Numbness was the predominant symptom (92.5%) followed by wrist pain (85%). The mean diameter of median nerve showed a significant decrease at different time points (P itivity of US in diagnosis of CTS was 87.5% in the preoperative assessment as compared to electrophysiological study. The sensitivity of US in detecting the improvement of CTS as compared to clinical examination increased from 28% at 1 month postoperative, 53.4% at 3 months to 92.1% at 6 months. Ultrasonography is a sensitive non-invasive diagnostic tool in diagnosis of CTS preoperative and diagnosis of improvement of the patients postoperatively especially at 6 months.

A Study of Heavy Metals in the Dust Fall around Assiut Fertilizer Plant

A study of an environmental assessment of dust fall and the associated heavy metal contents was conducted during the period from the first of March 2011 to the end of February 2012 at adjoining area of a phosphate fertilizer plant. Around the industrial area 8 dust fall stations were established and one of them was built upwind far from pollution activities to be taken as a control sample. Dust fall samples collected monthly weighed and then prepared to be analyzed through inductively Coupled Plasma-Mass Spectroscopy (iCP-MS) to obtain heavy metal concentration. Meteorological parameters influencing the distribution of dust fall such as wind speed and direction, temperature, humidity, rain fall and pressure were determined. Results showed that deposition flow rates were 38.2. 47.5, 57.7, 44.3, 39.4, 38.2, 42.7 and 5.9 g/m2·month for the sites No., 1, 2, 3, 4, 5, 6, 7 and 8 respectively, and were compared with the findings of other investigators of like industrial areas worldwide. Levels of heavy metal As, Cu, Pb, Zn, Cd, and Hg in the deposited dust fall were 3.30, 26.46, 22.33, 235.00, 4.53 and 3.80 μg/g respectively. Enrichment coefficients of the heavy metals in the dust fall were found to be significant and reached the values 1.81, 0.90, 0.85, 0.65, 0.41 and 0.35 for zinc, lead, cadmium, copper, mercury and arsenic respectively. The paper ends with results and recommendations suggesting a methodology to remediate the investigated area polluted with heavy metals and control measures for the fertilizer plant to reduce pollution into the surrounding environment.

Effect of Kind Rolling on the Properties of Plasma-Formed Nitride Layers on Fe93Ni4Ti3

The nitriding behavior of cold rolled Fe93Ni4Ti3 specimens was compared with that of hot rolled specimens of the same materials. Radio frequency (rf) nitriding was performed for 10 minutes in a 10-2 mbar nitrogen atmosphere. The continuous plasma power was varied from 300-550 W in steps 50 W or less. Results of optical microscopy (OM), x-ray diffraction (XRD) and microhardness measurements (Hv) are presented and discussed with regard to the influence of kind rolling on the nitriding behavior, particularly nitride formation and nitride layer growth on mechanical properties. The results show a remarkable increase of nitrogen diffusivity and microhardness of cold rolled nitride samples. These best results may be attributed to enhancement of the defect and/or a compressive stress.

A Modified interactive Stability Algorithm for Solving Multi-Objective NLP Problems with Fuzzy Parameters in its Objective Functions

This paper presents a modified method to solve multi-objective nonlinear programming problems with fuzzy parameters in its objective functions and these fuzzy parameters are characterized by fuzzy numbers. The modified method is based on normalized trade-off weights. The obtained stability set corresponding to α-Pareto optimal solution, using our method, is investigated. Moreover, an algorithm for obtaining any subset of the parametric space which has the same corresponding α-Pareto optimal solution is presented. Finally, a numerical example to illustrate our method is also given.

Clinical investigation: The Presence of Viral Meningitis without Pleocytosis among Pediatric Patients

Background:?Viral meningitis (VM) is mostly common among infants. its induction by enteroviruses (EVM) is associated with morbidity and is primarily diagnosed by lumbar puncture, which may yield false negatives. We evaluated the frequency of VM by polymerase chain reaction (PCR) among infants with no detected pleocytosis. Furthermore, as literature suggests EVM essentially occurs during summer and fall, we characterized the monthly distribution of EVM cases. Methods:?infants diagnosed with VM from June 2009 to May 2010 were enrolled in the current prospective study. Following each lumbar puncture, CSF was tested for enteroviruses by PCR. Outcome measures were i) the percentage of EVM cases revealed solely by PCR;and ii) the monthly distribution of EVM cases. Results:Enrolled were 173 VM-diagnosed infants, of whom 75 (43.4%) tested positive in CSF-PCR. Of these, no pleocytosis was indicated in 43 (57%), specifically in 70% and 42% of infants younger than 90 days and older than 1 year, respectively. Furthermore, 119 (69%) infants were admitted during June-November while 54 (31%) during December-May. Conclusions:Current findings stress the high frequency of infants who were tested negative for pleocytosis, yet were diagnosed with VM by PCR. This was especially noticeable among infants younger than 3 months, possibly reflecting their decreased ability to mount a robust inflammatory response to EV infection. CSF-PCR may be warranted in pediatric patients who test negative for pleocytosis. While most EVM cases occurred during the summer and fall, EVM-infants were admitted to the hospital all through the year.

Hydro Matrix Power Wheels Generate More than 5 GW/h from Main Branch Canals (River Nile) in Egypt

in this paper, a new hydromatrix power wheels generation unit is presented. This presented hydromatrix power unit is aimed to convert the water movement in the main river branches into electrical power. it is found that the generated power based on the proposed hydromatrix power can be generated about 5 GW/h in Egypt. The proposed hydromatrix power unit based on the under shot water wheel type and the generated electrical power using this proposed unit is calculated at head;difference equal to 1 meter is presented.

A novel cytogenetic abnormality r(7)(::p11.2->q36.3::) in a Philadelphia-positive chronic myeloid leukemia case

The so-calledPhiladelphia(Ph) chromosome is present in more than 90% of chronic myeloid leukemia (CML) cases. it results in juxtaposition of the 5' part of the BCR gene on chromosome 22 and the 3' part of the ABL1 gene on chromosome 9. An additional acquired monosomy 7 or deletion of 7q is associated with poor prognosis in a variety of myeloid disorders. Here we report a novel Ph chromosome positive CML case with a ring chromosome 7 [r(7)]. immunophenotyping was compatible with CML, although 4.5% of total leucocytes appeared like acute myelogeneous leukemia (AML) subtype M2. The r(7) was characterized in detail by array-proven multicolor banding (aMCB), the latter being of enormous significance to characterize breakpoint regions in detail. Underlying mechanisms and prognostic are discussed, as ring chromosomes are rare cytogenetic abnormalities in hematopoietic malignancies.

Soybean seed is a major source of phytochemical compounds that impact human health nutrition and livestock meal. Charcoal rot is a disease caused by the fungus Macrophomina phaseolina (Tassi) Goid, and thought to infect the plants through roots by a toxin-mediated mechanism, resulting in yield loss and poor seed quality, especially under drought conditions. Limited information is available on the effect of charcoal rot on seed phytochemical compounds and mineral nutrition in soybean. Therefore, the objective of this research was to investigate the effect of charcoal rot infection on seed phenol, seed coat lignin, isoflavones, and minerals using susceptible (S) (DK 3964) and moderately resistant (MR) (AG 3905) maturity group (MG)iii soybean cultivars to charcoal rot. A two-year field experiment was conducted, and infested soil with charcoal rot (infested soil conditions, iNF) or control (non-infested soil conditions, NiNF) was used. The results showed that the moderately resistant genotype had higher concentrations of seed phenolics, total isoflavones, and seed coat lignin under infested and non-infested conditions and under irrigated or non-irrigated conditions compared with the susceptible genotype. The same general trend was found for seed K, Ca, P, Mn, Zn, B, and Cu concentrations in the moderately resistant genotype compared with the susceptible genotype. Our research demonstrated that these seed phytochemical constituents may explain the differences between susceptible and moderately resistant cultivars and may play an important role in the resistance to charcoal rot.

This paper is devoted to the analysis of the two-layer shallow-water equations representing gravity currents. A similarity technique which is the characteristic function method is applied for this study. The application of the characteristic function method makes it possible to obtain the similarity forms depending on a group of infinitesimal transformations. Thus, the number of independent variables is reduced by one and the governing partial differential equations with the auxiliary conditions reduce to a system of ordinary differential equations with the appropriate auxiliary conditions. Numeric solutions are presented and discussed.
