We measured the main plasma parameters(density of electron,temperature of electron and ion confinement time)and beam intensity of various heavy ions as a function of B_(min).The B_(min) strongly affects the field gradient at the resonance zone,consequently the plasma parameters and beam intensity are changed. Based on these experimental results,we started to construct new 18GHz ECRIS and make a detailed design of the 28GHz SC-ECRIS for RIKEN RI beam factory project.

At RIKEN,three ECR ion sources(10GHz ECRIS,18GHz ECRIS and liquid He-free SC-ECRIS) are operated as external ion sources of heavy ion *** the last year,multi-charged uranium ion beam was produced from 18GHz ECRIS by using UF_6 and the ^(238)U ion was successfully accelerated by the accelerator complex which consists of the RFQ linear accelerator,RIKEN heavy ion linear accelerator(RILAC)and RIKEN ring cyclotron accelerator(RRC).The typical beam intensity of^(238)U^(14+) was about 2pμA on faraday cup after analysing magnet.^(70)Zn beam was still supplied for the new super-heavy element search experiment with insertion *** beam of^(70)Zn^(16+) was produced for long term(～43 days)without vacuum break and remarkably low material consumption rate(～100μgr/h).We already supplied Zn beam longer than 200 days for this experiment.^(48)Ca ion was also produced by insertion method using^(48)CaO rod for the nuclear physics *** this contribution,we will present ion source parameter and techniques for production of each of the metal ions.

Tuberculous otitis media(TOM) is a rare manifestation caused by Mycobacterium tuberculosis with low incidence rates among extrapulmonary tuberculosis cases. Diagnosis is often delayed because of the presence of several clinical manifestations and the high prevalence of secondary bacterial infections. Few reports have attributed secondary bacterial infections in patients with TOM to commensal Neisseria. Thus, understanding the pathogenic mechanisms and clinical features of commensal Neisseria is important, considering its recent presentation as an infection-causing pathogen. Neisseria mucosa is a commensal inhabitant in humans and is generally considered non-pathogenic but can cause infection in rare cases. Here, we report an atypical secondary infection caused by Neisseria mucosa in an 81-year-old woman with TOM being treated for pulmonary tuberculosis. Direct purulent otorrhea smear microscopy revealed no acid-fast bacilli using Ziehl-Neelsen staining, whereas the phagocytosis of gram-negative cocci by white blood cells was confirmed using Gram staining. Otorrhea culture revealed the growth of N. mucosa. Subsequently, M. tuberculosis infection in the otorrhea was identified using a culture-based method. Vigilance is critical for the early detection of TOM to prevent further complications. This report raises awareness regarding TOM and provides insight into the pathogenicity of N. mucosa in otitis media.

Missed Pancreaticobiliary Malignancy: The Flaw of the Expedited Cholecystectomy

Background: Early cholecystectomy has been recommended for patients with acute cholecystitis and gallstone pancreatitis. However, patients with pancreaticobiliary malignancy may present acutely with similar symptoms. We hypothesize that the diagnoses of these malignancies may potentially be delayed as an unintended consequence of expedited cholecystectomies. This study reviews a cohort of patients who underwent pancreaticoduodenectomy (PD) to identify those who underwent a separate cholecystectomy before their PD. Methods: We retrospectively reviewed 162 PDs performed between 2012 and 2022. Data collected included: demographics, disease etiology and the presence of cholelithiasis. We identified patients who had a previous cholecystectomy and the time elapsed before PD as well as procedures done during the interval. We reported detailed case summaries on those patients who had a cholecystectomy within 1 year of PD. Results: In the entire cohort, mean age was 65 years, 54% were males, and 83% had a malignant reason for PD. Thirty-one patients had cholelithiasis with 23 (14%) patients having had previous cholecystectomy. Six patients had cholecystectomy within 1 year of PD. They had the following malignancies: ampullary—3, pancreas—1, cholangiocarcinoma—1 and neuroendocrine—1. Four of these patients had expedited cholecystectomy on their index hospital admission and were later found to have a periampullary malignancy with further work up. Conclusions: Pancreaticobiliary malignancies can be difficult to diagnose, and surgeons should not overlook these potential diagnoses when considering expedited cholecystectomy. Future studies in large cohorts are needed to identify high risk candidates who should undergo more detailed testing to exclude malignancy before proceeding with cholecystectomy.
