BACKGROUND The incidence of diabetes mellitus type 1(DM1)has been rising worldwide because of improvements in diagnostic techniques and improved access to care in countries with lower socioeconomic status.A new anti-CD4 antibody,Teplizumab,has been shown to delay the progression of DM1 and is the only medication approved for this ***,more information is needed about the safety profile of this *** To identify the odds ratios(OR)of systems-based adverse effects for Teplizumab when compared to *** An extensive systematic review was conducted from the inception of the medication until December 31,*** clinical trials and studies that evaluated Teplizumab vs placebo were included in the initial *** study protocol was designed using Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines guidelines and was registered in PROSPERO(ID:CRD42024496169).Crude OR were generated using RevMan Software version *** After screening and review,5 studies were selected to determine the risk of adverse effects of teplizumab compared to placebo.A total of 561 patients were included in the study *** adverse effects and system-based adverse effects were studied and *** determined that patients receiving Teplizumab had a higher risk of developing gastrointestinal(GI)(OR=1.60,95%CI:1.01-2.52,P=0.04),dermatological(OR=6.33,95%CI:4.05-9.88,P<0.00001)and hematological adverse effects(OR=19.03,95%CI:11.09-32.66,P<0.00001).These patients were also significantly likely to have active Epstein-Barr Virus infection(OR=3.16,95%CI:1.51-6.64,P<0.002).While our data showed that patients receiving Teplizumab did have a higher incidence of total adverse effects vs placebo,this finding did not reach statistical significance(OR=2.25,95%CI:0.80-6.29,P=0.12).CONCLUSION Our systematic review suggests that Teplizumab patients are at risk for significant adverse effects,primarily related to GI,dermatological,and hematolog

Kikuchi-Fujimoto Disease: A Case of Lymphadenitis Following COVID-19 Infection

Introduction: Kikuchi-Fujimoto disease (KFD), also known as histiocytic necrotizing lymphadenitis, is a rare disease of uncertain origin proposed to be secondary to viral or autoimmune causes. It presents as unilateral cervical lymphadenopathy and fever, typically resolving within a few months, however, it mimics malignant lymphoma, leading to extensive work-up fearing malignancy. Case History: A 33-year-old female with a history of Sjögren’s syndrome, rheumatoid arthritis (not on immunosuppressive therapy) and recent COVID-19 infection two months ago presented with episodes of flu-like symptoms;fever (101˚F), chills, and myalgias for two months. Upon evaluation, she had leukopenia (WBC 1.8 k/uL), neutropenia (1.0 k/uL), elevated CRP (134 mg/L), and CT neck demonstrated multiple enlarged cervical lymph nodes with necrosis. The patient was started on Cefdinir and doxycycline for a possible atypical infection given neutropenic fevers and the patient continued to have fevers. A biopsy showed focal necrosis with no evidence of malignancy, suggesting KFD that was likely triggered by a recent COVID infection. She was treated with Prednisone and naproxen, which led to an improvement in symptoms and recovery of her pancytopenia. Discussion: This case is unique since the patient’s recent infection with COVID-19 may have triggered the clinical manifestations of KFD. There have been a few case reports of children who were diagnosed with KFD after a COVID-19 infection and adults diagnosed with KFD following COVID vaccinations;however, this is the first case report involving a young adult in her thirties who was diagnosed with KFD two months after COVID-19.

REAL CODED GENETIC ALGORITHM FOR STOCHASTIC HYDROTHERMAL GENERATION SCHEDULING

The intent of this paper is to schedule short-term hydrothermal system probabilistically considering stochastic operating cost curves for thermal power generation units and uncertainties in load demand and reservoir water inflows. Therefore, the stochastic multi-objective hydrothermal generation scheduling problem is formulated with explicit recognition of uncertainties in the system production cost coefficients and system load, which are treated as random variable. Fuzzy methodology has been exploited for solving a decision making problem involving multiplicity of objectives and selection criterion for best compromised solution. A real-coded genetic algorithm with arithmetic-average-bound-blend crossover and wavelet mutation operator is applied to solve short-term variable-head hydrothermal scheduling problem. Initial feasible solution has been obtained by implementing the random heuristic search. The search is performed within the operating generation limits. Equality constraints that satisfy the demand during each time interval are considered by introducing a slack thermal generating unit for each time interval. Whereas the equality constraint which satisfies the consumption of available water to its full extent for the whole scheduling period is considered by introducing slack hydro generating unit for a particular time interval. Operating limit violation by slack hydro and slack thermal generating unit is taken care using exterior penalty method. The effectiveness of the proposed method is demonstrated on two sample systems.

Local fluoride contamination and bacterial infections in potable water have dangerous effects on the human body and are today a global *** this study,we have synthesized a pH-responsive bifunctional biopolymer nanocomposite(HAZ)of humic acid with incorporating aluminum zirconium bimetallic oxide by deep freeze–drying *** nucleation and interconnection of nanoparticles form a highly porous network because of sublimation of frozen *** duo nanocomposite has efficiently worked for fluoride removal and showed potent antibacterial activity against the Escherichia coli Gram-negative and Staphylococcus aureus Gram-positive *** X-ray photoelectron spectroscopy(XPS)analysis demonstrates that the hydroxyl groups act as a pivot in the ion exchange process of adsorption,each element of bimetallic oxide primarily takes part in the adsorption *** maximum adsorption capacity of the adsorbent was 180.62 mg/g at pH *** parameters like Gibbs free energy change(△G0),entropy(△S0),and enthalpy(△H0)indicate that the process was endothermic,feasible,and taken place by a chemisorption *** is the first novel freeze–dried bifunctional biopolymer nanocomposite composed of humic acid natural polymer incorporated with Al–Zr metal oxide,and it exhibited three times higher adsorption efficacy with excellent antibacterial action at a concentration of 5μg/mL of the nanocomposite.

FUZZY SATISFYING INTERACTIVE MULTIOBJECTIVE THERMAL POWER DISPATCH： SWT APPROACH

In multiobjective optimization, trade-off analysis plays an important role in determining most preferred solution. This paper presents an explicit interactive trade-off analysis based on the surrogate worth trade-off function to determine the best compromised solution. In the multiobjective framework thermal power dispatch problem is undertaken in which four objectives viz. cost, NOx emission, SOx emission and COx emission are minimized simultaneously. The interactive process is implemented using a weighting method by regulating the relative weights of objectives in systematic manner. Hence the weighting method facilitates to simulate the trade-offrelation between the conflicting objectives in non-inferior domain. Exploiting fuzzy decision making theory to access the indifference band, interaction with the decision maker is obtained via surrogate worth trade-off （SWT） functions of the objectives. The surrogate worth trade-off functions are constructed in the functional space and then transformed into the decision space, so the surrogate worth trade-off functions of objectives relate the decision maker＇s preferences to non-inferior solutions through optimal weight patterns. The optimal solution of thermal power dispatch problem is obtained by considering real and reactive power losses. Decoupled load flow analysis is performed to find the transmission losses. The validity of the proposed method is demonstrated on 11-bus, 17-lines IEEE system, comprising of three generators.

BEST WEIGHT PATTERN EVALUATION BASED SECURITY CONSTRAINED POWER DISPATCH ALGORITHM

This paper presents a methodology which determines the allocation of power demand among the committed generating units while minimizes number of objectives as well as meets physical and technological system constraints. The procedure considers two decoupled problems based upon the dependency of their goals on either active power or reactive power generation. Both the problems have been solved sequentially to achieve optimal allocation of active and reactive power generation while minimizes operating cost, gaseous pollutants emission objectives and active power transmission loss with consideration of system operating constraints along with generators prohibited operating zones and transmission line flow limits. The active and reactive power line flows are obtained with the help of generalized generation shift distribution factors （GGDF） and generalized Z-bus distribution factors （GZBDF）, respectively. First problem is solved in multi-objective framework in which the best weights assigned to objectives are determined while employing weighting method and in second problem, active power loss of the system is minimized subject to system constraints. The validity of the proposed method is demonstrated on 30-bus IEEE power system.

Classification tool for the systematic histological assessment of hepatocellular carcinoma, macroregenerative nodules, and dysplastic nodules in cirrhotic liver

AIM： To design a classification tool for the histological assessment of hepatocellular carcinoma （HCC）, dysplastic nodules （DN）, and macroregenerative nodules （MRN） in cirrhotic liver. METHODS： Two hundred and twelve hepatocellular nodules （106 HCC; 74 IRN, 32 DN） were assessed systematically, quantitatively, and semiquantitatively as appropriate for 10 histological features that have been described as helpful in distinguishing small HCC, DN, and MRN in cirrhotic livers. The data were analyzed by multiple correspondence analysis （MCA）. RESULTS： HCA distributed HCC, DN, and HRN as defined by traditional histological evaluation as well as the individual histological variables, in a ＂malignancy scale＂. Based on the MCA data representation, we created a classification tool, which categorizes an individual nodular lesion as MRN, DN, or HCC based on the balance of all histological features （i.e., vascular invasion, capsular invasion, tumor necrosis, tumor heterogeneity, reticulin loss, capillarization of sinusoids, trabecular thickness, nuclear atypia, and mitotic activity）. The classification tool classified most （83%） of a validation set of 47 nodules in the same way as the routine histological assessment. No discrepandes were present for DN and MRN between the routine histological assignment and the dassification tool. Of 25 HCC assigned by routine assessment in the validation set, 8 were assigned to the DN category by the classification tool. CONCLUSION： We have designed a classification tool for the histological assessment of HCC and its putative precursors in cirrhotic liver. Application of this toolsystematically records histological features of diagnostic importance in the evaluation of small HCC.

Multiaxial fatigue life prediction of kiln roller under axis line deflection

This paper investigates the multiaxial fatigue life of the roller in rolling contact with wheels with respect to axis line deflection. The multiaxial fatigue criteria proposed by Wang and Brown, together with the ralnflow counting method and Miner- Palmgren＇s rule, are applied to the cumulative damage estimation and life prediction. As the axis line deflection of overlong kilns generally results in asymmetric load distribution on each roller, the load ratio is introduced to describe the deflection for quantitative stress analyses. The stress analyses are performed within the finite element code ANSYS. The tangential friction stress is calculated in terms ：of the condition of the rolling contact area. By taking one roller as an example, the plotted fatigue life versus load ratio curve discovers how the axis line deflection affects the fatigue life. This study is significant to prevent the fatigue failure of the roller and can provide basis to adjust and optimize the axis line of the rotary kiln.
