Objective The placebo response in depression studies is the change in symptoms amongst those who receive an inactive *** well-designed randomized controlled trials(RCTs)of depression have a high proportion of placebo responders,with little understanding as to *** present study assesses characteristics associated with the placebo response in a nutraceutical trial with a large proportion of placebo *** This is a secondary analysis of a nutraceutical depression RCT which identified no overall treatment benefit relative to placebo(n=69 in placebo group).We investigated participant characteristics such as socio-demographics,clinical features,and recruitment methods,and their association with the placebo *** genetic polymorphisms were also *** response was measured based on change in Montgomery-Asberg Depression Rating Scale *** association of these hypothesis-driven variables of interest and the placebo response was examined using linear mixed effects *** Greater levels of education,particularly pursuing post-high school education,better self-reported general health,marriage/de facto,greater improvement in the first trial week,and more failed antidepressant therapies in the current depressive episode were associated with greater placebo *** increased placebo response was not found in those recruited via social media nor in those with concomitant antidepressant *** nucleotide polymorphisms from the tryptophan hydroxylase 1(TPH1)gene(A779C and A218C)were weakly associated with greater placebo response,although the evidence was attenuated after accounting for multiple *** This is,to our knowledge,the first study within nutraceutical research for depression to assess the association between participant characteristics and variation in the placebo *** variables appeared to predict the placebo *** findings may encourage future trial design

分析2021-01-24南京医科大学附属儿童医院消化科1例发-肝-肠综合征(tricho-hepato-enteric syndrome,THES)患儿临床特征及基因变异情况,采集患儿及其父母外周血提取基因组DNA,使用二代测序对患儿进行基因检测,对检出的可疑变异进行一代Sanger验证及生物信息学分析。患儿临床特征为肝功能损害、腹泻、头发蓬松易断、生长迟缓。头发光镜下见脆性结节。基因测序提示患儿SKIV2L基因存在复合杂合新变异c.29C>T(p.P10L)和c.321C>G(p.H107Q)。两种变异既往均未见报道,位点较为保守,多种生物信息学预测为有害。由此得到SKIV2L基因复合杂合变异c.29C>T/c.321C>G为患者的致病原因的结论。新变异位点的检出丰富了SKIV2L基因的变异谱,为该家系的遗传咨询提供了依据。

Deep learning vs conventional learning algorithms for clinical prediction in Crohn's disease: A proof-of-concept study

Mechanism of delayed conduction of fellow eyes in patients with optic neuritis

To test the hypothesis that latency delay in the fellow eyes of optic neuritis（ON） patients and to compensate for delayed transmission of visual information, latency change of multi-focal visual evoked potential（mf VEP） traces in fellow eyes of 15 ON patients were analyzed. Patients with low risk（LR） for developing multiple sclerosis（MS） were examined separately from MS patients to isolate effect of cortical plasticity from potential pathological changes in disseminated disease. The small increase in latency in fellow eyes of LR group was statistically not significant. In MS patients, the latency was significantly delayed（P〈0.02）. The magnitude of the latency change in the fellow eyes did not correlate with the severity of latency delay in the affected eyes（R^2〈0.02, P=0.3）. The differences between ON patients with and without MS, reported here, suggest that the presence of disseminated disease plays critical role in latency delay of the fellow eye.

Role of bacterial and genetic factors in gastric cancer in Costa Rica

AIM： To evaluate several risk factors for gastric cancer （GC） in Costa Rican regions with contrasting GC incidence rate （GCIR）. METHODS： According to GCIR, 191 Helicobacter pylori （H pylori）-positive patients were classified into groups A （high GCIR, n = 101） and B （low GCIR, n = 90）. Human DNA obtained from biopsy specimens was used in the determination of polymorphisms of the genes coding for interleukin （IL）-1β and IL-10 by PCRRFLP, and IL-1RN by PCR. H pylori DNA extractions obtained from clinical isolates of 83 patients were used for PCR-based genotyping of H pylori cagA, vacA and babA2. Human DNA from gastric biopsies of 52 GC patients was utilized for comparative purposes. RESULTS： Cytokine polymorphisms showed no association with GCIR variability. However, gastric atrophy, intestinal metaplasia and strains with different vacA genotypes in the same stomach （mixed strain infection） were more frequently found in group A than in group B, and cagA and vacA s1b were significantly associated with high GCIR （P = 0.026 and 0.041, respectively）. IL- 1β＋3954_T/C （OR 2.1, 1.0-4.3）, IL-1RN^＊2/L （OR 3.5, 1.7-7.3） and IL-10-592_C/A （OR 3.2, 1.5-6.8） were individually associated with GC, and a combination of these cytokine polymorphisms with Hpylori vacA slb and ml further increased the risk （OR 7.2, 1.4-36.4）. conCLUSION： Although a proinflammatory cytokine genetic profile showed an increased risk for developing GC, the characteristics of Hpylori infection, in particular the status of cagA and vacA genotype distribution seemed to play a major role in GCIR variability in Costa Rica.

pH effect on the formation of THM and HAA disinfection byproducts and potential control strategies for food processing

Chlorine-based sanitizers have seen wide spread use in food sanitation. The reaction of chlorine species with organic matter is a concern for two reasons. Available chlorine can be “used up” by organic compounds resulting in a lower amount of chlorine available for disinfection. Another concern is that some forms of chlorine can react with some organic compounds to form toxic halogenated disinfection byproducts（DBPs）. Many studies have been conducted to evaluate the role of hypochlorous acid（HOCl） and hypochlorite ion（OCl–） in the production of DBPs with a particular interest in the production of trihalomethanes（THMs） and haloacetic acids（HAAs）. Since most of the chlorine reactions are pH dependent, pH is found to have a significant effect on the formation of chlorine DBPs. In many cases, the concentration of THMs decreases and HAAs increases as pH decreases. pH also plays an important role in the determination of the type and amount of DBPs formed, with lower, more acidic, pHs resulting in the formation of less chloroform. This review summarizes the information from the literature on the role of chlorine-based sanitizers as affected by pH in the formation of different types of DBPs. Alternative novel strategies to minimize the formation of DBPs are also discussed.

The SNW Domain of SKIP Is Required for Its Integration into the Spliceosome and Its Interaction with the Pall Complex in Arabidopsis

SKIP is a conserved protein from yeasts to plants and humans. In plant cells, SKIP is a bifunctional regulator that works in the nucleus as a splicing factor by integrating into the spliceosome and as a transcriptional activator by interacting with the Pall complex. In this study, we identified two nuclear localization signals in SKIP and confirmed that each is sufficient to target SKIP to the nucleus. The SNW domain of SKIP is required for both its function as a splicing factor by promoting integration into the spliceosome in response to stress, and its function as a transcriptional activator by controlling its interaction with the Pall complex to participate in flowering. Truncated proteins that included the SNW domain and the N- or C-terminus of SKIP were still able to carry out the functions of the full-length protein in gene splicing and transcriptional activation in Arabidopsis. In addition, we found that SKIP undergoes 26S proteasome-mediated degrada- tion, and that the C-terminus of SKIP is required to maintain the stability of the protein in plant cells. Together, our findings demonstrate the structural domain organization of SKIP and reveal the core domains and motifs underlying SKIP function in plants.

Effects of the urban heat island and climate change on the growth of Khaya senegalensis in Hanoi,Vietnam

Background: Recent projections expect that Vietnam will be affected most severely by climate change with higher temperatures, more precipitation and rising sea levels. Especially increased temperatures will worsen the situations in cities, amplifying the urban heat island effect. Green infrastructures, i.e. urban trees are a common rtool to improve the urban micro-climate for humans. Vital and well growing trees provide greatest benefits such as evaporative cooling, shading, air filtering and carbon storage. However, urban tree growth is often negatively affected by urban growing conditions such as high soil sealing with compacted tree pits providing small growing spaces with limited water, nutrient and oxygen supply, further warm temperatures and high pollution emissions. This study analyzed the growth of urban and rural African mahogany(Khaya senegalensis(Desr.) A. Juss.) trees in the city of Hanoi, Vietnam and the effects of the surrounding climate conditions on tree ***: The results showed that rural African mahogany trees grew better than trees situated in the city center, which is contrary to other results on tree growth of temperate and subtropical cities worldwide. Moreover tree growth was similar regardless of the time of growth. Other results regarding stem growth of African mahogany located in different areas of Hanoi(east, west, north, city center) revealed a better growth in the northern and western outskirts of the city compared to the growth of trees in the city ***: African mahogany trees in the urban centers of Hanoi showed a decreased growth compared to rural trees,which was likely induced by a low ground-water level and high pollution rates. In view of climate change and global warming, the decreased tree growth in the city center may also affect tree service provision such as shading and cooling.
