Automated biomedical signal processing becomes an essential process to determine the indicators of diseased *** the same time,latest develop-ments of artiﬁcial intelligence(AI)techniques have the ability to manage and ana-lyzing massive amounts of biomedical datasets results in clinical decisions and real time *** can be employed for medical imaging;however,the 1D biomedical signal recognition process is still needing to be ***(ECG)is one of the widely used 1-dimensional biomedical sig-nals,which is used to diagnose cardiovascular *** assisted diag-nostic modelsﬁnd it difﬁcult to automatically classify the 1D ECG signals owing to time-varying dynamics and diverse proﬁles of ECG *** resolve these issues,this study designs automated deep learning based 1D biomedical ECG sig-nal recognition for cardiovascular disease diagnosis(DLECG-CVD)*** DLECG-CVD model involves different stages of operations such as pre-proces-sing,feature extraction,hyperparameter tuning,and classiﬁ*** the initial stage,data pre-processing takes place to convert the ECG report to valuable data and transform it into a compatible format for further *** addition,deep belief network(DBN)model is applied to derive a set of feature ***,improved swallow swarm optimization(ISSO)algorithm is used for the hyper-parameter tuning of the DBN ***,extreme gradient boosting(XGBoost)classiﬁer is employed to allocate proper class labels to the test ECG *** order to verify the improved diagnostic performance of the DLECG-CVD model,a set of simulations is carried out on the benchmark PTB-XL dataset.A detailed comparative study highlighted the betterment of the DLECG-CVD model interms of accuracy,sensitivity,speciﬁcity,kappa,Mathew correlation coefﬁcient,and Hamming loss.

NanoHDA： A nanoparticle-assisted isothermal amplification technique for genotyping assays

Isothermal methods, such as helicase-dependent amplification （HDA）, have an advantage over polymerase chain reaction for DNA amplification owing to their ease of operation. Here, we developed a new HDA method that is nanoparticle-assisted, termed nanoHDA. This method uses gold nanoparticles （AuNPs） to improve the sensitivity and specificity of the isothermal method. In HDA, the denaturation of DNA templates is mediated by helicases, but this method is limited by the low denaturation efficiency of helicases. In this report, AuNPs with preferential affinity for single-stranded DNA （ssDNA） were utilized to improve the denaturation efficiency of helicases. The same affinity property of nanoparticles can also enhance specificity by suppressing primer-dimer formation. This nanoHDA method was employed to genotype the KRAS gene in genomic DNA samples from colorectal cancer patients, as achieved by the hybridization of nanoHDA amplicons using the NanoBioArray chip.

Cardiac remodeling as a consequence of atrial fibrillation： An anatomical study of perfusion-fixed human heart specimens

Background Atrial fibrillation （AF） causes a continuum of atrial anatomical remodeling. Methods Using a library of perfusion-fixed human hearts, specimens with AF were compared to controls. During this preliminary assessment study, direct measurements were taken of atrial volume, pulmonary vein （PV） circumference, and left atrial （LA） wall thicknesses. Results Hearts with AF typically had larger atrial volumes, as well as a much larger variation in volume compared to controls （range of 59.6-227.1 mL in AF hearts compared to 65.1-115.9 mL in controls）. For all hearts, right PVs were larger than left PVs （mean： 171.4 ± 84.6 mm^2 for right and 1182 ± 50.1 mm^2 for left, P 〈 0.005）. LA wall thicknesses ranged from 0.7 mm to 3.1 mm for both AF and control hearts. Conclusions Hearts with AF had a large range of sizes which is consistent with the progression of atrial remodeling during AF. The large range of thicknesses will influence the amount of energy needed to create transmural lesions during ablation procedures.

Hypertriglyceridemia-induced pancreatitis: A case-based review

Hypertriglyceridemia is an established cause of pancreatitis. In a case-based approach, we present a review of hypertriglyceridemJa and how it can cause pancreatitis. We outline how to investigate and manage such patients. A 35 year old man presented to the emergency department with abdominal pain and biochemical evidence of acute pancreatitis. There was no history of alcohol consumption and biliary imaging was normal. The only relevant past medical history was that of mild hyperlipidemia, treated with diet alone. Physical exam revealed epigastric tenderness, right lateral rectus palsy, lipemia retinalis, bitemporal hemianopsia and a delay in the relaxation phase of his ankle reflexes. Subsequent laboratory investigation revealed marked hypertriglyceridemia and panhypopituarism. An enhanced CT scan of the head revealed a large suprasellar mass impinging on the optic chiasm and hypothalamus. The patient was treated supportively; thyroid replacement and lipid lowering agents were started. He underwent a successful resection of a craniopharyngioma. Postoperatively, the patient did well on hormone replacement therapy. He has had no further attacks of pancreatitis. This case highlights many of the factors involved in the regulation of triglyceride metabolism. We review the common causes of hypertriglyceridemia and the proposed mechanisms resulting in pancreatitis. The incidence and management of hypertriglyceridemiainduced pancreatitis are also discussed.

Turnaround times for molecular testing of pediatric viral cerebrospinal fluid samples in United Kingdom laboratories

BACKGROUND Rapid molecular testing has revolutionized the management of suspected viral meningitis and encephalitis by providing an etiological diagnosis in<90 min with potential to improve outcomes and shorten inpatient ***,use of molecular assays can vary *** To evaluate current practice for molecular testing of pediatric cerebrospinal fluid(CSF)samples across the United Kingdom using a structured *** A structured telephone questionnaire survey was conducted between July and August *** was collected on the availability of viral CSF nucleic acid amplification testing(NAAT),criteria used for testing and turnaround times including the impact of the coronavirus disease 2019 *** Of 196/212(92%)microbiology laboratories responded;63/196(32%)were excluded from final analysis as they had no on-site microbiology laboratory and outsourced their *** 133 Laboratories included in the study,47/133(35%)had onsite facilities for viral CSF *** currently undertaking onsite NAAT(n=47)had much faster turnaround times with 39 centers(83%)providing results in≤24 h as compared to those referring samples to neighboring laboratories(5/86;6%).CONCLUSION Onsite/near-patient rapid NAAT(including polymerase chain reaction)is recommended wherever possible to optimize patient management in the acute setting.

Jasmonic acid-isoleucine formation in grapevine(Vitis vinifera L.) by two enzymes with distinct transcription profiles

The plant hormone jasmonic acid （JA） is essential for stress responses and the formation of reproductive organs, but its role in fruit development and ripening is unclear. Conjugation of JA to isoleucine is a crucial step in the JA signaling pathway since only JA-Ile is recognized by the jasmonate receptor. The conjugation reaction is catalyzed by JA-amido synthetases, belonging to the family of Gretchen Hagen3 （GH3） proteins. Here, in vitro studies of two grapevine （Vitis vinifera L. cv Shiraz） GH3 enzymes, VvGH3-7 and VvGH3-9, demonstrated JA-conjugating activ- ities with an overlapping range of amino acid substrates, including isoleucine. Expression studies of the correspond- ing genes in grape berries combined with JA and JA-lle measurements suggested a primary role for JA signaling in fruit set and cell division and did not support an involvement of JA in the ripening process. In response to methyl JA （MeJA） treatment, and in wounded and unwounded （distal）leaves, VvGH3-9 transcripts accumulated, indicating a participation in the JA response. In contrast, VvGH3-7 was unresponsive to MeJA and local wounding, demonstrating a differential transcriptional regulation of VvGH3-7 and VvGH3- 9. The transient induction of VvGH3-7 in unwounded, distal leaves was suggestive of the involvement of an unknown mobile wound signal.
